Abstract:Pancreatic duct adenocarcinoma is one of the most fatal malignancies, with R0 resection remaining the most important part of treatment of this malignancy. However, pancreatectomy is believed to be one of the most challenging procedures and R0 resection remains the only chance for patients with pancreatic cancer to have a good prognosis. Some surgeons have tried minimally invasive pancreatic surgery, but the short-and long- term outcomes of pancreatic malignancy remain controversial between open and minimally invasive procedures. We collected comparative data about minimally invasive and open pancreatic surgery. The available evidence suggests that minimally invasive pancreaticoduodenectomy (MIPD) is as safe and feasible as open PD (OPD), and shows some benefit, such as less intraoperative blood loss and shorter postoperative hospital stay. Despite the limited evidence for MIPD in pancreatic cancer, most of the available data show that the short- term oncological adequacy is similar between MIPD and OPD. Some surgical techniques, including superior mesenteric artery- first approach and laparoscopic pancreatoduodenectomy with major vein resection, are believed to improve the rate of R0 resection. Laparoscopic distal pancreatectomy is less technically demanding and is accepted in more pancreatic centers. It is technically safe and feasible and has similar short- term oncological prognosis compared with open distal pancreatectomy.

Abstract:Pancreatic cancer is a significant challenge in oncology due to its aggressive nature and complex management, leading to high mortality rates and a dismally low 5-year survival rate. Approximately 85% of cases manifest as adenocarcinoma, while endocrine tumors constitute less than 5%. Borderline resectable and locally advanced pancreatic cancers are particularly difficult to treat due to vascular involvement, which complicates complete resections and increases morbidity. Various therapeutic modalities aim to overcome these challenges and improve patient outcomes. Traditionally, upfront surgery was the standard for resectable tumors, with multimodal chemotherapy being central to treatment. Understanding surgical anatomy is pivotal in enhancing surgical outcomes and patient survival. Resectability challenges are several when seeking to achieve R0 resections, particularly for borderline resectable tumors. Various classification systems—the MD Anderson criteria, the NCCN criteria, the AHPA/SSAT/SSO consensus statement, and the Alliance definition—assess tumor involvement with major blood vessels, with the first of these systems being broadly accepted. Vascular staging integration is also important, with the Ishikawa staging system using preoperative imaging to assess venous involvement. Furthermore, neoadjuvant therapy enhances treatment effectiveness by addressing micro-metastatic disease early, increasing R0 resection chances, and downstaging tumors for optimal surgery. Insights from the Fox Chase Cancer Center's neoadjuvant treatment approach highlight the importance of a multidisciplinary strategy when advancing therapy and improving patient prognosis. This commentary, inspired by Dr. Sanjay S. Reddy's Keynote Conference during MedNews week, highlights current advancements and ongoing challenges in the treatment of pancreatic cancer, emphasizing the need for a comprehensive, multidisciplinary approach to improve outcomes.

Abstract:The incidence of pancreatic cancer has steadily risen in the past decades. Radical resection is still the best option for treatment of pancreatic cancer. Great progress has been achieved in pancreatic surgery; the mortality and morbidity have decreased dramatically, but long term survival after resection is still dismal. Standard pancreaticoduodenectomy is recommended now for pancreatic head cancer. Extended lymphadenectomy could not improve the prognosis of pancreatic cancer patients. Total pancreatectomy is suitable for some IPMNs patients, rather than a routine for pancreatic cancer patients. The current status and future of surgical treatment for pancreatic cancer will be summarized here.

Abstract:Radical resection is the only curative treatment for pancreatic cancer. However, only up to 20% of patients are considered eligible for surgical resection at the time of diagnosis. Although upfront surgery followed by adjuvant chemotherapy has become the gold standard of treatment for resectable pancreatic cancer there are numerous ongoing trials aiming to compare the clinical outcomes of various surgical strategies (e.g., upfront surgery or neoadjuvant treatment with subsequent resection). Neoadjuvant treatment followed by surgery is considered the best approach in borderline resectable pancreatic tumors. Individuals with locally advanced disease are now candidates for palliative chemo- or chemoradiotherapy; however, some patients may become eligible for resection during the course of such treatment. When metastases are found, the cancer is qualified as unresectable. It is possible to perform radical pancreatic resection with metastasectomy in selected cases of oligometastatic disease. The role of multi-visceral resection, which involves reconstruction of major mesenteric veins, is well known. Nonetheless, there are some controversies in terms of arterial resection and reconstruction. Researchers are also trying to introduce personalized treatments. The careful, preliminary selection of patients eligible for surgery and other therapies should be based on tumor biology, among other factors. Such selection may play a key role in improving survival rates in patients with pancreatic cancer.

