Abstract:Since coronavirus disease 2019 (COVID-19) might circulate in the following seasons, it is essential to understand how COVID-19 influences other respiratory diseases, especially influenza. In this study, we analyzed the influenza activity from mid-November 2019 to March 2020 in Chinese mainland and found that the influenza season ended much earlier than previous seasons for all subtypes and lineages, which may have resulted from the circulation of COVID-19 and measures such as travel control and personal protection. These findings provide rudimentary knowledge of the co-circulation patterns of the two types of viruses.

Abstract:Background: Influenza co-circulates with coronavirus disease 2019 (COVID-19) in the space and time. Given the current situation of COVID-19 pandemic and influenza seasonal epidemic worldwide, there are lots of concerns about the potential co-infections or interactions between these two respiratory viruses. We aimed to study the potential interactions between them in Wuhan. Methods: We obtained the epidemiological, demograhic and laboratory data from the COVID-19 and influenza cases visited the study hospital between January 2017 and February 2020. The co-infections of SARS-CoV-2 and influenza virus were retrospectively investigated in COVID-19 and influenza cases, and prospectively detected in the cases with fever and any other respiratory symptom visited the same hospital during February 14-24, 2020. Findings: COVID-19 incidence dramatically increased in the end of January 2020 and reached the peak on February 4 in the study hospital. The seasonal epidemic of influenza was interrupted in January. A sharp decline of influenza A incidence and a premature plummet of influenza B incidence were observed. Negative correlations between the epidemic curves of COVID-19 and influenza A (r=-0.49, p<0.05) or influenza B (r=-0.51, p<0.05) were uncovered. Nine co-infections were retrospectively identified in 1054 cases of COVID-19 or influenza (0.9%, 95% CI: 0.4-1.6%). No co-infection was prospectively detected in 179 patients with fever and any respiratory symptom. Interpretation: The emergence of COVID-19 interfered influenza epidemic in 2019-2020 season in Wuhan. Enhancing the surveillance of influenza could potentially facilitate better judgment about the real situation of COVID-19 pandemic, which could be hidden in massive influenza cases. Impact of Coinfection with SARS-CoV-2 and Influenza on Disease Severity: A Systematic Review and Meta-Analysis

Abstract:Background: Although coinfection with influenza in COVID-19 patients has drawn considerable attention, it is still not completely understood whether simultaneously infected with these two viruses influences disease severity. We therefore aimed to estimate the impact of coinfected with SARS-CoV-2 and influenza on the disease outcomes compared with the single infection of SARS-CoV-2. Materials and Methods: We searched the PubMed, Web of Science, Embase, Cochrane Library, China National Knowledge Infrastructure Database (CNKI) to identify relevant articles up to July 9, 2021. Studies that assessed the effect of SARS-CoV-2 and influenza coinfection on disease outcomes or those with sufficient data to calculate risk factors were included. Risk effects were pooled using fixed or random effects model. Results: We ultimately identified 12 studies with 9,498 patients to evaluate the risk effects of SARS-CoV-2 and influenza coinfection on disease severity. Results indicated that coinfection was not significantly associated with mortality (OR = 0.85, 95%CI: 0.51, 1.43; p = 0.55, I2 = 76.00%). However, mortality was found significantly decreased in the studies from China (OR = 0.51, 95%CI: 0.39, 0.68; I2 = 26.50%), while significantly increased outside China (OR = 1.56, 95%CI: 1.12, 2.19; I2 = 1.00%). Moreover, a lower risk for critical outcomes was detected among coinfection patients (OR = 0.64, 95%CI: 0.43, 0.97; p = 0.04, I2 = 0.00%). Additionally, coinfection patients presented different laboratory indexes compared with the single SARS-CoV-2 infection, including lymphocyte counts and APTT. Conclusion: Our study revealed that coinfection with SARS-CoV-2 and influenza had no effect on overall mortality. However, risk for critical outcomes was lower in coinfection patients and different associations were detected in the studies from different regions and specific laboratory indexes. Further studies on influenza strains and the order of infection were warranted. Systematic testing for influenza coinfection in COVID-19 patients and influenza vaccination should be recommended.

