Abstract:BACKGROUNDThe utility of T wave alternans (TWA) in identifying arrhythmia risk has been demonstrated. During myocardial ischemia (MI), TWA could be induced by cellular alternans. However, the relationship between cellular alternans patterns and TWA patterns in MI has not been investigated thoroughly.METHODSWe set MI conditions to simulate alternans. Either prolonging Ca2+ release or increasing spark-induced sparks (secondary sparks) can give rise to different patterns of APD alternans and TWA. In addition, different ischemic zones and reduced conduction velocity are also considered in one dimensional simulation.RESULTSDelay of Ca2+ release can produce discordant Ca2+-driven alternans in single cell simulation. Increasing secondary sparks leads to concordant alternans. Correspondingly, morphology and magnitude of TWA vary in two different cellular alternans. Epi ischemia results in alternans concentrating in the first half of T wave. Endo and transmural ischemia lead to fluctuations in the second half of T wave. In addition, slowing conduction velocity has no effect on TWA magnitude.CONCLUSIONSpecific ionic channel dysfunction and ischemic zones affect TWA patterns.

Abstract:T-wave alternans, a specific form of cardiac alternans, has been associated with the increased susceptibility to cardiac arrhythmias and sudden cardiac death (SCD). Plenty of evidence has related cardiac alternans at the tissue level to the instability of voltage kinetics or Ca2+ handling dynamics at the cellular level. However, to date, none of the existing experiments could identify the exact cellular mechanism of cardiac alternans due to the bi-directional coupling between voltage kinetics and Ca2+ handling dynamics. Either of these systems could be the origin of alternans and the other follows as a secondary change, therefore making the cellular mechanism of alternans a difficult chicken or egg problem. In this context, theoretical analysis combined with experimental techniques provides a possibility to explore this problem. In this review, we will summarize the experimental and theoretical advances in understanding the cellular mechanism of alternans. We focus on the roles of action potential duration (APD) restitution and Ca2+ handling dynamics in the genesis of alternans and show how the theoretical analysis combined with experimental techniques has provided us a new insight into the cellular mechanism of alternans. We also discuss the possible reasons of increased propensity for alternans in heart failure (HF) and the new possible therapeutic targets. Finally, according to the level of electrophysiological recording techniques and theoretical strategies, we list some critical experimental or theoretical challenges which may help to determine the origin of alternans and to find more effective therapeutic targets in the future.

Abstract:Myocardial ischemia is associated with pathophysiological conditions such as hyperkalemia, acidosis, and hypoxia. These physiological disorders may lead to changes on the functions of ionic channels, which in turn form the basis for cardiac alternans. In this paper, we investigated the roles of hyperkalemia and calcium handling components played in the genesis of alternans in ischemia at the cellular level by using computational simulations. The results show that hyperkalemic reduced cell excitability and delayed recovery from inactivation of depolarization currents. The inactivation time constant τ f of L-type calcium current (I CaL) increased obviously in hyperkalemia. One cycle length was not enough for I CaL to recover completely. Alternans developed as a result of I CaL responding to stimulation every other beat. Sarcoplasmic reticulum calcium-ATPase (SERCA2a) function decreased in ischemia. This change resulted in intracellular Ca (Cai) alternans of small magnitude. A strong Na+-Ca2+ exchange current (I NCX) increased the magnitude of Cai alternans, leading to APD alternans through excitation-contraction coupling. Some alternated repolarization currents contributed to this repolarization alternans.

Abstract:BACKGROUND:Macroscopic T wave alternans (macro-TWA) often heralds the onset of Torsades de Pointes in patients with QT prolongation. However, the mechanisms underlying macro-TWA remain unclear. We examined the cellular and ionic basis for macro-TWA in rabbits with left ventricular hypertrophy (LVH).METHODS:The renovascular hypertension model was used to induce LVH in rabbits. Action potentials were simultaneously recorded from epicardium and endocardium together with a transmural ECG and isometric contractility in arterially perfused left ventricular wedges. Late sodium current (INa-L) was recorded in single-isolated left ventricular myocytes with the whole cell patch-clamp technique.RESULTS:Macro-TWA and accompanied mechanical alternans occurred spontaneously in 8 of 33 LVH rabbits (P<0.05, versus 0/15 in controls) and were induced by an INa-L enhancer ATX-II at 1 to 3 nM in additional 7. Macro-TWA and mechanical alternans occurred discordantly, that is, that longer QT interval and larger T wave were associated with weaker isometric contvractility. Alternating early afterdepolarizations in the endocardium caused macro-TWA in 12 of 15 LVH rabbits and, therefore, early afterdepolarization-dependent R-from-T extrasystoles and Torsades de Pointes always originated from the beats with longer QT and larger T wave during macro-TWA. INa-L density was significantly larger in LVH myocytes than that of control myocytes. Macro-TWA, mechanical alternans, R-from-T extrasystoles, and Torsades de Pointes were all abolished by INa-L blocker ranolazine or mexiletine.CONCLUSIONS:LVH enhances INa-L density and promotes alternating early afterdepolarizations in the left ventricular endocardium that manifest as macro-TWA with discordant mechanical alternans. INa-L blockade abolishes macro-TWA, mechanical alternans, early afterdepolarization-dependent R-from-T extrasystoles, and Torsades de Pointes.

