Abstract:Abstract Objective: Melanoma is a highly heterogeneous disease. A certain subgroup of patients with primary melanomas exhibits a greater potential to develop metastatic disease and thus has worse survival. The present study attempts to identify the molecular features shared by metastatic melanomas and the subgroup of primary melanomas, and use the information to assist clinicians to design personalized therapy for primary melanoma patients. Patients and methods: Seven previously reported biomarkers, including BRAF, Dicer, Fbw7, KAI1, MMP2, p27 and Tip60, were investigated. A training cohort with 250 melanoma patients (145 primary melanomas and 105 metastatic melanomas) and an independent cohort with 92 primary melanoma patients were used for the study. Logistic regression analysis was used to identify discriminate biomarkers of metastatic melanoma from primary melanoma. Kaplan-Meier survival and multivariate Cox proportional hazard regression analysis were performed to assess the significance of the molecular signature in the prognosis of melanoma. Results: In the training cohort, we found the loss expression of KAI1 and p27 to be most significant between metastatic and primary melanoma. The primary melanoma patients with loss expression of both KAI1 and p27 had poor 5-year survival in both training cohort (P = 0.002) and independent cohort (P = 0.03). Multivariate Cox regression analysis showed that the KAI1-/p27- signature was an independent factor for disease- specific survival (P = 0.004). More important, compared to KAI1 and p27 as an individual prognostic marker, the KAI1-/p27- signature is more closely associated with melanoma patient survival: the P value is 0.004 for KAI1-/p27-, and 0.044, 0.181 for KAI1 and p27, respectively. Conclusion: Loss of both KAI1 and p27 defines a subgroup of primary melanoma patients with poor prognosis, and the combination signature is better than individual biomarker. The clinical significance of KAI1 and p27 warrants further assessment in prospective clinical trials. Loss of Tumor Suppressors KAI1 and P27 Identifies a Unique Subgroup of Primary Melanoma Patients with Poor Prognosis

Abstract:Primary melanoma, a highly aggressive malignancy, exhibits heterogeneity in biologic behaviors, clinical characteristics, metastasis potential and mortality. The present study sought to identify the molecular signatures that define a subgroup of primary melanomas with high risks of metastasis and mortality. First, we identified the markers that best differentiated metastatic melanomas from primary melanomas by examining the expression of seven previously reported biomarkers (BRAF, Dicer, Fbw7, KAI1, MMP2, p27 and Tip60) in a training cohort consisting of 145 primary melanomas and 105 metastatic melanomas. KAI1 and p27, both tumor suppressors, emerged as best candidates. Loss of both tumor suppressors occurred in the majority (74.29%) of metastatic melanomas. Further, a subset (metastatic like, or "ML", 33.10%) of primary melanomas also lost these two tumor suppressors. Kaplan-Meier analysis indicated that ML subgroup of primary melanoma patients had much worse 5 year survival compared with other primary melanoma patients (P = 0.002). The result was confirmed in an independent validation cohort with 92 primary melanomas (P = 0.030) and in the combined cohort with 237 melanoma patients (P = 3.00E-4). Additionally, compared to KAI1 and p27 as an individual prognostic marker, the combined signature is more closely associated with melanoma patient survival (P = 0.025, 0.264 and 0.009, respectively). In conclusion, loss of both KAI1 and p27 defines a subgroup of primary melanoma patients with poor prognosis. This molecular signature may help in metastatic melanoma diagnosis and may provide information useful in identifying high-risk primary melanoma patients for more intensive clinical surveillance in the future.
Whole-exome Sequencing Reveals Mutational Profiles of Anorectal and Gynecological Melanoma

