Abstract:OBJECTIVE:To investigate the surgical techniques of divided nevus of the eyelids with different locations and sizes.METHODS:Retrospective observational case series. Thirty cases (30 eyes) of divided nevus of the eyelids in Zhongshan Ophthalmic Center from July 1997 to November 2006 were retrospectively reviewed. There were three different surgical methods. Excision of the nevus and direct suture after excision in 3 cases. Excision of the nevus and free skin graft transplantation in 5 cases. Excision of the nevus and transfer of skin flap, which included a two stages reconstruction, was performed in 22 cases. The upper or lower eyelid was treated first and another eyelid was treated after three months. Lacrimal punctum and lacrimal canaliculus were excised if they were involved in 8 cases among 22 cases.RESULTS:The mean follow-up time was 60 months (ranging from 2 years to 11 years). Only one case recurred after two years during follow-up and was excised after five years. Pathologic examination indicated malignant transformation. Excision with histopathological control was performed and the nevus did not recur during follow-up time. Recurrence or malignant transformation did not occur in other cases. Aesthetical results of skin flap transplantation were better than skin graft transplantation.CONCLUSIONS:The results of two stages reconstruction and transfer of skin flap after excision of the divided nevus are better than free skin graft transplantation. Close observation is required after excision of the divided nevus for early detection of malignant transformation.

Abstract:Currently, there is a lack of comprehensive data regarding clinicopathologic characteristics and surgical treatment of divided nevus of the eyelids. Here clinicopathologic features and reconstructive procedures performed on 73 patients were described. We showed that the lesion was randomly located on both eyes with a predominant incidence in females. Affected regions included the following: eyelid only (n = 40, 54.8%); medial canthus involvement (n = 4, 5.5%); medial canthus, caruncle, and bulbar conjunctiva involvement (n = 5, 6.8%); lateral canthus involvement (n = 18, 24.7%); full eyelids (n = 6, 8.2%); and cheek extension (n = 9, 12.3%). Lesions presented as a pigmented macule (n = 18, 24.7%) or mass (n = 55, 75.3%). Ptosis was present in 26 (35.6%) cases leading to visual field defects (n = 9, 12.3%). The puncta were covered in 13 (17.8%) cases. Overall, surgical results were satisfactory. Pathologically, lesions were either intradermal (n = 40, 54.8%), junctional (n = 15, 20.5%), or compound (n = 18, 24.7%). Because of the lack of systemic data regarding the rare entity of divided nevus of the eyelids, this study improves our understanding of this lesion, and it should be treated with individualized reconstructive procedures. Incorporation of pediatric patient population in the future will further extend our knowledge in dealing with this disorder.
Reconstruction of Eyelid Defect Using Periosteal Stripe from Lateral Orbital Rim As Substitute of Tarsus

Abstract:OBJECTIVE:To evaluate the results of eyelid defect reconstruction using periosteal stripe of the lateral orbital rim.METHODS:This was a retrospective analysis of eyelid defects reconstruction in 13 patients with lateral canthus eyelid defect following the excision of the eyelid tumors including: seven patients with basal cell carcinoma, four patients with sebaceous cell carcinoma and two patients with squamous cell carcinoma. All eyelid defects were reconstructed with zygoma periosteal stripe as the posterior layer of the eyelid. The anterior layer of the eyelid was reconstructed by myocutaneous flap.RESULTS:After follow-up at 8 to 56 months, all reconstructed eyelids showed a normal appearance with good function and without deformities, e.g. eyelid notch, entropion and ectropion. There was no relapse of the eyelid tumor. All patients were satisfied with the results.CONCLUSIONS:This method is good for repairing lateral eyelid defect involving 1/3 to 2/3 eyelids and without the tumor invasion of lateral orbit rim. This procedure obtains satisfactory results, especially for simultaneous reconstruction of upper and lower eyelids defects.
Reconstruction of Hemifacial Congenital Giant Nevus with Pre-expanded Scalp Flaps and Deltopectoral Skin Flaps

