Abstract:Objectives To evaluate prospectively the efficacy of Extracorporeal Shock Wave Therapy (ESWT) as a conservative treatment for Peyronie's disease. Methods 112 patients from January 2022 to January 2023 were included in the prospective study. Each patient was treated with a minimum of three sessions of ESWT (3000 shock wavs at an energy density of 0.11-0.17 mJ/mm2) at 3-week intervals. The angulation was calculated by auto-photography: the mean angulation before treatment was 48 degrees (range 10°-100°). The severity of pain on erection was assessed by a visual analog scale (0-5): all patients had symptoms (60 scored 5). Moreover a self-evaluation questionnaire (International Index of Erectile Function) was used: 42 patients had erectile dysfunction (IIEF<18). The plaque was located by palpation. The mean disease duration was 18 months (4-60 mo). The mean follow up was 9 months (4-24). Results All patients completed the protocol. The tolerance and safety were excellent. The results were analysed using the Wilcoxon signed-rank test. Of the 60 patients with pain on erection, 54 (90%) noticed immediately relief after ESWT with mean reduction 2.9 on the visual analog pain scale (p<0.00001). An improvement in angulation greater than 10 degrees was observed in 64 patients (57.1%), with a mean reduction of 30 degrees (p<0.001). 11 patients with erectile dysfunction (26.2%) han an increased IIEF score greater than 4. 62 patients (55.4%) thought the plaque was smoother. Only 2 patients (1.8%) experimented minimal bruising at the site of treatment the day after, no major complications were reported. Conclusions ESWT represents an effective, feasable and safe conservative treatment for Peyronie's disease. It seems to reduce pain during erection and sometimes improve the penile angle. Abstract:Background: A systematic review of the evidence was conducted to assess the efficacy of low-intensity extracorporeal shock wave therapy (LI-ESWT) for patients with Peyronie`s Disease (PD). Methods: A comprehensive search of the Cochrane Registry, PubMed and Embase databases was conducted to identify all controlled trials, including randomised controlled trials (RCTs), cohort studies and case-control studies, focusing on the efficacy of LI-ESWT in treating PD, and published before February 2023. The size of plaques, curvature deviation, visual analog scale [VAS] and International Index of Erectile Function (IIEF) were the most commonly used tool to evaluate the treatment effectiveness of LI-ESWT. Results: There were 7 studies including 475 patients from 1999 to 2023. The meta-analysis of the data revealed that LI-ESWT could considerably enhance the proportion of men experiencing a reduction in penile plaques (RD 0.27, 95% CI: 0.04-0.50, P = 0.02), improvement in penile curvature (RD: 0.13; 95% CI, 0-0.26; p = 0.05), alleviation of pain (RD 0.22, 95% CI: 0.01-0.42, P = 0.04), and complete remission (RD 0.38, 95% CI 0.23-0.52, P < 0.00001). However, there were no significant differences in improvement of sexual function (MD: 1.44; 95% CI, -3.10-5.97; p = 0.53) between LI-ESWT and the placebo group. Conclusions: According to these studies, LI-ESWT has the potential to decrease plaque size and improve penile curvature or pain in men with PD. The publication of robust evidence from additional well-designed long-term multicenter randomized controlled trials would provide more confidence regarding use of these devices in patients with PD.