Abstract:T-wave alternans, characterized by a beat-to-beat change in T-wave morphology, amplitude, and/or polarity on the ECG, often heralds the development of lethal ventricular arrhythmias in patients with left ventricular hypertrophy (LVH). The aim of our study was to examine the ionic basis for a beat-to-beat change in ventricular repolarization in the setting of LVH. Transmembrane action potentials (APs) from epicardium and endocardium were recorded simultaneously, together with transmural ECG and contraction force, in arterially perfused rabbit left ventricular wedge preparation. APs and Ca(2+)-activated chloride current (I(Cl,Ca)) were recorded from left ventricular myocytes isolated from normal rabbits and those with renovascular LVH using the standard microelectrode and whole cell patch-clamping techniques, respectively. In the LVH rabbits, a significant beat-to-beat change in endocardial AP duration (APD) created beat-to-beat alteration in transmural voltage gradient that manifested as T-wave alternans on the ECG. Interestingly, contraction force alternated in an opposite phase ("out of phase") with APD. In the single myocytes of LVH rabbits, a significant beat-to-beat change in APD was also observed in both left ventricular endocardial and epicardial myocytes at various pacing rates. APD alternans was suppressed by adding 1 microM ryanodine, 100 microM 4,4'-diisothiocyanostilbene-2,2'-disulfonic acid (DIDS), and 100 microM 4-acetamido-4'-isothiocyanostilbene-2,2'-disulfonic acid (SITS). The density of the Ca(2+)-activated chloride currents (I(Cl,Ca)) in left ventricular myocytes was significantly greater in the LVH rabbits than in the normal group. Our data indicate that abnormal intracellular Ca(2+) fluctuation may exert a strong feedback on the membrane I(Cl,Ca), leading to a beat-to-beat change in the net repolarizing current that manifests as T-wave alternans on the ECG.

Abstract:We examined whether there is a relationship between repolarization abnormalities on electrocardiography (EKG) and deformation abnormalities by echocardiography. Analysis of baseline EKGs and mechanical (echo-based deformation) changes was performed in 128 patients with a clinical diagnosis of hypertrophic cardiomyopathy (HCM). Patients with left ventricular hypertrophy (LVH) or repolarization abnormalities had higher septal thickness when compared to patients with normal EKG. Patients with EKG evidence of LVH or QT c prolongation had lower systolic velocity, systolic strain, systolic strain rate, late diastolic velocity, and late diastolic strain rate than patients with a normal EKG. Patients with strain pattern or ST depression/T-wave inversion had lower systolic velocity, systolic strain, systolic strain rate, early diastolic velocity, and late diastolic velocity when compared to patients with normal EKGs. LVH and repolarization abnormalities on surface EKG are markers of impaired systolic and diastolic mechanics in HCM.

Abstract:INTRODUCTION:Evidence is accumulating that microvolt T wave alternans (TWA) is a marker of increased risk for ventricular tachyarrhythmias. Initially, atrial pacing was used to elevate heart rate and elicit TWA. More recently, a noninvasive approach has been developed that elevates heart rate using exercise.METHODS AND RESULTS:In 30 consecutive patients with a history of ventricular tachyarrhythmias, the spectral method was used to detect TWA during both atrial pacing and submaximal exercise testing. The concordance rate for the presence or absence of TWA using the two measurement methods was 84%. There was a patient-specific heart rate threshold for the detection of TWA that averaged 100 +/- 14 beats/min during exercise compared with 97 +/- 9 beats/min during right atrial pacing (P = NS). Beyond this threshold, there was a significant and comparable increase in level of TWA with decreasing pacing cycle length and increasing exercise heart rates.CONCLUSIONS:The present study is the first to demonstrate that microvolt TWA can be assessed reliably and noninvasively during exercise stress. There is a patient-specific heart rate threshold beyond which TWA continues to increase with increasing heart rates. Heart rate thresholds for the onset of TWA measured during atrial pacing and exercise stress were comparable, indicating that heart rate alone appears to be the main factor of determining the onset of TWA during submaximal exercise stress.
ST-T Alternans as a Risk Predictor for Fatal Ventricular Arrhythmia

