Abstract:Hepatocellular carcinoma (HCC) is the third highest cancer killer in the world, and it is ranked second in China. Surgery, which includes liver transplantation, remains the best treatment for a curative outcome of HCC. In the 20th century, progress of HCC surgery was based on the advances in pathology, anatomy, biochemistry, immunology, and physics. In the early 21st century, however, biology will be the key issue influencing our progress with HCC surgery.
Surgical Techniques in the Radical Resection of Hilar Cholangiocarcinoma

Abstract:Hilar cholangiocarcinoma remains a formi-dable challenge to hepatopancreatobiliary surgeons since the reported resection of a primary cancer originating at the hepatic duct confluence by Brown and Myers in 1954. Emerging evidence has indicated that aggressive surgery with a curative resection offers a better option for long-term survival compared with conservative therapy. Liver transplantation has also been considered as a management opportunity for the treatment of cholangiocarcinoma. However, the survival rate has been poor due to the high proportion of disease recurrence. This review highlights recent techniques in hilar cholangiocarcinoma resec-tion, with special attention to the management of the resection margin, clinical skills of liver resection, lymph node clearance, and portal vein or hepatic artery resection or reconstruction. In addition, technical advances have been proposed in hepatopan-creatoduodenectomy and liver transplantation for hilar cholangio-carcinoma treatment. In the current hepatic procedures, promis-ing survival outcomes have been obtained in patients with hilar cholangiocarcinoma, exhibiting a decreased operative mortality and a steady improvement in long-term survival. Overall, the correct clinical strategy and appropriate surgical techniques may provide an increased chance to cure patients with hilar cholan-giocarcinoma.

Abstract:This paper reviews the advances in surgery for hepatocellular carcinoma (HCC). Certainly, in the past 10 years great advances have been achieved in the field of liver surgery. However, three principal factors have become important for improving the prognosis of HCC: the early detection and resection of smaller asymptomatic HCC, re-resection of subclinical recurrence and metastasis, and cytoreduction and sequential resection for originally unresectable HCC. Encouraging changes in the prognostic pattern were observed when comparing the periods 1958-1977 (n = 132) and 1978-1989 (n = 508): the five-year survival rate after resection was 20.2% and 45.5%, respectively; and the 10-year survival rate after resection was 14.6% and 32.6%, respectively. Amongst the 210 patients with small HCC (�� 5 cm), the five-year and 10-year survival rates were 63.8%, and 43.5%, respectively. One hundred and thirteen patients survived for more than five years; 37 of them, more than 10 years; seven of them, more than 20 year. Indications and techniques of hepatic resection, and the value of liver transplantation for HCC are discussed in the paper.

Abstract:In the 1950s, hepatic lobectomy for huge hepatocellular carcinoma (HCC) has benefited 5-10% of HCC patients; in the 1970s, limited resection for small HCC and reresection for recurrence have benefited another 5-10% HCC patients. Cytoreduction and sequential resection for unresectable HCC might be of benefit to a further 5-10% HCC patients in the 1990s. Analysis of 1,642 patients with pathologically proven HCC in 1959-1991 demonstrated that the series 5-year survival has increased from 3.0% (n = 136) in the 1960s, to 12.2% (n = 440) in the 1970s, to 40.2% (n = 1,066) in the 1980s, which was correlated to the increasing number of limited resections for small HCC, reresections for subclinical recurrence, and cytoreductions and sequential resections for portions of unresectable HCC. With the advances in early detection, multimodality treatment, and changing concepts in surgical oncology, the role of surgery in the treatment of HCC has increased.