Abstract:Objectives Low-intensity extracorporeal shockwave therapy (LI-ESWT) is a recent introduction therapy in the treatment of Peyronie's disease (PD) with pain during erection that consists of 8 sessions of approximately 20 minutes each and a number of strokes applied varies from 2000 to 3000 for each session which appear to act by stimulating a pro-inflammatory response in the tissues on which it is applied. The key step is the stimulation of the expression of the VEGF (Vascular Endothelial Growth Factor) gene and so the consequent neoangiogenesis and blood overflow would lead to a series of modifications at the level of the plaque in PD that would ultimately lead to the inhibition of the inflammatory process in progress and this would seem to reduce the pain typical of the acute phase and it would also seem effective in reducing the size of the plaque itself and avoiding a worsening of the penile curvature in erection. Methods A prospective cohorts of 23 patients with PD presenting with pain during erection was evaluated. Every patient underwent standardized baseline visit at our Department, including physical examination, medical history, International Index of Erectile Dysfunction (IIEF) and pain evaluation. Pain during erection was the indication for treatment that was administered for 8 sessions. After treatment, all patients repeated the pain evaluation and IIEF questionnaire to evaluate the improvement through t-test. Results Median age of patients was 52.5 years. After treatment pain was improved in the whole cohort (100%). A significant improvement in the IIEF score was observed after Li-ESWT sessions (p value < 0.001). Conclusions This initial study shows that there is a significant improvement of pain after shock wave treatment at in patients affected by Peyronie's disease. An improved IIEF score after treatment was also observed. Larger cohort is needed to better evaluate Li-ESWT effect of pain and IIEF score in men with Peyronie's disease. Abstract:The present umbrella review of five systematic reviews and meta-analyses was conducted to investigate the effectiveness of Low-Intensity Extracorporeal Shock Wave Therapy (Li-ESWT) in the treatment of vascular origin Erectile Dysfunction (ED). A search was carried out in the databases of Pubmed, Scopus, Medline, Scielo and Embase. Participants were divided into two groups: an experimental group receiving Li-ESWT and a control group receiving simulated shock waves. The main variable of this study is ED, measured using the International Index of Erectile Function-Erectile Function (IIEF-EF) and the Erection Hardness Score (EHS) scale. The results showed a statistically significant increase in the mean IIEF-EF score in the experimental group. Overall, four out of five articles reported an increase in the EHS score in the Li-ESWT group compared to the placebo. Concerning the treatment parameters, better outcomes were observed with an energy density of 0.09 mJ/mm2 and the application of 1500-2000 pulses. Additionally, a follow-up of 6-12 months resulted in greater improvement in ED compared to 3 months, although more studies investigating follow-ups beyond 12 months are needed. Obtaining conclusive and clear results is challenging; however, everything indicates that Li-ESWT is an innovative therapeutic alternative for vascular-origin ED due to its low risk and improvement in erectile function.

Abstract:Introduction: Extracorporeal shock wave therapy (ESWT) for erectile dysfunction (ED) presents a challenging paradox. While numerous clinical studies, systematic reviews, and meta-analyses have been published, indicating a substantial body of evidence supporting the efficacy and safety of ESWT, significant questions remain. Notably, the American Urological Association (AUA) continues to classify ESWT for ED as investigational (Evidence Level: Grade C), suggesting that the true therapeutic effect of ESWT may differ considerably from current estimates. This review aims to critically assess the evidence and propose strategies to address this unresolved discrepancy. Data sources: We systematically searched two electronic databases (PubMed and Ovid/Embase) and published systematic reviews on ESWT for ED and compiled a systematic literature review and meta-analysis based on 87 relevant studies. Areas of agreement: There is clear evidence that ESWT for ED is effective and can therefore be a valuable treatment modality in the management of ED. Areas of controversy: Current assessments of ESWT for ED as investigational by, e.g., the AUA may not stem from a lack of clinical studies, insufficient related basic science, or an inadequate number of systematic reviews and meta-analyses. Instead, the deficits lie in the area of the scientific quality of the clinical studies published to date. Growing points: We hypothesize that this unfortunate situation will only change if the following aspects will be rigorously considered in future clinical studies on ESWT for ED: adequate characterization and reporting of extracorporeal shock waves, appropriate handling of missing data and intercurrent events, and comprehensive classification of ESWT in the overall context of the available treatment options for ED. Areas for developing research: We are convinced that the consistent implementation of these aspects will significantly contribute to establishing ESWT as the first truly regenerative therapy in the management of ED. This overall aim justifies the corresponding efforts, for the benefit of our patients.

Abstract:Objective: To describe the evidence of Platelet Rich Plasma (PRP), Stem cells therapy (SCT) and Extracorporeal shockwave therapy (ESWL) for the treatment of Peyronies disease (PD), including information from the main urological society guidelines. Materials and methods: A literature review of PubMed articles published between 2000 and 2023 was conducted, utilizing keywords such as "Peyronie's Disease", "Penile curvature", "Platelet Rich Plasma", "Stem cells", and "Extracorporeal shockwave therapy". Only full-text articles in English were included, excluding case reports and opinions. Results: A considerable number of clinical trials were conducted using PRP penile injections for therapy of PD, showing reduction of curvature, plaque size and improvement in quality of life. Preclinical studies in rats have shown the potential benefit of adipose-derived stem cells, with improvements in erectile function and fibrosis. Human studies with mesenchymal stem cells demonstrated promising results, with reduction of curvature and plaque size. ESWL effects on PD were investigated in randomized clinical trials and demonstrated no significant impact in curvature or plaque size, but reasonable effect on pain control. Conclusion: Restorative therapies has emerged as an innovative treatment option for PD and the results from current studies appear to be promising and demonstrated good safety profile. Unfortunately, due to scarce evidence, PRP and SCT are still considered experimental by American Urological Association (AUA) and European Association of Urology (EAU) guidelines. ESWT is recommended, by the same guidelines, for pain control only. More high-quality studies with long-term follow-up outcomes are needed to evaluate efficacy and reproducibility of those therapies.