Abstract:In this study, we performed a single‐centred study of 307 SARS‐CoV‐2 infected patients. It was found that co‐infection of SARS‐CoV‐2 and influenza virus was common during COVID‐19 outbreak. Influenza and Other Respiratory Viruses

Abstract:The COVID‐19 pandemic caused by the novel coronavirus SARS‐CoV‐2 continues to have a major impact on healthcare and social systems throughout the world. As the clinical and epidemiological features of COVID‐19 have many parallels with influenza, it is important to ensure optimal management of both respiratory diseases as we anticipate their continued co‐circulation. In particular, there is a need to ensure that effective surveillance and diagnostic capacities are in place to monitor these and other respiratory viruses, as this will underpin decisions on the appropriate clinical management of the respective diseases. As such, we propose a series of key recommendations for stakeholders, public health authorities, primary care physicians and surveillance bodies that will help mitigate the combined risks of concurrent influenza epidemics and the COVID‐19 pandemic. We advocate the judicious use of influenza vaccines and antivirals, particularly among groups at high risk of complications, with healthcare workers also considered a priority for vaccination. It is likely that the increased use of emerging technologies such as telemedicine and contact tracing will permanently change our approach to managing infectious disease. The use of these technologies, alongside existing pharmaceutical strategies, will ensure that we achieve a holistic approach to the global public health measures needed to deal with the combined threat of influenza and COVID‐19. Ensuring that this approach is optimal will be key as we move from a reactive pandemic response towards preparing for the long‐term management of the remarkable clinical burden associated with these respiratory pathogens.

Abstract:In this Journal, Tang et al. have report the symptoms of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection appear very similar to influenza (1). We would like to share our findings for co-infection of SARS-CoV-2 and influenza virus. In December, 2019, a novel coronavirus (SARS-CoV-2) caused Coronavirus disease 2019 (COVID-19) in Wuhan City, Hubei province, China (2,3). The epidemic of SARS-CoV-2 has rapidly spread worldwide and affected more than 4 million patients with more than 300 000 deaths in more than 230 countries (4).

Abstract:Influenza is an infectious respiratory disease caused by the influenza viruses. Older people, infants and people with underlying medical conditions could have a higher risk of severe influenza symptoms and complications. The co-infection of Coronavirus Diseases 2019 (COVID-19) with influenza viruses could lead to the complication of prevention, diagnosis, control, treatment, and recovery of COVID-19. Influenza vaccine and COVID-19 vaccine overlapped in target populations, vaccination time, and inoculation units. Although there was insufficient evidence on the immunogenicity and safety of co-administration of influenza vaccine and COVID-19 vaccine, World Health Organization and some countries recommended co-administration of inactivated influenza vaccine and COVID-19 vaccine. This review summarized domestic and international vaccination policies and research progress, and put forward corresponding suggestions in order to provide scientific support for the formulation of vaccination strategy on seasonal influenza vaccine and COVID-19 vaccine.

Abstract:The COVID-19 pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has resulted in a serious public health burden. As the COVID-19 epidemic in China would coincide with a seasonal outbreak of influenza, there were serious concerns about whether influenza would be aggravated by the SARS-CoV-2 infection and COVID-19 pandemic. This article provides a brief overview of the impacts of the COVID-19 epidemic on influenza activity in China. The percentage of positive influenza tests decreased during the COVID-19 pandemic. During the first stage of the COVID-19 outbreak, the percentage of positive influenza tests reached to a peak of 47.7%. At the second stage, the percentage of positive influenza tests was dramatically decreased from 40.4% to 14.0%. Thereafter, it remains at a low level of less than 6.2%. In addition, the possible causes of this phenomenon have been summarized, including prevention and control measures and ecological competition. Lastly, this article suggests that the public health approach to preventing COVID-19 may also help to control other respiratory infectious diseases. Public health measures need to be maintained even in the later stages of the COVID-19 epidemic.

Abstract:The COVID-19 pandemic remains a serious public health problem globally. During winter influenza seasons, more aggressive SARS-CoV-2 infections and fatalities have been documented, indicating that influenza co-infections may significantly impact the disease outcome of COVID-19. Both influenza and SARS-CoV-2 viruses share many similarities in their transmission and their cellular tropism for replication in the human respiratory tract. However, the complex intricacies and multi-faceted dynamics of how the two pathogens interact to ensure their survival in the same lung microenvironment are still unclear. In addition, clinical studies on influenza co-infections in COVID-19 patients do not provide conclusive evidence of how influenza co-infection mechanistically modifies disease outcomes of COVID-19. This review discusses various viral as well as host factors that potentially influence the survival or synergism of these two respiratory pathogens in the infected lung microenvironment.

Abstract:Respiratory pathogen coinfections pose significant challenges to global public health, particularly regarding the intersecting epidemics of COVID-19 and influenza. This study investigated the incidences of respiratory infectious pathogens in this unique context. We collected throat swab samples from 308 patients with a fever from outpatient and emergency departments at sentinel surveillance hospitals in Xiamen, southeast of China, between April and May 2023, testing for SARS-CoV-2 and 26 other respiratory pathogens. The coinfection rate of the XBB SARS-CoV-2 variant with other respiratory pathogens was higher than that observed during the Alpha and Delta phases. Among patients with influenza, bacterial coinfections were more prevalent. Only 0.65% (2/308) of the patients were concurrently infected with both COVID-19 and influenza. Age-stratified analysis showed a clear pattern, with a higher incidence of coinfections in children under 18 years of age. These findings highlight the need for the timely detection of respiratory pathogen coinfections and for the implementation of appropriate interventions, crucial for reducing disease burden during intersecting respiratory epidemics.