Abstract:Mucosal melanoma is a rare and highly malignant type of melanoma. Among the sites that mucosal melanoma arises, anorectal and gynecological melanoma has more aggressive behavior and worse prognosis. There was no effective therapy for mucosal melanoma at present. Only a small number of mucosal melanoma patients which harbor mutations in BRAF or KIT benefit from targeted therapy. So it's an urgent need to identify more actionable mutations in mucosal melanoma. To identify more potential therapeutic targets in mucosal melanoma, 48 samples were collected from 44 patients with anorectal or gynecological melanoma and subjected to whole-exome sequencing. The tumor mutation burden was low with a median of 1.75 mutations per Mb. In chromosomal level, 1q, 6p and 8q of mucosal melanoma were significantly amplified while 9p, 10p, 10q, 16p and 16q were significantly deleted. Muc16 was the most frequently mutated oncogene in our samples(25%). The mutation frequency of KIT(20%) was comparable to the "triple-wild" genes-NRAS(20%), NF1(20%), and BRAF(11%). KMT2D mutation was found in 18.18% patients, which is previously unidentified. MAPK signaling pathway and lysine degradation were the most frequently mutated pathways. Moreover, patients with TP53 mutations tend to have worse clinical outcome (median survival time 19 vs. 50 months, log-rank P = 0.006). 2000 ore mutated genes involved in MAPK signaling pathway were identified, which expand the patients potentially benefit from ample MAPK inhibitors. KMT2D could be a potential therapeutic target. Moreover, TP53 could be a potential prognosis marker for mucosal melanoma.

Abstract:Mucosal melanoma is rare and associated with extremely poor prognosis. Mucosal melanoma has historically been refractory to traditional therapeutic approaches. Recently molecularly based targeted drugs show great success in melanoma. The success of these drug strategies can be partially attributed to the identification of the genetic alterations responsible for the development and progression of metastatic melanoma. This review will focus on genes involved in two major mucosal melanoma-related signaling pathways, the RAS/RAF/mitogen activated protein kinase (MAPK) pathway and the phosphatidylinositol 3-kinases (PI3K)-AKT pathway, and detail the current understanding of their roles in melanoma progression. Additional mutations in key genes, such as KIT, GNAQ and MITF, in mucosal melanoma will also be introduced. Finally, an overview of the current targeted therapy landscape will be provided.

Abstract:Knowledge of key drivers and therapeutic targets in mucosal melanoma is limited due to the paucity of comprehensive mutation data on this rare tumor type. To better understand the genomic landscape of mucosal melanoma, here we describe whole genome sequencing analysis of 67 tumors and validation of driver gene mutations by exome sequencing of 45 tumors. Tumors have a low point mutation burden and high numbers of structural variants, including recurrent structural rearrangements targeting TERT, CDK4 and MDM2. Significantly mutated genes are NRAS, BRAF, NF1, KIT, SF3B1, TP53, SPRED1, ATRX, HLA-A and CHD8. SF3B1 mutations occur more commonly in female genital and anorectal melanomas and CTNNB1 mutations implicate a role for WNT signaling defects in the genesis of some mucosal melanomas. TERT aberrations and ATRX mutations are associated with alterations in telomere length. Mutation profiles of the majority of mucosal melanomas suggest potential susceptibility to CDK4/6 and/or MEK inhibitors.