Abstract:With the rapid development of liver surgery,minimally invasive techniques have been widely used in liver surgery. Many challenging liver can be performed laparoscopically to decrease the surgical trauma. At the same time,the efficiency and accuracy of liver surgeries have been highly improved by the advanced assisted technology of liver surgery. Abstract:Hepatocellular carcinoma (HCC), the second cause of cancer death in China, is responsible for 130,000 deaths every year. However, as a result of efforts in early detection and small HCC resection, re-resection for subclinical recurrence, cytoreduction and second-stage resection for unresectable HCC, and aggressive palliative surgery other than resection (hepatic artery ligation/cannulation, cryosurgery, etc.), and encouraging improvemental of long-term survival of inpatients has been observed in the authors' institution. In the entire series of 2672 HCC inpatients, the 5-year survival was 4.8% in 1958-70, 12.2% in 1971-83, and 46.7% in 1984-95. The 5-year survival rates were: 61.3% for small HCC resection (n = 645), 33.6% for large HCC resection (n = 950), 48.9% for re-resection (n = 147) calculated from the first resection, 67.9% for second-stage resection (n = 73), and 19.8% for palliative surgery (n = 574, including 73 with second-stage resection). By 1995, 239 HCC patients survived for more than 5 years. Recurrence and metastasis remained the major obstacles to more prolonged survival after HCC surgery. Molecular studies of HCC invasiveness, experimental intervention in the newly established highly metastatic model of human HCC in nude mice in the authors' institution have also been delineated. It is concluded that early detection and small HCC resection remain the major approach to improve survival. Aggressive surgical treatment, such as re-resection and second-stage resection, are also important, and invasiveness-related recurrence will be the next target to be studied.

Abstract:The incidence of hepatocellular carcinoma (HCC) has increased worldwide over the past two decades. Surgical resection and liver transplantation have been demonstrated as potentially curative treatment options, which could be considered in 30% -40% of HCC patients. Recent advancements of surgical treatment have focused not only on the surgical techpiques, but also the hepatic functional reserve evaluation, resectability assessment and the effects of biological characteristics of tumor on prognosis. There is no single variable to evaluate the hepatic functional reserve accurately. Combined Child-Pugh classification, ICGI5, portal vein pressure detection and remanent liver volume measurement are required prior to liver resection. The 5-year survival rate after liver resection for HCC is about 50%. The results are acceptable for some selected patients that underwent tumor resection with thrombectomy, including HCC with portal vein tumor thrombus or bile duct thrombosis. The choice of local resection or regular hepatectomy is still controversial although the former is commonly performed to treat HCC with cirrhosis, and the latter is applied to HCC patients without liver cirrhosis. The results of liver transplanta-tion for HCC are better than liver resection, and the Milan criteria is generally accepted. Any attempts to expand the selection criteria should be cautious because of organ shortage. Salvage transplantation for intrabepatic recurrence after liver resection may be a good choice in some resectable HCC. The recurrence and metastasis after surgical treatment are the main obstacles to achieve better results. Identification of predictive factors could be helpful to develop prevention strategies. Due to the importance of biological characteristics in tumor recurrence and metastasis, a molecular classification to predict prognosis of HCC patients will lead to a more personalized medicine. Targeting key molecules of biological pathways could optimize the therapeutic modality in HCC.
Small Hepatocellular Carcinoma: Current Status and Prospects.

Abstract:BACKGROUND:More than two decades have gone by since the early report of resection for small hepatocellular carcinoma (HCC), which resulted in improved prognosis of HCC.OBJECTIVE:To review the past and recent data, and prospect the future in this field.DATA SOURCES:Literature and recent data from the Liver Cancer Institute of Fudan University, Shanghai, China.DATA SYNTHESIS:1232 patients with small HCC from the institute were analyzed between 1960-1984 (n=107) and 1985-1999 (n=1125). The increase of limited resection rate from 69.5% to 82.5% contributed in part to the increase of resectability from 76.6% to 95.5%, decrease of operative mortality from 2.4% to 1.2%, and improvement of 5-year survival after resection (from 53.1% to 64.0%). The 5-year survival was higher after limited resection than after lobectomy, being 64.4% versus 55.9%. The 5-year survival after resection was superior to that after cryosurgery and other regional cancer therapies (32.8%). However, molecular studies found that biological characteristics were only slightly better in small HCC than in large HCC.CONCLUSIONS:Resection remains the treatment choice for small HCC with compensated liver function, while regional cancer therapies and liver transplantation are alternatives for patients with incompensated liver function. Biological characteristics remain the leading factor influencing prognosis of small HCC.
Recent Advances in the Studies of Hepatocellular Carcinoma

