Abstract:Background: E-cigarettes are increasingly used by smokers and ex-smokers, often to support smoking cessation, but also among those who have never regularly smoked. The aim of our study is to estimate time trends in vaping prevalence among adults who have never regularly smoked and describe the profile of adult never-regular-smokers who vape. Methods: In this nationally representative, monthly cross-sectional survey in England, 153 073 participants (aged ≥18 years) were recruited from July, 2016, to April, 2024. 94 107 (61·5%) of the 153 073 participants had never regularly smoked. Logistic regression estimated associations between survey wave and current vaping, overall, and by sociodemographic characteristics and alcohol consumption. Findings: Vaping prevalence among never-regular-smokers was relatively stable up to 2021, at an average of 0·5% (95% CI 0·5-0·6) between July, 2016, and December, 2020; it then increased rapidly, reaching 3·5% (2·8-4·4) in April, 2024. This increase was largely driven by young adults (19·0% [13·9-25·4] among individuals aged 18 years) and those with higher alcohol consumption levels (22·1% [11·7-37·8%] among the heaviest drinkers). By contrast, trends among all adults (including current and former smokers) suggested the rise in vaping prevalence plateaued among all ages by early 2023. In 2023-24, 55·6% (48·2-62·8) of never-regular-smokers who vaped reported vaping daily; 81·7% (75·4-86·7) had been vaping for 6 months or more. The most used devices in 2023-24 were disposables (50·2% [42·9-57·5]) and the most commonly used e-liquids contained 20 mg/mL or more nicotine (44·6% [37·4-52·0]). Interpretation: The number of adults in England who vape but have never regularly smoked rose rapidly between 2021 and 2024, particularly in younger age groups and most of these individuals reported vaping regularly over a sustained period. The public health impacts of this finding will depend on what these people would otherwise be doing: it is likely that some might have smoked if vaping were not an available option (exposing them to more harm), whereas others might not have smoked or vaped. Abstract:INTRODUCTION: Although e-cigarettes can be an effective form of nicotine substitution for adults attempting to quit smoking, their use among children and young people is a concern. Accurate data about this are needed to inform debates over policy and regulation in the UK and elsewhere.METHODS: Using data from an online survey of 2613 youth aged 11-18 years, conducted by the market research company YouGov in March 2022, we present prevalence estimates of e-cigarette and tobacco use. We use logistic regression models to assess differences in e-cigarette use, tobacco use and use of disposable e-cigarettes across a range of covariates including age, sex, tobacco smoking status, social class, and country.RESULTS: Among the 18.0% of those surveyed who reported ever having smoked a cigarette, 83.9% were not regular (at least once per week) smokers and 16.1% were (15.1% and 2.9% of the total sample, respectively). Among the 19.2% of those surveyed who had ever used an e-cigarette, 79.2% were not regular users, while 20.8% were (15.2% and 4.0% of the total sample, respectively). Regular e-cigarette use was more common than regular tobacco smoking (4.0% vs 2.9%). E-cigarette use was more common among those who also smoked tobacco, with 9.0% of never e-cigarette users ever smoking tobacco, compared with 89.4% of regular e-cigarette users. Both smoking and e-cigarette use were associated with increasing age and use by others within the home, but not with social class. Use of disposable e-cigarettes was reported by 53.8% of those who have ever used an e-cigarette, and more common among females than males.CONCLUSIONS: Regular e-cigarette use is now more common than smoking in children and youth, though the majority of this is among those who have also smoked tobacco. Measures to reduce the appeal of both e-cigarettes and tobacco to children and young people are warranted.

