Abstract:Failure Modes and Effects Analysis (FMEA) is an effective technique for identifying and eliminating possible failures. FMEA has been widely used in the manufacturing industry and in various phases of the life cycle of a product. The aim of FMEA is to ensure high quality and reliability. Design FMEA (DFMEA) is used to identify and analyze potential failure modes and their associated causes for product design. It is a verification activity that can help avoid a large percentage of product design problems before the design is finalized. When dealing with new generations of a product including design changes on the basis of existing ones, DFMEA of earlier generations can be used as a reference for such design modifications. Traditionally, criticality or risk assessment in FMEA is performed by developing a risk priority number (RPN). Failure modes that have high RPN values are assumed to be more critical and deserve more attention. In this paper, we propose a method to assess failure modes in DFMEA considering two RPNs for diesel engines. In addition to the traditional RPN, a new RPN is proposed. A diesel engine cylinder head is used as an example to illustrate the application of the two RPNs in a design FMEA.

Abstract:Maintainability analysis method combined with FMECA and FTA is proposed for the failure analysis of a type of diesel engine combustion system. Failure modes and cause analysis for this type of diesel engine combustion system are identified, and the hazard is analyzed. Finally, the real failure situation of this diesel engine is presented to validate the analysis results from the proposed method, which can be used to conduct the maintenance activities.

Abstract:The failure mode and effect analysis (FMEA) is an effective tool to analyze risks and potential effects of complex systems, and it is one of the most widely used risk analysis methods for complex systems as there often exists various factors that could affect the operation of the complex systems. Conventional FMEA methods have been limited to using crisp values to represent the assessments, which has been criticized for having many deficiencies. Marine diesel fuel injection system is an important part of marine diesel engine, and its failure could directly affect the performance of the marine diesel engine and even impact the safe operation of the ship. However, little attention has been paid to the FMEA of the marine diesel fuel injection system. To this end, this paper presents a novel FMEA method based on the best-worst method (BWM) and TOPSIS method with probabilistic linguistic term set (PLTS). Firstly, the PLTS is used to represent the uncertain and linguistic judgments of experts. Then, the BWM is extended with PLTS to determine the weights of different elements for FMEA, and the TOPSIS is extended with PLTS to assess and rank different failure modes. Finally, a case study on marine diesel fuel injection is presented, and the most critical failures are identified for improvement measures. The results show that the proposed method could help managers and engineerings identify the most important failure modes for marine diesel fuel injection system.

In this paper, a series of reliability indexes were applied in diesel engine reliability evaluation based on maintenance records. Several methods including life cycle distribution and FMEA (Failure Mode and Effects Analysis) were also presented. At the end, a diesel engine (Model 6108ZQ) was used as a case study.

Abstract:This study aims at improving the effectiveness of failure mode and effect analysis (FMEA) technique. FMEA is a widely used technique for identifying and eliminating known or potential failures from system, design, and process. However, in conventional FMEA, risk factors of Severity (S), Occurrence (O), and Detection difficulty (D) are simply multiplied to obtain a crisp risk priority number without considering the subjectivity and vagueness in decision makers’ judgments. Besides, the weights for risk factors S, O, and D are also ignored. As a result, the effectiveness and accuracy of the FMEA are affected. To solve this problem, a novel FMEA approach for obtaining a more rational rank of failure modes is proposed. Basically, two stages of evaluation process are described: the determination of risk factors’ weights and ranking the risk for the failure modes. A rough group ‘Technique for Order Performance by Similarity to Ideal Solution’ (TOPSIS) method is used to evaluate the risk of failure mode. The novel approach integrates the strength of rough set theory in handling vagueness and the merit of TOPSIS in modeling multi‐criteria decision making. Finally, an application in steam valve system is provided to demonstrate the potential of the methodology under vague and subjective environment. Copyright © 2013 John Wiley & Sons, Ltd.

