Abstract:The constitution of traditional Chinese medicine was established in 1970s by Chinese scholars, in which the constitutions of Chinese people were classified into nine types for study. The phlegm-dampness constitution is one of the nine constitutions and is the most common type in constitution study. Genomics studies found four upregulated genes: COPS8, GNPDA1, CD52 and ARPC3; and six downregulated genes: GSPT2, CACNB2, FLJ20584, UXS1, IL21R and TNPO in the phlegm-dampness constitution. Gene functional analyses on genes affecting the differences between the phlegm-dampness constitution and the balanced constitution indicated that people with phlegm-dampness constitution were susceptible to hyperlipemia and diabetes. Results of epidemiological surveys also revealed that people with phlegm-dampness constitution have a much higher risk of obesity, metabolic syndrome, hypertension and diabetes than people with a balanced constitution. Therefore, differentiation of phlegm-dampness constitution could be performed in the normal population with the Constitution of Chinese Medicine Scale to estimate the risks of those diseases for prediction. For people with phlegm-dampness constitution, Chinese medicine could be used to reduce risk of related diseases. Constitution-based strategies in disease prevention and treatment are consistent with the current proposed 4P medical mode (personalized, predictive, preventive and participatory). With the rising burden of global disease and increasing medical expenditure, the objectives of medicine are transforming from treatment to prevention. Thus, studies on the phlegm-dampness constitution of traditional Chinese medicine are significantly important for the prediction and prevention of related diseases and maintenance of human health.
Analysis on Constitution Ratio of TCM Constitution of 2 332 Cases in Zhengzhou Area

Abstract:Objective:To understand the distribution of TCM Constitution in general population in Zhengzhou,and to provide basis for further health management.Methods:analyzed according to 2 332 cases of different age,sex and so on.Results:2 332 cases of moderate constitute of 771 people,accounting for 33.06％,biased physique of 1 561 people,accounting for 66.94％.In the top 3 of the types of biased constitute for the quality of Yang deficiency,damp heat,phlegm-dampness,accounting for 18.57％,10.33％,9.31％ respectively.The physical distribution differences between men and women,2 332 cases of population,1 279 were male,and moderate constitute of 566 people,accounting for 44.25 ％,the biased constitution of 713 people,accounting for 55.75％,with the top three was the damp heat,Yang deficiency,phlegm-dampness,accounted for 13.21％,11.10％ and 9.70％ respectively.Conclusion:The constitute of Yang deficiency,damp heat and phlegm dampness is a common type of biased constitution in the general population of Zhengzhou area,and a variety of accompanying physical is also physical characteristics of Zhengzhou residents.

Abstract:Diseases caused by latent pathogenic factors are very common.The syndromes of them are complicated,the courses are very long and the diseases are easy to recur.How to completely cure and prevent recurrence is a difficult problem.Professor WANG Qi considers there will be correlation between diseases caused by latent pathogenic factors and allergic constitution,he put forword that desensitization and recuperating allergic constituion can improve the environment of the latent pathogenic factors.He uses dark plum,periostracum cicadae,sessile ganoderma lucidum and polygonum capitatum,etc,to regulate allergic constitution from the beginning to end,and enhance the adaptability of human body to stimulate factors from external environment gradually.This achieves good effects,provides a new perspective on clinical prevention and treatment of the diseases caused by latent pathogenic factors.

Abstract:Hypertension is a major public-health issue. Much consensus has been reached in the treatment, and considerable progress has been made in the field of antihypertensive drugs. However, the standard-reaching rate of blood pressure is far from satisfaction. Considering these data and the seriousness of the effects of hypertension on the individual and society as a whole, both economically and socially, physicians must look for more effective and alternative ways to achieve the target blood pressure. Could treatment of hypertension be improved by insights from traditional Chinese medicine? As one of the most important parts in complementary and alternative therapies, TCM is regularly advocated for lowering elevated blood pressure. Due to the different understanding of the pathogenesis of hypertension between ancient and modern times, new understanding and treatment of hypertension need to be reexplored. Aiming to improve the efficacy of Chinese herbal medicine in treating hypertension, the basis of treatment is explored through systematically analyzing the literature available in both English and Chinese search engines. This paper systematically reviews the trends in emerging therapeutic strategies for hypertension from the perspective of traditional Chinese medicine.