Abstract:To the Editor Xu et al present the electrocardiogram findings of a patient in their 50s with syncope and palpitation and amaurosis, followed by loss of consciousness for several minutes without preceding position change, cough, chest pain, or shortness of breath. The authors highlight that "giant R-wave syndrome is distinguished by delayed depolarization and repolarization of the myocardium, which is a predictor of the onset of ventricular fibrillation." The authors focused only on giant R-wave syndrome as a predictive factor for ventricular arrhythmias in patients with acute myocardial infarction and ignored the presence of cardiac electrical alternances. Obvious in the Holter recording (Figure in the case report) is a clear macro–ST-T–wave alternans observed in leads V2 through V4. Therefore, it is reasonable to conclude that, in addition to the presence of giant R-wave syndrome, the electrical alternans of ST-T may play a more crucial role in polymorphic ventricular tachycardia. ST-T alternans have been associated with spontaneous ventricular arrhythmias during myocardial ischemia.

Abstract:Identification of individuals at risk for sudden cardiac death (SCD), the main cause of adult mortality in developed countries, remains a major challenge. The main contemporary noninvasive marker, left ventricular ejection fraction (LVEF), has not proved adequately reliable, as the majority of individuals who die suddenly have relatively preserved cardiac mechanical function. Monitoring of T-wave alternans (TWA), a beat-to-beat fluctuation in ST-segment or T-wave morphology, on ambulatory electrocardiogram (AECG) is an attractive approach on both scientific and clinical grounds. Specifically, TWA's capacity to assess risk for malignant arrhythmias has been shown to rest on sound electrophysiologic principles and AECG-based TWA monitoring can be performed in the flow of routine clinical evaluation. This review addresses: (1) electrophysiologic and ionic mechanisms underlying TWA's predictivity, (2) principles and practical aspects of AECG-based TWA monitoring, (3) clinical evidence supporting this approach to SCD risk stratification, and (4) current and potential applications in guiding medical therapy.

Abstract:Background: Arhythmogenic right ventricular dysplasia (ARVD) carries the risk of ventricular arrhythmias and sudden cardiac death (SCD). The modified moving average microvolt T-wave alternans (MMA TWA) has been explored to predict arrhythmic events and mortality in ischemic heart disease. However, the clinical application of TWA in ARVD remains unclear. Methods: The cohort enrolled consecutive 33 patients with ARVD based on the new Task Force (TF) criteria. The Holter ECG-based MMA TWA was obtained and TWA≥65 µV was defined as positive. Regarding the demographic, electrocardiographic, structural, and invasive electrophysiological characteristics, several factors were explored to predict mortality and malignant ventricular arrhythmias. Results: During a mean follow-up duration of 18.09±12.59 months (1–53 months), a total of 10 (30.3%) patients developed events, including 3 patients (30%) with SCD and 7 (70%) non-sustained or sustained ventricular arrhythmias. Ten patients underwent an implantable cardioverter defibrillator placement for secondary prevention. There were no differences in baseline characteristics and invasive electrophysiological results, except more patients with positive TWA in the event group. On multivariate analysis, independent predictors of events included positive TWA (P=0.011, OR: 11.677, CI: 1.77∼77.09) and daily ventricular extrasystole percentage (P=0.047, OR: 1.101, CI: 1.001∼1.21). Conclusion: The MMA TWA could predict the risk of ventricular arrhythmia and mortality in patients with ARVD in addition to the TF criteria.