Abstract:To review the evidence of clinical efficacy of low-intensity extracorporeal shock wave therapy (Li-ESWT) for the treatment of erectile dysfunction (ED). A search on PubMed using Medical Subject Headings terms [((low intensity extracorporeal shockwave therapy) OR (Li-ESWT)) AND (erectile dysfunction)] was conducted in August 2022, to obtain studies on the use of Li-ESWT for the treatment of ED. Its success rate in terms of International Index of Erectile Function-5 (IIEF-5) score and Erection Hardness Score (EHS) improvement was recorded and analysed. A total of 139 articles were reviewed. Overall, 52 studies were included in the final review. 17 studies were on vasculogenic ED, 5 on post pelvic surgery ED, 4 specifically on ED in diabetic patients, 24 on non-specified origin ED and 2 on mixed pathophysiological origin ED. The mean age of patients was 55.87±7.91 (standard deviation) years and the duration of ED was 4.36±2.08 years. The mean IIEF-5 score went from 12.04±2.67 at baseline to 16.12±5.72, 16.30±3.26 and 16.85±1.63 respectively at 3, 6 and 12 months. The mean EHS went from 2.00±0.46 at baseline to 2.58±0.60, 2.75±0.46 and 2.87±0.16 respectively at 3, 6 and 12 months. Li-ESWT may be a safe and efficacy option for the treatment and cure of ED. Further studies are needed to assess which patients are more suitable for this procedure and which Li-ESWT protocol can lead to the best outcomes.

Abstract:Background: Low-intensity extracorporeal shock wave therapy (Li-ESWT) is a new method for treating erectile dysfunction (ED), but there are no standards yet for its indications. Aim: The study aimed to suggest the early clinical efficacy of Li-ESWT and explore its related factors in young and middle-aged patients with ED who responded to phosphodiesterase type 5 Inhibitors (PDE5Is). Methods: Data from 61 patients with ED who had previously responded to oral PDE5Is and subsequently underwent Li-ESWT were collected. This included information on age, body mass index, total testicular volume, sex hormones, as well as IIEF-EF scores before treatment and at 1, 3, and 6 months after treatment. The treatment regimen involves a weekly session for four consecutive weeks, with each session administering 5000 shock wave pulses. Linear regression analysis was utilized to identify factors associated with the efficacy of Li-ESWT treatment. Additionally, the improvement in different severity groups of ED before and after treatment, along with their IIEF-EF scores, was compared. Outcomes: Li-ESWT was more targeted and effective for young and middle-aged patients with erectile dysfunction who responded to PDE5Is. Results: The age of enrolled patients ranges from 22 to 53 years old, and the IIEF-EF scores at 1 month, 3 months, and 6 months after treatment were compared to baseline for efficacy assessment, showing significant improvements (P < .0001) in all instances. Linear regression analysis using baseline data revealed predictive factors associated with treatment efficacy: treatment efficacy was negatively correlated with baseline IIEF-EF scores (t = -2.599, P = .013) and positively correlated with baseline LH levels (t = 2.170, P = .036). Clinical implications: Given the considerable cost of Li-ESWT treatment and the emphasis on treatment continuity, we hope to identify the most suitable candidates for Li-ESWT therapy, thereby optimizing its application. Strengths and limitations: Our findings provide a better solution for nonelderly ED patients who are responsive to PDE5Is. This study was limited by our sample size and follow-up time. Conclusion: After 3 months of Li-ESWT, the IIEF-EF score gradually stabilizes and short-term maintenance of PDE5Is medication increases the responsiveness to shock wave therapy.