Abstract:In the recent decades, advances have been made in the studies of hepatocellular carcinoma (HCC). Based on the further understanding of etiological factors involved in HCC, prevention of HCC is possible. Using hepatitis B vaccination in newborn babies, interferon therapy to control viral hepatitis B and C and improve quality of drinking water, HCC incidence has decreased. A prospective randomized control screening trial has indicated that screening decreased HCC mortality. Currently a 1 cm HCC is not difficult to be detected based on the progress of tumor marker and medical imaging . Surgical resection remains to play a key role to improve HCC prognosis. Of the 415 HCC patients who survived more than 5 years, 95.4％ of them received either 1st stage or 2nd stage resection.The 5-year survival of 1106 patients with small HCC resection was up to 63.7％ .In a group or 109 patients who had initially unresectable HCC treated by palliative surgery other than resection, and received 2nd stage resection after shrinkage of tumor, the 5-year survival was up to 62.2％. Comparison of large series of liver transplantation and resection for HCC demonstrates that transplantation is only suitable for small HCC, not suitable for resection due to liver dysfunction. The treatment pattern of HCC has been changed from surgery to a combination of surgery, regional cancer therapies and their multimodality combination." Evidence based" is currently advocated in clinical research, unfortunately, very few effective drug for HCC has been confirmed. Recurrence and metastasis will be an important target to be studied in the 21 century.

Thirty Years’ Studies on Surgery and Related Basic Research of Hepatocellular Carcinoma

Abstract:To emphasize the role of clinical innovation and basic research on the advances of hepatocellular carcinoma (HCC) surgery, related papers published by the author's institution in the last three decades and recent data were analysed. In the 1970s−1980s, diagnosis using alpha fetoprotein assay and innovation of limited resection instead of lobectomy have resulted in early diagnosis and resection of small HCC. These have led to a marked increase of long‐term survivors of HCC. In the 1980s−1990s, research works including establishment of human HCC nude mice model, experimental multimodality therapy and studies on targeting therapy have been conducted. These have made resection possible in some patients after downstaging of initially unresectable HCC. The 5‐year survival of unresectable HCC was improved. Since the early 1990s, research works have been carried out on metastatic recurrence of HCC, including the establishment of metastatic human HCC model, the molecular basis of HCC metastasis, angiogenesis and experimental intervention. These provide clues for prediction and prevention of metastatic recurrence of HCC. Translation from bench to bedside has resulted in improved disease free survival after curative HCC resection in a randomized control trial using interferon alpha. It is concluded, clinical innovation and basic research are essential for progress of HCC surgery.
Three Decades' Experience In Surgery Of Hepatocellular Carcinoma

Abstract:In the author's institution, 2254 patients with hepatocellular carcinoma (HCC) have been treated during 1958-1994. The overall 5-year survival increased from 5.4% (1958-1970), to 11.9% (1971-1982), to 46.2% (1983-1984), which correlated well with the increasing proportion of small HCC in the series (2.6%, 12.1%, and 33.4%, respectively); with the increasing percentage of limited resection (3.1%, 32.2%, and 58.3%); with the increasing number of re-resections for recurrence (0, 27, and 114 patients); and with the increasing number of second stage resections (0, 5, and 67 patients). In our institution, surgical approaches that resulted in significantly prolonging survival included: small HCC resection, re-resection, and cytoreduction followed by sequential resection for initially unresectable HCC. Experience in these 3 aspects suggests: (a) Small HCCs are mainly found by screening using AFP and ultrasonography (US) in a high risk population, and limited resection is the best treatment in patients with compensated liver cirrhosis, the 5-year survival after resection being 62.9% (n = 549). (b) Postoperative monitoring using AFP/US every 2-3 months for 5-10 years after curative resection is needed to detect subclinical recurrence. Limited re-resection is indicated for liver recurrence less than 3 nodules, and lung lobectomy is of proven merit to prolong survival for solitary lung metastasis. Re-resection of subclinical recurrence has resulted in a 10-20% further increase in 5-year survival after curative resection. (c) Palliative surgery other than resection such as hepatic artery ligation (HAL) and cannulation with arterial infusion (HAI), cryosurgery, etc. are superior to palliative resection with residual cancer. (d) Cytoreduction and sequential resection have provided hope for localized unresectable HCC, particularly in the right cirrhotic liver. Multimodality combination treatments such as HAL+HAI+radioimmunotherapy/regional radiotherapy are acceptable cytoreductive therapies. Repeated transcatheter hepatic arterial chemoembolization (TACE) is an alternative nonsurgical approach. Sequential resection is important to eradicate residual cancer after cytoreduction. The 5-year survival of 72 patients with cytoreduction and sequential resection for initially unresectable HCC was 62.1% and resulted in improving 5-year survival in the entire series of unresectable HCC over the 3 periods from 0% to 7.4% to 25.7%, respectively. However, multicentric origin and tumor invasiveness are two major targets to be studied in the control of recurrence and metastasis.
[Surgical Treatment of 1450 Patients with Hepatocellular Carcinoma].