Young people's use of disposable vapes: A qualitative study

Abstract:Background and aims: Youth use of disposable vapes has increased markedly in the United Kingdom in recent years, yet little is known about the motivations, experiences and perceptions of young people themselves. This study aimed to explore young people's experiences and use of disposable vapes. Methods: This was a qualitative study recruiting young people reporting regularly vaping disposables, collecting data via dyad guided, individual and group interviews. Data analysis was theoretically informed by the Social Ecological Model. Inductive and deductive coding approaches were used, with resolution of coded interpretations by consensus. Results: Twenty-nine young people aged 16-20 years participated in qualitative interviews. At the individual level, participants discussed how characteristics of disposable vapes were important to them-particularly price, accessibility and the attractive designs, colours, names and flavours. Young people frequently engaged in both vaping and tobacco smoking, seeing the behaviours as interchangeable dependent on context, and having inaccurate relative harm perceptions of vaping compared with smoking. Experimentation was widespread and many used vapes as a way of managing stress and anxiety. Vaping was positioned as a social behaviour, common among peers. Parental influence on vaping behaviour was minimal, although vaping initiation could be influenced by family vaping norms. Culturally, vaping was a widespread normalized behaviour. Young people were aware of media reports and potential harms, but were less aware of smoking related harms as a consequence. Conclusions: Disposable vapes appear to be attractive and accessible to young people in the United Kingdom. Vaping is normalized in this population, despite being seen as potentially damaging to health, and vaping and smoking are engaged in interchangeably. Underage sales of vapes are reportedly widespread. Strict regulation, such as banning products or increasing prices, may prompt UK youth to switch from vaping to smoking.
Trends and patterns of dual use of combustible tobacco and e-cigarettes among adults in England: a population study, 2016-2024

Abstract:Background/Aims: E-cigarettes are frequently used by people who smoke. This study examined how the prevalence and patterns of smoking and vaping ('dual use') in England have changed as the vaping market has rapidly evolved. Design: Representative monthly cross-sectional survey, July 2016 to April 2024. Setting: England. Participants: 128,588 adults (≥18y). Measurements: Logistic regression estimated associations between survey wave and dual use. Descriptive statistics were used to analyse patterns of smoking and vaping, overall and by sociodemographic, smoking, and vaping characteristics and harm perceptions of e-cigarettes vs. cigarettes. Results: Across the period, the overall prevalence of dual use increased non-linearly from 3.5% to 5.3% of adults (prevalence ratio [PR]=1.49 [1.25-1.76]). Among adults who smoked, the proportion who also vaped was relatively stable up to mid-2021, at an average of 18.6% between July-2016 and May-2021, then increased rapidly to 34.2% by April 2024 (PR=1.76 [1.48-2.09]). This increase was greatest at younger ages (e.g., from 19.6% to 59.4% among 18-24-year-olds; PR=3.04 [2.28-4.23]). The most common pattern of dual use across the period was daily cigarette smoking with daily vaping (49.0% [47.3-50.8%]). Over time, the proportion of dual users reporting daily cigarette smoking with non-daily vaping decreased (from 35.2% to 15.0%; PR=0.43 [0.29-0.63]), offset primarily by an increase in the proportion reporting non-daily cigarette smoking with daily vaping (from 7.6% to 21.5%; PR=2.84 [1.71-4.72]). Daily cigarette smoking with daily vaping was more common (and non-daily cigarette smoking with daily vaping less common) among dual users who were older, less advantaged, mainly smoked hand-rolled cigarettes, had stronger urges to smoke, and had been vaping for ≤6 months. Daily vaping was more common among dual users who thought e-cigarettes were less/equally harmful as cigarettes, or were unsure. Conclusions: The proportion of smokers in England who vape has increased rapidly since 2021, which was when disposable e-cigarettes started to become popular. Since 2016, patterns of dual use have shifted away from more frequent smoking towards more frequent vaping. This may be the result of increasing prevalence of dual use among younger adults, who are more likely than older dual users to smoke non-daily and vape daily.

While overall smoking rates declined between 2013 and 2023, there appears to have been a rise in the proportion of middle class women under the age of 45 who smoke in England. The findings come from the Smoking Toolkit study in which a different representative sample of 1700 adults in England are interviewed each month.1There goes that theory up in smokeIndeed. The study, funded by Cancer Research UK and published in BMC Medicine, found that the proportion of women aged 18 to 45 from higher socioeconomic backgrounds who smoke rose from 12% (95% confidence interval 10.2% to 13.5%) to 15% (95% CI 13.4% to 16.6%) over the decade. Trends in long term vaping among adults in England, 2013-23: population based study