Abstract:ObjectivesBoth depression and hypertension (HTN) are prevalent, costly, and destructive, and frequently coexist among the aging population of China. This study aimed to examine the role that treatment adherence plays in blood pressure control in older adult Chinese with depression.MethodsData for these analyses were taken from a randomized control trial of a collaborative depression care management intervention conducted in rural villages of Zhejiang Province, China with older adults who had comorbid depression and HTN. They included baseline assessments of 2362 subjects ages ≥60 years, whose blood pressure and depression were measured using a calibrated manual sphygmomanometer and the Chinese version of the 17‐item Hamilton Depression Rating Scale (HDRS‐17), respectively. Treatment adherence was identified by a single question asking whether patients on occasion did not take their medicine.ResultsUncontrolled HTN was associated with older age (t = 3.10, P<0.01), higher HDRS‐17 score (t = 5.76, P<0.01), and higher rates of non‐adherence to HTN treatment (χ2 = 21.34, P<0.01). Logistic regression models indicated that adherence accounted for 39.4% of the total effect between depression and HTN. Specifically, those with poor adherence were at 1.417 greater odds of having their HTN uncontrolled compared with those with good adherence.ConclusionsHypertension control in older adults with depression is complicated by nonadherence to treatment. In addition to diagnosing and treating depression in their older adult patients, primary care physicians can optimize blood pressure control by identifying and addressing their patients' adherence to recommendations for HTN management.
Traditional Chinese Medicine constitution types of high-normal blood pressure: A meta-analysis

Abstract:Objective We determined the distribution of constitutional types of high-normal blood pressure in Traditional Chinese Medicine (TCM) and provided evidence for the prevention of high-normal BP and hypertension. Methods Eight digital databases were searched from January 2011 to November 2022, including PubMed, EMBASE, Web of Science, EBSCOhost, CNKI, CBM, Wangfang, and CQVIP. We performed a meta-analysis with the random-effects model or fixed-effects model to describe the distribution of constitutional types of high-normal blood pressure in TCM. The studies were assessed based on heterogeneity testing and the potential for publication bias. The meta-analysis was performed on Stata software 15.0. Results A total of 17 studies with 8118 participants were included in this meta-analysis. The proportion of the biased constitution (82.3%; 95% CI: 75.6%–89.1%, p < 0.001) was higher than the balanced constitution (17.3%; 95% CI: 10.7–23.8%, p < 0.001). Phlegm-dampness constitution, Yin -deficiency constitution, and damp-heat constitution accounted for 16.0% (95%CI: 10.5–21.5%, p < 0.001), 14.8% (95% CI: 11.0–18.6%, p < 0.001), 11.3% (95% CI: 8.0–14.5%, p < 0.001) of the total high-normal blood pressure cases, respectively. The subgroup analyses performed that region, age and gender were positively associated with the distribution of constitution types of high-normal blood pressure in TCM. Compared with the general population, the risk of high-normal blood pressure in people with the phlegm-dampness constitution, Yin -deficiency constitution, and blood-stasis constitution was 2.665 (95%CI: 2.286–3.106, p < 0.001), 2.378 (95%CI: 1.197–4.724, p = 0.013), 1.965 (95%CI: 1.634–2.363, p < 0.001) times of the general population, respectively. Meanwhile, the risk of high-normal blood pressure was lower in people with a balanced constitution (0.248, 95%CI: 0.165–0.372, p < 0.001). Conclusions Phlegm-dampness constitution, Yin -deficiency constitution, and damp-heat constitution were the common constitution types of high-normal blood pressure. There might also be differences in the distribution characteristics of TCM constitution among people with high-normal blood pressure in different regions, ages, and genders. Finally, a balanced constitution might be a protective factor for hypertensive people.

Abstract:The prevention and treatment of hypertension is valued globally. The World Health Organization advocates combining traditional medicines in the prevention and treatment of diseases. Traditional Chinese medicine (TCM) assumes that diseases originate from the attenuation of one's body constitution. A few studies have found that hypertension is correlated with TCM body constitution. However, body constitution is also affected by living environment. Therefore, investigating the correlation between deviations in body constitution and essential hypertension in different living environments is necessary to provide the basis for using TCM in combination with conventional Western medicine to prevent and treat hypertension.