Abstract:Mucosal melanoma is a rare disease, accounting for approximately 1.4% of all Caucasians’ melanomas and 22% of Asian melanomas, which are commonly found in the nasal cavity or sinuses, anorectum, and vulvovagina. Nearly half of the mucosal melanomas are amelanotic. The most common sites of metastatic disease are the liver and the lungs. The prognosis may be similar regardless of anatomic site of origin. The genetics of mucosal melanoma thatg originates from melanocytes within mucosal epithelia has not been fully explored. Data suggest BRAF may occur in only 3–15% of all cases of mucosal melanoma. Mutations in KIT can be identified in 7–17% of all patients with mucosal melanoma. As to the adjuvant therapy, the only published randomized evidence supporting adjuvant therapy after resection of mucosal melanoma was from Lian et al. They randomized 189 patients with resected mucosal melanoma to observation, 1 year of high-dose IFN-α2b, or thrice-weekly chemotherapy with temozolomide plus cisplatin for six cycles. Adjuvant therapy was associated with a trend toward improved survival and HDI was less effective than temozolomide-based chemotherapy for patients with resected mucosal melanoma in respect to RFS. The preliminary interim analysis of a phase III trial (NCT03435302) of 204 patients with mucosal melanoma comparing temozolomide-based chemotherapy with HDI showed that the chemo was superior to HDI. Several clinical trials on adjuvant therapy of mucosal melanomas are ongoing, including NCT03178123 (a phase III trial of PD-1 inhibitors VS HDI) and NCT03241186 (phase II trial of nivolumab + ipilimumab). The clinical trials for systemic treatment of unresectable or metastatic mucosal melanoma were limited. A pooled analysis of the outcomes of patients with metastatic mucosal melanoma treated in trials either with single-agent nivolumab or the combination of ipilimumab and nivolumab compared outcomes to those of patients with cutaneous melanoma. The ORR for mucosal melanoma receiving nivolumab monotherapy or nivolumab combined with ipilimumab were lower than cutaneous melanoma, 23.3% vs. 40.9%, 37.1% vs. 60.4%. PD-1 monoclonal antibody was less effective in mucosal melanomas maybe because of low PD-L1 expression and tumor mutation burden (TMB). At the 2018 ASCO meeting, the results of phase II study of JS001 (PD-1 inhibitors) were reported. 128 melanoma patients were enrolled. Subgroup analysis showed that patients with mucosal origin were less effective (mucosal 0%). How to increase the efficacy for the mucosal melanoma? As we all know, liver metastases are common in mucosal melanomas and difficult to handle. A phase Ic study of oncolytic virus (OrienX010) injected into liver metastases demonstrated good tolerance and efficacy in Chinese patients. The next step is to combine oncolytic virus injected into liver lesions with PD-1 antibody to improve the efficacy of liver metastasis. On the other hand, TKIs are found to have an influence on microenvironment in tumor and facilitate immunotherapy. We conducted a phase Ib study of JS001 in combination with axitinib in metastatic mucosal melanoma. The results were reported in ASCO 2018, which were encouraging to achieve ORR of 66.7%. Recurrent activating mutations in KIT have been identified in 15% of patients, but KIT inhibitors have not produced durable clinical responses. Patients with mucosal melanoma have a poor prognosis in part due to the lack of targeted therapies. Imatinib may remain the first-choice treatment option for patients with a KIT mutation. Abstract:9576 Background: The incidence of melanoma subtypes differs significantly among ethnicities. Ultraviolet (UV) radiation-driven melanomas are common in Caucasians, while non-cutaneous melanomas including acral and mucosal melanomas are more frequent in Asians. It will be of great interest and clinical relevance to decipher the molecular pathogenesis of different melanoma subtypes. Methods: We retrospectively studied a cohort of 89 Chinese melanoma patients who underwent surgical resection of their primary tumors followed by chemotherapy. Genomic profiling of primary melanomas was performed using next generation sequencing by targeting 422 cancer-relevant genes. The Kaplan-Meier method and logrank test were used for survival analysis, and a cox model was used for multivariate survival analysis. Results: Acral melanomas (54/89, 60%) were the most common subtype of this cohort, while cutaneous and mucosal subtypes accounted for 25% and 15%, respectively. Mutation profiling revealed that BRAF was most frequently mutated in cutaneous melanomas, but aberrant BRAF, RAS, KIT, and NF1 were almost evenly represented in acral and mucosal melanomas; of note, mucosal melanomas had a propensity for concurrent driver mutations. Chromosomal alterations were detected across all subtypes, and chr7p amplification significantly correlated with poor prognosis while independently of melanoma subtypes. Furthermore, acral and mucosal melanomas demonstrated higher rates of focal copy number variations (CNVs) than cutaneous melanomas. The amplification of CDK4/CCND1 and NOTCH2 was observed predominantly in acral melanomas , and RAD51 loss was significantly enriched in mucosal melanomas correlating with poor survival. In addition, the tumor mutation burden (TMB) was significantly lower in acral or mucosal melanomas than in cutaneous melanomas. Conclusions: Our findings revealed distinct patterns of driver mutations and chromosomal alterations in acral and mucosal melanomas in contrast to cutaneous melanoma, and highlighted the association of chromosome 7p amplification and RAD51 deletion with unfavourable survival in melanoma patients treated with standard chemotherapy.

