The Role of Gut-Derived Endotoxin in Early MODS of Severe Acute Pancreatitis

Abstract:Objective To investigate the role of gut-derived endotoxin and gut barrier dysfunction in early MODS of animal severe acute pancreatitis(SAP).Method Twenty-four SD rats were randomly divided into 4 groups with 6 rats(3 male and 3 female) in each group.① In the SAP group,retrograde injection of 5% sodium taurocholate into the pancreatic duct was performed;② In the SAP+endotoxin-in-gut group,retrograde injection of 5% sodium taurocholate(the same as SAP group)was performed similarly,followed by endotoxin injection into the colon;③ In the endotoxin-in-gut group,only endotoxin was injected into the colon;④ A control group.The maximum blood volume of the abdominal aorta(reflecting effective blood circulation) and the volume of ascites were measured 8 hours after each model was established,and the biochemical parameters,cytokines and diamine oxidase(DAO) both in the blood and ascites were detected simultaneouly.Pathological sections of the pancreas,lung,kidney liver and colon were made and graded.Results The rats in the SAP+endotoxin-in-gut rats had more ascites,less ciraclating blood,worse kidney function,more elevated amylase and TNF-α in the ascites compared with those of the SAP group,and the organ structures were damaged more seriously.There were no organ damage in the rats of the endotoxin-in-gut group,and there was no difference of parameters both in the blood and the ascites as compared with the control group.Conclusions The presence of endotoxin in the gut accompanied by the existence of gut barrier dysfunction in the SAP rats leads to the translocation of gut-derived endotoxin;this play an important role in the production of early MODS in SAP.
