A Pilot Study on Detection of PRA in the Children with Severe-Thalassemia Using Enzyme-Linked Immunosorbent Assay in China

Abstract:[Abstract] Objective The panel reactive antibodies (PRA) in the children with severe -thalassemia were detected by enzyme-linked immunosorbent assay for evaluating sensitization in HSCT. Methods Fifteen patients with -thalassemia major were monitored from March 2005 to December 2005; 9 of them were males and other females, the means age was 6.15±4.01. All of subjects enrolled were exsanguinated 3 ml vein blood. The serum was collected and stored below -20℃.Serum PRA was detected by enzyme-linked immunosorbent assay (ELISA) including Lambda antigen tray mixed (LATM) and Lambda antigen tray (LAT) . Results Six of 15 patients serum were PRA positive; the strength ranged from 14% to 75% respectively, with the median PRA strength 47.5%; the positive rate of PRA was 40%. Children’s age and the period of blood transfusion were(8.67±3.95)and (96.00±44.99)months in the positive group which were significantly higher than those of the negative group; the later was (4.47±3.24)years and (40.33±35.14)months respectively,( P 0.05). By the Spearman test, the correlation between the PRA positive、PRA strength and child’s age, the starting time of blood transfusion, the period of blood transfusion, the times of blood transfusion was analyzed. There was correlation between the PRA positiveness and the times of transfusion, child’s age, the period of transfusion (r s =0.819, 0.536, 0.567; P 0.05). Conclusion The PRA positive rate and PRA strength were high in the children with severe -thalassemia. The times of transfusion, the child’s age, the period of transfusion in the positive group were significantly higher than those of the negative group. The older of the child’s age, the longer of period of transfusion, the more times of transfusion, the more chances of positive PRA and high PRA strength in the children with severe -thalassemia were in our study. PRA can cause graft rejection, it may be the result of the low engraft rate in children with -thalassemia for HSCT.