Abstract:Neuroendocrine tumor of the pancreas (pNET) is a rare group of neoplasms with widely heterogeneity.Complete surgical resection remains the only potentially curative treatment for pNET.Compared to open surgery,minimal invasive approach treating pNET could decrease bleeding and shorten postoperative hospital stay,while didn't show difference in operation time,postoperative complication rate and overall mortality.Therapeutic strategy was formulated by synthesizing clinical symptoms,tumor size and tumor stage.Minimal invasive approach can achieve radical curve of oncology and provide refine manipulation condition to improve surgical safety and efficacy,meanwhile realize function preservation.Minimal invasive approach is a safe and efficient method in treating pNET,but further prospective,randomized control trials are required.

Abstract:Pancreatic duct carcinoma remains a common disease with a poor prognosis. More than 30,000 Americans will die of the disease in 2004, making it the fourth leading cause of cancer death. Despite significant advances in the treatment of many other human tumors, the 5-year survival rate for persons diagnosed with pancreatic cancer has not changed in decades and remains <5%. This is due both to the inherently aggressive biology of the disease and to its late diagnosis in most cases. Surgical resection of localized disease remains the only hope for cure of pancreatic cancer. Over the past 2 decades, significant advances in diagnostic imaging, staging, surgical technique, and perioperative care have led to marked improvement in the surgical management of pancreatic cancer patients. Operative mortality rates for pancreaticoduodenectomy are now <5% at major centers, and the average length of hospital stay has been reduced to <2 weeks. Improvements in patient outcome after pancreatic cancer surgery have made possible, for the first time, the design and conduct of large adjuvant therapy studies in pancreatic cancer. Such clinical trials are critical for improving outcomes for pancreatic cancer patients.

Abstract:Over the past 20 years, the concept of pancreatic surgery has been updated and surgical skills has improved dramatically. With the significant improvement of surgical safety and increase of resection rate for pancreatic cancer, some traditional surgical issues such as surgical indications, timing and extent of resection are being re-evaluated. The improvement of patients′ prognosis is the gold standard for judging the surgical indications. The traditional criteria of “unresectable” based on morphology will be constantly broken through, and the estimation of resectability will transition from “what can we resect” to “what should we resect”. Except for clinical research, standard extent of lymph node dissection is recommended. However, for borderline resectable and locally advanced pancreatic cancer after neoadjuvant treatment, extended lymph node dissection is recommended. All kinds of surgical approaches are feasible. The approach is subject to the needs of anatomy, anatomy is subject to the needs of radical treatment, and radical treatment is subject to the needs of improving prognosis. For some patients with locally advanced pancreatic cancer, sub-adventitial divestment of superior mesenteric artery and “Heidelberg triangle” cleaning are helpful to improve the resection rate and reduce the local recurrence rate,however, the impact on the long-term prognosis still needs to be further observed clinically. The quality of pancreaticojejunostomy has more influence on the incidence of pancreatic fistula than the type of pancreaticojejunostomy. For the centers with high volume patients and the surgeons with rich personal experience, laparoscopic or robot assisted surgery has the advantages of minimally invasive, but for pancreatic head carcinoma, it is not enough evidence to prove the oncological advantages of laparoscopic pancreaticoduodenectomy and robotic-assistant pancreaticoduodenectomy.

Abstract:Pancreatic ductal adenocarcinoma (PDAC) remains associated with poor outcomes with a 5-year survival of 12% across all stages of the disease. These poor outcomes are driven by a delay in diagnosis and an early propensity for systemic dissemination of the disease. Recently, aggressive surgical approaches involving complex vascular resections and reconstructions have become more common, thus allowing more locally advanced tumors to be resected. Unfortunately, however, even after the completion of surgery and systemic therapy, approximately 40% of patients experience early recurrence of disease. To determine resectability, many institutions utilize anatomical staging systems based on the presence and extent of vascular involvement of major abdominal vessels around the pancreas. However, these classification systems are based on anatomical considerations only and do not factor in the burden of systemic disease. By integrating the biological criteria, we possibly could avoid futile resections often associated with significant morbidity. Especially patients with anatomically resectable disease who have a heavy burden of radiologically undetected systemic disease most likely do not derive a survival benefit from resection. On the contrary, we could offer complex resections to those who have locally advanced or oligometastatic disease but have favorable systemic biology and are most likely to benefit from resection. This review summarizes the current literature on defining anatomical and biological resectability in patients with pancreatic cancer.