Abstract:Abstract Objective To examine trends in long term (>6 months) vaping among adults in England. Design Population based study. Setting England. Participants 179 725 adults (≥18 years) surveyed between October 2013 and October 2023. Main outcome measures Time trends in prevalence of long term vaping using logistic regression, overall and by vaping frequency (daily or non-daily), and main type of device used (disposable, refillable, or pod). Results The proportion of adults reporting long term vaping increased non-linearly, from 1.3% (95% confidence interval 1.1% to 1.5%) in October 2013 to 10.0% (9.2% to 10.9%) in October 2023, with a particularly pronounced rise from 2021. This rise included an increase in long term daily vaping, from 0.6% (0.5% to 0.8%) to 6.7% (6.0% to 7.4%). Absolute increases in long term vaping were larger among people with a history of regular smoking (current smokers: 4.8% (4.0% to 5.8%) to 23.1% (20.4% to 25.9%); recent former smokers: 5.7% (3.4% to 9.2%) to 36.1% (27.6% to 45.4%); long term former smokers: 1.4% (1.0% to 1.9%) to 16.2% (14.2% to 18.4%)), but an increase also occurred among people who had never regularly smoked (0.1% (0.0% to 0.2%) to 3.0% (2.3% to 3.8%)). Growth was also more pronounced in young adults (eg, reaching 22.7% (19.2% to 26.5%) of 18 year olds v 4.3% (3.6% to 5.2%) of 65 year olds), including among those who had never regularly smoked (reaching 16.1% (11.1% to 22.7%) of 18 year olds v 0.3% (0.1% to 0.6%) of 65 year olds). Between October 2013 and March 2021, most long term vapers mainly or exclusively used refillable electronic cigarettes (2.5% to 3.3% of adults) and few (0.1% of adults) used disposable devices. However, prevalence of long term vaping using disposable devices subsequently rose rapidly, and by October 2023 similar proportions of adults mainly or exclusively used disposable and refillable devices (4.9% (4.2% to 5.7%) and 4.6% (4.0% to 5.3%), respectively). Conclusions The prevalence of long term vaping increased substantially among adults in England during 2013-23. Much of this increase occurred from 2021, coinciding with the rise in popularity of disposable e-cigarettes. Half of long term vapers now mainly or exclusively use disposable devices. The growth was concentrated among people with a history of regular smoking, but an increase also occurred among people who never regularly smoked, especially young adults.

Trends in where people buy their vaping products and differences by user and device characteristics: A population study in England, 2016-23

Abstract:Aims: To explore where people in England typically buy e-cigarettes/vaping products, how their characteristics and the types of products purchased differ according to the source of purchase and whether this is changing over time. Design: This was a nationally representative monthly cross-sectional survey. Setting: The study was conducted in England, 2016-23. Participants: A total of 6507 adults (≥ 18 years) who reported current vaping. Measurements: Participants were asked where they usually buy their vaping products: vape shops, supermarkets/convenience stores, on-line or other sources ('other' or cheap from friends). Findings: Up to 2020, vape shops were the most common source of purchase (mean monthly proportion = 43.3%), followed by supermarkets/convenience stores (29.7%) and on-line retailers (20.5%). In 2020, these purchasing patterns shifted: on-line purchasing increased and vape-shop purchasing fell. The rise in on-line purchasing was short-lived, peaking at 33.6% [95% confidence interval (CI) = 31.0-36.5%] in July 2021, and soon returned to baseline levels. However, the fall in vape-shop purchasing persisted, remaining below 31% from July 2021 onwards, displaced by a rapid rise in purchasing from supermarkets and convenience stores from 31.6% (95% CI = 29.6-33.7%) in January 2021 to 48.5% (95% CI = 42.9-54.8%) by April 2023. This rise was most pronounced among younger adults (aged 18-35 years; Pinteraction < 0.001) and those mainly using disposable devices (Pinteraction = 0.013). Purchasing from other sources was relatively rare, and declined from 9.1% (95% CI = 6.6-12.7%) in July 2016 to 4.3% (95% CI = 2.6-7.1%) in April 2023. Conclusions: Supermarkets and convenience stores have recently overtaken vape shops to become the most popular places to buy vaping products in England. This change appears to have been driven by the rising popularity of new disposable e-cigarettes among younger adults, who tend to buy these products from supermarkets/convenience stores.
Changes in vaping trends since the announcement of an impending ban on disposable vapes: a population study in Great Britain