Abstract:<p>This paper proposes a two-stage Failure Mode and Effect Analysis (FMEA) technique as a basis for implementing the failure analysis of offshore wind turbines. At the first stage, critical failure causes and failure modes of each component of offshore wind turbines are identified. In the next stage, critical components and systems of offshore wind turbines are ascertained by a cost-and-risk-based index that considers both risk priority and failure costs of components. The objective is to overcome some weaknesses of the traditional FMEAs including: (i) Risk-based FMEA ignores practical information extracted in the operation stage of offshore wind turbines such as failure cost and, (ii) Cost-based FMEA addresses mainly failures of components and systems and cannot deepen to failure modes and failure causes of offshore wind turbines. A methodology towards conducting uncertainty analysis of FMEA results is developed to provide a new insight into a good understanding of FMEAs and their results. The developed uncertainty analysis methodology reveals that the proposed two-stage FMEA technique is adequate to reduce the uncertainty of FMEA results and is superior in failure analysis of offshore wind turbines. The application of the methodology can provide recommendations toward corrective actions and condition-based maintenance implementations.</p>

Abstract:Failure mode and effects analysis (FMEA) is a systematic bottom-up structured method used originally in aerospace systems then electronic systems to implement failure analysis, criticality analysis, and failure clearance. With its effective application, FMEA has been extended rapidly into mechatronic systems and others to improve system reliability. However, there exist different compositions, loading forms, operation modes, and failure mechanisms for diverse kinds of systems, so the formulaic extension of FMEA usually is not suitable for the actual situation of followers. In this paper, we propose a function-motion-action (FMA) hierarchy structure especially suitable for mechatronic systems, which focuses on describing and analyzing failure behaviors such as identifying failure modes, analyzing failure causes, and mapping failure effects at each system structure layer without vague and overlapping interpretations. Three risk factors namely occurrence, detection or non-detection, and severity are both defined for each of failure behaviors at each system structure layer, in which the occurrence describes all failure behaviors and the severity is reliant on the condition of the detection and non-detection about all failure behaviors. A primary risk calculation model using quantitative risk data is established to garner the risk level of each failure, based on which multiple and specific risk levels of each failure are explained when considering different levels of failure effects and different combinations of control methods.

Abstract:Diesel engine is a multi-disciplinary complex system with the harsh working environment and high reliability requirements. Its design for satisfying the basic reliability requirement is a high-demanding work. Correspondingly the reliability prediction is served as an important tool for reliability design. However, in the traditional reliability method, the analytical outcome obtained from the conventional FMECA was not always sufficient. This increases the workload of designers in charge of reliability, and reduces the working efficiency. Therefore, this paper conducts an in-depth research with the emphasis on the diesel engine’s reliability prediction, in which the failure mode and the information of criticality provided by FMECA will be made full use of to carry out the reliability prediction. Meanwhile, to make the prediction results more objective and reasonable, this paper introduces the weight of experts, and provides a prediction model, in which the fuzzy analytic hierarchy process and the fuzzy comprehensive evaluation are combined to analyze the criticality of FMECA.

Abstract:Failure mode and effect analysis FMEA technique has been extensively used as a powerful tool for identifying and assessing potential failures in different phases of the product life cycle. However, the conventional FMEA has been criticised much for its deficiencies in measurement scale, computation of risk priority numbers RPN, risk factors’ weights, etc. In this work, the authors propose a more reasonable failure evaluation structure using fuzzy weighted Technique for Order Preference by Similarity to Ideal Solution TOPSIS. To fully reflect the importance of risk factors severity S, occurrence O and detection D, the authors consider integrating both subjective weights and objective weights, which is not presented in literature before, avoid failure modes from being underestimated or overestimated. For ranking fuzzy TOPSIS is adopted to get the closeness coefficient for each failure mode. Then, all failure modes can be ranked according to the closeness coefficients. A case of nuclear reheat valve system is provided to illustrate the applications and benefits of proposed FMEA method.

