Abstract:Introduction: Therapeutic hypothermia improves the outcomes following resuscitation. The current hypothermia range (32-34°C) recommended by the American Heart Association is based mainly on two clinical studies. The benefits and the adverse effects of the temperature management at this range are still controversial. In the present study, we compared the effects of different body temperatures (33°C, 36°C, 37°C and 38°C) after resuscitation on the outcomes of CPR in a rat model of cardiac arrest. Methods: Ventricular fibrillation was electrically induced and untreated for 8 mins. Defibrillation was attempted after 8 mins of CPR. The animals were then randomized into 4 groups ( n =10): hypothermia at 33°C (T33), hypothermia at 36°C (T36), normothermic (T37) and hyperthermia (T38). The variable core temperatures were maintained for 6 hours. Myocardial function and sublingual microcirculation were measured at baseline, 1, 2, 3 and 6 hours after the return of spontaneous circulation (ROSC). Survival and neurological function were evaluated at 24, 48 and 72 hours after ROSC. Results: The measurements were significantly better in the animals treated with hypothermia regardless of the cooling depth when compared with the normothermic group. Better sublingual microcirculation at 6 hours post-resuscitation was observed in the T36 group when compared with the T33 group. Significantly worse myocardial function and sublingual microcirculation were observed in the T38 animals when compared to the T37 group. Conclusions: Mild reduction of core temperature to 36°C early after ROSC improved post-resuscitation myocardial and neurological functions and survival to the same extent as 33°C but with less impairment of microcirculation.

In the present study, we investigated the effect of therapeutic hypothermia on oxygenation index, a sensitive index of pulmonary function,in a rat model of cardiac arrest and resuscitation. Hypothesis: We hypothesize that during therapeutic mild hypothermia in a rat model of cardiac arrest, the pulmonary function following resuscitation is less impaired when compare to normothermia. Methods: Twenty-one male Sprague-Dawley rats were randomized into three groups: 1) control group (control, n=5): the normothermic rats only received anesthesia and the surgical procedure as the other groups without ventricular fibrillation (VF); 2) normothermia group (NT, n=7): the normothermic rats were subjected to induced VF for 8 mins followed by 8 mins of cardiopulmonary resuscitation (CPR); 3) Mild hypothermia group (HT, n=9): the rats were subjected to induced VF for 8 mins followed by 8 mins of CPR. Mild hypothermia of 33±0.5°C was started 5 mins after return of spontaneous circulation (ROSC) and maintained for 8 hrs. The oxygenation indexes were measured at baseline, 2, 4 or 8 hours after ROSC with a conventional blood gas analyzer (PHOX plus L; Nova Biomedical Corporation, Waltham, MA, USA). Results: Compared to the control group, the oxygenation indexes of both the NT and HT groups were significantly decreased at 2 hrs after ROSC. However, more significant reduction in oxygenation index was observed in the NT group (Figure). Conclusions: There is an early pulmonary dysfunction after successful resuscitation from cardiac arrest. Hypothermia reduces the impairment of pulmonary function.