Abstract:An analysis of 1450 patients with pathologically and surgically proved hepatocellular carcinoma (HCC) revealed a steadily increased 5-year survival rate of 2.8% (1958-1968), 10.5% (1969-1979), and 36.6% (1980-1990), owing to increased number of cases with small HCC (0.9%, 9.9%, 25.2%, respectively), more and more patients undergoing tumor resection (0, 14, 59) and preoperative tumor bulk reduction for otherwise unresectable HCC (0, 1, 33 cases). Small HCC resection (n = 250) resulted in a high 5-year survival rate as compared to non-small HCC resection (n = 491) (66.3% versus 31.2%). The 5-year survival of 73 patients undergoing tumor re-resection was 40.6%, and the 5-year survival of 34 patients having second stage resection was as high as that of those having small HCC resection (62.0%). 125 patients survived more than 5 years; of these 65 underwent small HCC resection, and 43 non-small HCC resection. Thus, early resection, re-resection for subclinical recurrence, and second stage resection for originally unresectable HCC play an important role in improving prognosis of HCC.

Abstract:Surgical treatment is the classic treatment modality for focal liver diseases. In 140 years, liver surgery is constantly evolving, along with advances in understanding anatomy, physiology, and emergence of technologies. During the last 30 years, many surgical techniques used in the modern ages became available since the 1990s, and liver surgery is becoming a routine procedure in many hospitals in China because of the significant decrease in surgical mortality. Furthermore, liver surgery is playing an increasingly important role in multimodality treatment for liver cancer while the treatment pattern is also changing because of progresses in systemic treatment. The progresses in liver surgery are summarized in the article.

Abstract:Hepatocellular carcinoma (HCC) is a significant global health challenge, requiring innovative methods to improve patient survival. Due to different disease backgrounds, different HCC management guidelines have been developed, especially in the field of surgical treatment, with the aim of reducing the risk of incidence and enhancing the therapeutic effect. Focusing on the progress and challenges in the development of surgical management strategies for HCC in recent years, this article systematically elaborates on the research and clinical application of precision surgical treatment, including improvement of the surgical evaluation system, breakthroughs in surgical techniques, and updates in perioperative treatment concepts. In addition, clinical research on surgical treatment for HCC has received unprecedented attention. The conclusions of innovative clinical trials in surgery will provide important guidance for the development of practice guidelines and the selection of appropriate treatment strategies for HCC patients.
Surgery of the small hepatocellular carcinoma in China | DIE CHIRURGIE DES KLEINEN HEPATOZELLULAREN KARZINOMS IN CHINA

Abstract:Surgical treatment of the small hepatocellular carcinoma (HCC) is associated with lower operative mortality rate, higher resectability rate and better long-term survival rate. The keystones of better prognosis are early detection and diagnosis made by ultrasonography in the high-risk group, early surgical treatment, limited resection technique as well as reresection in cases of subclinical recurrences. Results of surgical treatment in patients with small HCC over a period of 32 years are presented and compared with the literature. Out of 381 patients, 345 (90.6%) underwent surgical resection, 34 (8.9%) received palliative surgery other than resection, and 2 (0.5%) were treated conservatively following surgical exploration. The operative mortality rate within 1 month after resection was 2.0% (7/345). The 5-year recurrence rate after curative resection was 43.5%. The 5-year survival rates of entire series of small HCC populations and of patients who underwent surgical resection only were 58.5 and 62.7%, respectively.