Abstract:Background/Aim: There has been a rapid rise in vaping prevalence among youth and young adults in Great Britain since disposable vapes started to become popular in 2021. In January 2024, the government announced plans to introduce a number of new vaping policies, including a ban on disposable vapes, to tackle youth vaping. This study examined whether trends in current vaping and use of disposable vapes have changed since this announcement. Design, setting, and participants: Segmented regression analysis of data collected monthly between January 2022 and November 2024 as part of the Smoking Toolkit Study; a representative household survey in Great Britain. We ran generalised additive models using data from all participants aged 16+ (n=83,764; 'adults') and from a subset aged 16-24 (n=8,846; 'young adults'). Main outcome measures: Changes in trends in (i) the prevalence of current vaping and (ii) the proportion of vapers mainly using disposable devices. Results: Before January 2024, vaping prevalence was increasing by 24.5% per year (RRtrend=1.245 [95%CI 1.193-1.299]) and use of disposable vapes was increasing by 18.0% per year (RRtrend=1.180 [95%CI 1.106-1.258]). These trends changed after the new policy measures were announced (RR for change in trend=0.718 [0.623-0.827] and 0.558 [0.460-0.677], respectively). Instead of increasing, there was an uncertain decrease in vaping prevalence from 13.7% [12.7-14.7%] in January 2024 to 12.4% [11.4-13.6%] in November 2024) and a substantial decline in the proportion of vapers mainly using disposables from 43.7% [40.3-47.3%] to 30.8% [27.6-34.4%]. Similar changes were observed among young adults (vaping prevalence: RR for change in trend=0.647 [0.495-0.845]; use of disposable vapes: RR for change in trend=0.526 [0.400-0.693]), with an uncertain decline in vaping prevalence from 27.7% [24.5-31.3%] to 23.6% [20.2-27.6%] between January and November 2024 and the proportion of vapers mainly using disposables falling from 63.4% [59.0-68.2%] to 37.8% [32.3-44.2%]. Conclusions: Following the announcement of an impending ban on disposable vapes and other potential vaping policies, recent increases in vaping prevalence in Great Britain stalled - or may have even reversed - including among young adults. In addition, there was a shift away from using disposable vapes, with people increasingly opting to use devices that can be refilled and recharged.
Prevalence of vaping and smoking among adolescents in Canada, England, and the United States: repeat national cross sectional surveys

Abstract:Abstract Objective To examine differences in vaping and smoking prevalence among adolescents in Canada, England, and the United States. Design Repeat cross sectional surveys. Setting Online surveys in Canada, England, and the US. Participants National samples of 16 to 19 year olds in 2017 and 2018, recruited from commercial panels in Canada (n=7891), England (n=7897), and the US (n=8140). Main outcome measures Prevalence of vaping and smoking was assessed for use ever, in the past 30 days, in the past week, and on 15 days or more in the past month. Use of JUUL (a nicotine salt based electronic cigarette with high nicotine concentration) and usual vaping brands were also assessed. Logistic regression models examined differences in vaping and smoking between countries and over time. Results The prevalence of vaping in the past 30 days, in the past week, and on 15 days or more in the past month increased in Canada and the US between 2017 and 2018 (P<0.001 for all), including among non-smokers and experimental smokers, with no changes in England. Smoking prevalence increased in Canada (P<0.001 for all measures), with modest increases in England, and no changes in the US. The percentage of ever vapers who reported more frequent vaping increased in Canada and the US (P<0.01 for all), but not in England. The use of JUUL increased in all countries, particularly the US and Canada—for example, the proportion of current vapers in the US citing JUUL as their usual brand increased threefold between 2017 and 2018. Conclusions Between 2017 and 2018, among 16 to 19 year olds the prevalence of vaping increased in Canada and the US, as did smoking in Canada, with little change in England. The rapidly evolving vaping market and emergence of nicotine salt based products warrant close monitoring.

Abstract:Background and Objectives: In 2022, the Centers for Disease Control and Prevention (CDC) discovered that 2.55 million middle and high school students were using electronic cigarettes (e-cigarettes) in the US alone. E-cigarettes allow people to inhale a nicotine solution (e-liquid) into their bodies. While e-cigarettes are often advertised as a way to help people quit traditional tobacco products, the World Health Organization (WHO) has stated that there is no proof of e-cigarettes being effective at reducing an individual's tobacco use. The objective of this study is to determine why adolescents start smoking e-cigarettes. Materials and Methods: For the following study, the National Youth Tobacco Survey (NYTS) 2021 was used. This is a nationally representative cross-sectional survey that includes middle and high school students. The data were analyzed using SAS v.9.4. The odds ratio for smoking e-cigarettes was evaluated for both sex and race via a logistic regression. Results: It was discovered that the percentage of only e-cigarette smokers (12.10%) was 5.5 times the percentage of only traditional smokers (2.19%). Additionally, the median age at which adolescents started smoking e-cigarettes was fifteen years with an IQR (Inter Quartile Range) of two. We used a logistic regression to show that biological sex and race were significant predictors of e-cigarette usage. Conclusions: In general, we saw that adolescents were mostly starting to smoke e-cigarettes because their friends were doing it, out of curiosity, they were depressed or anxious, and/or to get a "high", implying that adolescents mostly started smoking in order to fit in and/or to numb themselves. Interestingly, reasons like cost, using them to quit smoking, seeing them in the media, and ease of attainment were ranked the lowest when it came to smoking e-cigarettes. However, their lower costs naturally lead to increased accessibility compared to traditional cigarettes, making them easier to reach the hands of teenagers. Overall, in this paper, we aim to identify if adolescents are choosing e-cigarettes as their first foray into tobacco products and why.
Trends in non-daily cigarette smoking in England, 2006-2024

