Abstract:Hypertension,as one of the most common chronic diseases,has become a major risk factor of cardiovascular diseases.Extensive studies indicate that aerobic exercise has the ability to lower blood pressure and to ameliorate the risk factors of hypertension,especially in individuals with higher baseline blood pressure and hypertensive patients.First,this paper reviews the beneficial effects of aerobic exercise on hypertension and the possible potential mechanisms.It means that aerobic exercise improves peripheral pressure,central artery pressure and cardiac function,having beneficial effects on arteriosclerosis,endothelial function and sympathetic nervous system activity.Then,the paper introduces the rules of establishing exercise prescriptions among hypertension patients and its differences between different human populations,such as gender,age,baseline blood pressure,etc.The risks brought by excessive exercise and certain avoidance methods are also discussed.Finally,according to the requirements of precision medicine,combined with the considerations of security,we provide certain directions of future comprehensive studies to prescribe personalized exercise programs.

Abstract:In conclusion, the findings of most recent studies show that moderate-intensity aerobic exercise training can lower BP in patients with stage 1 and 2 essential hypertension. The average reduction in BP is 10.5 mm Hg for systolic and 7.6 mm Hg for diastolic BP. The reductions do not appear to be gender- or age-specific. Significant reductions in BP and LVH regression in patients with stage 3 hypertension have also been reported following aerobic exercise training. Resistance training exercise has not consistently shown to significantly lower BP and is not recommended as the only form of exercise for hypertensive patients. The exercise training program for optimal benefits should consist of 3 to 5 times per week, 30 to 60 minutes per session, at 50% to 80% of PMHR. However, exercise programs should be individualized to meet the patient's needs and abilities. Exercise intensity and duration should be manipulated to promote a safe and effective antihypertensive program. Initially, the exercise intensity should be low and the duration short. Both intensity and duration should progressive increase over a period of weeks until the desired goal, is achieved. The rate of progression must be tailored to meet individual patient needs and abilities. The exercise program for overweight or obese hypertensive patients should aim to promote a caloric expenditure of 300 to 500 Kcal per day and 1000 to 2000 Kcal per week. Such an approach, combined with a prudent diet, is likely to reduce body weight. The mechanisms mediating exercise-induced BP reduction are poorly understood. BP reductions appear to be independent of changes in body weight or body composition. There are also no indications of age- or gender-related differences in BP response to exercise. The use of ambulatory blood pressure measuring devices in exercise studies is not extensive. The few studies available indicate a more moderate reduction in BP than that reported by casual observations.

Abstract:Main characteristics of essential hypertension are high arterial blood pressure with functional and organic injury of heart,brain,kidney,blood vessel and so on. Human health is threatened by hypertension. Exercise therapy is an important hypertension therapy and its effects have been confirmed. Recently,(improved) remodeling of target organ plays an important role in hypertension therapy. Whether injury of target organ can be improved and even reversed by exercise therapy need to be discussed on a thorough and profound level.

Abstract:Hypertension poses a significant health burden, necessitating effective interventions to manage blood pressure and mitigate associated cardiovascular risks. Exercise has emerged as a cornerstone non-pharmacological strategy in the management of hypertension. This comprehensive review synthesizes current evidence regarding the impact of various exercise modalities on blood pressure regulation in hypertensive individuals. The review commences by elucidating the physiological mechanisms underpinning exercise-induced blood pressure alterations, highlighting factors influencing blood pressure responses during different exercises. It delineates the differential impacts of aerobic and resistance training, examining their effectiveness in blood pressure reduction among diverse populations, considering age, fitness levels, and comorbidities. Moreover, the review explores the emerging prominence of high-intensity interval training (HIIT), showcasing its unique effects on blood pressure regulation and cardiorespiratory fitness improvement compared to traditional continuous aerobic exercises. Additionally, alternative mind-body exercises, including tai chi, qigong, and mindfulness-based interventions, are investigated for their potential to influence blood pressure levels among hypertensive populations. Furthermore, the review identifies novel exercise approaches such as deep breathing exercises and multi-modal isometric exercise training (MIET) as promising strategies for blood pressure control in hypertensive individuals, emphasizing their potential benefits and implications for tailored exercise prescriptions.