Abstract:With progress of technique and accumulation of experience, laparoscopic liver resection (LLR) has become a standard operation. China is a country with high incidence of hepatocellular carcinoma (HCC). In the past 20 years, LLR has overcome "double barriers of concept and technique" . After exploration, development and optimization, it evolves from initial wedge resection to hemihepatectomy, extended hemihepatectomy, mesohepatectomy, followed by precise anatomical hepatectomy. With the continuous improvement of technical approaches, the standardized operation system of LLR has preliminarily taken shape. From the perspectives of surgical and oncological view, it has been proven to bring "dual advantages of minimal invasion and survival" to patients. At present, surgical techniques develop with each passing day, and systemic therapeutic drugs emerge in an endless stream. Along with new pattern and new challenge, China’s laparoscopic liver resection for HCC is striding into a new era with a brilliant future.
Current Management of Hepatocellular Carcinoma.

Abstract:After a long, hopeless period in the management of hepatocellular carcinoma (HCC), the diagnosis and treatment of HCC have progressed remarkably in the past decade. In particular the discovery of asymptomatic HCC in the early 1970s opened up a new era in clinical research of HCC. With the progress in the diagnostic imaging of liver tumors, a 1 cm hepatic mass can now be detected. It is especially worth noting that a 5-year survival rate of 72.9% has been achieved after the resection of the tumor in asymptomatic HCC patients. The role of surgery in the treatment of HCC has become more important. Various modalities of medical treatment and combination therapy have been recommended and used. Despite the progress in the early diagnosis and treatment of HCC, a complete cure is very rare. Problems to be studied include new tumor markers for the early detection of alpha-fetoprotein (AFP)-negative HCC, the development of more specific treatments for unresectable HCC with uncompensated cirrhosis, and an effective approach to preventing recurrence and metastasis after radical resection.

Abstract:<p>The global incidence of liver cancer continues to grow. Liver cancer, especially hepatocellular carcinoma, has high recurrence and mortality rates. Here, we review the past decade's diagnostic, therapeutic, and management strategies for hepatocellular carcinoma, and summarize new patient management approaches, including enhanced recovery after surgery, targeted therapy, and immunotherapy. We compare traditional and innovative management methods, which comprise developments in precision medicine, and consider their limitations. Ongoing innovation and technological advances enable surgeons to gain deeper understandings of the multidimensionality of hepatocellular carcinoma, thereby promoting the continuous development of precision therapy.</p>

Abstract:OBJECTIVE:To summarize the progress of surgical treatment of hepatocellular carcinoma (HCC) and related basic research at the Liver Cancer Institute of Shanghai Medical University in the recent years, with special reference to recurrence and metastasis.METHODS:Published and unpublished update clinical and experimental data in the above-mentioned areas are summarized.RESULTS:Surgical resection has played an important role in improving prognosis of HCC, the 5-year survival were 63.4% for small HCC resection (n = 806), 39.6% for large HCC resection (n = 1061), 64.7% for cytoreduction (using hepatic artery cannulation and ligation) and sequential resection of initially unresectable HCC (n = 93), 56.0% for cytoreduction using transcatheter arterial chemoembolization (TACE) and followed by resection (n = 65), and 22.4% for hepatic resection with removal of tumor thrombi in portal vein (n = 103). Unfortunately, the 5-year recurrent rate after curative resection of HCC was up to 61.5%, which was mainly a result of intrahepatic "metastasis" and multicentric origin of HCC. Clinically, re-resection of subclinical recurrence yielded 56% of 5-year survival (n = 202); prevention of recurrence by transcatheter arterial chemoembolization (TACE) + Interferon, or LAK/IL-2 therapy have decreased 3-year recurrent rate from 33% to 11%-18%. In experimental aspect, metastatic human HCC model in nude mice (LCI-D20) and HCC cell line with metastatic potential (MHCC97) have been established; studies on HCC invasiveness in the molecular level revealed similar results that reported in other solid cancers, and small HCC showed slightly better biological characteristics as compared with large HCC; microvessel density (MVD) that reflecting angiogenesis adversely correlated with 5-year survival of small HCC; experimental interventions using antisense H-ras, bispecific antibody, BB94, as well as anti-angiogenic agents (TNP470, suramin, CAI, heparin, antisense VEGF, etc.) have been demonstrated to inhibit tumor growth and lung metastasis in nude mice model.CONCLUSIONS:Recurrence and metastasis are the major obstacle to further improve prognosis of HCC, studies should be conducted both in clinical and experimental aspects, "HCC invasiveness" will be the major target to be studied, particularly in the molecular level, and anti-angiogenesis will be one of the important approach.