A Current Status Between Laparoscopic and Robotic Resection of Pancreatic Malignant: A Systematic Review

Abstract:Background: Laparoscopic and Robotic approaches have become increasingly used for pancreatic surgery.The aim of this study is to evaluate compile and evaluate existing literature on the comparison of laparoscopic pancreatic surgery and robotic pancreatic surgery in the resection of Pancreatic neoplasms.The outcomes of each technique were quantified using meta-analysis.Study Design: A systematic review of articles in both PubMed and Embase comparing laparoscopic and robotic colorectal procedures was performed.Approaches were evaluated in terms of operative time, length of stay, estimated blood loss, conversion, number of lymph nodes harvested, and mobidity and mortality and pancreatic fistula.Mean net differences and effect of each group.Results: 232 were full-text articles were identified, and 47 met the inclusion criteria, representing 2753 patients: 690 patients who underwent robotic pancreatic surgery procedures and 2063 patients who underwent laparoscopic pancreatic surgery procedure.Operative time for the robotic approach was 20-45 minutes longer.The robotic group had lower estimated blood loss (57 ml), and patients were 1.85 times more likely to be converted procedure and average length of stay in hospital (14,395 vs 11.85) laparoscopic group was longer then robotic.But there was no real difference between the 2 groups in terms of number of day.There was no much real difference between groups with respect to number of lymph nodes harvested, mortality, or morbidity rate. Conclusions:The laparoscopic and robotic approach to pancreatic surgery is as safe and similar outcomes for both procedures.Further the robotic approach tended to have longer operating times, less blood loss, and a higher rate of conversion to an open procedure compared with laparoscopic procedure.On the basis of the findings of this meta-analysis, there does not appear to be any clear advantage of a robotic approach over a laparoscopic one for pancreatic surgery.

Abstract:Background and objective: Pancreatic ductal adenocarcinoma (PDAC) is the third-leading cause of cancer-related death in the United States and is projected to become the second-leading cause of cancer-related death by 2030. Despite advances in systemic and radiation therapy, for patients with surgically resectable PDAC, complete surgical resection is the only potentially curative treatment option. The conduct of a safe, technically excellent pancreatectomy is essential to achieve optimal perioperative outcomes and long-term survival. In this narrative review, evidence from large, well-executed studies and clinical trials examining the technical aspects of pancreatectomy is reviewed. Methods: A search was conducted in PubMed, Medline, and Cochrane Review databases to identify English-language randomized clinical trials, meta-analyses, and systematic reviews assessing surgical aspects of pancreatectomy for PDAC published between 2010 to 2023. Key content and findings: We identified retrospective and prospective studies evaluating the technical aspects of surgery for PDAC. In this review, we evaluate data on surgical techniques of pancreatectomy for PDAC, including the role of minimally invasive techniques, extent of lymphadenectomy, reconstruction options after pancreatoduodenectomy, and the role of surgical drainage. Conclusions: Surgical resection has a critical role in the treatment of operable PDAC. While pancreatic cancer surgery is an active area of research, conducting a technically excellent surgical resection maintains paramount importance for both oncological and perioperative outcomes. In this review, we summarize the latest evidence on surgical technique for operable PDAC.

Abstract:The incidence of pancreatic neuroendocrine tumors (PNETs) is on the rise primarily due to the increasing use of cross-sectional imaging. Most of these incidentally detected lesions are non-functional PNETs with a small proportion of lesions being hormone-secreting, functional neoplasms. With recent advances in surgical approaches and systemic therapies, the management of PNETs have undergone a paradigm shift towards a more individualized approach. In this manuscript, we review the histologic classification and diagnostic approaches to both functional and non-functional PNETs. Additionally, we detail multidisciplinary approaches and surgical considerations tailored to the tumor's biology, location, and functionality based on recent evidence. We also discuss the complexities of metastatic disease, exploring liver-directed therapies and the evolving landscape of minimally invasive surgical techniques.

Abstract:Pancreatic carcinoma is still a challenging disease. Assessment of resectability of the tumor is of importance in patient evaluation. The extent of lymphadenectomy in pancreaticoduodenectomy remains controvisal. Several RCT reports recommended dissection of the 2nd group of lymphnodes according to the JPS classification. Pancreaticoduodenectomy with resection and reconstruction of the vein is a common practice for the removal of pancreatic carcinoma if a R0/R1 resection is reasonably expected. To achieve an R0 margin is the main objective of pancreaticoduodenectomy as it is of great importance for extended survival. The clinical application of the concept of the mesopancreas will help to improve the status of retroperitoneal margins via standardized surgical procedures. The effect of a microscopically positive (R1) resection on ultimate clinical outcome is uncertain. Multimodality therapy may "recover"a R1 margin and improve survival similar to R0 resection. To improve the clinical outcome of the patient underwent pancreaticoduodenectomy,a standard system should be built to guide clinical practice.