Abstract:Hypertension is a clinical common disease, with high mortality and disability. Although there have also been significant advances in therapeutic concepts and measures, it has shown a certain value and significance in the treatment of Chinese medicine. The control strategy on hypertension is described from the following aspects such as differentiation of symptoms, pathogenesis, formula syndrome, and herb syndrome. As the common clinical manifestations of hypertension are dizziness, headache, fatigue, lassitude in the loins and knees, and so on, the pathogeneses of them are analysed. The author found that the main pathogenesis of the disease is heat, excessive fluid, and deficiency, which occurred incorporatively and interacted with each other in patients. Although the pathogenesis of the disease is complicated, the distribution of formula syndromes and herb syndromes is regular. The common formula syndromes include Banxia Baishu Tianma Tang (Decoction of Pinellia ternata, Atractylodes and Gastrodia elata ), Da Chaihu Tang (Major Bupleurum Decoction), and Liu Wei Dihuang Wan (Pill of Rehmannia ). And the common herb syndromes include Tian Ma ( Gastrodia elata ) syndrome, Sheng Di Huang (Radix Rehmanniae) syndrome, Niu Xi ( Achyranthes Root ) syndrome, and Chuan Xiong ( Ligusticum wallichii ) syndrome.

Abstract:Objective To study the syndrome factors of hypertension and supply the foundation of syndrome differentiation and treatment for hypertension.Methods A perspective observation plan was designed and 428 hypertensive patients were selected according to the unified criteria of diagnosis,inclusion and exclusion.The information of TCM four examination methods was collected and syndrome factors were extracted through frequency statistics and factor analysis.Results There were 26 common factors(syndrome factors) extracted by factor analysis.The analysis on the distribution of syndrome factors in 428 patients showed that blood stasis accounted for 17.5%,yin deficiency and heat for 17.5%,qi deficiency for 17.2%,blood deficiency for 14%,fire for 11.9%,phlegm for 9.2%,qi deficiency and phlegm for 8.4%,and qi deficiency and blood stasis for 4.2%.The synthetic analysis showed that deficiency syndrome accounted for 61.3%.Conclusion The main syndrome factors of hypertension include qi deficiency,fire,blood stasis and phlegm,and secondary factors include yin deficiency and blood deficiency.The main viscera and bowels involved include liver and kidney,secondly,heart.The most important pathological factor of hypertension is deficiency,or combining fire,phlegm and stasis and there are many complex factors.

Abstract:Objective To discuss the pathogenic factors of hypertension on the base of TCM constitution science.Method The multiple Logistic regression analysis was carried out in hypertension patients in the case group(n=271) and those without hypertension in the control group(n=302) by applying case-control study of clinical epidemiology.The correlative degree between relevant constitutional factors and hypertension was explored.Result There were 3 constitutional factors enclosed in the regression equation,including mild constitution,phlegm-damp constitution and blood-stasis constitution.The partial regression coefficients were -0.602,1.437 and 0.592 respectively.Conclusion The risk factors of hypertension include phlegm-damp constitution and blood-stasis constitution.The phlegm-damp constitution is more important in the onset of hypertension and mild constitution is a protective factor.