Abstract:Hypertension is a fatal but preventable risk factor for cardiovascular disease and an important cause of death from cardiovascular disease. With the increasing aging of the population, the number of patients with hypertension continues to increase, and effective treatment is becoming increasingly urgent. [ 1 ] In addition to drug therapy, current evidence [ 2–4 ] shows that exercise training has a definite clinical effect on blood pressure control. It can not only optimize the treatment of hypertension but also prevent or delay the progression of hypertension and its complications. However, choosing an appropriate type of exercise is clinically critical. Inappropriate exercise is ineffective and may also cause disease. [ 5 ] Exercise hypertension (EH) refers to abnormal hypertension during and/or after exercise, also known as hypertensive response to exercise. [ 6 ] It is a manifestation of prehypertension. The ideal blood pressure for exercise should achieve the therapeutic effect of exercise without causing injury to target organs or related clinical diseases.

Abstract:The effects of exercise interventions on blood pressure control in hypertensive patients have received extensive attention and recognition. However, the influence of exercise-related behaviors on the effects of blood pressure control and the management factors that affect exercise behaviors remain inconclusive. Therefore, this study aimed to explore the relationships between exercise and exercise-related behaviors and blood pressure control, expecting to provide a scientific basis for further improvement of exercise intervention programs for patients with hypertension.Long-term residents in a community in Fuzhou, Fujian Province, China who were diagnosed with hypertension at a medical facility were recruited for this study. Hypertensive patients with uncontrolled blood pressure were placed in the case group, and those with controlled blood pressure were placed in the control group. The count data were analyzed with the χ test based on α = 0.05. A univariate logistic regression analysis was performed to screen significant variables; independent variables were selected based on professional knowledge and were tested for co-linearity. These variables were incorporated into a non-conditional multivariate logistic regression analysis to analyze relevant factors for blood pressure control, while calculating odds ratio (ORs), 95% confidence intervals (CIs), and P values. Differences with a P < .05 were considered statistically significant.A total of 598 patients were included in the present study. Univariate and multivariate analyses showed that exercise (OR = 0.194; 95%CI: 0.133-0.283), frequency of self-monitoring of blood pressure (OR = 2.412; 95%CI: 1.137-5.117), and regular housework (OR = 0.397, 95%CI: 0.248-0.633) were statistically significant factors for blood pressure control. In addition, there was a relationship between exercise-related behaviors and blood pressure control: walking (OR = 0.189, 95% CI: 0.065-0.551), 60 to 120 min/day of exercise time (OR = 0.317, 95% CI: 0.118-0.856). Sex (OR = 2.248, 95% CI: 1.279-3.954), health belief (OR = 0.016, 95% CI: 0.004-0.059), and company of a family member during exercise (OR = 0.048, 95% CI: 0.025-0.094) were significant factors predictive of regular exercise in hypertensive patients.Improving exercise-related behaviors and their management can help improve the effects of exercise interventions on blood pressure control in middle-aged and elderly hypertensive patients.

Effect of T'ai Chi Exercise on Hypertension in Young and Middle-Aged In-Service Staff

Abstract:Objective: This study aims to investigate the effect of t'ai chi exercise on hypertension in young and middleaged in-service staff. Methods: A total of 208 subjects with grade 1 hypertension were enrolled into this study. These subjects were randomly divided into two groups: research group and control group (n = 104, each). On the basis of general daily lifestyle intervention, subjects in the research group underwent 24-Style Simplified t'ai chi exercise for 3 months, whereas subjects in the control group underwent general daily lifestyle intervention. All subjects were followed up at the first and third month of intervention. The body mass index (BMI), blood pressure, blood lipid, and other indexes were measured before and after the intervention, and quality of life was evaluated. Results: (1) In the research group, after 1 month of exercise, systolic blood pressure (SBP), heart rate (HR), triglyceride (TG), total cholesterol (TC), and low-density lipoprotein cholesterol (LDL-C) significantly decreased (p < 0.05), while BMI, blood glucose (Glu), diastolic blood pressure (DBP), and pulse pressure (PP) did not significantly change. Furthermore, after 3 months of exercise, BMI, HR, SBP, DBP, PP, TG, TC, LDL-C, and Glu all significantly decreased (p < 0.05). (2) Moreover, the quality of life of subjects in the research group obviously improved after 3 months of t'ai chi exercise (p < 0.05). Conclusions: t'ai chi exercise can reduce the level of blood pressure in young and middle-aged in-service staff with grade 1 hypertension, control weight, slow down the HR, improve metabolism, and improve quality of life. t'ai chi is an exercise suitable for in-service hypertension subjects.