Abstract:An enhanced failure mode effect analysis (FMEA) based risk assessment for subsea compressor systems was proposed in this study. The enhanced model was established using a combination of fuzzy analytic hierarchy process (FAHP), fuzzy comprehensive evaluation, and FMEA. Different from the traditional FMEA model, the model improved the capability to identify system faults and to further measure the risk of a subsea compressor system through effective qualitative and quantitative analyses. A case study was then conducted to demonstrate the capability of the developed method in a complete risk assessment for a subsea compressor system and the subsystems and subcomponents with low reliabilities were identified. Comparing the enhanced model with previous models, it is found that the potential risks of some components are changed, and the components with higher potential risks in the system are identified. Sensitivity analysis to investigate the impact of parameters of subcomponents on system reliability was also performed.

Abstract:Failure mode and effects analysis (FMEA) has been widely used in product design process as a reliability analysis technique. Design FMEA (DFMEA), which is used in the product development phase to identify and mitigate product risk, is one of the three application scenarios of FMEA. During the DFMEA process, verification and validation (V&V) activities are proposed to mitigate the risk of the identified potential failure modes. The V&V activities can be further planned and implemented to improve the product reliability under development. However, the DFMEA report usually contains rich text descriptions of potential failure modes and causes, and it is difficult and nonintuitive to fully understand these information for design improvements. In addition, it is also very challenging to optimize the planning of V&V activities by selecting a set of V&V activities to achieve expected reliability improvement effectiveness with the minimum resource consumption requirement. To address these two challenges, this article first proposes a method of applying text mining to the DFMEA report to obtain two types of hidden reliability information, including the classifications of failure modes/causes and the correlation between keywords. Then, a mathematical model is proposed to optimize the product V&V planning by selecting an optimal set of V&V activities. The application of the above proposed methods is illustrated through the product development of a diesel engine power generation system.

Abstract:The purpose of this study is to determine the type of failures, the causes, as well as efforts and proposed actions that can be taken to improve the performance of the pneumatic system of CNC milling machines. The Failure Mode and Effect Analysis (FMEA) method is used to find priority problems through Risk Priority Number (RPN) for pneumatic system of components of CNC milling machine. The results of critical component analysis were a leakage occurs in pneumatic valve component, which the most critical risk of the RPN value. It potential failure was caused the high humidity and poor air quality from the compressor, which the RPN of 392 and 384 respectively. The proposed improvement was the replacement of damaged components and optimizing the preventive maintenance, which follow the right operation and maintenance procedures recommended as suggested by the manufacturer.

Abstract:Failure mode and effects analysis (FMEA) is a widely used technique for assessing the risk of potential failure modes in designs, products, processes, system, and services. One of the main problems with FMEA is the need to address a variety of assessments given by FMEA team members and the sequence of the failure modes according to the degree of risk factors. Many different methods have been proposed to improve the traditional FMEA, which is impractical when the risk assessments given by multiple experts to one failure mode are imprecise, incomplete, or inconsistent. However, the existing methods cannot adequately handle these types of uncertainties. In this paper, a new risk priority model based on D numbers and technique for the order of preference by similarity to ideal solution (TOPSIS) is proposed to evaluate the risk in FMEA. In the proposed model, the assessments given by the FMEA team members are represented by D numbers, where a new feasible and effective method can effectively represent the uncertain information. The TOPSIS method, a multicriteria decision‐making method is presented to rank the preference of failure modes with respect to risk factors. Finally, an application of the failure modes of the rotor blades of an aircraft turbine is provided to illustrate the efficiency of the proposed method.