Abstract:Introduction: Mild hypothermia is one of the effective treatments in comatose patients after cardiac arrest. However, its neuroprotective mechanism is not fully clear. In the present study, we investigated the effect of mild hypothermia on cerebral cortex microcirculation and cerebral oxygen extraction ratio (CERO2). Hypothesis: Mild hypothermia is associated with an improved cerebral cortex microcirculation and a decreased CERO2. Methods: Twenty-five male Sprague-Dawley rats were randomized into 3 groups: hypothermic (HT, n=10), normothermic (NT, n=10) or sham control group (SC, n=5). Ventricular fibrillation was electrically induced and untreated for 8 mins, followed by 8 mins of precordial compressions and mechanical ventilations. Core temperature was reduced to 33±0.5°C at 14 mins after return of spontaneous circulation (ROSC) with a combination of ice packs, an electrical fan and a cooling blanket. The temperature was maintained at 33°C for 8 hrs. Hemodynamics, jugular venous and aortic blood gas, and cerebral cortex microcirculation were measured at baseline, 2, 4 and 8 hrs after ROSC. Results: Of the 20 experimental rats, 15 were successfully resuscitated. There was no significant difference in the rate of ROSC in the NT and HT groups (7/10 vs. 8/10; p>0.05). Microvascular flow index was significantly reduced at 2, 4 and 8 hrs after ROSC but was significantly improved by mild hypothermia (Figure 1). Mild hypothermia significantly reduced the CERO2 after ROSC (Figure 2). Conclusions: Mild hypothermia improves the cerebral cortex microcirculatory blood supply/oxygen uptake mismatching after ROSC, which may provide one additional mechanism of cerebral protection.
Effects of Therapeutic Hypothermia on Cerebral Tissue Oxygen Saturation in a Swine Model of Post-Cardiac Arrest

Abstract:Since the introduction of therapeutic hypothermia (TH), trends have changed in the monitoring indicators used during and after cardiac arrest. During hypothermia, the cerebral metabolic rate of oxygen is reduced, which leads to uncertainty in regional cerebral tissue oxygen saturation (SctO2). The aim of the present study was to evaluate the effect of TH on changes in SctO2 using near-infrared spectroscopy. A total of 23 male domestic pigs were randomized into three groups: TH (n=9), normothermia (NT; n=9) and control (n=5). Animals in the control group underwent surgical preparation only. The animal models were established using 8 min of ventricular fibrillation and 5 min of cardiopulmonary resuscitation. In the TH group, at 5 min after resuscitation, the animals were cooled with a cooling blanket and ice packs for 24 h. SctO2 was recorded throughout the experiment. In all groups, The mean arterial pressure, arterial carbon dioxide partial pressure, arterial oxygen partial pressure, lactate, neuron-specific enolase (NSE) and S100B were measured at baseline and at 1, 3, 6, 12, 24 and 30 h after resuscitation. SctO2 significantly decreased after ventricular fibrillation, compared with the baseline. Following resuscitation, the SctO2 values gradually increased to 55.6±3.8% of baseline in the TH group and 51.2±3.5% in the NT group (P=0.039). Significant differences between the two groups were observed, starting at 6 h after cardiac arrest. Throughout the hypothermic period, NSE and S100B showed an increasing trend, then decreased during rewarming in the TH and NT groups. NSE and S100B showed greater improvement in the TH group compared with the NT group at 6 and 24 h after resuscitation. Following cardiac arrest, therapeutic hypothermia could increase SctO2 after resuscitation and could improve neurological outcome. In conclusion, SctO2 may be a feasible marker for use in the early assessment of brain damage during and after cardiac arrest.

Post-resuscitation functional vital organ recovery is largely dependent on microcirculatory function. The effects of hypothermia on the peripheral and cerebral microcirculation following CA are not fully understood. Hypothesis: Hypothermia reduces the peripheral microcirculatory flow. However, the microcirculatory flow of cerebral cortex is preserved during hypothermia in a rat model of CPR. Methods: Twenty-one male Sprague-Dawley rats were randomized into three groups: hypothermic (HT, n=8), normothermic (NT, n=8) or sham-operated group (SO, n=5). Ventricular fibrillation was induced electrically and untreated for 8 mins, followed by 8 mins of precordial compressions and mechanical ventilations. The core temperature was reduced to 33±0.2°C at 5 mins following successful resuscitation and maintained for 8 hrs in the HT group. Normothermic animals were maintained at 37±0.2°C for 8 hrs. Sublingual and cerebral cortex microcirculation was measured by a sidestream dark-field imaging device at baseline, 1, 2, 3, 4, 6 and 8 hrs post-resuscitation. Results: All animals were resuscitated successfully. The sublingual microcirculatory flow index (MFI) was decreased significantly compared to the cerebral cortex MFI in the HT group following resuscitation (P< 0.05) (Figure 1). However, sublingual and cerebral microcirculation were impaired equally in the NT group after resuscitation.(Figure 2) Conclusions: A similar reduction in both sublingual and cerebral cortex microcirculatory flow was observed following resuscitation under conditions of normothermia. Hypothermia preserved the cerebral cortex microcirculation, but not the peripheral microcirculation, after resuscitation.