Abstract:Abstract Coronavirus disease 2019 (COVID-19) was detected in China during the 2019–2020 seasonal influenza epidemic. Non-pharmaceutical interventions (NPIs) and behavioral changes to mitigate COVID-19 could have affected transmission dynamics of influenza and other respiratory diseases. By comparing 2019–2020 seasonal influenza activity through March 29, 2020 with the 2011–2019 seasons, we found that COVID-19 outbreaks and related NPIs may have reduced influenza in Southern and Northern China and the United States by 79.2% (lower and upper bounds: 48.8%–87.2%), 79.4% (44.9%–87.4%) and 67.2% (11.5%–80.5%). Decreases in influenza virus infection were also associated with the timing of NPIs. Without COVID-19 NPIs, influenza activity in China and the United States would likely have remained high during the 2019–2020 season. Our findings provide evidence that NPIs can partially mitigate seasonal and, potentially, pandemic influenza.

Abstract:The coronavirus 2019 (COVID-19) pandemic, caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has spread across the world and is responsible for over 1,686,267 deaths worldwide. Co-infection with influenza A virus (IFV-A) during the upcoming flu season may complicate diagnosis and treatment of COVID-19. Little is known about epidemiology and outcomes of co-infection. Data for 213 COVID-19 patients treated at Tongji Hospital in Wuhan from January 28, 2020 to March 24, 2020 were retrospectively analyzed. Ninety-seven of the patients (45.5%) tested positive for anti- IFV-A immunoglobulin M antibodies. The clinical characteristics were described and analyzed for patients with SARS-CoV-2 infection only and patients with SARS-CoV-2/IFV-A co-infection. Patients with co-infection showed similar patterns of symptoms and clinical outcomes to patients with SARS-CoV-2 infection only. However, an increased expression of serum cytokines (interleukin-2R [IL-2R], IL-6, IL-8, and tumor necrosis factor-alpha) and cardiac troponin I, and higher incidence of lymphadenopathy were observed in patients with SARS-CoV-2 infection only. Male patients and patients aged less than 60 years in the SARS-CoV-2 infection group also had significantly higher computed tomography scores than patients in co-infection group, indicating that co-infection with IFV-A had no effect on the disease outcome but alleviated inflammation in certain populations of COVID-19 patients. The study will provide a reference for diagnosing and treating IFV-A and SARS-CoV-2 co-infection cases in the upcoming flu season.

Abstract:The World Health Organization (WHO) named the disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) as coronavirus disease 2019 (COVID-19) and declared the outbreak a Public Health Emergency of International Concern (PHEIC) on January 30, 2020 and a pandemic on March 11, 2020. Globally, there have been 239,437,517 confirmed cases of COVID-19 reported to WHO, including 4,879,235 (2.1%) deaths as of October 15, 2021 (1). The COVID-19 pandemic continues to cause an unparalleled impact on global public health security and economic well-being in the context of previous influenza pandemics as well as other emerging infectious diseases in history (2). As the epidemiology, clinical presentations, and control measures for SARS-CoV-2 and influenza share many features, there is a need to develop strategies to address additional challenges arising in the continued surveillance, prevention, and clinical management of influenza in conjunction with COVID-19 pandemic responses. SARS-CoV-2 and influenza are expected to be circulating during the upcoming influenza season and may lead to situation where an amplified respiratory disease burden occurs due to both viruses spreading, causing overlapping symptoms or severe clinical illness particularly in the case of co-infections (3). Therefore, it is essential that effective public health control measures are in place for the forthcoming influenza season to protect those at risk (e.g., the elderly and patients with underlying chronic diseases), prevent severe illness, and minimize additional impact on the healthcare system and a surge in hospital admissions.

Abstract:Background Non-pharmaceutical interventions (NPIs) to mitigate COVID-19 can impact the circulation of influenza viruses. There is a need to describe the activity of influenza and its subtypes during the COVID-19 pandemic to aid in the development of influenza prevention and control measures in the next influenza season. Method Data from pathogenic surveillance performed by the Chinese National Influenza Center from January 2016 to August 2021 were extracted and stratified by type and subtype for northern China and southern China. The distribution of influenza activity and circulating subtypes were described during the COVID-19 pandemic, and data from 2016 to 2019 were used for comparisons. Results Influenza activity declined rapidly and then rose slowly during the COVID-19 pandemic in China. The distribution of influenza subtypes changed from A-dominant to B/Victoria-dominant after the COVID-19 outbreak. Discussion Whether the B/Yamagata lineage has disappeared from China deserves more attention in future virologic monitoring programs. The influenza vaccination campaign in the 2021–2022 season is an important means by which to reduce the proportion of susceptible people and limit the damage that potentially greater and earlier circulation of the virus could cause.

