Analysis of Factors Affecting the Onset of Symptomatic Vasospasm of Postoperative Patients with Aneurysm

Abstract:Objective To analyze all the factors affecting the onset of symptomatic vasospasm of postoperative patients with aneurysm,so as to find protective ones.Methods Between October 2004 and March 2006,82 Patients were randomized into two groups:the routine remedy group(n=41);the edaravone remedy group(n=41).In the routine remedy group,the aneurysm was clipped within 3 days from SAH onset and blood clot in basal cistern was evacuated as more as possible,then they treated routinely including tri-H remedy(hypertensive hypervolemic hemodilution).In the edaravone remedy group,edaravone was used by intravenous injection for 14 days once diagnosed.We recorded patients conditions,Glasgow grade and incidence of symptomatic vasospasm,including Fisher and Hunter's grade made at admission.Logistic regression was used on patients'age、gender、Fisher grade、Hunter's grade at admission,and edaravone remedy.Results Incidence of symptomatic vasospasm was 14.63% in edaravone remedy;36.59% in routine remedy respectively.Logistic regression analyses showed Fisher grade,Hunter's grade at admission and edaravone remedy had effects on the onset of symptomatic vasospasm.Both Fisher grade and Hunter's grade were adverse factors with OP=6.55 and 5.62 respectively,while edaravone remedy a protective one with OR=0.11.Conclusions Many factors affected the onset of symptomatic vasospasm.Fisher grade,Hunter's grade at admission was adverse factors while edaravone remedy a protective one.When accompanied with routine remedy,edaravone can decrease the onset of symptomatic vasospasm more effectively.
