Abstract:OBJECTIVE:To describe the histologic and clinical findings of recurrent intraosseous dentinogenic ghost cell tumor (DGCT) and its malignant transformation.METHODS:The clinical features, treatment, pathology and prognosis of recurrent intraosseous DGCT patients from 53 cases of mixed odontogenic tumors with ghost cells and mesenchymal inductive components were retrospectively reviewed.RESULTS:Ten recurrent cases from 14 DGCT were all male and the age at diagnosis ranged from 18 to 60 years with an average of 32.4 years. The main manifestation was progressive bone bulging. Radiographically the tumor was characterized by ill-defined radiolucency. Most recurrent tumors had entrenched the surrounding tissues. Seven cases occurred in the maxilla among which 2 cases were transformed into ghost cell odontogenic carcinoma and 2 showed features of increased cell proliferation. Three cases occurred in the mandible among which 1 case showed a high proliferative activity. The initial surgery of all recurrent cases was curettage.CONCLUSION:The microscopic findings and prognosis suggested that intraosseous DGCT was locally aggressive. Multiple recurrences increased the risk of malignant transformation. Curettage alone might not be adequate for the management of DGCT. Maxillary cases with ill-defined borders, especially those showing an increased proliferative activity in biopsy or frozen section examination, should be treated more radically.

Abstract:Dentinogenic ghost cell tumor is a rare benign tumor that accounts for less than 3% of all cases and consists of the stellate reticulum, which is made up of enamel epithelioid and basaloid cells. Although DGCT is a benign tumor, the local infiltration of the odontogenic epithelium or recurrences have been reported, and its detailed pathology and treatments remain unclear.

Abstract:We are reporting two cases of dentinogenic ghost cells tumor (DGCT). Case 1: A 55-year-old female, who is a smoker, presented with a painless lesion in the left maxilla following a tooth extraction. Physical examination revealed an enlargement in the left maxillary region with buccal and palatal bone protrusion. A tomography scan was performed, which showed a hypodense image causing bulging/thinning (BT) of the floor of the maxillary sinus and left orbital area. An incisional biopsy was carried out, followed by the enucleation of the lesion.

Abstract:Dentinogenic ghost cell tumor (DGCT) is an infrequent locally invasive odontogenic neoplasm, recognized as a solid variant of the calcifying odontogenic cyst. An 11-year-old female patient presented with an intraosseous mandibular lesion, which was incidentally discovered on a panoramic radiograph taken for orthodontic documentation. No intraoral swelling was noted clinically; however, tooth 47 was conspicuously absent. Further evaluation through cone-beam computed tomography revealed a well-defined mixed mass, measuring 2 cm, in the region of tooth 47 and confirmed its absence.

Abstract:Dentinogenic ghost cell tumor (DGCT) is a rare tumor of odontogenic origin. A locally invasive lesion can be described by the presence of ameloblast-like epithelial islands, ghost cells, and dentinoid material. It is one of the few lesions with a predilection for the Asian population. The available literature has revealed that only 131 cases to date have been reported and published from 1968 to 2022. The following is a case report of a 25-year-old male with a tumor in the left lower back teeth region for the past 1 month. Orthopantomogram (OPG) reveals a well-defined unilocular radiolucency extending anteroposteriorly and crossing the midline. Histopathology revealed basal ameloblast-like cells and central stellate reticulum-like cells with the characteristic presence of ghost cells. The diagnosis was made based on the clinical, radiographical, and histopathological correlation and was confirmed using immunohistochemical analysis as a DGCT.

Abstract:A dentinogenic ghost cell tumor (DGCT) is a rare and locally aggressive benign mixed odontogenic tumor, histologically made up of ameloblast-like epithelial islands, ghost cells, and dentin-like material. This is a highly unusual example of DGCT combined with an odontoma in a 13-year-old female patient affecting the right maxilla. On radiographic examination, ill-defined radiolucency with right maxillary sinus obliteration and involvement of permanent maxillary right first and second molars were noted. The second molar was pushed towards the orbital fossa, and root resorption of the first molar is associated with multiple radiopaque masses. The excision specimen showed multiple tooth-like structures histologically, including dentin-like areas and dental pulp. Only five cases of a DGCT with an odontoma have been documented in the literature, making it a very unusual condition.