Prevalence and associated outcomes of coinfection between SARS-CoV-2 and influenza: a systematic review and meta-analysis

Abstract:Objectives: To estimate the prevalence of influenza coinfection in COVID-19 patients and investigate its association with severe clinical outcomes.Methods: We systematically searched the Web of Science, PubMed, Scopus, Embase, The Cochrane Library, and CNKI for studies published between January 01, 2020, and May 31, 2023. Meta-analysis was performed to estimate the pooled prevalence of coinfection and the impact on clinical outcomes. Systematic review registered in PROSPERO (CRD42023423113).Results: A total of 95 studies involving 62,107 COVID-19 patients were included. The pooled prevalence of coinfection with influenza virus was 2.45% (95% confidence interval [CI]: 1.67-3.58%), with a high proportion of influenza A. Compared with mono-infected patients (COVID-19 only), the odds ratio (OR) for severe outcomes (including intensive care unit admission [OR = 2.20, 95% CI: 1.68-2.87, P < 0.001], mechanical ventilation support [OR = 2.73, 95% CI: 1.46-5.10, P = 0.002], and mortality [OR = 2.92, 95% CI: 1.16-7.30, P = 0.022]) was significantly higher among patients coinfected influenza A.Conclusion: Although the prevalence of coinfection is low, coinfected patients are at higher risk of severe outcomes. Enhanced identification of both viruses, as well as individualized treatment protocols for coinfection, are recommended to reduce the occurrence of serious disease outcomes in the future.(c) 2023 Published by Elsevier Ltd on behalf of International Society for Infectious Diseases. Abstract:The outbreak of 2019 novel coronavirus (COVID‐19) infection emerged in Wuhan, China on December 2019. Since then the novel coronavirus pneumonia disease has been spreading quickly and many countries and territories have been affected, with major outbreaks in China, South Korea, Italy and Iran. Influenza virus has been known as a common pathogen in winter and it can cause pneumonia. It was found clinically that very few patients were diagnosed with both COVID‐19 and influenza virus. 5 out of the 115 patients confirmed with COVID‐19 were also diagnosed with influenza virus infection, with three cases being influenza A and two cases being influenza B. In this study, we describe the clinical characteristics of those patients who got infected with COVID‐19 as well as influenza virus. Common symptoms at onset of illness included fever (5 [100%] patients), Cough (5 [100%] patients), shortness of breath (5 [100%] patients), nasal tampon (3 [60%] patients), pharyngalgia (3 [60%] patients), myalgia (2 [40%] patients), fatigue (2 [40%] patients), headache (2 [40%] patients), and expectoration (2 [40%] patients). The laboratory results showed that compared to the normal values, the patients' lymphocytes were reduced (4 [80%] patients), and liver function ALT and AST (2 [40%] patients, 2 [40%] patients) and C‐reactive protein (4 [80%] patients) were increased when admitted to hospital. They stayed in hospital for 14, 30, 17, 12, and 19 days (28.4±7.02), respectively. The main complications for the patients were acute respiratory distress syndrome (ARDS) (1 [20%] patients), acute liver injury (3 [60%] patients), and diarrhea (2 [40%] patients). All patients were given antiviral therapy (including oseltamivir), oxygen inhalation, and antibiotics. Three patients were treated with glucocorticoids including two treated with oral glucocorticoids. One of the five patients had transient hemostatic medication for hemoptysis. Fortunately, all patients did not need ICU care and were discharged from hospital without death. In conclusion, those patients with both COVID‐19 and influenza virus infection did not appear to show a more severe condition because based on the laboratory findings, imaging studies, and patient prognosis, they showed similar clinical characteristics as those patients with COVID‐19 infection only. However, it is worth noting that the symptoms of nasal tampon and pharyngalgia may be more prone to appear for those co‐infection patients.This article is protected by copyright. Abstract:COVID-19 has been a topic of interest since a pandemic struck in 2019. The morbidity of influenza tended to decrease due to the measures to prevent COVID-19. Indeed, influenza seems to be "ignored" in this era of COVID-19. However, influenza has not disappeared from the scene. Presented here are two examples of recent influenza epidemics in China and Australia. Possible interactions between COVID-19 and influenza are discussed. Measures against COVID-19 may reduce contact with influenza, subsequently reducing adaptive immunity against influenza in the general population. Influenza might not be center stage right now, but insufficient adaptive immunity in the population may potentially trigger a future influenza pandemic. Coinfection with COVID-19 and influenza might potentially be a thorny problem. Hence, influenza cannot be ignored. Governments around the world should take measures to prepare for a possible influenza pandemic in the future.