Abstract:e13508 Background: Murine double minute-2 (MDM2) has increasing clinic relevance as preclinical and early clinical evidences have showed that the MDM2 inhibitors are promising alternative treatments for patients with MDM2-amplified (-amp) and TP53-wild-type (-WT) tumors by preventing TP53 protein degradation. However, exploration into genomic landscape of MDM2/TP53 across various solid tumors in Chinese patients is still limited. Methods: Formalin-fixed, paraffin-embedded (FFPE) tumor samples were collected from 10010 Chinese patients with solid tumors and subjected to a clinical-grade next-generation sequencing (NGS)-based 450 gene panel testing carried out by a College of American Pathologists (CAP) accredited and Clinical Laboratory Improvement Amendments (CLIA) certified laboratory. Genomic alterations (GAs) including single base substitutions, short and long insertion/deletions (Indels), copy number variations, gene fusions, and rearrangements were analyzed. Tumor mutational burden (TMB) was measured by a NGS algorithm. Results: Amplification of MDM2 occurred in 3.3% of Chinese patients. MDM2-amp was detected most frequently in soft tissue sarcoma (STS, 11.4 %), melanoma (10.3 %) and osteosarcoma (6.3 %). Notably, MDM2-amp was revealed to be mutually exclusive with TP53-mut in most tumor types other than esophageal carcinoma (93.9 %), head and neck carcinoma (68.4%) and ovarian carcinoma (50 %). Tumors with highest frequencies of MDM2-amp/ TP53-WT were melanoma (100%), osteosarcoma (100%) and urothelial carcinoma (100 %). The most common co-altered genes accompanied with MDM2-amp were FRS2 (84%), CDK4 (38%), EGFR (26%), TP53 (17%) and GLI1 (14%). 82.8% patients who had one or more co-alterations potentially targetable with either FDA-approved or investigational agent. Moreover, TMB of MDM2-amp tumors were significantly lower than that of MDM2-WT and MDM2-mutation (-mut) tumors (median TMB: 3.1 vs. 4.6 vs. 10.8, p < 0.001 respectively). Conclusions: MDM2 amplification was found in 3.3% of 10010 Chinese cancer patients, 83.4% of whom harbored wild-type TP53 that were potentially responsive to MDM2 inhibitors. In addition, 82.8% of the patients with MDM2-amp harbored genomic co-alterations and were potentially therapy targetable. This study provided insights into identifying patients who might potentially benefit from anti-MDM2 therapies.

Abstract:Purpose: PD-1 checkpoint blockade immunotherapy induces long and durable response in patients with advanced melanoma. However, only a subset of patients with melanoma benefit from this approach. The mechanism triggering the innate resistance of anti–PD-1 therapy remains unclear. Experimental Design: Whole-exome sequencing (WES) and RNA sequencing (RNA-Seq) analyses were performed in a training cohort ( n = 31) using baseline tumor biopsies of patients with advanced melanoma treated with the anti–PD-1 antibody. Copy-number variations (CNVs) for the genes CDK4, CCND1 , and CDKN2A were assayed using a TaqMan copy-number assay in a validation cohort ( n = 85). The effect of CDK4/6 inhibitors combined with anti–PD-1 antibody monotherapy was evaluated in PD-1–humanized mouse (C57BL/6-hPD-1) and humanized immune system (HIS) patient-derived xenograft (PDX) models. Results: WES revealed several significant gene copy-number gains in the patients of no clinical benefit cohort, such as 12q14.1 loci, which harbor CDK4 . The association between CDK4 gain and innate resistance to anti–PD-1 therapy was validated in 85 patients with melanoma ( P < 0.05). RNA-Seq analysis of CDK4 -normal cell lines and CDK4 -normal tumors showed altered transcriptional output in TNFα signaling via NF-B, inflammatory response, and IFN response gene set. In addition, CDK4/6 inhibitor (palbociclib) treatment increased PD-L1 protein levels and enhanced efficacy ( P < 0.05) in the C57BL/6-hPD-1 melanoma cell and the HIS PDX model. Conclusions: In summary, we discovered that genetic aberrations in the CDK4 pathway are associated with innate resistance to anti–PD-1 therapy in patients with advanced melanoma. Moreover, our study provides a strong rationale for combining CDK4/6 inhibitors with anti–PD-1 antibody for the treatment of advanced melanomas.

Prevalent Aberrations of CDK4 Pathway in Acral Melanoma and Implications for Targeted Therapy.