Abstract:Dentinogenic ghost cell tumor (DGCT), also referred to as odontogenic ghost cell tumor, is a rare tumor classified as a neoplastic variant of calcifying odontogenic cyst (COC). Ghost cell odontogenic carcinoma (GCOC) is the especially rare malignant counterpart of DGCT and COC. The case of a middle-aged male with a maxillary DGCT which transformed to GCOC after 5 recurrences during a 21-year period is presented. When the diagnosis of GCOC (first identified as malignant after 17-year recurrence) was made, the tumor cells showed more obvious atypia, necrosis, and numerous mitoses, and interestingly the ghost cell was hard to find and even disappeared. Abstract:Dentinogenic ghost cell tumor (DGCT) is a rare benign neoplasm form of calcifying odontogenic cyst (COC) characterized by ghost cells. Although benign, it presents an aggressive behavior. DGCT accounts for 2% to 14% of all COCs and less than 0.5% of all odontogenic tumors. It is a benign odontogenic tumor despite its local invasion and the likelihood of recurrence. To detect recurrence, central DGCT patients must be monitored long-term. We present the case of a 51-year-old male who reported pain in the right upper back tooth region. On examination, a soft to firm, bright red swelling was present in the buccal vestibule and gingival margin of the maxillary right first and second molar, which extended up to the palate. Histopathological analysis confirmed the diagnosis of a DGCT, which occurred in a previously treated calcifying odontogenic cyst. The case is reported here, along with a review of the literature update of such recurred instances in the past.

Abstract:A dentinogenic ghost cell tumor (DGCT) is a rare benign odontogenic tumor that commonly shows characteristics of solid proliferation and has a relatively high risk of recurrence after surgical treatment. We herein report a case of a central DGCT that occurred in the maxilla and resulted in bone expansion. This study highlights new imaging findings (particularly magnetic resonance imaging) along with histopathological observations. In addition, we conducted a review of the existing literature on this rare tumor. A 37-year-old man developed swelling around the right cheek. A benign odontogenic tumor such as ameloblastoma was suspected based on the imaging examination findings (including bone expansion and the internal characteristics of the tumor) on panoramic imaging, computed tomography, and magnetic resonance imaging. The lesion was surgically excised from the right maxilla. Postoperative histopathological examination led to a definitive diagnosis of central DGCT. The tumor comprised epithelial neoplastic islands, resembling ameloblastoma, inside tight fibroconnective tissue; masses of ghost cells and formation of dentin were also observed. We had suspected that the minute high-density region around the molars on the imaging examinations represented alveolar bone change; however, it represented dentin formation. This led to difficulty diagnosing the lesion. Although DGCT may present characteristic findings on imaging examinations, its occurrence is infrequent, and in some cases, the findings may include the presence or absence of an impacted tooth without obvious calcification. The present case suggests that we should consider the possibility of an odontogenic tumor with calcification when high-density structures are observed inside the lesion.

Abstract:The clinical data of duodental stromal tumors were analyzed retrospectively.Most of the duodental gastrointestinal stromal tumors were located in the horizontal part of duodenum(47.05%)and descending part of duodenum(29.41%),alimentary tract hemorrhage was the common clinical manifestation(64.70%).The positive rates of CD117 and CD34 were 94.12%(16/17),88.23%(15/17).5 patients who were middle and high in Fletcher classification died to the end of visitation.

Abstract:We present a rare case of a dentinogenic ghost cell tumor (DGCT) associated with a compound odontoma (CO) in a 13-year-old male patient, affecting the anterior region of the maxilla. A computed tomography scan revealed a well-defined, unilocular hypodense lesion that involved tooth-like structures of varying shapes and sizes, leading to divergence of the roots of teeth 11 and 12. An excisional biopsy was performed, and the histopathological analysis revealed the presence of dentin, enamel matrix, and loose connective tissue resembling dental pulp.

