Abstract:An 81-year-old male presented with eruptions on his scalp and face for 10 months. The clinical features revealed numerous violaceous papules, nodules and plaques. Histopathology and immunochemical positive staining for CD31, FⅧ and CD34 supported the diagnosis of angiosarcoma. He also had chest distress for 3 months, the imageology examination revealed numerous nodules in his lung. The final diagnosis was cutaneous angiosarcoma of the scalp and face with lung metastasis.
A rare case of angiosarcoma with skull masses and erythropenia and thrombocytopenia: A case report and review of literature.

Abstract:Rationale: Primary splenic angiosarcoma (PSA) is a rare, fatal neoplasm originating from sinusoidal vascular endothelial cells, and usually metastasizes and almost always has a poor prognosis. Surgical excision is the main treatment of this highly malignant disease. Patient concerns: We reported a special case of a 68-year-old female who had a 6-month history of scalp masses. Diagnosis: The patient was found to have 2 skull masses on computed tomography (CT). Laboratory findings revealed erythropenia and thrombocytopenia. Enhanced abdomen magnetic resonance imaging (MRI) showed multiple masses in liver and spleen. The pathological result of the skull masses was revealed to be metastatic angiosarcoma. Interventions: The patient underwent surgical excision of skull masses, and no subsequent radiotherapy or chemotherapy was done. Outcomes: The patient died due to dyscrasia at August 12, 2015, with a survival of nearly 1 month. Lessons: We highlight the importance for clinicians to be aware of this rare neoplasm, and to consider it in the differential diagnosis when encountering a skull mass. Early confirmation and treatment may improve the prognosis.
Angiosarcoma of the Face and Scalp in the Elderly: an Analysis of Seven Cases

Abstract:Objective To assess clinical features and treatment of angiosarcoma.Methods A retrospective study was carried out among seven patients hospitalized for angiosarcoma in the Department of Plastic Surgery,Zhongshan Hospital of Fudan University from January 2004 to March 2012.Clinical behaviors,therapeutic strategies and outcomes of angiosarcoma were analyzed in these patients.Results Of the seven patients,four were male,and three were female,with a median age of 67 years (range,37-77 years).Clinical presentations were various,including irregular ulcerous nodular masses in four patients,localized ulcer in one patient,and irregular erythematous nodules in two patients.All the patients were treated surgically with or without postoperative adjuvant therapy.Pathological examination revealed various degrees of differentiation and atypia of tumor cells,as well as obvious proliferation of endothelial cells,and all of these patients were diagnosed as angiosarcoma.Immunohistochemical analysis revealed a positive staining for CD31 and CD34.After surgical treatment,angiosarcoma recurred in four patients with the median time to the first recurrence being 3.5 months (range,1-6 months),cerebral metastasis occurred in one patient 5 months later,and two patients died.Conclusions Angiosarcoma is a rare and heterogeneous sarcoma with diverse clinical presentations.Local wide resection is the cornerstone of angiosarcoma treatment,and adjuvant therapy may improve its prognosis.
Angiosarcoma of the Head and Face: a Retrospective Study of 15 Patients

Abstract:OBJECTIVE:To investigate the clinical and pathological characteristics of angiosarcoma of the head and face.METHODS:The data of 15 patients with angiosarcoma of the head and face underwent treatment between January 1993 and January 2003 were reviewed. Of 15 cases 10 were men and 5 women; 11 tumors in scalp and 4 in face; 6 cases of stage I, 3 cases of stage II, 1 case of stage III and 5 cases of stage IV. Clinical symptoms included a focal mass or bruiselike patches. Surgery alone in 5 cases, chemotherapy alone in 3 cases, comprehensive treatment in 5 cases, no treatment in 2 cases. Immunohistochemical staining with CD31, CD34 and FVIIIRag was performed in 8 cases.RESULTS:Follow-up time was 3-240 months with a median of 28 months. One patient of stage I by surgery alone and 2 patients of stage I with comprehensive treatment survived for 121, 209 and 240 months respectively and no recurrence. One patient of stage I with comprehensive treatment was survival for 127 months with tumor burden. Eleven cases died in 3-78 months after diagnosis, the median follow-up time was 20 months. Three year and five year survival rates were 46.7% and 33.3%, respectively. Immunohistochemical staining showed CD31- and CD34-positive for 8 cases and FVIIIRag-positive for 6 cases.CONCLUSIONS:Angiosarcoma of the head and face is a high-risk sarcoma. It is rare and easily misdiagnosed but can be diagnosed properly based on pathological feature. Comprehensive treatment including surgery, radiotherapy and chemotherapy can improve survival rate for the patients with early stage angiosarcoma.