Abstract:9512 Background: Therapeutics for metastatic acral melanoma have not been successfully established. This study aims to investigate gene aberrations of CDK4 pathway in acral melanoma and evaluate the potential application of CDK4 inhibitors in targeted therapy of acral melanoma. Methods: A total of 428 cases of primary acral melanoma samples were included. The copy number variations of CDK4 pathway-related genes, including CDK4, CCND1 and p16INK4a, were examined by QuantiGenePlex DNA Assay. The aberrations of these genes were further correlated with the clinicopathological features of patients. And the sensitivity of established cell lines and patient-derived xenograft (PDX) models containing typical CDK4 aberrations to CDK4 inhibitors was also analyzed. Results: Among the 428 samples, 173 cases (62.0%), 117 cases (27.3%) and 265 cases (62.0%) showed CDK4 amplification,CCND1 amplification and p16INK4a loss respectively. Moreover, 36.4% of acral melanomas contained more than two simultaneous aberrations of these three genes, and 9.1% of the samples contained three aberrations. The frequency of acral melanomas that contain at least one aberrations in CDK4, CCND1 and p16INK4a was about 84.1%. The median survival time for acral melanoma patients with simultaneous CDK4 amplification and p16INK4a losswas significantly shorter than that for patients without CDK4 amplification and p16INK4a loss(P = 0.003). In acral cells with concurrent CDK4 amplification and p16INK4a loss, concurrent CCND1 amplification and p16INK4a loss, AT7519 and PD033299l dramatically inhibited the cell proliferation. AT7519 and PD033299l also abrogated tumor growth in a PDX model with concurrent CDK4 amplification and p16INK4a loss. Conclusions: It is the first study indicates that CDK4 pathway aberration is predominant in acral melanoma (more than 80%). The acral melanoma cells and PDX model containing CDK4 pathway aberration are sensitive to CDK4 inhibitors, thus providing key evidence for the significance of CDK4 inhibitors for acral melanoma and facilitating the establishment of strategy for targeted therapy to acral melanoma.
Frequency of CDK2 , CCNE1 Copy Number Variation in Acral Melanoma and Implications for CDK Inhibitor in Targeted Therapy

Abstract:Abstract Background: Acral melanoma have a high frequency of cell cycle-related gene copy number variation. However, the status and clinical significance of CNVs of CDK 2 and CCNE1 have not been fully elucidated. Methods: A total of 490 acral melanoma samples were examined for CNVs of CDK 2 and CCNE1 using QuantiGenePlex DNA Assay. Correlations of CDK2 and CCNE1 CNVs to clinicopathologic features and prognosis of acral melanoma were evaluated.The sensitivity of cell lines and cell-derived xenograft (CDX) containing CCNE1 CNVs to CDK inhibitor AT7519,Dinaciclib and proteasome inhibitor Bortezomib were also analyzed. Results: Among the 490 samples,140 cases, 139 cases and 39 cases respectively showed CDK2 gain (28.5%), CCNE1 gain (28.3%) and CDK2 gain plus CCNE1 gain (8.0%).The median progression-free survival (PFS) time for acral patients with CCNE1 gain was significantly shorter than that for patients without CCNE1 gain (17.0 versus 27.0 months; P =0.002). Furthermore, CCNE1 gain was an independent prognostic factor for patients receiving chemotherapy. The pan-CDK inhibitor AT7519 could inhibit the cell proliferation, induce apoptosis and cause cell cycle arrest in G2 phase of acral melanoma cells and inhibit the tumor growth of CDX with CCNE1 gain. Dinaciclib and Bortezomib showed CCNE1 copy number independent inhibitory effects on the proliferation of melanoma cells. Conclusions: CDK2 and CCNE1 copy number variations were frequent in acral melanoma and CCNE1 gain may be a useful biomarker to predict the outcome of receiving chemotherapy in patients with acral melanoma. Abstract:The incidence of melanoma is rising globally including China. Comparing to Caucasians, the incidence of non-cutaneous melanomas is significantly higher in Chinese. Herein, we performed genomic profiling of 89 Chinese surgically resected primary melanomas, including acral (n = 54), cutaneous (n = 22), and mucosal (n = 13), by hybrid capture-based next-generation sequencing. We show that mucosal melanomas tended to harbor more pathogenic mutations than other types of melanoma, though the biological significance of this finding remains uncertain. Chromosomal arm-level alterations including 6q, 9p, and 10p/q loss were highly recurrent in all subtypes, but mucosal melanoma was significantly associated with increased genomic instability. Importantly, 7p gain significantly correlated with unfavorable clinical outcomes in non-cutaneous melanomas, representing an intriguing prognostic biomarker of those subtypes. Furthermore, focal amplification of 4q12 (KIT, KDR, and PDGFR alpha) and RAD51 deletion were more abundant in mucosal melanoma, while NOTCH2 amplification was enriched in acral melanoma. Additionally, cutaneous melanomas had higher mutation load than acral melanomas, while mucosal melanomas did not differ from other subtypes in mutation burden. Together, our data revealed important features of acral and mucosal melanomas in Chinese including distinctive driver mutation pattern and increased genomic instability. These findings highlight the possibilities of combination therapies in the clinical management of melanoma.