Abstract:Dentinogenic ghost cell tumors are rare tumors, and few cases of them were reported in the literature. The presence of pigment in odontogenic lesions is a rare unexplained histological finding. In this report, we describe a unique case of a 7-year-old girl that was referred to the Department of Oral and Maxillofacial Surgery complaining of a left mandibular swelling. Clinical examination revealed a huge, ulcerated mass. Both incisional and excisional biopsies revealed a benign infiltrative odontogenic tumor with admixed ameloblast-like cells and pigmented ghost cells, consistent with a pigmented dentinogenic ghost cell tumor. To the best of our knowledge, this is the youngest case of intraosseous dentinogenic ghost cell tumor reported in the English literature and the second report of a pigmented variant. This rare variant should be included in the differential of pigmented odontogenic lesions to avoid misinterpretation, especially in small biopsies.

Abstract:The objective of this study was to summarize diagnostic points and treatment strategies for diffuse tenosynovial giant cell tumours (D-TSGCTs) of the temporomandibular joint (TMJ), and to evaluate the expression of proteins related to bone destruction and recurrence. The clinical and histopathological characteristics of 24 cases were analysed retrospectively. TRAP staining and immunohistochemical staining for MMP-9, MMP-13, and Ki-67 were performed. The median age of the patients was 45.5 years; the female to male ratio was 1.7:1. In 11 cases (45.8%), skull base destruction seen on computed tomography was confirmed by surgery. Computer-assisted navigation was performed in six cases. Four patients received adjuvant radiotherapy after first surgery. Five patients had recurrent lesions. Multinucleated giant cells were positive for TRAP, MMP-9, and MMP-13. The average Ki-67 index of the recurrent cases was significantly higher than that of the non-recurrent ones (P<0.05). This study demonstrates the aggressive and recurrent nature of D-TSGCT occurring in the TMJ. Computer-assisted navigation is helpful to protect vital structures and determine margins. Adjuvant postoperative radiotherapy is recommended for local control of residual or recurrent tumour. In conclusion, MMP-9 and MMP-13 may play a role in bone destruction of D-TSGCT, and the Ki-67 index has predictive significance for recurrence.
Diffuse Tenosynovial Giant Cell Tumours in the Temporomandibular Joints

Abstract:OBJECTIVE:To retrospectively analyze the clinical features, treatment and prognosis to the diffuse tenosynovial giant cell tumor (D-TSGCT) arising from the temporomandibular joint (TMJ), and to give a reference for the early diagnosis and treatment of this disease.METHODS:In this study, 15 patients finally diagnosed as D-TSGCT of TMJ histopathologically at the Peking University Hospital of Stomatology from October 2003 to August 2015 were selected and reviewed. Their clinical manifestations, imaging and histological features, diagnoses and differential diagnoses, treatments and follow-ups were summarized and discussed.RESULTS:D-TSGCT of TMJ showed obvious female predominance (12/15), the main symptoms included painful preauricular swelling or mass, limited mouth-opening and mandibular deviation with movement. D-TSGCT on computed tomography (CT) scan often showed ill-defined soft tissue masses around TMJ, enhancement after contrast administration, usually with widening of the joint spaces and with bone destruction of the condyle, the fossa and even the skull base. On magnetic resonance images (MRI), the majority of lesions on T1 weighted images and T2 weighted images both showed the characteristics of low signals (6/11). The lesions could extend beyond the joints (9/11) and into the infratemporal fossa (4/11) and the middle cranial fossa (4/11). Surgical resection was performed in 14 cases and biopsy in 1 case. Postoperative radiotherapy was performed in 3 cases. In follow-ups, 3 cases showed recurrence postoperatively.CONCLUSION:D-TSGCT arising from TMJ should be differentiated with TMJ disorders, other tumors and tumor-like lesions of TMJ and parotid neoplasms, etc. CT and MRI examinations have important values in the diagnosis and treatment design of D-TSGCT. Because of the local aggressive and extensive behavior, complete resection should be performed as soon as possible. Postoperative radiotherapy was helpful for the extensive lesions including destruction of skull base and may be a good supplementary therapy. Because of the possibility of recurrence and malignancy, long-term follow-up was suggested.