Abstract:Background: The hemifacial congenital giant nevus impacts both physical and mental health of the patients. Excision is typically the most suitable option in these situations, but reconstructing the subsequent surgical defects is always a serious challenge. Methods: Between February 2012 and January 2021, a retrospective review of 4 patients who suffered from hemifacial congenital giant nevus was conducted, and they were treated by pre-expanded scalp flap and deltopectoral flap simultaneously. All patients receive tissue expansion, nevus resection, expanded skin flap transfer, and pedicle division. Results: Four patients with hemifacial congenital giant nevi were successfully treated with no major complications. One patient with a transferred deltopectoral flap experienced distal necrosis of the flap, and healed after dressing changes. No recurrence of the nevus was found during the follow-up period, and the transferred skin flaps match well with facial skin in contour and color. Conclusion: This modified pre-expanded scalp flap combined with a deltopectoral flap provides an easy and reliable way for hemifacial reconstruction in patients with a congenital giant nevus.

[The Design and Technique of One Stage Treatment of Eyelid Divided Nevus with Island Skin Flap].

Abstract:OBJECTIVE:To investigate the design and technique of the one stage treatment of eyelid divided nevus with adjacent and distant island skin flap.METHODS:Under local or general anesthesia, the divided nevus on the upper and lower eyelid were excised totally or partially. According to the location, shape and size of the defect, orbicularis oculi musculocutaneous flap, postauricular SMAS-pedicled flap or reversal superficial temporal artery flap was chosen to repair the defect in one stage. The donor site was closed directly.RESULTS:Since 2003, 16 patients were treated in this group, with 10 orbicularis oculi musculocutaneous flaps, 3 postauricular SMAS-pedicled flaps, and 3 reversal superficial temporal artery flaps. The largest size of the nevus on the upper and lower eyelid were 2.5 cm x 2.0 cm and 4.0 cm x 3.0 cm, respectively. One postauricular SMAS-pedicled flap and 1 reversal superficial temporal artery flap showed distal venous refluence obstruction and epidermal necrosis in early postoperative stage, which healed through dressing. The other flaps survived completely.CONCLUSIONS:Treating eyelid divided defects with adjacent or distant island skin flap is a suitable method with satisfactory result and less morbidity in donor sites.

Abstract:Purpose: Three different techniques used in 10 cases of divided eyelid nevus and the results are reported and discussed. Methods: A retrospective study of 10 patients who had undergone treatment of divided eyelid nevus with skin flap between March 2003 and March 2008 was conducted. According to the location, shape, and size of the upper and lower eyelids nevus orbicularis oculi musculocutaneous flap, postauricular superficial musculoaponeurotic system (SMAS)-pedicled flap, or reversal superficial temporal artery (STA) flap was designed to repair the eyelid defects. Results: Ten patients were treated in this series with 10 oculi musculocutaneous flaps, 3 postauricular SMAS-pedicled flaps, and 3 reversal STA flaps. One postauricular SMAS-pedicled flap and I reversal STA flap showed distal flap venous refluence (reflux) obstruction and partial epidermal necrosis in the early postoperative stage, but after active treatment, they healed well and needed no surgical revision. All the other flaps survived well. Four flaps showed slight trapdoor deformation needing no revision. No other complication developed during the follow-up periods of 6 to 36 months. All the donor sites were closed directly and healed well with inconspicuous scarring. This series has exhibited satisfactory cosmetic and functional results. Conclusions: Treating divided eyelid nevus with well-selected skin flaps is satisfying with excellent color and texture matching, normal eyelid function, and inconspicuous donor deformity.
Clinical and Immunohistochemical Analysis of the Verrucous and Non-Verrucous Divided Nevus of the Eyelids