Abstract:Introduction : In an earlier report, our group demonstrated that when head cooling was started during CPR, outcomes were improved. We had also reported in a series of studies the adverse effects on ultimate outcomes of adrenergic amines, both á 1 and âadrenergic agonists and improved outcomes withá 1 and âadrenergic blockades. We therefore hypothesized that the benefits of rapid head cooling may at least in part relate to reduced adrenergic stimulation. In a small exploratory study, we therefore measured endogenous plasma concentration of norepinephrine. Methods: Pilot studies were conducted in 7 domestic male pigs weighing 40±5 kg. After 15 minutes of untreated ventricular fibrillation, in 4 animals head cooling was induced with a Rhino Chill device coincident with start of CPR. An additional 3 animals served as identically treated controls without hypothermia. CPR was continued for 5 minutes in both group prior to defibrillation. Endogenous plasma norepinephrine levels were measured at baseline and 5 minutes after start of CPR. Results: None of the control animals were successfully resuscitated, in contrast to 3 of 4 brain cooled animals. Jugular vein temperature was reduced from 38.0°C to 37.0°C in brain cooled animals ( P <0.001). Endogenous norepinephrine levels were greatly increased in comparing to baseline levels and was significantly less in brain cooled animals (153 vs 381 μg/L, P <0.05) Conclusions: The mechanism by which rapid head cooling improves outcome of resuscitation may, at least in part, relate to inhibition of the adverse effects of endogenous adrenergic stimulation.
Effect Of Mild Hypothermia On Cerebral Microcirculation In A Murine Cardiopulmonary Resuscitation Model

Abstract:Background We hypothesized that mild hypothermia may improve brain microcirculation by reducing cerebral microvascular endothelial cells apoptosis, and this effect may be maximized by moving up the initiation of mild hypothermia from after return of spontaneous circulation (ROSC) to the start of cardiopulmonary resuscitation (CPR). Methods A total of 35 rats were randomized into the intra-arrest hypothermia group (IAH), post-resuscitation hypothermia group (PRH), normothermia group (NT), or the sham control group. A craniotomy exposed the parietal cortex for visualization of microcirculation. Ventricular fibrillation was electrically induced and untreated for 8 minutes, followed by 8 minutes of precordial compression and mechanical ventilation. Hypothermia (33 +/- 0.5 degrees C) in the IAH and PRH group was induced and maintained for 6 hours at the beginning of CPR or after ROSC, respectively. At baseline, 1, 3, and 6 hours, hemodynamic parameters were measured and the pial microcirculations were visualized with a sidestream dark field imaging video microscope. Microvascular flow index and perfused microvessel density (PMD) were calculated. Rats were euthanized, and brain tissues were removed at 3 and 6 hours separately. Expression of Bax, Bcl-2, and Caspase 3 in brain microvascular endothelial cells was examined by Western blot. Results Microvascular flow index and PMD were significantly reduced after cardiac arrest and resuscitation (all P < 0.05), and the former was largely preserved by hypothermia regardless when the hypothermia treatment was induced (P < 0.05). Bax and Caspase 3 increased and Bcl-2 decreased significantly after resuscitation, and hypothermia treatment reversed the trend partly (all P < 0.05). A moderate correlation was observed between MFI and those proteins (Bcl-2/BAX: 3 hours: r = 0.730, P = 0.002; 6 hours: r = 0.743, P = 0.002). Conclusion Mild hypothermia improves cerebral microcirculatory blood supply, partly by inhibiting endothelial cell apoptosis. Mild hypothermia induced simultaneously with CPR has shown no additional benefit in microcirculation or endothelial cell apoptosis.

