Abstract:Objective:To investigate the changes of lung function and inflammatory markers in children with asthma after standardized treatment.Methods:A retrospective study was performed on the clinical data of 198 children with asthma who were followed up for more than 3 years from January 2010 to October 2018, including 112 males and 86 females.The age was(8.92±3.41)years old, ranging from 4 to 14 years old.Children were divided into control group and difficult control group according to the level of disease control.According to whether or not the co-rhinitis was divided into asthma group and asthma with rhinitis group.According to the age of the first onset of children, divided into onset(first wheezing attack)≤3 years old group and onset > 3 years old group.Comparing inflammatory markers[immunoglobulin E(IgE), exhaled nitric oxide(FeNO)]and lung function levels[forced vital capacity in percent predicted values(FVC%pred), the forced expiratory volume in the first second in percent predicted values(FEV 1%pred), forced expiratory volume one second/forced vital capacity in percent predicted values(FEV 1/FVC%pred), peak expiratory flow in percent predicted values(PEF%pred), forced expired flow at 50% of forced vital capacity in percent predicted values(FEF50%pred), forced expired flow at 75% of forced vital capacity in percent predicted values(FEF75%pred)]. Results:After three years of standardized treatment, 154 patients were in control and 44 patients were in poor control.There were 42 asthmatic patients and 156 asthmatic patients with rhinitis.There were 138 cases with onset less than 3 years old, 60 cases more than 3 years old.The FEV 1/FVC%pred of the children in the refractory group was lower than that of the control group( P<0.05). In the third year of standardized treatment, the differences of FVC%pred, FEV 1%pred, FEV 1/FVC%pred, PEF% pred, FEF50% pred and FEF75%pred between the two groups were statistically significant( P<0.05). Before the beginning of treatment and three years of standardized treatment, there were no statistically significant differences in lung function indicators between the asthmatic group with rhinitis and the asthmatic group with asthma alone( P>0.05). At the beginning of treatment and the second year of standardized treatment, FVC%pred in the more than 3-year-old group was significantly lower than that in the group with onset ≤3 years old( P<0.05). In the first year of standardized treatment, FEV 1%pred, FEF50%pred and FEF75%pred were all higher than the group with onset ≤3 years old at the first year of standardized treatment( P<0.05). FeNO in more than 3-year group at the beginning of treatment and the third year of standardized treatment was higher than that in the group with onset less than 3 years old( P<0.05). Before treatment, IgE level in the control group[(185.00±251.43)IU/L]was significantly lower than that in the refractory group[(482.11±403.90)IU/L], ( P<0.001). After one year of standardized treatment, IgE levels[(275.40±281.36)IU/L]in the control group and[(224.48±252.04)IU/L]in the refractory group showed no statistically significant difference( P=0.596). Correlation analysis between FeNO and FEV 1%pred: FeNO was negatively correlated with FEV 1%pred in the first year of treatment initiation and standardized treatment( P<0.05). There was no correlation between the two at 2 and 3 years of follow-up. Conclusion:Early intervention and standardized management of allergic rhinitis and asthma are of great significance to improve the prognosis of children with asthma.