Abstract:Purpose Divided nevus with verrucous hyperplasia will always recur after surgery but non-verrucous divided eyelid nevus rarely recur. This study analyzed the differential expression of Ki-67, S100, Melan A and HMB45 and identified the correlation between the clinical and histopathological features of verrucous and non-verrucous divided eyelid nevus. Methods This study included 29 patients, of whom 8 patients had verrucous divided nevus. Immunohistochemistry labeling was used to assess the expression of Ki-67, S100, Melan A and HMB45 after excision. The difference between verrucous and non-verrucous divided eyelid nevus was analyzed. Results The patients’ ages ranged from 2 to 59 years, with a mean age of 19 years. The lesion size ranged from 1.5 to 2.0 cm in diameter and invaded the eyelid margins and the posterior lamella of the eyelids. Immunohistochemistry labeling showed strong positivity for approximately 98.5% of S100 and positive staining for approximately 27.6% of Ki-67, 72.4% of Melan A and 6.8% of HMB45. However, Ki-67 was significantly upregulated in verrucous divided nevus of the eyelids compared with non-verrucous divided nevus, with approximately 38.8% upregulation in verrucous and 18.3% upregulation in non-verrucous nevus. Conclusions This study correlated the clinic-pathologic features of verrucous divided eyelid nevus by means of statistically analyzing the varied clinical features and pathological impressions. The significant over-expression of S100 may be used as an indicator of divided nevus of the eyelids, and the over-expressed Ki-67 may contribute to the recurrence of verrucous divided nevus. High expression of HMB45 and Melan A may represent malignant transformation.

Abstract:Tissue expansion is an invaluable reconstructive strategy after surgical excision of a congenital naevus. However, reducing the number of operations is very important for patients with a giant congenital naevus. We used a large pedicle expanded flank flap to successfully treat a giant circumferential naevus that extended from the left upper arm to the wrist, which also provided excellent contour and colour match. We consider this an effective and convenient method to treat a circumferential naevus covering almost the entire upper limb of the paediatric patient.

Abstract:Congenital melanocytic naevi are neuroectodermal lesions that are mainly composed of melanocytes. They are present in 1% to 6% of all newborns. These lesions carry the risk of transforming into melanomas; however, the psychological effect of such disfiguring naevi is potentially of greater concern to both the child and parent. Several classifications for congenital melanocytic naevi have been proposed, the most common of which is the sub-classification according to their size as this affects the choice of treatment. Many treatment modalities have been utilized including surgical excision followed by reconstruction, curettage, dermabrasion, laser therapy and chemical peels. In this report, we present a case of an otherwise healthy eight-year-old girl with a giant congenital melanocytic naevus on the central face. The lesion was mostly excised with remaining satellite lesions dermabraded. The defect was then reconstructed with a full thickness skin graft harvested from the expanded supraclavicular skin, in addition to the ReCell (non-cultured epithelial autograft) technique. Within six months post-operation, excellent skin pigmentation and texture was achieved.

Abstract:Objective: The reconstruction of nasal alar defects after resection of a melanocytic nevus becomes one of the most challenging procedures for surgeons. Choosing the most appropriate technique is still difficult sometimes. The authors reviewed our cases and published experience on choosing optimal surgical methods to repair different types of nasal alar defect following melanocytic nevus resection. Methods: A total of 152 patients who were treated between 2016 and 2019 in Shanghai Ninth People's Hospital were evaluated. The surgical methods included primary closure, full-thickness skin graft, composite auricular graft, interpolated melolabial flap, paramedian forehead flap including hair-bearing paramedian forehead flap. Results: Among the 152 patients with a melanocytic nevus, 49 underwent primary closure, 38 were treated with a full-thickness skin graft, 16 composite auricular graft, 28 were treated with interpolated melolabial flap, and 21were treated with paramedian forehead flap including 14 with hair-bearing paramedian forehead flap. Most skin graft, composite auricular graft, and flaps were survived. Patients who completed follow-up were satisfied with the contour of the nasal ala. Conclusions: After resection of the melanocytic nevus, the surgeon should choose the most appropriate surgical method for alar reconstruction according to the characteristics of the defect and personal desire of patient or the guardian.
Minimally invasive operation for treatment of divided nevus of eyelid