Abstract:While most forms of alopecia neoplastica are attributable to cutaneous metastases from visceral primary malignancies, rarely a diffuse primary skin cancer may present as alopecia. Herein, we present a case of angiosarcoma which clinically mimicked an inflammatory alopecia and was diagnosed by examination of alopecia‐protocol horizontal histologic sections. A 72‐year‐old female presented to her dermatologist with a chief complaint of hair loss and pruritus. She was noted to have alopecia with erythema of the scalp and was diagnosed clinically with lichen planopilaris. However, when multiple topical treatments were unsuccessful, her dermatologist performed a scalp biopsy to guide further alopecia therapy. Horizontally oriented histologic sections were prepared from the biopsy in standard alopecia fashion. Histopathologic examination revealed a proliferation of dissecting vascular channels lined by atypical endothelial cells, diagnostic of angiosarcoma. The patient underwent radiation and chemotherapy with clinical response and had stable disease for several years thereafter. This case highlights the importance of considering unexpected malignancies in the clinical and pathologic examination of cases of suspected inflammatory alopecia.

Abstract:Cutaneous angiosarcoma is a rare, yet a highly aggressive skin and soft tissue tumor with a predilection to the head and neck region in white-skinned elderly males1.Cutaneous angiosarcomas have varied clinical presentations and could mimic several clinical entities making the diagnosis a challenge. Upon clinical suspicion, diagnosis is confirmed with histopathology examination and immunohistochemistry. Early diagnosis improves prognosis. We report a case of cutaneous angiosarcoma mimicking facial deep fungal infection.

Abstract:Angiosarcoma is a rare, heterogeneous malignant tumor that derives from endothelial cells, and it has aggressive characteristics with a marked tendency for distant metastasis. Diffuse alveolar hemorrhage (DAH) is a catastrophic clinical syndrome, however, it is rare as the presentation of pulmonary angiosarcoma. To increase awareness with regard to angiosarcoma and DAH, the current study presents a case of angiosarcoma that originated from the subcutaneous soft tissue of the mastoid process, but was subject to a delayed diagnosis and rapid invasion into the brain and lung. The metastatic angiosarcoma of the lung presented with DAH as the initial manifestation. The pathological examination of a biopsy of the subcutaneous mass and pulmonary lesions confirmed the diagnosis of angiosarcoma. The patient succumbed to respiratory failure at 1 month post-diagnosis.

Abstract:Objective To investigate clinicopathological features of scalp angiosarcoma, and to analyze the relationship of tumor stage and treatment with prognosis. Methods Clinical and follow-up data were collected from 16 patients with non-metastatic primary scalp angiosarcoma treated in the Department of Plastic and Reconstructive Surgery of Zhongda Hospital, Southeast University from September 2002 to June 2013. According to the seventh edition American Joint Committee on Cancer (AJCC)TNM staging system for soft tissue sarcomas (2010), staging of scalp angiosarcoma was performed for the 16 patients. Statistical analysis was carried out by the Kaplan-Meier method for survival rates and by the Log-rank test for survival curve. The Cox regression model was used for multivariate regression analysis. Results Of the 16 patients, 4 had stageⅠangiosarcoma, 4 stage Ⅱangiosarcoma, and 8 stage Ⅲ angiosarcoma. The tumor usually began as ecchymosis-like lesions on the head or face in early stage, and progressed into dark red infiltrative plaques, nodules and ulcers which easily ruptured and bled in later stage. Histopathological examination showed generalized vascular proliferation and infiltration with high histological morphological diversity. Cytologic atypia was commonly seen. The median time to recurrence was 15 months, and local recurrence occurred in 7 patients. The median time to metastasis was 20.5 months, and distant metastasis was observed in 8 cases, including 4 cases of pulmonary metastasis, 2 lymph node metastasis, 1 liver metastasis and 1 bone metastasis. The survival time was 33.0 ± 4.4 months (median, 32 months)in 4 patients with stage Ⅰangiosarcoma, 24.0 ± 7.9 months(median, 15 months)in 4 patients with stage Ⅱangiosarcoma, and 23.9 ± 3.9 months (median, 24 months)in 8 patients with stage Ⅲ angiosarcoma. Additionally, the survival time was 23.4 ± 5.2 months(median, 21 months), 24.4 ± 5.7 months(median, 24 months)and 35.8 ± 9.7 months(median, 26 months) in 5 patients receiving surgical treatment alone, 7 patients receiving surgical treatment and radiotherapy, and 4 patients receiving surgical treatment, radiotherapy and immunotherapy, respectively. Conclusions Surgical treatment combined with radiotherapy and immunotherapy may serve as the first-choice treatment for scalp angiosarcoma. Tumor size and treatment regimens are main factors influencing prognosis of scalp angiosarcoma.
A Rare Cause of Pulmonary Nodules Diagnosed As Angiosarcoma Was Misdiagnosed As Vasculitis and Wegener's Granuloma in an Elderly Man: A Case Report.