Abstract:Background: Cigarette smoking is incredibly harmful, even for people who do not smoke every day. This study aimed to estimate trends in non-daily smoking in England between 2006 and 2024, how these differed across population subgroups, and to explore changes in the profile of non-daily smokers in terms of their sociodemographic and smoking characteristics and vaping and alcohol consumption. Methods: Data were collected monthly between November-2006 and April-2024 as part of a nationally-representative, repeat cross-sectional survey of adults (≥18y; n=353,711). We used logistic regression to estimate associations between survey wave and non-daily smoking and used descriptive statistics to characterise the profile of non-daily smokers across 3-year periods. Results: The proportion of cigarette smokers who smoked non-daily was relatively stable between November-2006 and November-2013, at an average of 10.5% [10.1-10.9%], then increased to 27.2% [26.0-28.4%] (4.0% [3.7-4.2%] of adults) by April-2024. This increase was particularly pronounced among younger adults (e.g., reaching 52.8%, 20.4%, and 14.4% of 18-, 45-, and 65-year-old cigarette smokers by April-2024) and those who vape (reaching 34.2% vs. 23.1% among non-vapers). Over time, there were reductions in non-daily smokers' mean weekly cigarette consumption (from 34.3 in 2006-09 to 21.1 in 2021-24), urges to smoke (e.g., the proportion reporting no urges increased from 29.2% to 38.0%), and motivation to stop smoking (e.g., the proportion highly motivated to quit within the next three months decreased from 30.8% to 21.0%). Conclusions: An increasing proportion of adults in England who smoke cigarettes do not smoke every day, particularly younger adults. Although non-daily smokers report smoking fewer cigarettes and weaker urges to smoke than they used to, which may make it easier for them to stop smoking, they appear to be decreasingly motivated to quit.

Trends in e-cigarette brands, devices and the nicotine profile of products used by youth in England, Canada and the USA: 2017–2019

Abstract:Background The e-cigarette market has rapidly evolved, with a shift towards higher nicotine concentration and salt-based products, such as JUUL ; however, the implications for youth vaping remain unclear. Methods Repeat cross-sectional online surveys were conducted in 2017, 2018 and 2019, with national samples of youth aged 16–19 years recruited from commercial panels in Canada (n=12 018), England (n=11 362) and the USA (n=12 110). Regression models examined differences between countries and over time in the types of e-cigarette products used (design and nicotine content), reasons for using brands and differences in patterns of use, sociodemographics and dependence symptoms by brand/nicotine content. Results In 2019, the use of pod- or cartridge-style e-cigarettes was greater in Canada and the USA than England, with Smok and JUUL the leading brands in all countries. In 2019, youth vapers in England were less likely to report using e-cigarettes with ≥2% nicotine (12.8%) compared with Canada (40.5%; adjusted OR (AOR)=4.96; 95% CI 3.51 to 7.01) and the USA (37.0%; AOR=3.99, 95% CI 2.79 to 5.71) and less likely to report using nicotine salt-based products (12.3%) compared with Canada (27.1%; AOR=2.77, 95% CI 1.93 to 3.99) and the USA (21.9%; AOR=2.00, 95% CI 1.36 to 2.95). In 2019, self-reported use of products with higher nicotine concentration was associated with significantly greater frequency of vaping, urges to vape and perceived vaping addiction (p<0.05 for all). Conclusions The use of high-nicotine salt-based products is associated with greater symptoms of dependence, including JUUL and other higher-nicotine brands. Greater use of high-nicotine salt-based products may account for recent increases in the frequency of vaping among youth in Canada and the USA.