Abstract:OBJECTIVE To discuss the techniques and the effect of minimally invasive surgery on divided nevus of the eyelid.METHODS Six cases(6 eyes) of divided nevus of eyelids in ophthalmology department of our hospital from 2002 to 2012 were retrospectively reviewed.All the nevus involved both the palpebral margins and the conjunctiva.The excision of the nevus under the surgical microscope could be completed by two or three times of operation.During the operation,we tried to protect the eyelash follicle and the posterior limbi.All the excision tissue was examined by pathology.RESULTS The pathological diagnoses of all cases were intradermal nevus.The follow-up time was 4-48 months.All eyelids function was normal with satisfactory appearance.CONCLUSIONS Compared with original surgical methods,the minimally invasive surgical techniques on divided nevus of the eyelid were simpler,with less damage and more satisfactory appearance.

Abstract:Giant facial neurofibroma leads to disfigurement and functional and neurological deficits. Surgical resection is the mainstay of treatment and poses a great challenge to the surgeon with regard to the restoration of the defects arising from tumour resection. The cases of three male and three female patients diagnosed with giant facial neurofibroma, who underwent radical resection and reconstruction with soft tissue free flaps between 2008 and 2015, were analyzed retrospectively. Clinical data including patient sex, age, preoperative embolization of the nutrient artery, volume of blood loss, type and size of flaps used for reconstruction, and complications were recorded. Three of the six patients underwent preoperative embolization of the nutrient artery. The average volume of blood loss was 2850ml. Reconstruction was performed with anterolateral thigh flaps in four patients and latissimus dorsi myocutaneous flaps in two patients. All free flap reconstructions were successful. Partial necrosis of the scalp and wound dehiscence occurred in one patient each. All complications were managed successfully. In conclusion, the soft tissue free flap is a good choice for the coverage of defects after giant facial neurofibroma resection. Multi-disciplinary treatment should be strengthened to minimize the risks of complications, as well as improving quality of life.
Serial tissue expansion and excision for reconstruction of giant dorsal congenital melanocytic nevus: A case report

Abstract:Giant congenital melanocytic nevus often necessitates meticulous planning and multiple treatment stages for complete surgical excision. This report presents a case of giant congenital melanocytic nevus on the back managed through serial tissue expansion and excision. Initially, two expanders were placed at the deep fascia level. Sequential outpatient expansions over 10 weeks were followed by expander removal, partial nevus excision, defect coverage with expanded skin flaps, and simultaneous placement of a new expander. The subsequent single expander expansion over 12 weeks involved a total of 600 mL of saline. After three operations spanning approximately 6 months, 54 cm × 36 cm of giant congenital melanocytic nevus skin, covering 65% of the patient's back, was completely excised. Serial tissue expansion and excision may be an effective surgical approach for managing dorsal giant congenital melanocytic nevus, reducing the need for multiple surgeries and achieving favorable aesthetic outcomes.
[Reconstruction of Large External Nose Defect with Forehead Flap].

Abstract:OBJECTIVE:To discuss the role of forehead flap in large external nasal defect reconstruction.METHOD:We retrospectively reviewed the clinical and follow-up data of 11 patients with large or full-thickness external nasal defects, which were reconstructed with single-or two-stage interpolated or island forehead flaps from January 2007 to June 2011. All patients were male,defects of 9 cases were resulted from resection of skin malignant tumor and traumatic defect in 2 cases. The average diameter of defects was 3.3 cm (2.5-5.0 cm). Four cases had alar full thickness defect. The supratrochlear and angular artery of the elders (>70 yrs) and patients with suspicious peripheral blood vessel lesions were scanned before the surgery by Doppler ultrasonic. The defects were reconstructed by two-stage interpolated flap in 7 cases; five cases were reconstructed by single-stage island flap technique. A nasolabial flap based on piriform aperture was turned into the nasal cavity to reconstruct the lining.RESULT:All defects were repaired successfully. All tissue flap survived and the scar was not conspicuous. One patient had nostril stenosis with mild nasal congestion complain at the 6th postoperative month.CONCLUSION:The forehead flap, interpolated or island, can be used safely and effectively to repair the large external nasal defect in experienced hands.
Surgical Reconstruction in Malignant Eyelid Tumors