Abstract:Background Recent high-throughput sequencing studies have revealed frequent CDK4 and TERT amplification in mucosal melanoma, suggesting that they are potential therapeutic targets. In this study, we investigated the statuses of CDK4 and TERT in head and neck mucosal melanoma (HNMM) with the aim of providing preclinical data to support future clinical trials. Methods In total, 29 HNMM samples were collected, including 16 oral mucosal melanoma (OMM) samples and 13 nasal cavity/sinuses melanoma (SNMM) samples. Fluorescence in situ hybridization was used to analyze CDK4 and TERT amplification, and immunohistochemistry was used to analyze CDK4 and TERT protein expression patterns. CDK4 expression was knocked down in the ME cells (an OMM cell line), and changes in cell cycle were analyzed. Cell viability assays were performed to determine the sensitivity of ME to abemaciclib (a CDK4 inhibitor) combined with dacarbazine (an anti-melanoma chemotherapy drug). Results We detected five samples exhibited CDK4 amplifications and nine samples exhibited TERT amplifications in our HNMM series, and found that CDK4 amplification tended to occur in combination with TERT amplification. Amplifications of CDK4 and TERT were more common in OMM than in SNMM. Amplifications of CDK4 and TERT were associated with greater CDK4 and TERT protein expression levels. CDK4 knockdown led to delayed G1/S phase transition in ME cells. Furthermore, ME cells were sensitive to abemaciclib (IC50 = 5.23 nM). Abemaciclib and dacarbazine synergistically inhibited ME cells' viability. Conclusion We confirmed high frequencies of CDK4 and TERT amplification in OMM. Combined therapy with a CDK4/6 inhibitor and anti-melanoma chemotherapeutic agents will be a reasonable strategy for future clinical trials concerning unresectable or metastatic OMM.

Abstract:Mucosal melanoma (MM) is a rare melanoma subtype that originates from melanocytes within sun-protected mucous membranes. Compared with cutaneous melanoma (CM), MM has worse prognosis and lacks effective treatment options. Moreover, the endogenous or exogenous risk factors that influence mucosal melanocyte transformation, as well as the identity of MM precursor lesions, are ambiguous. Consequently, there remains a lack of molecular markers that can be used for early diagnosis, and therefore better management, of MM. In this review, we first summarize the main functions of mucosal melanocytes. Then, using oral mucosal melanoma (OMM) as a model, we discuss the distinct pathologic stages from benign mucosal melanocytes to metastatic MM, mapping the possible evolutionary trajectories that correspond to MM initiation and progression. We highlight key areas of ambiguity during the genetic evolution of MM from its benign lesions, and the resolution of which could aid in the discovery of new biomarkers for MM detection and diagnosis. We outline the key pathways that are altered in MM, including the MAPK pathway, the PI3K/AKT pathway, cell cycle regulation, telomere maintenance, and the RNA maturation process, and discuss targeted therapy strategies for MM currently in use or under investigation.

Abstract:<p>Melanoma is the deadliest type of cancer that affects the largest organ of our body, the skin. In recent years, there is an increase in the incidence and aggressiveness of melanomas. The number of treatment options has grown considerably in the past few years, leading to significant improvements in both overall and progression-free survival. One of the attractive candidates in this wave of treatment options is a cell cycle controller: cyclin-dependent kinases (CDK) 4/6 inhibitors. CDK4/6, a class of serine/threonine kinases expressed in most cell types, controls the first gap phase (G1 to S) of the cell cycle, indicating its vital importance in both normal cellular processes as well as tumorigenesis. Up to 90% of melanoma patients have genomic mutations affecting various parts of CDK4/6 pathway. Noticeably, with the help of next-generation sequencing technology, mutations with high frequency in the CDK4 pathway were also identified in relatively rare subtypes of melanoma including acral melanoma and mucosal melanoma. Therefore, CDK4/6 inhibitors have emerged as powerful and promising anticancer therapies, especially in combination treatment with immunotherapies or other targeted therapies. In this review, we will provide an overview of current scientific knowledge regarding the oncogenic properties of CDK4/6 in melanomas, we mainly discuss the latest genomic and preclinical findings of CDK4 signaling in melanoma, the progress of CDK4 inhibition as combined with other therapies for overcoming resistance and summarize recent advances from clinical trials as well as ongoing studies which gives us a better scope into the effectiveness of CDK4/6 therapy in treating malignant melanomas.</p>