Abstract:US teen tobacco use has reached a 25 year low with some 2.25 million young people in middle school and high school reporting they used tobacco products in 2024, down from 2.8 million in 2023.The fall was described by the US Centre of Disease Control and Prevention (CDC) as "an extraordinary milestone for public health."For a report the CDC and the Food and Drug Administration (FDA) analysed data from the National Youth Tobacco Survey to look at use of nine tobacco products among US young people aged 11-18.12 Middle school students in the US are aged 11-13 and high school students are aged 14-18.The decline was attributed to a drop in use of e-cigarettes among high school students, although e-cigarettes remained the most used tobacco product among US young people. Nicotine strength of e‐liquids used by adult vapers in Great Britain: A population survey 2016 to 2024

Abstract:Background and aims In March 2024, the UK government announced plans to introduce a Vaping Products Duty that will tax e‐liquids based on their nicotine strength. This study examined trends in the nicotine strength of e‐liquids used by adult vapers and differences in those currently used across relevant subgroups. Design Nationally‐representative, cross‐sectional household survey, July 2016 to January 2024. Setting Great Britain. Participants 7981 adult vapers. Measurements Participants were asked whether the e‐cigarette they mainly use contains nicotine (yes/no) and the e‐liquid strength (no nicotine, >0–≤ 6, 7–11, 12–19 or ≥20 mg/ml). We also collected information on the main device type used (disposable/refillable/pod), age, gender, occupational social grade, history of ≥1 mental health conditions, smoking status and (among past‐year smokers) level of cigarette addiction. Findings The proportion of vapers in England using high‐strength (≥20 mg/ml) e‐liquids increased from an average of 3.8% (95% confidence interval [CI] = 2.9–5.0) up to June 2021 to 32.5% (CI = 27.9–37.4) in January 2024 (the vast majority of whom [93.3% in January 2024] reported using exactly 20 mg/ml; the legal limit). This rise was most pronounced among those using disposable e‐cigarettes, those aged 18‐24 years and all smoking statuses (including never smokers) except long‐term (≥1y) ex‐smokers. Of those surveyed in 2022–2024 in Great Britain, overall, 89.5% (CI = 88.1–90.8) said they usually used e‐cigarettes containing nicotine, 8.7% (CI = 7.5–10.0) used nicotine‐free e‐cigarettes, and 1.8% (CI = 1.2–2.4) were unsure. The proportion using ≥20 mg/ml was higher among those mainly using disposable (47.9%) compared with pod (16.3%) or refillable (11.5%) devices; never smokers (36.0%), current smokers (28.8%) or recent (<1y) ex‐smokers (27.4%), compared with long‐term ex‐smokers (13.9%); and younger (16–24y; 44.2%) compared with older (≥25y; range 9.4–25.1%) age groups. There were no notable differences across other subgroups of interest. Conclusions Use of high‐strength nicotine e‐liquids in England appears to have increased sharply in recent years. Most adult vapers in Great Britain appear to use e‐cigarettes that contain nicotine but different subgroups use different strengths: nicotine strengths tend to be higher among those who mainly use disposable devices and those aged 16–24y, and lower among long‐term ex‐smokers.

Impacts of EU Tobacco Products Directive regulations on use of e-cigarettes in adolescents in Great Britain: a natural experiment evaluation