Abstract:Primary prevention of sudden cardiac death is hampered by the inability to accurately identify high risk patients. Various noninvasive methods such as determination of left ventricular function or heart rate variability as well as invasive electrophysiologic testing are currently used for risk stratification. Noninvasive measurement of microvolt T wave alternans (TWA) is a promising new method to assess repolarization abnormalities; in experimental studies, TWA was associated with an increased incidence of ventricular tachyarrhythmias. Since the occurrence of TWA is heart rate-dependent, it is measured either during atrial pacing or during exercise stress testing. The first clinical validation of the method was performed in patients undergoing invasive EP testing to assess prediction of inducibility of ventricular tachyarrhythmias. A first prospective validation of the noninvasive method was performed in patients surviving out-of-hospital cardiac arrest fitted with an ICD. Further studies have shown a good concordance between invasive and noninvasive TWA determination. The occurrence of TWA in this population was of predictive value with respect to arrhythmia recurrence. Recently published data confirm the value of TWA assessment with respect to identification of patients with congestive heart failure at high risk of malignant ventricular tachyarrhythmias. The use of this method in post myocardial infarction risk stratification is currently under prospective evaluation.
Two Types of T Wave Alternans in Long‐QT Syndrome

Abstract:A 5-year-old female child with bilateral hand syndactyly, QT prolongation, and 2:1 atrioventricular (AV) block was seen in consultation (Fig. 1). The genetic testing identified a mutation in CACNA1C gene, confirming type 1 Timothy syndrome (LQT8). During ambulatory EKG monitoring, she had two types of T wave alternans (TWA): one occurred during a steady state in which RR intervals remained relatively unchanged (type 1), whereas the other developed during transition from 2:1 AV conduction to 1:1 conduction in which a sudden change in RR intervals and aberrant conduction were associated (type 2). During type 1 TWA, there was no significant beat-to-beat variation in PP interval (or RR interval) as well as in QRS duration (Fig. 2A). In addition, there was an identifiable isoelectric interval between the end of previous T wave and P wave. Type 2 TWA, as shown in Figure 2B and C, was profound and developed likely secondary to a sudden change in heart rate and aberrant ventricular conduction from resumption of 1:1 AV conduction from 2:1 AV block during sinus arrhythmia (Fig. 2C). Type 2 TWA disappeared after a few seconds of 1:1 AV conduction from 2:1 AV block when the RR intervals remained stable and the mean QT ventricular became shorter. The relationship between QRS and QT during type 2 TWA is shown in Figure 2D. Previous studies have shown that TWA may herald development of malignant ventricular arrhythmia. However, two types of TWA in our case obviously developed at different mechanisms, from which different arrhythmogenic risks would be expected. Type 1 TWA, commonly reported in the literature, is unrelated to ventricular conduction and a change in RR intervals. It is probably due to abnormal intracellular Ca2+ cycling that influences ventricular repolarization in an alternating pattern.1 Our recent basic research has shown that early afterdepolarization (EAD) alternans in left ventricular hypertrophy manifests as type 1 TWA on the ECG, which often precedes the development of EAD-initiating R-on-T ectopic beats.2 On the other hand, type 2 TWA was less commonly reported. It may occur as a consequence of an abrupt change in ventricular rate or aberrant ventricular conduction. "TWA" resulting from intermittent pre-excitation is the best illustration of activation-dependent change in T wave (Fig. 2E). A sudden change in RR intervals may also contribute importantly to type 2 TWA (Fig. 2B and C). Facilitation of TWA by an abrupt change in RR intervals in long-QT syndrome may be related to adaptation of time-dependent repolarizing currents to a new rate. Surprisingly, this female child was free of syncope and documented ventricular arrhythmias despite the fact that both types of TWA were commonly seen prior to the drug therapy. Even though, potential risks of both types of TWA to trigger TdP should not be ignored.

Abstract:BACKGROUND Atrial tachycardia (AT) with cycle length (CL) alternans is a rare phenomenon. We aimed to identify the characteristics and precise mechanism of this special category of ATs by using an ultrahigh density mapping system. METHODS We identified 7 ATs with alternating CL in a total of 478 ATs from 2 institutions mapped with an ultrahigh density mapping system. Activation maps were performed for long CL (289±35 ms; mapping points, 21 520±11 103) and short CL (251±18 ms; mapping points,17 594±8059) separately. RESULTS We classified ATs with CL alternans into 2 types. Type 1: There existed 2 potential loops with different routes. CL alternans resulted from an intermittently 2:1 conducting block within the channel of the smaller loop. Type 2: CL alternans resulted from different conduction velocity through 2 closely spaced gaps within preexisting linear lesions. Catheter ablation successfully terminated all the 7 ATs. CONCLUSIONS Ultrahigh density mapping provides an opportunity to delineate the precise mechanism of AT with CL alternans. Intermittent conduction block or slowing of a channel was essential for the maintenance of AT.