Clinical Presentation, Systemic Therapy and Prognosis of Mucosal Melanoma, A Study of 463 Consecutive Cases

Abstract:e20036 Background: Mucosal melanoma (MM) is the second common subtype in Asians with a percentage of 22-25%; but there have been few epidemiologic studies. So we performed a retrospective analysis of MM about its clinical presentation, gene mutation, systemic therapy (tx) and prognosis. Methods: Using institutional databases, we identified patients (pts) with MM who were treated in Chinese melanoma centers between 2006-2014 with basic demographics and clinical outcomes. Results: 463 MM pts were included. Median age at diagnosis 56 years (range, 17-86); 61% female; primary site: 149 anal-rectal, 125 genitourinary, 174 head/neck, 15 others. Mutation (mut) status: 14.1% (45/319) KIT mut; 9.7% (31/319) BRAF mut; 10.3% (33/319) NRAS mut. Stage I-II/ III/IV at referring – 36%/17%/47%, M1a/b/c – 17%/18%/65%. The median overall survival (mOS) was 28.0 mo (95% CI 25.1-30.9 mo). Multivariate analysis showed that clinical stage and LDH level were significant prognostic factors for OS. The application of adjuvant therapy and LDH level were significant prognostic factors for disease free survival. For 293 pts who developed stage IV, mOS was 14.0 mo, for M1a, M1b, M1c, 20.2, 16.0, 11.1 mo respectively (p = .004). In 1st-line setting, the objective response rate (RR), disease control rate (DCR) and median progress free time (PFS) was 11.8%, 54.8%, and 3.1 mo (95% CI 2.6-3.8 mo), most of the pts using dacarbazine/temozolomide, cisplatin plus rhEndostatin had RR, DCR and PFS, 12.3%, 64.6%, and 4.5 mo respectively. In 2nd-line setting, paclitaxel plus carboplatin was common regimen, RR, DCR and PFS was 8.3% 61.1% and 3.3 mo (95% CI 2.0-4.4 mo). The mPFS of 3rd-line tx (including Abraxane, investigational targeted drugs, etc) was 1.3 mo without complete regression and partial regression. 8/20 pts harboring c-kit mutation used imatinib, 3/8 pts had SD for a median PFS of 6.7 mo. 5/15 pts harboring BRAF mutation used vemurafenib, 3/5 pts got response, mPFS cannot be evaluated. Conclusions: The prognosis for MM is still poor, clinical stage has its significance for OS. For pts with stage IV, the efficacy of systemic tx includes chemotherapy and targeted therapy is modest. Further investigation into the biology and treatment of MM is needed.

Abstract:9588 Background: PD-1 checkpoint blockade immunotherapy induces long durable responses in patients with advanced melanoma. However, only a minor subset of acral melanoma patients could gain benefit from it. In this study, we aim to explore factors that may influence innate sensitivity or resistance to anti-PD-1 therapy and screen drugable targets with potential to be combined with anti-PD-1 antibody monotherapy in acral melanoma. Methods: Whole exome sequencing (WES) and RNAseq analyses were performed in 13 biopsies from baseline lesions of advanced melanoma patients who were treated with anti-PD-1 antibody (JS001, Shanghai Junshi Biosciences, China). The copy number variations (CNV) of candidate genes were identified by TaqMan Copy Number Assay in an independent cohort. The effect of CDK4/6 inhibitors combined with anti-PD-1 antibody monotherapy was evaluated by PD-1 humanized mouse (C57BL/6-hPD-1) and patient-derived xenograft (PDX) models. Results: WES revealed parallel CNV amplifications in both the CR+PR and PD cohorts, such as 4q22.1, 10q23.32, 15q26.3, 22q13.1. However, several significant CNV amplifications notably only occurred in the PD cohort, such as 1q23.1, 5p15.33, 7p22.3, 11q13.3, 12q14.1, which harbor Cdk4, Ccnd1, CARD11 and mTOR genes. The association between Cdk4 or/and Ccnd1 amplification with innate resistance to anti-PD-1 therapy was validated in 46 acral melanoma patients (P < 0.01). The CDK4/6 inhibitor treatment increased PD-L1 protein levels in primary acral melanoma cell line and PDX model. Moreover, CDK4/6 inhibitor augments the response to PD-1 blockade in PD-1 humanized mouse (C57BL/6-hPD-1) and PDX models. Tumor growth curves for each treatment group demonstrate the improved efficacy of combining PD-1 blockade with the CDK4/6 inhibitor (P < 0.05). Conclusions: In summary, we discovered the genetic aberrations in the CDK4 pathways were associated with innate resistance to anti-PD-1 therapy in patients with advanced acral melanoma. Moreover, our study provides evidence for the testing of CDK4/6 inhibitors combined with anti-PD-1 antibody monotherapy in acral melanoma.
Analysis of TSC1 Mutation Spectrum in Mucosal Melanoma