Abstract:Background: E-cigarettes are a popular smoking-cessation tool. Although less harmful than tobacco, use of e-cigarettes by non-smokers should be prevented. There is concern about the use of e-cigarettes by young people and that e-cigarettes may renormalise smoking. In May 2016, Tobacco Products Directive regulations aimed to reduce e-cigarettes' appeal to young people. Aims: To examine the effects of the Tobacco Products Directive regulations on young people's use of e-cigarettes, and the role of e-cigarettes in renormalising smoking. Design: A mixed-method natural experimental evaluation combining secondary analyses of survey data, with process evaluation, including interviews with young people, policy stakeholders, retailers and trading standards observers, and observations of retail settings. Settings: Wales, Scotland and England. Participants: Survey participants were aged 13-15 years, living in England, Scotland or Wales and participated in routinely conducted surveys from 1998 to 2019. Process evaluation participants included 14- to 15-year-olds in England, Scotland and Wales, policy stakeholders, trading standards offices and retailers. Intervention: Regulation of e-cigarettes, including bans on cross-border advertising, health warnings and restrictions on product strength. Comparison group: Interrupted time series design, with baseline trends as the comparator. Main outcome measures: The primary outcome was ever e-cigarette use. Secondary outcomes included regular use, ever and regular smoking, smoking attitudes, alcohol and cannabis use. Data capture and analysis: Our primary statistical analysis used data from Wales, including 91,687 young people from the 2013-19 Health Behaviour in School-aged Children and School Health Research Network surveys. In Scotland, we used the Scottish Schools Adolescent Lifestyle and Substance Use Survey and in England we used the Smoking Drinking and Drug Use surveys. The process evaluation included interviews with 73 young people in 2017 and 148 young people in 2018, 12 policy stakeholders, 13 trading standards officers and 27 retailers. We observed 30 retail premises before and after implementation. Data were integrated using the Medical Research Council's process evaluation framework. Results: Ever smoking continued to decline alongside the emergence of e-cigarettes, with a slight slowing in decline for regular use. Tobacco Products Directive regulations were described by stakeholders as well implemented, and observations indicated good compliance. Young people described e-cigarettes as a fad and indicated limited interaction with the components of the Tobacco Products Directive regulations. In primary statistical analyses in Wales [i.e. short (to 2017) and long term (to 2019)], growth in ever use of e-cigarettes prior to Tobacco Products Directive regulations did not continue after implementation. Change in trend was significant in long-term analysis, although of similar magnitude at both time points (odds ratio 0.96). Data from England and Scotland exhibited a similar pattern. Smoking followed the opposite pattern, declining prior to the Tobacco Products Directive regulations, but plateauing as growth in e-cigarette use stalled. Limitations: Alternative causal explanations for changes cannot be ruled out because of the observational design. Conclusions: Young people's ever and regular use of e-cigarettes appears to have peaked around the time of the Tobacco Products Directive regulations and may be declining. Although caution is needed in causal attributions, findings are consistent with an effect of regulations. Our analysis provides little evidence that e-cigarettes renormalise smoking. More recent data indicate that declines in smoking are plateauing. Future work: International comparative work to understand differences in use of e-cigarettes, and tobacco, within varying regulatory frameworks is a priority. Reasons for current E-cigarette use among U.S. adults

Abstract:E-cigarette use has increased rapidly among U.S. adults. However, reasons for use among adults are unclear. We assessed reasons for e-cigarette use among a national sample of U.S. adults. Data were collected via online surveys among U.S. adults aged 18 or older from April through June 2014. Descriptive and multivariate regression analyses were conducted to assess reasons for e-cigarette use among 2448 current e-cigarette users, by sociodemographic characteristics and product type. Assessed reasons included cessation/health, consideration of others, convenience, cost, curiosity, flavoring, and simulation of conventional cigarettes. Among current e-cigarette users, 93% were also current cigarette smokers. The most common reasons for e-cigarette use were cessation/health (84.5%), consideration of others (71.5%), and convenience (56.7%). The prevalence of citing convenience (adjusted prevalence ratio [aPR]=1.49) and curiosity (aPR=1.54) as reasons for e-cigarette use were greater among current cigarette smokers than nonsmokers (P<0.05). The prevalence of citing flavoring as a reason for use was greater among adults aged 18 to 24 (aPR=2.02) than 55 or older (P<0.05). Tank use was associated with greater prevalence of citing every assessed reason except convenience and curiosity. Cessation- and health-related factors are primary reasons cited for e-cigarette use among adults, and flavorings are more commonly cited by younger adults. Efforts are warranted to provide consumers with accurate information on the health effects of e-cigarettes and to ensure that flavoring and other unregulated features do not promote nicotine addiction, particularly among young adults.

Abstract:Despite widespread evidence suggesting that e-cigarettes are harmful to youth, the prevalence of adolescent vaping continues to rise. The first aim of this cross-sectional study was to determine which methods adolescents have previously used to quit vaping and their interest in using various methods to quit vaping in the future. The second aim was to investigate the associations between vaping-related characteristics and 1) motivation to quit vaping and 2) previous use of pharmacological methods to quit vaping or previous use of unassisted quitting. Participants were 185 current or former vapers, aged 14-19 (M age = 16.9 years, SD = 1.1; 52% female), recruited online from different U.S. regions. Data were collected through online questionnaires. Most participants reported at least one prior attempt to quit vaping (81.3%). Unassisted quitting was the most prevalent method to quit vaping (78.4%), followed by "advice from a friend" (51.9%). Participants were interested in using a variety of methods for quitting vaping in the future, including medication (28.8%), and mobile apps (34.6%). Greater perceived harm of vaping was significantly associated with motivation to quit vaping, while greater perceived risk of addiction and higher SES were significantly associated with ever use of nicotine replacement to quit vaping. Adolescents may be open to trying methods to quit that are different from what they have used previously. These findings could help direct resources toward the development of vaping cessation programs that are acceptable to adolescents.