Abstract:Hypertension is recognised as a leading attributable risk factor for cardiovascular disease and premature mortality. Global initiatives towards the prevention and treatment of arterial hypertension are centred around non-pharmacological lifestyle modification. Exercise recommendations differ between professional and scientific organisations, but are generally unanimous on the primary role of traditional aerobic and dynamic resistance exercise. In recent years, isometric exercise training (IET) has emerged as an effective novel exercise intervention with consistent evidence of reductions in blood pressure (BP) superior to that reported from traditional guideline-recommended exercise modes. Despite a wealth of emerging new data and endorsement by select governing bodies, IET remains underutilised and is not widely prescribed in clinical practice. This expert-informed review critically examines the role of IET as a potential adjuvant tool in the future clinical management of BP. We explore the efficacy, prescription protocols, evidence quality and certainty, acute cardiovascular stimulus, and physiological mechanisms underpinning its anti-hypertensive effects. We end the review with take-home suggestions regarding the direction of future IET research.

Abstract:OBJECTIVE To explore the rehabilitative effect of physical exercise on hypertensive in community for longer period. METHODS 60 hypertensive with exercise training during 2000 to 2001 and 97 patients without that as control cases sampled randomly in same community were interviewed by the unique questionnaire and blood pressure and constitution indexes were measured and recorded in the study. RESULTS In the exercise group,the blood pressure had the declined trends and the systolic blood pressure had a significant reduction of 10.56 mmHg compared with that one of the study base:and in this survey,there were significant differences in both systolic and diastolic blood pressure between the exercise group and the control group. CONCLUSIONS Moderate physical exercise could lower blood pressure level in patients with established hypertension for longer period.

Abstract:<h2>Abstract</h2><p>A growing understanding of the benefits of exercise over the past few decades has prompted researchers to take an interest in the possibilities of exercise therapy. Because each sport has its own set of characteristics and physiological complications that tend to appear during exercise training, the effects and underlying mechanisms of exercise remain unclear. Thus, the first step in probing exercise effects on different diseases is the selection of an optimal exercise protocol. This review summarizes the latest exercise prescription treatments for 26 different diseases: musculoskeletal system diseases (low back pain, tendon injury, osteoporosis, osteoarthritis and hip fracture); metabolic system diseases (obesity, type 2 diabetes, type 1 diabetes, nonalcoholic fatty liver disease); cardio-cerebral vascular system diseases (coronary artery disease, stroke and chronic heart failure); nervous system diseases (Parkinson's disease, Huntington's disease, Alzheimer's disease, depression, and anxiety disorders); respiratory system diseases (chronic obstructive pulmonary disease, interstitial lung disease and after lung transplantation); urinary system diseases (chronic kidney disease and after kidney transplantation); and cancers (breast cancer, colon cancer, prostate cancer, and lung cancer). Each exercise prescription is displayed in the corresponding table. The recommended type, intensity, and frequency of exercise prescriptions are summarized, and the effects of exercise therapy on the prevention and rehabilitation of different diseases are discussed.</p>

Exercise Training Attenuates Hypertension and Cardiac Hypertrophy by Modulating Neurotransmitters and Cytokines in Hypothalamic Paraventricular Nucleus