Abstract:BACKGROUND:Angiosarcoma is a rare, highly malignant tumor prone to recurrence and metastasis. Angiosarcoma is insidious in the initial stage, and its clinical manifestation lacks specificity. The diagnosis is based on histopathology and immunohistochemistry findings.CASE PRESENTATION:A 73-year-old man was hospitalized following complaints of persistent cough 6 months and hemoptysis for 2 months. Anti-infective treatment was ineffective. A CT-guided percutaneous core needle biopsy of pulmonary lesions revealed organized pneumonia, and the removed skin of purpuric rash area on the left calf revealed vasculitis. Chest CT was used during the patient follow-up. Hormonal therapy combined with immunoglobulins did not lead to improvement, and there was rapid progression of the lung lesions. Subsequently, the patient underwent a surgery, the diseased tissue was separated and removed completely beside the left submandibular gland under local anaesthesia. The immunohistochemical staining indicated CD31 (+) and CD34 (+) confirming a diagnosis of metastatic angiosarcoma. The expression of PD-L1 was 70%, therefore, anlotinib and pembrolizumab treatments were initiated. The patient eventually died.CONCLUSION:Angiosarcoma is a malignant tumor in the clinic that lacks typical and specific signs and symptoms. The diagnosis depends on immunohistochemistry, which requires repeated biopsies of multiple sites in highly suspected cases.
Long-term Survival of a Patient with Scalp Angiosarcoma and Multiple Metastases Treated Using Combination Therapy: A Case Report.

Abstract:Angiosarcoma is a rare and deadly malignancy originating from the vascular endothelial cells. Surgery is the most effective method to cure this disease, but for metastatic angiosarcoma, a chemotherapy-based treatment is the main therapeutic choice. However, there is currently no standard chemotherapy regimen. The current study reports the case of a 66-year-old male with post-operative scalp angiosarcoma recurrence and multiple metastases. The patient obtained a complete response to first-line combination chemotherapy consisting of cyclophosphamide, epirubicin, vincristine and dacarbazine, with a progression-free survival time of eight months. After benefitting from subsequent comprehensive treatment including, cyclophosphamide, epirubicin, vincristine, dacarbazine, docetaxel, cisplatin, gemcitabine and radiotherapy and anti-angiogenic therapy, the patient obtained an overall survival time of 38 months following initial diagnosis.

Epithelioid Sarcoma of the Scalp: a Case Report and Literature Review.

Abstract:Epithelioid sarcoma (ES) is an aggressive mesenchymal tumor exhibiting bi-directional phenotypes. According to its proclivity for different sites, ES can be subdivided into two categories: distal and proximal variants. Proximal ES often affects the truncal tissue, thighs, head, and neck. Accumulating evidence indicates that several locations in the head, including the orbital, gingival, and nasal cavities, are involved in ES. However, the underlying mechanisms of ES carcinogenesis and progression are largely unknown, including and especially the reason why the tumor cells are positive for both epithelial and mesenchymal classical markers. Thus, we wish to share a rare case of ES in the scalp and its clinical and molecular features. Only 9 cases to date have been reported. An 80-years-old man had sustained a painful swollen mass in his scalp for three months. A diagnosis of epithelioid sarcoma was established based on the combination of the histopathological and immunohistochemical findings. The tumor cells were positive for both mesenchymal (vimentin and S100) and epithelial markers (pan-cytokeratin). This case suggests that ES can be derived from the soft tissue of the scalp. The tumor cells co-expressed biomarkers of epithelial and mesenchymal cells, suggesting the mesenchymal-epithelial transition (MET) may be involved.

