Sirolimus is a Successful Treatment for Recurrent Iron Deficiency Anaemia in Blue Rubber Bleb Naevus Syndrome

Abstract:To the Editor: We read with great interest the article by Warner et al, in which the authors reported a case of blue rubber bleb naevus syndrome (BRBNS) treated with sirolimus (1). Nevertheless, we would like to discuss some points on this case. In BRBNS, gastrointestinal bleeding is a common symptom. Therefore, iron deficiency anaemia can be the result of recurrent gastrointestinal bleeding. In the treatment of BRBNS, the severity of gastrointestinal bleeding represents something of a response to sirolimus (2). The severity of gastrointestinal bleeding was, however, not described in the article. Sirolimus has not been reported to be effective for iron deficiency anaemia. Instead, it has been reported that a case with BRBNS showed no gastrointestinal bleeding and muscular haematoma after sirolimus therapy. Our centre's experience is consistent with this result (2). In this article the authors, however, emphasised improvement of iron deficiency anaemia and not mentioned change of gastrointestinal bleeding after sirolimus treatment. In addition, authors did not provide direct evidence that sirolimus was effective for iron deficiency anaemia. Consequently, we think that the title “Sirolimus is a successful treatment for recurrent iron deficiency anaemia in blue rubber bleb naevus syndrome” is inappropriate. In addition, the data of gastrointestinal bleeding after sirolimus treatment should be supplemented.