Abstract:Background: The concepts of "individualization" and "preventive treatment" should be incorporated into the precise diagnosis and treatment of coronary heart disease (CHD). Both hemodynamics and Chinese medicine constitution studies align with these two concepts. Methods: This study utilized data from 81 patients with CHD, including 12 patients with balanced constitution (BC), 20 patients with blood stasis constitution (BSC), 17 patients with phlegm-dampness constitution (PDC), 15 patients with qi-deficiency constitution (QDC), and 17 patients with other constitutions. Clinical data provided information on the patients' blood property, heart function, degree of coronary stenosis, coronary hemodynamics, and so on. These parameters were compared between patients with balanced constitution vs. biased constitutions as well as between those with blood stasis constitution, phlegm-dampness constitution, and qi-deficiency constitution. Results: Compared to biased constitution (BC), patients with balanced constitution exhibited lower total cholesterol (TC) levels and low-density lipoprotein (LDL) levels. Additionally, they had lighter stenosis degrees in the Left anterior descending branch (LAD) and Left circumflex branch (LCX) branches. The hemodynamic condition of the LAD and LCX was better for those with balanced constitution; however there was no difference in heart function. Among the groups categorized by blood stasis, phlegm dampness or qi deficiency constituions, patients classified under phlegm dampness had higher levels of LDL compared to those classified under blood stasis or qi deficiency, while patients classified under qi deficiency had higher levels of blood glucose compared to those classified under blood stasis or phlegm dampness. Hemodynamic environments also differed among the LAD and LCX for each group but there were no significant differences observed in heart function or degree of coronary stenosis among these three groups. Conclusion: The balanced constitution demonstrates superior blood property, degree of coronary artery stenosis, and coronary hemodynamics compared to the biased constitution. Furthermore, among the three constitutions with CHD, variations in blood property and certain hemodynamic parameters are observed. These findings emphasize the significant clinical value of incorporating physical factors into the diagnosis and treatment of patients with CHD.

Abstract:Objective To get know the distribution of TCM constitutional types in the patients with hyperthyroidism or hypothyroidism.Methods Constitution Scale in Chinese Medicine (CSCM) was used to survey the TCM constitutional types in 117 patients with hyperthyroidism and 172 ones with hypothyroidism,and to compare the compound constitution in the patients of two groups.Results The frequency of TCM constitutional types in the patients with hyperthyroidism were orderly qi stagnation (63 times),yin deficiency (41),qi deficiency (37),normal (28),phlegm-dampness (28),yang deficiency (25),damp-heat (15),blood stasis (9),and special (7).The frequency of TCM constitutional types in the patients with hypothyroidism were orderly qi deficiency (91 times),yang deficiency (82),qi stagnation (58),yin deficiency (49),phlegm-dampness (46),normal (29),damp-heat (25),blood stasis (15),and special (13).The difference between two groups was significant.The main constitutional type was compound constitution in the patients of two groups.Conclusion The distribution of TCM constitutional types of hyperthyroidism or hypothyroidism had laws and difference.

Abstract:BACKGROUND: Whether antihypertesive remedy is given or not is not only according to the blood pressure grade, but also to its risk stratification. Recently the relation of syndrome types to hypertension grades and risk stratification in primary hypertension has been investigated at home. OBJECTIVE: To observe the relation of syndrome types to hypertension grades and risk stratification in primary hypertension by the method of syndrome differentiation in traditional Chinese medicine combining with achievements in modern medicine. DESIGN: Descriptive investigation. SETTING: Department of Traditional Chinese Medicine, Jing'an District Central Hospital of Shanghai City and Shanghai Dahua Hospital. PARTICIPANTS: Sixty-four patients, in different genders and ages, with primary hypertension from Department of Traditional Chinese Medicine of Jing'an District Central Hospital of Shanghai City METHODS: The primary hypertension was classified into four types of syndrome: up disturbing of wind-yang, up clouding of turbid phlegm, deficiency of qi and blood and yin deficiency of liver and kidney. The relation of syndrome type to hypertension grade and risk stratification in primary hypertension was investigated combining syndrome differentiation in traditional Chinese medicine (TCM) and disease differentiation in Western Medicine. MAIN OUTCOME MEASURES: The relation of syndrome types to the age, course, blood pressure grade and risk stratification in primary hypertension RESULTS: All 64 patients entered the result analysis. ① Relationship of syndrome type with the age, course and blood pressure grade: The age of patients in up-disturbing of wind-yang group was younger than those in both up-clouding of turbid phlegm and yin deficiency of liver and kidney groups [(59.0±10.9), (72.7±9.1), (71.6±10.1)years, P ＜ 0.01]; the disease course of patients in yin deficiency of liver and kidney group was longer than those in up-disturbing of wind-yang group and up-clouding of turbid phlegm group [( 160.50±143.51 ), (64.83±70.77), (80.56±108.69)months,P ＜ 0.05];the difference of systolic pressure among different syndrome types was not significant, but the diastolic pressure in up-disturbing of wind-yang group was higher than those in other groups [(99±8), (92±9),(89±11 ), (89±12)mmHg, P ＜ 0.05]. ② The relationship of syndrome type with blood pressure grade and risk stratification: The difference of blood pressure grades among patients with different syndrome types was not significant, but the risk stratification in up-disturbing of wind-yang group was lower than those in up-clouding of turbid phlegm and yin deficiency of liver and kidney groups (There were respectively 3, 7, 5 and 3 cases; 15, 1, 1and 1 cases; 16, 1, 2 and 1 cases at very high risk, high risk, medium risk and low risk, P ＜ 0.05). CONCLUSION: Different syndrome types in TCM may present differences in hypertension grade and risk stratification, which can provide basis for determining therapeutic principle.