Abstract:Angiosarcoma is a soft tissue sarcoma of vascular origin, with more than half of the cases arising in the skin and affecting primarily the face and scalp of elderly males. Furthermore, cutaneous angiosarcoma exhibits a higher incidence of lymph node metastases than other types of sarcomas. Angiosarcomas are rarely aspirated and are occasionally encountered on cytological samples. It is a diagnostic challenge in evaluating fine needle aspiration (FNA) from a metastatic angiosarcoma without the knowledge of prior history. We present a case of scalp angiosarcoma with disease progression to erythroderma and cervical lymphadenopathy 20 months after. FNA of the cervical node revealed vasoformative features, including hemophagocytosis, formation of an intracytoplasmic lumen/vacuole, endothelial wrapping, and cell grasping. The diagnosis of nodal metastasis by angiosarcoma was confirmed with immunohistochemistry (IHC) using two vascular markers on cell block sections. Our case demonstrates the recognizable cytomorphologic clues for this rare metastatic malignancy.

A Case Of Retiform-Hemangioendothelioma With Unusual Presentation And Aggressive Clinical Features

Abstract:Retiform hemangioendothelioma (RH) is an extremely rare low-grade angiosarcoma mainly involving the skin and subcutaneous tissue. Clinically patients often present with an asymptomatic slow-growing solitary nodular or plaque-like lesion. RH is characterized by frequent local recurrences but a very low metastatic rate. Here we reported a case of RH in a 61-year-old Chinese woman who presented with a rapid growing cutaneous plaque-like lesion on her right scalp, followed by another lesion behind the right ear. The lesions were associated with paroxysmal sharp needle-stabbing like headache. She underwent wide excision and skin engraftment. Three months post surgery, she experienced tumor recurrence, and died 9 months after the initial diagnosis.

Abstract:Synovial sarcoma (SS) arising primarily in the scalp was scarcely reported. Here we described a subcutaneous mass in the right parietal region of the scalp of a 39year-old male patient. It has been present for 10years and enlarged recently. It was initially suspected as hemangioma by magnetic resonance imaging (MRI), but the histological examination revealed a sarcoma rich of spindle cells reminiscent of SS. Therefore, immunohistochemistry, florescence in site hybridization (FISH), reverse transcription-polymerase chain reaction (RT-PCR) and DNA sequencing were performed to determine the final diagnosis as biphasic SS. The detailed description of primary scalp SS might help differential diagnosis of scalp soft tumors.
Clinical outcomes in primary scalp angiosarcoma.

Abstract:Scalp angiosarcoma is a rare, extremely aggressive cutaneous malignancy with poor patient prognosis. The present study reviewed the cases of 42 patients who presented scalp angiosarcoma and were treated at the Zhongshan Hospital of Fudan University between January 2002 and December 2013. The clinical characteristics, demographics, treatment regimens and outcomes of patients were analyzed, and the overall survival (OS) and recurrence-free survival (RFS) rates were calculated. A total of 42 patients were examined in this study. Surgery was the most common therapeutic measure, and was performed in 39 patients, alone (12 patients), in combination with chemotherapy (14 patients), radiotherapy (6 patients) or 3-modality-therapy (7 patients). The median follow-up time of patients was 28.5 months. The 5-year OS rate was 19%, and the 5-year RFS rate was 10%. Taken together, the results of the present study suggested that patients whose tumor presented a nodular localized lesion had a significantly improved OS rate (P=0.0078). Patients aged >= 70 years were associated with a lower 5-year OS (P=0.0071) and RFS rates (P=0.0095) vs. patients aged <70 years. Different treatments were not identified to be significantly associated with an improved OS or RFS. The present results also indicated that if the tumor presented nodular localized lesions, the patients exhibited a better prognosis than those with a diffuse lesion. Although younger patients had better clinical outcomes, the likelihood of recurrence and mortality remained high for all patients.
Surgical Treatment and Prognosis of Angiosarcoma of the Scalp: A Retrospective Analysis of 14 Patients in a Single Institution