Abstract:Pancreatic ductal adenocarcinoma (PDAC) is a challenging disease process with a 5-year survival rate of only 11%. Neoadjuvant therapy in patients with localized pancreatic cancer has multiple theoretical benefits, including improved patient selection for surgery, early delivery of systemic therapy, and assessment of response to therapy. Herein, we review key surgical considerations when selecting patients for neoadjuvant therapy and curative-intent resection. Accurate determination of resectability at diagnosis is critical and should be based on not only anatomic criteria but also biologic and clinical criteria to determine optimal treatment sequencing. Borderline resectable or locally advanced pancreatic cancer is best treated with neoadjuvant therapy and resection, including vascular resection and reconstruction when appropriate. Lastly, providing nutritional, prehabilitation, and supportive care interventions to improve patient fitness prior to surgical intervention and adequately address the adverse effects of therapy is critical.

Abstract:Pancreatic cancer is one of the most lethal malignancies and is associated with a poor prognosis. Surgery is considered the only potential curative treatment for pancreatic cancer, followed by adjuvant chemotherapy, but surgery is reserved for the minority of patients with non-metastatic resectable tumors. In the future, neoadjuvant treatment strategies based on molecular testing of tumor biopsies may increase the amount of patients becoming eligible for surgery. In the context of non-metastatic disease, patients with resectable or borderline resectable pancreatic carcinoma might benefit from neoadjuvant chemo- or chemoradiotherapy followed by surgeryPatients with locally advanced or (oligo-/poly-)metastatic tumors presenting significant response to (neoadjuvant) chemotherapy should undergo surgery if R0 resection seems to be achievable. New immunotherapeutic strategies to induce potent immune response to the tumors and investigation in molecular mechanisms driving tumorigenesis of pancreatic cancer may provide novel therapeutic opportunities in patients with pancreatic carcinoma and help patient selection for optimal treatment.

Abstract:Pancreatic head carcinoma is considered to have the worst prognosis , while curative pancreaticoduodenectomy ( PD ) is the only potentially curative treatment .Newly surgical concepts and therapeutic principles have been updated , including resection approach , modus as well as extent , to improve the R0 resection rate and to decrease the incidence of local recurrence and metastasis .No-touch isolation technique could prevent the invasion of tumor cells into the portal vein , lymphatic system or abdominal cavity, to decrease the incidence of local recurrence and metastasis .Various“artery first” approaches could help to evaluate the resectability , retroperitoneal lymphadenectomy and the early recognition of the hepatic artery.Total mesopancreas excision in PD could improve R 0 resection.These modified techniques were more advantageous than conventional surgical techniques .Because of retrospective data from a single center , further clinical investigation is necessary to improve the long-term outcome .

Abstract:The prognosis of patients with pancreatic carcinoma is the poorest of all the digestive system cancers, and the managements are very challenging. Now the treatment mode of pancreatic carcinoma is converting into “MDT” from “surgery first”. Neoadjuvant therapy is advocated for borderline resectable pancreatic carcinoma so as to increase rates of R0 resection. The standard lymphadenectomy is recommended in routine clinical practice except in special clinical trials. Pancreatic fistula is the severest complication of pancreatoduodenectomy. There are no significant differences in relation of methods of pancreatoenteric anastomosis to the occurrence of postoperative pancreatic fistula, so the quality of the anatomosis and the technical skills should be emphasized in order to decrease the incidence of pancreatic fistula.

Abstract:Pancreatic cancer is characterized by high malignancy,low early diagnosis rates,and poor prognosis.Approximately 15%-20%of patients at initial diagnosis have resectable pancreatic cancer,20%-30%have borderline resectable pancreatic cancer,and over 50%are present with unresectable.Surgical resec-tion remains the only potentially curative treatment option available,but the unique anatomical position of the pancreatic head necessitates complex gastrointestinal reconstruction after resection,which may lead to severe complications such as grade B/C pancreatic fistulas and bleeding.Comprehensive preoperative preparation and individualized postoperative management are crucial to reducing perioperative complications,enhancing patient quality of life,and expediting the initiation of adjuvant therapy.This article reviews perioperative integrated management strategies for patients with pancreatic cancer,drawing on literature and our center's experience,in-cluding nutritional therapy,psychological support,preoperative jaundice reduction,neoadjuvant treatment,the prevention and management of pancreatic fistulas and postoperative bleeding,with the aim of improving clinical outcomes and overall survival rates.

Abstract:With an overall 5-year survival rate of 12%, pancreas ductal adenocarcinoma (PDAC) is an aggressive cancer that claims more than 50,000 patient lives each year in the United States alone. Even those few patients who undergo curative-intent resection with favorable pathology reports are likely to experience recurrence within the first two years after surgery and ultimately die from their cancer. We hypothesize that risk factors for these early recurrences can be identified with thorough preoperative staging, thus enabling proper patient selection for surgical resection and avoiding unnecessary harm. Herein, we review evidence supporting multidisciplinary and multimodality staging, comprehensive neoadjuvant treatment strategies, and optimal patient selection for curative-intent surgical resections. We further review data generated from our standardized approach at the Mayo Clinic and extrapolate to inform potential future investigations.