Abstract:Objective To investigate the clinical effects of extracorporeal shock wave in the treatment of chronic prostatitis / chronic pelvic pain syndrome.Methods Twenty patients with CP/CPPS who were diagnosed by NIH clinical criteria from August of 2009 to January of 2001 received ESWT with each by 2000 impulses, operating voltage range from 11-15KV, 25 times in one minutes.Each patient was treated four times once a week, ESWT. The parameters such as NIH-CPSI, IPSS,IIEF, MPQ were investigated using validated questionnaires before and after treatment.Results All patients received outpatient treatments and had no problems during follow-ups.THE effective power of NIH-CPSI, IPSS,IIEF was 62.50%,31.25%,18.75% respectively.The efficacy of ESWT was statistically significant in relieving pain and improving the quality of life, but no improving in sexual function status and alleviating the symptoms of lower urinary tract obstruction.Conclusion ESWT is a safe, effective and cheap method for treatment of the patients with CP/CPPS.It can relieve pains of patients and improve their life quality.

Abstract:In recent times, low-intensity shockwave therapy (LISWT) has become one of the therapeutic modalities for the treatment of andrological disorders with controversial findings, which makes it difficult to recommend them in the guidelines (1-3). In this scenario this interesting paper aimed to evaluate the applicability of the LISWT in the treatment of erectile dysfunction (ED), Peyronie’s disease (PD) and chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). During their review, the authors analyzed 11 RCTs and 5 meta-analyzes that investigated LISWT for ED, 4 RCTs and 1 meta-analysis for PD and 5 RCTs for CP / CPPS. For erectile dysfunction purposes, although there is a tendency in this direction, the review made clear a heterogeneity among treatment protocols, with controversial findings and indication being restricted to vasculogenic ED. Before starting treatment, patients should be aware that the scientific evidence is controversial and that the expected improvement may not be clinically relevant. In relation to PD, data available from RCTs is poor. Patient inclusion criteria vary from stable disease to non-stable disease and follow-up assessment varies too much (24 weeks to 1 year) using different sources of energy and heterogeneous protocols proposed, making any comparison difficult. Nevertheless, in a large prospective, randomized, double-blind, placebo-controlled study, with four weekly treatment sessions of ESWT, they observed a significant improvement in penile pain and thus Vol. 46 (1): 134-135, January February, 2020

Abstract:Background and objective: Erectile dysfunction (ED) is a prevalent and impactful complication post definitive management of prostate cancer. The mechanism of ED is thought to be secondary to vascular and neural injury as well as corporal smooth muscle damage with resultant fibrosis. The use of penile rehabilitation in ED following treatment for prostate cancer has been studied. Low-intensity extracorporeal shockwave therapy (Li-ESWT) is a novel treatment for ED thought to stimulate neovascularization and nerve regeneration, and as such, has gained interest in treatment of ED related to radical prostatectomy or radiation therapy. Herein, we performed a narrative review on the use of Li-ESWT in management of ED following treatment for prostate cancer. Methods: A literature review was performed using PubMed and Google Scholar. Studies evaluating Li-ESWT following prostate cancer treatment were included. Key content and findings: We identified three randomized controlled trials and two observational studies that assessed use of Li-ESWT for ED after prostate surgery. Use of Li-ESWT across most studies showed improvements in the International Index of Erectile Function-erectile function (IIEF-EF) domain scores, but this improvement was not statistically significant. Additionally, use of Li-ESWT in an early versus delayed fashion does not appear to affect changes in long-term sexual function scores. No data on use of Li-ESWT after radiotherapy were identified. Conclusions: There is a paucity of data regarding use of Li-ESWT for penile rehabilitation in treatment of ED post-prostate cancer therapy. Current protocols for Li-ESWT are not standardized and have a limited number of participants with short duration of follow-up. Additional evaluation is needed to determine optimal Li-ESWT protocols. Ideally, studies should have longer follow-up to truly evaluate the clinical significance of Li-ESWT in the treatment of post-prostatectomy ED. Furthermore, the role of Li-ESWT after radiotherapy remains elusive.