Abstract:AIMS: Regular exercise as an effective non-pharmacological antihypertensive therapy is beneficial for prevention and control of hypertension, but the central mechanisms are unclear. In this study, we hypothesized that chronic exercise training (ExT) delays the progression of hypertension and attenuates cardiac hypertrophy by up-regulating anti-inflammatory cytokines, reducing pro-inflammatory cytokines (PICs) and restoring the neurotransmitters balance in the hypothalamic paraventricular nucleus (PVN) in young spontaneously hypertensive rats (SHR). In addition, we also investigated the involvement of nuclear factor-κB (NF-κB) p65 and NAD(P)H oxidase in exercise-induced effects.METHODS AND RESULTS: Moderate-intensity ExT was administrated to young normotensive Wistar-Kyoto (WKY) and SHR rats for 16 weeks. SHR rats had a significant increase in mean arterial pressure and cardiac hypertrophy. SHR rats also had higher levels of glutamate, norepinephrine (NE), phosphorylated IKKβ, NF-κB p65 activity, NAD(P)H oxidase subunit gp91(phox), PICs and the monocyte chemokine protein-1 (MCP-1), and lower levels of gamma-aminobutyric acid (GABA) and interleukin-10 (IL-10) in the PVN. These SHR rats also exhibited higher renal sympathetic nerve activity (RSNA), and higher plasma levels of PICs, and lower plasma IL-10. However, ExT ameliorates all these changes in SHR rats.CONCLUSION: These findings suggest that there are the imbalances between excitatory and inhibitory neurotransmitters and between pro- and anti-inflammatory cytokines in the PVN of SHR rats, which at least partly contributing to sympathoexcitation, hypertension and cardiac hypertrophy; chronic exercise training attenuates hypertension and cardiac hypertrophy by restoring the balances between excitatory and inhibitory neurotransmitters and between pro- and anti-inflammatory cytokines in the PVN; NF-κB and oxidative stress in the PVN may be involved in these exercise-induced effects.

Association between Exercise and Blood Pressure in Hypertensive Residents: A Meta-Analysis

Abstract:Background. Exercise is recommended as an effective lifestyle behaviour for adults to prevent and treat hypertension. In this study, a randomized-effect meta-analysis was used to analyse the influence of exercise interventions on blood pressure in patients with hypertension. Methods. Candidate papers were retrieved from PubMed, Web of Science, Embase, and Cochrane Library electronic databases, and 46 studies were finally included and analysed. Results. It was shown that preplanned walking (systolic blood pressure (SBP): WMD (weighted mean difference) = −5.94, 95% CI: −8.57, −3.30; diastolic blood pressure (DBP): WMD = −2.66, 95% CI: −3.66, −1.67), yoga (SBP: WMD = −5.09, 95% CI: −9.28, −0.89; DBP: WMD = −3.06, 95% CI: −5.16, −0.96), aquatic sports (SBP WMD = −7.53, 95% CI: −11.40, −3.65; DBP: WMD = −5.35, 95% CI: −9.00, −1.69), and football (SBP: WMD = −6.06, 95% CI: −9.30, −2.82; DBP: WMD = −5.55, 95% CI: −8.98, −2.13) had significant effects on blood pressure reduction. However, Tai Chi (SBP: WMD = −8.31, 95% CI: −20.39, 3.77; DBP: WMD = −3.05, 95% CI: −6.96, 0.87) and Qigong (SBP: WMD = −4.34, 95% CI: −13.5, 4.82; DBP: WMD = −3.44, 95% CI: −7.89, 1.01) did not significantly reduce blood pressure. The heterogeneity of the meta-analysis was high. Conclusion. Walking, yoga, aquatic sports, and football were feasible and independent lifestyle interventions, and they were effective options for treating hypertension. More scientifically designed randomized controlled trials are needed in the future to further compare different forms of exercise for the treatment of hypertension.

Abstract:Introduction: In the last few decades, the prevalence of hypertension has been drastically increased in India. Hypertension is defined by the presence of a chronic elevation of systemic arterial pressure above a certain threshold value. Proper management of hypertension may require both pharmacological and non-pharmacological interventions. Aerobic exercise and Jacobson’s Progressive Muscular relaxation (JPMR) technique both are Non-pharmacological interventions for reducing blood pressure. Method: Interventional study was done by Random sampling method and conducted on 20 hypertensive subjects. Subject willing to participate, age group 20-55 year both male and female and blood pressure above 140/90 mmHg were included. Subjects not able to understand, psychologically unstable, systemic illness and subject who were not regularly taking medications were excluded. 20 hypertensive subjects, randomly divided into 2 groups. The first group was asked to do 5 sessions of Aerobic exercise for 30-45 minutes per week for 4 week and the second group was asked to do 5 sessions of JPMR for 30 minutes per week for 4 week. In both the groups pre and post systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) were measured in sitting position. Subjective results were established. Results: Statistical analysis was done using SPSS16. There was statistically significant difference between pre and post value of SBP (p<0.05), DBP (p<0.05) in both groups, but the aerobic exercise showed more significant results as compared to the JPMR. Conclusion: Hence it can be concluded that Aerobic Exercise is more effective in hypertensive than Jacobson’s Progressive Muscular relaxation technique. Prescription of exercise training for hypertensives