Abstract:Therapeutic hypothermia is well known for its protective effect against brain injury after cardiac arrest, but the exact mechanism remains unclear. Cold-inducible RNA-binding protein (CIRP), a member of cold shock protein, enables mammalian cells to withstand decreased temperature by regulating gene translation. However, the role of CIRP in global cerebral ischemia after therapeutic hypothermia has not been clearly elucidated. In the present study, rats resuscitated from 4 min of cardiac arrest were separately treated with therapeutic hypothermia (immediately after return of spontaneous circulation (ROSC); targeted temperature at 33 °C) and therapeutic normothermia (targeted temperature at 36.8 °C) for 6 h. The hippocampus was harvested at 0 h (baseline), 6 h, 12 h, 1 day, 3 days, and 7 days after ROSC. The expression of CIRP messenger RNA (mRNA) was assessed by real-time PCR. CIRP and mitochondrial apoptosis-associated proteins were tested by Western blot. The histological changes and neurological function were respectively evaluated by hematoxylin-eosin staining and neurological deficit score (NDS). Compared with baseline, rats resuscitated from cardiac arrest showed increased expression of CIRP, Bax, Caspase 9, and Caspase 3 and decreased expression of Bcl-2 in hippocampus (P < 0.05). However, therapeutic hypothermia after ROSC alleviated the alterations of Bax, Caspase 9, Caspase 3, and Bcl-2, while further increased the hippocampal expression of CIRP mRNA and protein, when compared with the normothermia rats (P < 0.05). In addition, compared with the therapeutic normothermia rats, histopathological damage in CA1 zone and NDS were respectively decreased and increased in the hypothermia rats (P < 0.05). Our findings suggest that 32 °C therapeutic hypothermia exerts an important neuroprotective effects by up-regulating CIRP expression and inhibiting mitochondrial apoptosis factor production in the cardiac arrest rat model.

Abstract:Laboratory and clinical studies have demonstrated that therapeutic hypothermia (TH), when applied as soon as possible after resuscitation from cardiac arrest (CA), results in better neurological outcome. This study tested the hypothesis that TH would promote cerebral blood flow (CBF) restoration and its maintenance after return of spontaneous circulation (ROSC) from CA. Twelve Wistar rats resuscitated from 7-min asphyxial CA were randomized into two groups: hypothermia group (7 H, n = 6), treated with mild TH (33–34°C) immediately after ROSC and normothermia group (7 N, n = 6,37.0 ± 0.5°C). Multiple parameters including mean arterial pressure, CBF, electroencephalogram (EEG) were recorded. The neurological outcomes were evaluated using electrophysiological (information quantity, IQ, of EEG) methods and a comprehensive behavior examination (neurological deficit score, NDS). TH consistently promoted better CBF restoration approaching the baseline levels in the 7 H group as compared with the 7 N group. CBF during the first 5–30 min post ROSC of the two groups was 7 H:90.5% ± 3.4% versus 7 N:76.7% ± 3.5% ( P < 0.01). Subjects in the 7 H group showed significantly better IQ scores after ROSC and better NDS scores at 4 and 24 h. Early application of TH facilitates restoration of CBF back to baseline levels after CA, which in turn results in the restoration of brain electrical activity and improved neurological outcome.

Abstract:Objective. Previously, we demonstrated that remote ischemic postconditioning (RIpostC) improved postresuscitation myocardial and cerebral functions in rat. Here, we investigated the effects of RIpostC alone and combined with therapeutic hypothermia (TH) on cardiac and neurological outcomes after CPR in swine. Methods. Twenty-one pigs were subjected to 10 mins of VF and then 5 mins of CPR. The animals were randomized to receive RIpostC alone, or its combination with TH, or sham control. RIpostC was induced by 4 cycles of limb ischemia followed by reperfusion. TH was implemented by surface cooling to reach a temperature of 32-34°C. Results. During 72 hrs after resuscitation, lower level of cardiac troponin I and greater stroke volume and global ejection fraction were observed in animals that received RIpostC when compared to the control. RIpostC also decreased serum levels of neuron-specific enolase and S100B and increased neurologic alertness score after resuscitation. The combination of RIpostC and TH resulted in greater improvement in cardiac and neurological outcomes than RIpostC alone. Conclusion. RIpostC was conducive to improving postresuscitation myocardial and cerebral functions and reducing their organ injuries. Its combination with TH further enhanced its protective effects.