Abstract:Hypertension has become one of the major public health problems in the world. At present, the pathogenesis of hypertension has still not been completely elucidated. In recent years, an increasing evidence shows that intestinal microecology is closely related to hypertension, which provides a new thinking for the prevention and treatment of hypertension. Traditional Chinese medicine (TCM) has unique advantages in the treatment of hypertension. Taking intestinal microecology as the target, it is possible to interpreting the scientific connotation of TCM prevention and treatment of hypertension by updating the treatment concept of hypertension, so as to improve the therapeutic effect. In our study, the clinical evidence for TCM treatment of hypertension was systematicly summarized. And the relationship among TCM, intestinal microecology and hypertension was analyzed. In addition, the methods by which TCM regulates intestinal microecology to prevent and treat hypertension were presented, to provide new research ideas for prevention and treatment of hypertension.

Abstract:Cardiovascular disease (CVD) represents enormous challenges to population health. Body constitution, from the perspective of traditional Chinese medicine (TCM), differentiates how individuals feel and behave, and ultimately affects how different bodies respond to dietary intake. We hypothesise that the TCM constitution-based dietary intervention would be an effective approach to achieving favourable clinical and TCM-related outcomes when compared to usual care alone among the Chinese grade 1 hypertensive patients. This is a parallel-group, randomised trial (registered as ChiCTR2000034749; http://www.chictr.org.cn). Participants consist of 274 primary care adults, aged 35 years or older, with uncomplicated grade 1 hypertension and at low risk of cardiovascular and renal events. Participants will be randomly assigned to either control or intervention group with a 1:1 allocation as per computer-generated random numbers. Permuted blocks of random sizes will be used. Subjects in the invention group will receive an on-site constitution assessment based on physical signs, personality, body symptoms, and the susceptibility to environmental changes. This will be conducted by the attending TCM practitioners trained in a prior session to improve the inter-rater reliability. A colour-printed constitution-specific brochure will be provided onsite. Health education materials will be made available to patients by courier service thereafter. All participants receive routine care provided by the ‘family doctor team’ in the context of the national basic public health service delivery. Measurements will be undertaken at three time-points, i.e., at baseline, at 6- and 12-month follow-ups. At each follow-up interview, subjects in the intervention group will be questioned about the constitution types of their own, and a full awareness of the constitution profile will be seen as an adherence to the protocol. The primary outcome of the study is the difference between the two arms in the proportion of participants who had optimal control of blood pressure at 12 months. A point estimate of 25% inferiority in effectiveness would be acceptable assuming a gain that is considered to be clinically relevant. The secondary outcome measures include differences in the profile of clinical parameters and TCM symptoms between the two arms. We believe that this study offers new insights for cardiovascular health management through TCM constitution-based strategies in primary care settings to target those who need continuous attention in dietary modifications. The findings may advance our knowledge of the practical contribution of complementary and alternative medicine in the prevention and control of CVD.

