RE-EVALUATION OF SULFADIAZINE

Abstract:Sulfadiazine(SD)is a broad-spectrum antimicrobial agent with strong bacteriostatic activity, low serum protein binding, high cerebro-spinal fluid and tissue concentrations. It is the drug of choice for the treatment of meningococcal meningitis, and is also quite effective against pneumonia and urinary tract infections. Owing to misuse of the drug in the past, the side effects, such as hematuria and crystalluria, occured rather frequently, so its clinical application was greatly restricted. In this article, we reported the antibacterial activities and pharmacokinetics of SD, sulfamethoxazole(SMZ) and trime-thoprim(TMP). We found that SD has similar antibacterial activity to that of SMZ, but is less active than TMP. When combined with TMP, the antibacterial activities of SD and SMZ are greatly enhanced. The half-lives as well as other pharmacokinetics of SD, SMZ and TMP are quite similar. Both SD and SMZ are medium-acting sulfon-amides, the tissue concentrations of SD are higher than those of SMZ, and the tissue affinity of SD is closer to TMP than SMZ.The clinical efficacy and side effect of SD-TMP compound and SMZ-TMP compound are about the same. In view of the above merits of SD, its cheap price and easier production, it is considered that SD-TMP compound may be used instead of SMZ-TMP compound in the management of various bacterial infections.