Abstract:Giant cell tumour (GCT) of bone is still one of the most obscure and intensively studied tumours of bone. The histogenesis of GCT remains unclear. The recommended therapy of GCT evolved during the 20th century. The best treatment should ensure local control and maintain function. Curettage has been the preferred treatment for most GCTs. Good results have also been published on the use of high-speed burr and local adjuvants. Local tumour control can be satisfactorily achieved by wide excision. However, treatment options for GCT have remained fairly static over the past 30 years and there is no widely held consensus regarding the standard treatment selection for all patients. This challenge may result from the fact that there are no single clinical, radiographic, histological or morphological aspects that allow surgeons to accurately predict the trend of a single lesion to recur. In this research, a comprehensive review of the previously described radiographic staging systems by Enneking and Campanacci et al. and the shortfalls associated with them are provided, and then the possible risk factors of predicting local recurrence or evaluating functional outcome of GCT are also discussed. A new preoperative evaluating system of GCT may be necessary and feasible, so that surgeons may accurately assess the aggressiveness or severity of GCT in order to reliably guide treatment decisions and predict outcomes.

Abstract:Objective:To investigate the possibility of pulp regeneration in vivo with absorbale gelatin sponge as the scaffold,and dental seed cells,induced molecules and MTA closed.Method:Clinical Irreducible pulpitis patients 143 cases,removed the crown infection pulp and retained the root living pulp,randomly divided into two groups,experimental group,absorbable gelatin sponge as the scaffold and MTA closed,control group,calcium hydroxide coverage,obsered symptoms situation,pulp situation Result:Symptom improved situation,pulp vitality situation 3 months after surgery,the experimental group was significantly better than the control group(P 0.05).Conclusion:Pulp regeneration in vivo is possible with absorbable gelatin sponge as the scaffold and MTA induced.
Application of Dental Applicator Treating Malignant Tumors Around Jaw

Abstract:Objective:To study the application of dental applicator containing 125I to treat malignant tumors around jaw.Methods:8 patients who had been treated in Peking university hospital of stomatology since 2004.All patients had pathology diagnosis before implantation.There were 3 squamous cell carcinomas of gingiva,3 adenoid cystic carcinomas and 2 mucousepidermal carcinomas of hard palate.Before implantation,all patients accepted CT examination.Then the plan through TPS was made.Only three patients received intertissual 125I radioactive seeds implantation under local anesthesia.All patients wore the dental applicators.All patients accepted CT examination in one week,2 months and 6 months after implantation.Patients were asked to answer a form about the life quality during treatment period.Results:After 9～31 months(median 12 months),the malignant tumor of the patient who just received brachytherapy reduced 92% in volume,and the pain was significantly decreased.Malignant tumors of other patients didn't relapse.Three patients had ulcers in mucosa of oral cavity.All patients didn' t have other complications.Furthermore the quality of life was improved.Conclusion:The brachytherapy of dental applicator containing iodine-125 seeds is a feasible way to treat malignant tumors of gingival and hard palate,and it is an effective and convenient method in combined therapy to treat malignant tumors of tissues around jaws,with the advantage of preserving jaw's functions and reducing complications.

Diagnosis and Treatment of A Hereditary Gingival Fibromatosis Case

Abstract:Hereditary gingival fibromatosis (HGF) is a rare condition characterised by severe gingival hyperplasia, which could result in serious aesthetic and emotional problems and functional impairment. Here the present authors report a case of a 28-year-old female patient with generalised severe gingival enlargement covering almost all of the teeth and diagnosed as HGF. Her family history was of significance, since her father and 3-year-old daughter suffered from the same symptoms. Many studies have agreed that surgical removal should be used in the treatment ofHGF, and gingivectomy is the most common method. This study tried both external and internal bevel incisions. The results suggest that the former is better for shaping gingival contour, if the attached gingiva is adequate. Correct physiological contour of the marginal gingiva, good oral hygiene and periodic recall can decrease recurrence risk. Post-surgical follow-up after 26 months demonstrated no recurrence and the patient was satisfied with her appearance.