Abstract:Purpose Mucosal melanoma is a relatively rare subtype of melanoma for which no clearly established therapeutic strategy exists. The genes of the mTOR signalling pathway have drawn great attention as key targets for cancer treatment, including melanoma. In this study, we aimed to investigate the mutation status of the upstream mTOR regulator TSC1 and evaluated its correlation with the clinicopathological features of mucosal melanoma. Methods We collected 91 mucosal melanoma samples for detecting TSC1 mutations. All the coding exons of TSC1 were amplified by PCR and subjected to Sanger sequencing. Expression level of TSC1 encoding protein (hamartin) was detected by immunohistochemistry. The activation of mTOR pathway was determined by evaluating the phosphorylation status of S6RP and 4E-BP1. Results The overall mutation frequency of TSC1 was found to be 17.6% (16/91 patients). TSC1 mutations were more inclined to occur in advanced mucosal melanoma (stages III and IV). In the 16 patients with TSC1 mutations, 14 different mutations were detected, affecting 11 different exons. TSC1 mutations were correlated with upregulation of S6RP phosphorylation but were unrelated to 4E-BP1 phosphorylation or hamartin expression. Mucosal melanoma patients with TSC1 mutations had a worse outcome than patients without TSC1 mutations (24.0 versus 34.0 months, P = 0.007). Conclusions Our findings suggest that TSC1 mutations are frequent in mucosal melanoma. TSC1 mutations can activate the mTOR pathway through phospho-S6RP and might be a poor prognostic predictor of mucosal melanoma. Our data implicate the potential significance of TSC1 mutations for effective and specific drug therapy for mucosal melanoma.

Abstract:BACKGROUND: Mucosal melanoma has characteristically distinct genetic features and typically poor prognosis. The lack of representative mucosal melanoma models, especially cell lines, has hindered translational research on this melanoma subtype. In this study, we aimed to establish and provide the biological properties, genomic features and the pharmacological profiles of a mucosal melanoma cell line that would contribute to the understanding and treatment optimization of molecularly-defined mucosal melanoma subtype.METHODS: The sample was collected from a 67-year-old mucosal melanoma patient and processed into pieces for the establishment of cell line and patient-derived xenograft (PDX) model. The proliferation and tumorigenic property of cancer cells from different passages were evaluated, and whole-genome sequencing (WGS) was performed on the original tumor, PDX, established cell line, and the matched blood to confirm the establishment and define the genomic features of this cell line. AmpliconArchitect was conducted to depict the architecture of amplified regions detected by WGS. High-throughput drug screening (HTDS) assay including a total of 103 therapeutic agents was implemented on the established cell line, and selected candidate agents were validated in the corresponding PDX model.RESULTS: A mucosal melanoma cell line, MM9H-1, was established which exhibited robust proliferation and tumorigenicity after more than 100 serial passages. Genomic analysis of MM9H-1, corresponding PDX, and the original tumor showed genetic fidelity across genomes, and MM9H-1 was defined as a triple wild-type (TWT) melanoma subtype lacking well-characterized "driver mutations". Instead, the amplification of several oncogenes, telomerase reverse transcriptase (TERT), v-Raf murine sarcoma viral oncogene homolog B1 (BRAF), melanocyte Inducing transcription factor (MITF) and INO80 complex ATPase subunit (INO80), via large-scale genomic rearrangement potentially contributed to oncogenesis of MM9H-1. Moreover, HTDS identified proteasome inhibitors, especially bortezomib, as promising therapeutic candidates for MM9H-1, which was verified in the corresponding PDX model in vivo.CONCLUSIONS: We established and characterized a new mucosal melanoma cell line, MM9H-1, and defined this cell line as a TWT melanoma subtype lacking well-characterized "driver mutations". The MM9H-1 cell line could be adopted as a unique model for the preclinical investigation of mucosal melanoma.