Abstract:Importance: Comprehensive surveillance of e-cigarette use behaviors among youth is important for informing strategies to address this public health epidemic.Objective: To characterize e-cigarette use behaviors among US youth in 2020.Design, Setting, and Participants: The 2020 National Youth Tobacco Survey, a nationally representative, cross-sectional, school-based survey of middle school (grades 6-8) and high school (grades 9-12) students, was conducted from January 16, 2020, to March 16, 2020. A total of 14 531 students from 180 schools participated in the 2020 survey, yielding a corresponding student-level participation rate of 87.4% and school-level participation rate of 49.9%. The overall response rate, a product of the school-level and student-level participation rates, was 43.6%.Exposures: Current (past 30-day) e-cigarette use.Main Outcomes and Measures: Self-reported current e-cigarette use behaviors (frequency of use, usual e-cigarette brand, and access source) by school level and flavored e-cigarette use and flavor types among current e-cigarette users by school level and device type. Prevalence estimates were weighted to account for the complex survey design.Results: Overall, 14 531 students completed the survey, including 7330 female students and 7133 male students with self-reported grade level and sex. In 2020, 19.6% (95% CI, 17.2%-22.2%) of high school students and 4.7% (95% CI, 3.6%-6.0%) of middle school students reported current e-cigarette use. Among them, 38.9% (95% CI, 35.2%-42.6%) of high school users and 20.0% (95% CI, 16.0%-24.8%) of middle school users reported e-cigarette use on 20 to 30 days within the past 30 days. Among current users, JUUL was the most commonly reported usual brand (high school: 25.4%; 95% CI, 18.8%-33.4%; middle school: 35.1%; 95% CI, 27.9%-43.1%). Among current users, the most common source of obtaining e-cigarettes was from a friend (high school: 57.1%; 95% CI, 52.6%-61.4%; middle school: 58.9%; 95% CI, 51.4%-66.1%). Among current users, 84.7% (95% CI, 82.2%-86.9%) of high school students and 73.9% (95% CI, 66.9%-79.8%) of middle school students reported flavored e-cigarette use. Fruit-flavored e-cigarettes were the most commonly reported flavor among current exclusive e-cigarette users of prefilled pods or cartridges (67.3%; 95% CI, 60.9%-73.0%), disposable e-cigarettes (85.8%; 95% CI, 79.8%-90.3%), and tank-based devices (82.7%; 95% CI, 68.9%-91.1%), followed by mint-flavored e-cigarettes.Conclusions and Relevance: These results suggest that although current e-cigarette use decreased during 2019 to 2020, overall prevalence, frequent use, and flavored e-cigarette use remained high. Continued actions are warranted to prevent and reduce e-cigarette use among US youth.

Abstract:Introduction: It is illegal in the UK to sell tobacco or nicotine e-cigarettes to people under the age of 18 years, as is displaying tobacco cigarettes at the point of sale. This paper examined changes in exposure to display of these products in shops and sources of these products among children and adolescent users over time METHODS: Data from representative repeated online cross-sectional surveys of youth in Great Britain (11-18 years) were used (2018-2022; n=12 445). Outcome measures included noticing product displays and sources of e-cigarettes and tobacco cigarettes. Logistic regressions examined the associations of these outcome variables over time and with sociodemographic variables. Results: Of 12 040 participants with complete data, 10.1% used some form of nicotine product (4.2% cigarettes, 2.9% e-cigarettes, 3.0% both) at least occasionally. The likelihood of noticing tobacco cigarettes on display fell over time for both supermarkets (2018: 67.1% to 2022: 58.5%) and small shops (2018: 81.3% to 2022: 66.3%), but the likelihood of noticing e-cigarettes in supermarkets rose (2018: 57.4% to 2022: 66.5%). Sources of tobacco cigarettes did not differ over time, but e-cigarette users were more likely to get their e-cigarettes from small shops in 2022 (51.2%) vs 2019 (34.2%) (OR 2.02, 95% CI 1.24, 3.29). Conclusion: This study provides evidence that current policies to limit awareness of and access to both tobacco and e-cigarettes among adolescents in the UK may not be effective. UK policies on the advertising, promotion and sale of both tobacco and e-cigarettes need to be reinforced to deter use among children and adolescents.