Introduction: The current standard for arrhythmic risk stratification is electrophysiologic (EP) testing, which, due to its invasive nature, is limited to patients already known to be at high risk. A number of noninvasive tests, such as determination of left ventricular ejection fraction (LVEF) or heart rate variability, have been evaluated as additional risk stratifiers. Prospective evaluation of noninvasive risk markers in low‐ or moderate‐risk populations requires studies involving very large numbers of patients, and in such studies, documentation of the occurrence of ventricular tachyarrhythmias is difficult. In the present study, we identified a high‐risk population, recipients of an implantable cardioverter defibrillator (ICD), and prospectively compared microvolt TWA with invasive EP testing and other risk markers with respect to their ability to predict recurrence of ventricular tachyarrhythmias as documented by ICD electrograms. Methods and Results: Ninety‐five patients with a history of ventricular tachyarrhythmias undergoing implantation of an ICD underwent EP testing, assessment of TWA, as well as determination of LVEF, baroreflex sensitivity, signal‐averaged ECG, analysis of 24‐hour Holter monitoring, and QT dispersion from the 12‐lead surface ECG. The endpoint of the study was first appropriate ICD therapy for electrogram‐documented ventricular fibrillation or tachycardia during follow‐up. Kaplan‐Meier survival analysis revealed that TWA (P < 0.006) and LVEF (P < 0.04) were the only significant univariate risk stratifiers. EP testing was not statistically significant (P < 0.2). Multivariate Cox regression analysis revealed that TWA was the only statistically significant independent risk factor. Conclusions: Measurement of microvolt TWA compared favorably with both invasive EP testing and other currently used noninvasive risk assessment methods in predicting recurrence of ventricular tachyarrhythmias in ICD recipients. This study suggests that TWA might also be a powerful tool for risk stratification in low‐ or moderate‐risk patients, and needs to he prospectively evaluated in such populations.
The characteristics of T wave alternans in aortic stenosis in school-aged children

Abstract:Objective To explore the characteristics of T wave alternans (TWA) in aortic stenosis (AS) in school-aged children. Methods TWA was analyzed in 15 AS patients and 60 age-matched normal children using Microvolt T-Wave Alternans and the incidence of sustained TWA was compared between two groups. Myocardial ischemia during treadmill test was studied in sustained TWA and non-sustained TWA in AS patients. Results Compared with normal children, the incidence of sustained TWA was signiifcantly increased in AS group (P<0.01). In AS school-aged patients, the incidence of myocardial ischemia is higher in sustained TWA than in non-sustained TWA during treadmill test (P<0.05). Conclusions AS school-aged children have electrophysiologic basis for sunstained TWA. TWA is closely related to myocardial ischemia during treadmill test. Sustained TWA could be used as an important electrophysiologic index to assess the cardiac function in children with AS.

Abstract:Cardiac alternans, a beat-to-beat alternation in action potential duration (at the cellular level) or in ECG morphology (at the whole heart level), is a marker of ventricular fibrillation, a fatal heart rhythm that kills hundreds of thousands of people in the US each year. Investigating cardiac alternans may lead to a better understanding of the mechanisms of cardiac arrhythmias and eventually better algorithms for the prediction and prevention of such dreadful diseases. In paced cardiac tissue, alternans develops under increasingly shorter pacing period. Existing experimental and theoretical studies adopt the assumption that alternans in homogeneous cardiac tissue is exclusively determined by the pacing period. In contrast, we find that, when calcium-driven alternans develops in cardiac fibers, it may take different spatiotemporal patterns depending on the pacing history. Because there coexist multiple alternans solutions for a given pacing period, the alternans pattern on a fiber becomes unpredictable. Using numerical simulation and theoretical analysis, we show that the coexistence of multiple alternans patterns is induced by the interaction between electrotonic coupling and an instability in calcium cycling.

Abstract:T wave alternans (TWA) is a risk factor for life-threatening arrhythmic events (LAEs) in long QT syndrome(LQTS) patients. Few small-sample studies have quantified the TWA value by 24-hour ambulatory recording or exercise stress test, and the cut-off point of TWA≥47 mV was based on myocardial infarction patients(ATRAMI study). In our study, we aimed (1) to compare the predictive model of LAE according to the TWA value measured by 24-hour ambulatory recording and exercise stress test; (2) to propose a new TWA cutpoint for abnormality in LQTS patients; (3) to analyse the association of TWA with LAE.