Abstract:Objective . We describe specific surgical methods for angiosarcoma regarding extent of resection and reconstructive options and assess their effect on patients’ prognosis. Patients and Methods . We retrospectively examined 14 patients undergoing treatment for angiosarcoma of the scalp at our institute between January 2000 and June 2015. Surgical treatment comprised wide excision of the tumor and reconstruction using a free flap with skin graft. Kaplan-Meier survival analysis was used to assess the survival parameters. Univariate and multivariate analyses were performed to evaluate the association between risk factors and outcome parameters. Results . Mean patient age at diagnosis was 69 years, and the mean follow-up period was 17 months. The overall 5- and 2-year survival rates were 15% and 75%, respectively, whereas the 5- and 2-year disease-free survival rates were 7.7% and 38.7%, respectively. The mean survival duration was 32 months. Metastatic tumor dissemination to the lung or brain was closely associated with the major cause of death. Only a deep excision margin was significantly related to the recurrence rate. Conclusions . Cases of angiosarcoma had a poor prognosis despite the aggressive treatments. Sufficient resection margins are essential for controlling local recurrence. The effect of multidisciplinary approaches needs to be explored.

A Case of Nasal Big Angiosarcoma

Abstract:Angiosarcoma also called malignant hemangioendothelioma, caused by vascular endothelial cells or in the direction of vascular endothelial cell differertiation of mesenchymal malignant tumor, is a rare tumor found in clinical, can be in any place of the body, such as head and neck, skin and tissue, breast, bone, liver and other organs and so on, also in the nose, and sinuses rich blood vessels. But this tumor has a low incidence in the nose. Because the nose and sinuses location is hidden, the patient often with nasal bleeding, stuffy nose, headache and so on. With the increase of tumor involvement, the patient can appears the symptoms of corresponding adjacent tissue. Therefore this tumor is difficult to be early detected timely. Early complete surgical excision and supplemented by radiotherapy are preferred methods as the treatments of the nasal and sinuses angiosarcoma without distant metastase.
Diagnosis and treatment of pulmonary angiosarcoma(1 case report and a literature review)

Abstract:Objective To review the clinical features,pathological characteristics,diagnosis and differential diagnosis and treatment of pulmonary angiosarcoma. Methods The clinical data in 5 cases was retrospectively analyzed with a literature review. Results The clinical symptoms of pulmonary angiosarcoma were chest pain、cough and emptysis.Multiple nodules and plaque may be seen on Chest CT.Microscopically,the tumor was composed of blood vessel endothelium cells with obvious dysplasia,many irregular blood vessels varied in size were inosculated each other and formed vascular web.The immunohistochemical results showed positive CD34、CD31 and vimentin positive in cases. Conclusion Pulmonary angiosarcoma is a rare and high malignant soft tumor with poor prognosis,the diagnosis mainly depends on pattology.It needs to be differential diagnosted with lung adenocarcinoma,pulmonary tuberculosis,and mycosis etc.
Skull Sclerosing Epithelioid Fibrosarcoma: A Case Report and Review of the Literature.

Abstract:Sclerosing epithelioid fibrosarcoma (SEF) is an unusual variant of fibrosarcoma that was previously considered to be a low-grade tumor with an indolent course. The tumor occurs most commonly in the soft tissue of the limb, trunk, head and neck, and occasionally in the bone and visceral organs. The skull is a rare primary site for SEF, with only 3 cases reported to date. The current study reports a case of SEF occurring in the occipital bone of a 24-year-old man, who lacked neurological symptoms. Imaging revealed a large mass emanating from the occipital bone and involving the superior sagittal sinus, torcular herophili and adjacent brain tissue. Histological and immunohistochemical characteristics confirmed the diagnosis of SEF. The patient experienced local recurrence and distant metastasis at 10 and 15 months, respectively, subsequent to the resection of the primary mass. The current case and review of the literature suggest that skull SEF may behave clinically as an aggressive malignant sarcoma. Radiological findings indicated the biological and histopathological characteristics of the tumor. Thus, its clinical behavior and certain imaging features may suggest this diagnosis.