It decreases cerebral metabolism and protects the brain after ischemia by reduction of brain metabolism, attenuation of neuroexcitotoxic cascade, abolition of reactive oxygen species, and inhibition of apoptosis. This article addresses the mechanism of neuroprotection, phases of hypothermia, and noninvasive and invasive cooling methods.

Abstract:Introduction: Studies have demonstrated that Win55,212-2 induced pharmacological hypothermia and protected the neurological function after cardiopulmonary resuscitation (CPR) in a rat cardiac arrest (CA) model. However, the mechanism is still unclear. In this study, we hypothesized brain derived neurotrophic factor (BDNF) was involved in this mechanism. Methods: CA was induced by transoesophageal ventricular pacing in SD rats. 5min after onset of CA, CPR was started. After 30 min of ROSC, 30 animals were randomized into either Win55, 212-2 group (animals in drug groups were received continuous intravenous infusion of Win55,212-2 with 1 mg/kg/h for 4 h) or Normothermia group (animals were received 5% DMSO). Rats in normothermia group were maintained at 37°C with warming lamp though the whole experiment. Rats in drug group were warmed to 37°C increasingly after 4h of Win55,212-2 treatment. Brains of five animals in each group were harvested for detecting BDNF by RT-PCT and Westen blot at 24 h after ROSC respectively. Results: Temperatures of animals in Win55,212-2 group decreased to 34°C in 4 hours after ROSC, and the survival of which were longer than that of the control group. The expression of BDNF was significantly higher in the Win55, 212-2 group than that of control group. Conclusions: BDNF paticipated the neuroprotection of Win 55,212-2 after CPR in a rat CA model.

Abstract:Introduction: A strong relationship between lactate clearance rate and better outcomes in patients of cardiac arrest (CA) has been demonstrated. Mild hypothermia improves neurological outcomes and survival after CA. In the present study, we investigated the effect of mild hypothermia on cerebral lactate metabolism in a rat model of CA. Hypothesis: Mild hypothermia accelerates cerebral lactate clearance rate in a rat model of CA. Methods: Ten male Sprague-Dawley rats were randomized into two groups: hypothermic (HT, n=5) and normothermic (NT, n=5) groups. After 8 mins of untreated ventricular fibrillation, precordial compression and mechanical ventilations were initiated for 8 mins. The core temperature was decreased to 33±0.5°C after 14 mins of resuscitation with a combination of ice packs, an electrical fan and a cooling blanket. Abstract:Objective:We investigated the dynamic changes of Nogo-A protein in brain and the effects of mild therapeutic hypothermia (MTH) on its expression after cardiopulmonary resuscitation (CPR).Methods:Western-blotting and neurological scoring of 45 rats subjected to cardiac arrest and CPR with and without MTR were performed to investigate the changes in the expression of Nogo-A protein in the hippocampus and cortex over a period of time ranging from 6 h to 72 h after restoration of spontaneous circulation (ROSC).Results:Nogo-A expression levels were increased at 6 h after CPR in the hippocampus and cortex, peaked at 24 h in the cortex, and at 48 h in the hippocampus. The expression of Nogo-A in the MTR group was significantly lower at 12 h (p < 0.05) compared to those with no MTR after ROSC.Conclusions: MTR blunts the expression of Nogo-A protein in the hippocampus and cortex after cardiac arrest and resuscitation, and MTR may provide cerebral protection after ischemia.
