Study on Genotype and Resistance of ESBLs-producing Escherichia Coli Causing Biliary Tract Infection

Abstract:OBJECTIVE To realize the bacterial spectrum and drug resistance in patients with biliary infection and to study the gene type of the ESBLs-producing Escherichia coli.METHODS Biliary specimens from patients with biliary tract infections were cultured for bacterial identification,and the antimicrobial activity tests were determined.Meanwhile,the special primer was designed to detect the ESBL gene type by PCR.RESULTS Of 240 biliary specimens,200 strains bacteria and 25 strains fungus were isolated.There were 80 strains E.coli,which was 40%,the antimicrobial activity tests show most of the E.coli were multidrug resistant,however,all of them were sensitive to imipenem,meropenem and amikacin.Phenotypic confirmatory test showed that 20 strains all had the TEM gene,almost of the strains carried 5 kind ESBL gene type,among them,there were 10 strains CTX-M-9,6 strains CTX-M-1,4 strains OXA-1 and 5 strains SHV gene.Some strains carried more than two kinds ESBL gene.CONCLUSION The main pathogenic microbes of biliary infection is mainly E.coli,some are with multidrug resistance in high levels in our hospital.most were ESBL strains.The main gene type is TEM gene,however,there are CTX-M,OXA and SHV gene type in the strains.