Abstract:Select patients diagnosed of hypertension in the first place in 16 hospitals and the patients were described by association rules analysis and distribution for the analysis in the study,in order to understand the information of diagnosis and treatment in hypertension patients in real world. The information include age, gender, admission condition, inpatient department, hospitalization expenses, western medicine comorbidities, the traditional Chinese medicine (TCM) syndrome and the medicine. The average age of patients was 64 years in the study. The proportion of men to women about 1. 10: 1. Admission condition is critical for about 10.12% of the total and more concentrated in the cardiovascular department. Hospital stay ranged from 8 to 14 d. Inpatients with medical insurance is 62.93%. Total hospitalization cost distribution most is the 5 000-10 000 RMB. Hypertension complicated with coronary heart disease, blood lipoprotein disorder disease; phlegm and blood stasis and Yin deficiency of liver and kidney are the most in TCM syndrome type. Promoting blood circulation and removing blood stasis is the major in Chinese medicine treatment. Western medicine treatment basically conforms to the guidelines. The most antihypertensive drugs is calcium antagonistst, accounting for about 81.2% of the total number. Class ACEI, class ARB, beta blockers, diuretics class are accounted for 43.0%, 43.4%, 42.4%, 42.4%, fixed compound is only 2 393, accounting for about 8%. the total frequency of five classes of antihypertensive drugs is 78 206 times. The principal conclusions of this analysis are as follows: the elderly people is the most in hypertension, more men than women; medical insurance is the majority type in hospitalization cost; nearly half of the population is combined with coronary heart disease; phlegm and blood stasis and yin deficiency of liver and kidney are the most in TCM syndrome type; western medicine treatment basically conforms to the guidelines. Combination therapy is more, less fixed compound preparation in western medicine. Chinese medicine is mainly to promoting blood circulation. Suggesting that the clinical treatment in strict blood pressure and at the same time, pay attention to combination therapy and the use of fixed compound preparation,it's useful to control the blood pressure better. Combined with traditional Chinese medicine and resolving phlegm and removing blood stasis, reduce the target organ damage, reduce complications, the prevention and control work of hypertension in China still faces enormous challenges at present. The result provides reference information for clinical medicine and medical research.

Abstract:This paper summarizes the pathogenesis and dialectical classification of hypertension from the perspective of traditional Chinese medicine, and integrates the research results of hypertension from the previous schools of traditional Chinese medicine based on the theory of "one qi circulation", as well as the key points of syndrome differentiation, and concludes that under the theory of "one qi circulation", the main pathogenesis of hypertension is related to soil dampness and wood depression and poor operation of qi and machine, and this theory can be used to better carry out the diagnosis and treatment of hypertension in traditional Chinese medicine, in order to provide new ideas for the diagnosis and treatment of hypertension in traditional Chinese medicine.
Correlation between Cardiovascular Risk Factors and TCM Constitutions in 400 Patients with Atherosclerosis in Guangzhou City

Abstract:Objective To study the distribution of traditional Chinese medicine (TCM) constitutions and the correlation between TCM constitutions and cardiovascular risk factors in patients with atherosclerosis in Guangzhou,to provide the theoretical basis for early intervention of arteriosclerosis through TCM.Methods The TCM constitutions and clinical information of 400 patients with arteriosclerosis in Guangzhou were collected by questionnaire investigation,and the distribution of TCM constitutions and the correlation between TCM constitutions and cardiovascular risk factors were analyzed.Results The main TCM constitution in patients with atherosclerosis was phlegm-dampness constitution (23.0％),22.0％ with blood stasis constitution,15.0％ with qi-deficiency constitution,13.5％ with damp-heat constitution and,11.5％ with yang-deficiency constitution.There were significant differences in the levels of uric acid,triglyceride.and high-density lipoprotein-cholesterol between the patients with qi-deficiency,damp-heat,phlegmdampness and blood-stasis constitution (P ＜0.05).Smoking,alcohol consumption were significantly correlated with qi-deficiency constitution(P ＜0.05).Smoking,alcohol consumption and hypertension were significantly correlated with damp-heat constitution (P ＜0.05).Men,smoking,lack of exercise,overweight/obese,hypertension were significantly correlated with the phlegm-dampness constitution (P ＜0.05).Hyperuricemia and dyslipidemia were significantly correlated with blood stasis constitution (P ＜0.05).The Binary Logistic regression analysis was used to analyze correlation between TCM constitutions and cardiovascular risk factors.The results showed that smoking and alcohol consumption may be risk factors of damp-heat constitution,while males,lack of exercise,overweight or obesity may be risk factors of phlegm-dampness constitution.Conclusion Phlegm-dampness,blood-stasis,qi-deficiency,and blood-stasis constitution are the main risk factors of atherosclerosis.The correlations of different TCM constitutions and cardiovascular risk factors are different.