Abstract:Failure Modes and Effects Analysis (FMEA) is an engineering technique widely used to identify, evaluate, and eliminate known or potential failures, problems, and errors and inform risk management decisions. Traditional FMEA uses the risk priority number (RPN) to identify potential failure modes and their levels. However, it displays shortcomings in determining risk factor weights, ranking risk priorities of failure modes, and dealing with uncertainties during the risk assessment process. To address these issues, this paper proposed an improved FMEA risk assessment method by integrating FMEA, the analytic hierarchy process (AHP), the technique for order preference by similarity to an ideal solution (TOPSIS), and the cloud model (CM) to determine a more reasonable failure mode level. First, the CM was used to improve the FMEA framework structure to comprehensively assess ambiguity and randomness during the evaluation process. Second, the cloud weights of the risk factor severity (S), occurrence (O), and detectability (D) were calculated using AHP and CM to determine the relative risk factor importance. Then, the cloud distance measurement algorithm and TOPSIS were combined into the TOPSIS-CM risk ranking method to objectively identify the failure modes requiring improvement. Finally, a case study of a flange connection system at a hydrogen blending transmission site in northern China was conducted to verify the adaptability of the proposed method in failure mode assessment. Compared with traditional FMEA methods, the decision scores obtained using the improved FMEA were more discriminating and distinguished the risk of each failure mode, providing more accurate information to decision makers (DMs).

Abstract:： Failure modes and effects analysis(FMEA) is an important means of design review and reliability evaluation, but conventional FMEA is tedious and error-prone due to its informalization. Formal methods are introduced to FMEA for electromechanical systems and an approach to FMEA for electromechanical systems based on model checking is proposed. Based on the states and events in electromechanical systems, formal modeling for the behavior process of the systems is studied and formal model for FMEA with respect to component failures is established. Computation tree logic(CTL) formulas are employed to make formal specification of the potential faults in electromechanical systems, and the negative propositions of potential fault are used to form system properties to be verified. By traversing system space, a system is checked if it satisfies the system properties and thereby the relations between component failures and the system potential faults are identified, so model checking based FMEA for electromechanical systems is realized. The above approach is applied to feed system of a CNC machine tool, and the system faults caused by limit switches are identified successfully, which demonstrates the feasibility of the proposed approach.

Abstract:Systems reliability analysis method has a wide application in product reliability analysis. It can describe failure mode of system more clearly. When analyzing product using FMECA method, we often use system reliability analysis method. However, this method has its difficulty to use. As it describes fault in way of function, we should know the system function completely before the analysis. Meanwhile, to collect enough products FMECA reports from product manufacturers is also needed. Based on that, we can carry out the system FMECA. This paper chooses an electronic system of aircraft to illustrate this analysis process, and some discussions are given on FMECA application.

Abstract:In this paper, an improved Failure Modes, Effects and Criticality Analysis (FMECA) method for wind turbine reliability investigation is proposed.According to previous work, the main deficiencies of Risk Priority Number (RPN) based criticality analysis are presented.For the qualitative analysis, failure modes and main causes of a gearbox are analyzed on subcomponents level with available data.After that, inspection approaches are provided and mitigation methods are recommended from both design and maintenance stage.For criticality analysis, another index, Probable Profit Loss (PPL) is created and all the parameters are carefully defined to overcome the limits of conventional RNP method.Moreover, to make PPL applicable for maintenance tasks, different maintenance types are considered for profit loss calculation.The criticality rank shows the efficiency of the index and can be used as reference for making maintenance schedules.

Abstract:Traditionally, failure mode and effects analysis (FMEA) only considers the impact of single failure on the system. For large and complex systems, since multiple failures of components exist, assessing multiple failure modes with all possible combinations is impractical. Pickard et al. [1] introduced a useful method to simultaneously analyze multiple failures for complex systems. However, they did not indicate which failures need to be considered and how to combine them appropriately. This paper extends Pickard’s work by proposing a minimum cut set based method for assessing the impact of multiple failure modes. In addition, traditional FMEA is made by addressing problems in an order from the biggest risk priority number (RPN) to the smallest ones. However, one disadvantage of this approach is that it ignores the fact that three factors (Severity (S), Occurrence (O), Detection (D)) (S, O, D) have the different weights in system rather than equality. For examples, reasonable weights for factors S, O are higher than the weight of D for some non-repairable systems. In this paper, we extended the definition of RPN by multiplying it with a weight parameter, which characterize the importance of the failure causes within the system. Finally, the effectiveness of the method is demonstrated with numerical examples.