Abstract:Abstract Previous published studies have shown an improvement of penile hemodynamic parameters after low-intensity extracorporeal shockwave therapy (Li-ESWT). However, the clinical significance of these findings remains unclear, and definitive selection criteria for Li-ESWT based on preexisting comorbidities have yet to be established. This was an observational study of 113 patients with ED, evaluated between January 2019 and December 2021 in Andrology Unit at the Department of Urology and Renal Transplantation, University of Foggia (Foggia, Italy). Penile dynamic Doppler was performed to evaluate vascular parameters and 5-item version of the International Index of Erectile Dysfunction (IIEF-5) questionnaire was administered to assess the severity of ED. This was repeated 1 month after treatment. Patients with a peak systolic velocity (PSV) <30 cm s −1 were considered eligible for Li-ESWT. Our protocol consisted of 8 weekly sessions with 1500 strokes distributed in 5 different locations along the penis. After treatment, a significant mean (±standard deviation [s.d.]) PSV increase of 5.0 (±3.4) cm s −1 was recorded and 52/113 (46.0%) patients reached a PSV >30 cm s −1 at posttherapeutic penile dynamic Doppler. A clinically significant IIEF-5 score improvement was observed in 7 patients, 21 patients, and 2 patients with mild-to-moderate, moderate, and severe pretreatment ED, respectively. No different outcomes were assessed based on smoking habits, previous pelvic surgery, or use of oral phosphodiesterase-5 inhibitor (PDE5i). On the other side, only 1 (6.7%) in 15 patients with diabetes mellitus showed an IIEF-5 score improvement after Li-ESWT. Shockwave treatment determined a significant increase in PSV and correlated IIEF-5 improvement in ED patients. This advantage seemed particularly evident for moderate ED and was not affected by smoking habits, previous pelvic surgery, and use of PDE5i. Conversely, diabetic patients did not benefit from the treatment.

Abstract:Background: An umbrella review was conducted to compare the effectiveness of extracorporeal shock wave therapy (ESWT) versus non-ESWT in the treatment of knee osteoarthritis (KOA). Materials and methods: Three databases including PubMed, Embase and Web of science were searched up to September 2023. Literature screening, quality evaluation, and data extraction were performed according to inclusion and exclusion criteria. Meta-analysis of outcome indicators was performed using Revman 5.4 software. Results: A total of eight meta-analysis were included in this umbrella review. All meta-analysis were graded against a Measurement Tool to Assess Systematic Reviews 2 (AMSTAR 2) and scored between 8 and 11. Compared to the sham group, the ESWT group showed better results in WOMAC (Western Ontario and McMaster Universities Arthritis Index) [mean difference (MD)=−2.94, 95% CI: −5.52, −0.37, P =0.03, I2=60%], Visual Analog Scale (VAS) (MD=−2.0, 95% CI: −2.5, −1.5, P <0.01, I2=0%), range of motion (ROM) (MD=17.55, 95% CI: 13.49, 21.61, P <0.00001, I2=0%), and Lequesne index (MD=−2.85, 95% CI: −3.64, −2.07, P <0.00001, I2=48%). Conclusion: Based on the results of our analysis, ESWT is now an effective therapy for improving pain and function in patients with KOA.

Abstract:Objectives Low intensity - Extracorporeal Shock Wave Therapy (Li-ESWT) can be considered a novel promising method for treatment men with erectile dysfunction (ED), in particular after failure of medical therapy. The purpose of this study was to investigate feasibility, safety and effectiveness of this kind of therapy through a follow-up period of 12 and 18 months. Methods We included 102 patients with ED of vascular origin who failed to respond to oral PDE5-I. The International Index of Erectile Function (IIEF) and Erection Hardness Score (EHS) questionnaires were used to evaluate erectile function. All patients had an EHS of 2 or less with PDE5-I. The patients underwent to one session of Li-ESWT per week for 6 weeks. During each session 3000 shocks (energy density 0.09 mJ/mm2 and frequency 120/min) were erogated. Patients were considered responders to ESWT whenever they showed improvement in erection parameters (IIEF and EHS) 3 or greater: erection hard enough to permit vaginal penetration. Side effects were recorded. Results All patients completed follow up. Mean age was 63.9 years (31-82), mean duration of ED was 22 months (5-40). IIEF score was 11 at baseline, 21 at 12 months follow up, 18 at 18 months; EHS improved in 72 patients at 12 months (70.6%). Younger men (<45 years), short ED duration (<12 months) and moderate ED severity, responded better to the treatment (p<0.05). However, during follow up, a gradual decrease in efficacy was observed. Efficient parameters were mantained by 66 patients (64.7%) at 12 months and by 54 patients (52.9%) at 18 months. No adverse events were reported during or after treatment. Conclusions Li-ESWT offers a valid therapeutic option in patients with erectile dysfunction, particularly within 12 months. The beneficial effect seems to diminish in the final assessment. Multicentre controlled studies and longer follow up are needed. Conflicts of Interest No.

(2015 to 2022) Studies that includes subjects with Tennis Elbow. RESULTS: 20 studies were reviewed from which 10 studies concluded that ESWT is effective in Tennis Elbow patients. CONCLUSIONS: Based on the analysis of these 10 articles, it can be concluded that ESWT is an effective treatment regimen in individual with Tennis Elbow patients.