Abstract:Objective: To report the histopathological subtypes of malignant eyelid tumors treated surgically in the author's clinic and to present the author's graft and flap preferences in the reconstruction of the eyelid defect after surgical excision. Methods: This retrospective study included 33 patients who underwent malignant eyelid tumor excision, followed by eyelid reconstruction. A frozen section was performed during surgery to assess surgical margins in all patients. Demographic characteristics, histological classification, localization, and size of the tumor, postoperative defect size, follow-up period, complications, and recurrence rate were recorded. Results: Of the patients with a mean age of 66.6 ± 15.3 (range: 23-94), 15 (45.5%) were females and 18 (54.5%) were males. The lower eyelid was the most commonly involved site in 27 (81.8%) patients. Basal cell carcinoma was the most frequently diagnosed type of malignant cancer in 81.8% (n = 27) of the cases. Among the tumor subtypes, nodular type basal cell carcinoma was the most frequently observed subtype in 10 (30.3%) patients. Wide excision with different flap and graft techniques was performed in 26 (78.8%) patients with defect sizes of 40% or larger. In the author's study, the most preferred flap technique for anterior lamella reconstruction was the Tenzel semicircular flap (30.3% n = 10). The most frequently preferred flap for posterior lamella reconstruction was the Hughes tarsoconjunctival flap (30.3% n = 10). There were no flap or graft failures in any of the author's cases. Conclusion: In the treatment of malignant eyelid tumors, different local flap and graft techniques selected according to the size of the defect have successful anatomical results in reconstructive surgery after frozen section controlled excision.

Abstract:BACKGROUND:Large involuted upper lip infantile hemangioma is a challenge for plastic surgeon because of high visibility of disfigurement. To achieve an optimal reconstruction with matched color and texture, nasolabial flap based on the upper lateral lip subunit is used for the upper lip reconstruction.METHODS:Records of 8 patients who underwent upper lip reconstruction from 2014 to 2018 at Shanghai Ninth People's Hospital were reviewed. After flap design according to the contralateral upper lip subunit, nasolabial flap was dissected and rotated to the defect. The donor site was subsequently closed by extensive undermining cheek skin tissue. The incisions were hidden in the border of lateral upper lip subunit.RESULTS:There were a few deformities observed, including scar hypertrophy (4/8), alar flaring (2/8), and lower lid bulking (1/8). Five patients received following trimming procedures because of residual scars or deformity. All the patients were satisfied with the postoperative results. No obvious complications were observed.CONCLUSIONS:The nasolabial flap based on the upper lateral lip subunit is a safe, effective, and reliable technique for correction of severe upper lip defect caused by large involuted infantile hemangioma. Although subsequent trimming procedures are required in some complicated cases, satisfactory results can still be achieved.

Abstract:Background: Removal of large facial benign cutaneous lesions remains challenging. Serial or complete excisions together with local flaps or expander-based reconstructions are required. However, those techniques are time-consuming and may contribute to poor cosmetic and functional outcomes. Objective: The authors describe the resection and reconstruction of large facial benign cutaneous lesions by using Stepwise, Multi-Incisional, and Single-Stage (SMISS) approach. Methods: The authors performed a retrospective review from all patients with large facial benign cutaneous lesions who underwent "SMISS'' approach for reconstruction between September 2013 and December 2014. Results: The authors treated 47 patients (32 female and 15 male; mean age 23.5 years, range 9-50 years). Follow-up was for 12 months or longer. The mean length of major axis was 43.91 mm, minor axis 32.10 mm, and scar 66.91 mm. Good to excellent outcomes were achieved in all patients with a mean Vancouver scar scale score of 3.46 +/- 0.39 (Cronbach alpha = 0.890) and mean visual analog scale score of 8.02 +/- 0.69 (Cronbach alpha = 0.946). Limitations: This was a nonrandomized, unblinded clinical case series with a limited sample size. Conclusion: For the excision and reconstruction of large facial benign cutaneous lesions, "SMISS'' technique can be considered as a suitable option, leading to excellent results and a high patient satisfaction.