Abstract:In Japan, there were an estimated 43 million patients with hypertension in 2010. The management of this condition is given the highest priority in disease control, and the importance of lifestyle changes for the prevention and treatment of hypertension has been recognized in Japan. In particular, emphasis has been placed on increasing the levels of activities of daily living and physical exercise (sports). In this literature review, we examined appropriate exercise prescriptions (e.g., type, intensity, duration per session, and frequency) for the prevention and treatment of hypertension as described in Japanese and foreign articles. This review recommends safe and effective whole-body aerobic exercise at moderate intensity (i.e., 50-65% of maximum oxygen intake, 30-60 min per session, 3-4 times a week) that primarily focuses on the major muscle groups for the prevention and treatment of hypertension. Resistance exercise should be performed at low-intensity without breath-holding and should be used as supplementary exercise, but resistance exercise is contraindicated in patients with hypertension who have chest symptoms such as chest pain.

Abstract:In China, hypertension has become a dangerous factor to public health in recent years. To study its impact on health status, exercise testing was developed to examine the changes of physiological parameters. 107 hypertension patients and 68 healthy persons were volunteers for this testing. During testing, ECG, pulse wave, respiratory wave and blood pressure were recorded. According to the parameters measured, the WHOQOL-BERF results completed by all subjects, and the evaluation based on physical function, statistical analysis was carried out using the student's t-test and logistic regression. The results show that the recovery time of BP, maximal systolic pressure, and the score of physical function could be used to reflect the difference between hypertension patients and healthy persons. In conclusion, the logistic regression model could be used to detect the hypertension status by exercise testing which did not depend merely on the sphygmomanometer.
Effects of different aerobic exercise programmes with nutritional intervention in sedentary adults with overweight/obesity and hypertension: EXERDIET-HTA study

Abstract:Background Both exercise training and diet are recommended to prevent and control hypertension and overweight/obesity. Purpose The purpose of this study was to determine the effectiveness of different 16-week aerobic exercise programmes with hypocaloric diet on blood pressure, body composition, cardiorespiratory fitness and pharmacological treatment. Methods Overweight/obese, sedentary participants ( n = 175, aged 54.0 ± 8.2 years) with hypertension were randomly assigned into an attention control group (physical activity recommendations) or one of three supervised exercise groups (2 days/week: high-volume with 45 minutes of moderate-intensity continuous training (MICT), high-volume and high-intensity interval training (HIIT), alternating high and moderate intensities, and low-volume HIIT (20 minutes)). All variables were assessed pre- and post-intervention. All participants received the same hypocaloric diet. Results Following the intervention, there was a significant reduction in blood pressure and body mass in all groups with no between-group differences for blood pressure. However, body mass was significantly less reduced in the attention control group compared with all exercise groups (attention control –6.6%, high-volume MICT –8.3%, high-volume HIIT –9.7%, low-volume HIIT –6.9%). HIIT groups had significantly higher cardiorespiratory fitness than high-volume MICT, but there were no significant between-HIIT differences (attention control 16.4%, high-volume MICT 23.6%, high-volume HIIT 36.7%, low-volume HIIT 30.5%). Medication was removed in 7.6% and reduced in 37.7% of the participants. Conclusions The combination of hypocaloric diet with supervised aerobic exercise 2 days/week offers an optimal non-pharmacological tool in the management of blood pressure, cardiorespiratory fitness and body composition in overweight/obese and sedentary individuals with hypertension. High-volume HIIT seems to be better for reducing body mass compared with low-volume HIIT. The exercise-induced improvement in cardiorespiratory fitness is intensity dependent with low-volume HIIT as a time-efficient method in this population. ClinicalTrials.gov Registration: NCT02283047.

Abstract:Background: The prevalence of prehypertension and hypertension has been increasing over the years, and is closely related to cardiovascular and cerebrovascular diseases. Exercise is an effective method of lifestyle intervention, and it aims to lower blood pressure and control other risks. Studies have shown that different modes of exercise have varying effects on blood pressure, and individuals with prehypertension or hypertension need to carry out this intervention by using personalized modes of exercise. Methods: We conducted a systematic review and meta-analysis to evaluate the effects of different modes of exercise regimens on systolic blood pressure, diastolic blood pressure and heart rate in individuals with high-normal blood pressure and hypertension. We included 27 trials, and 2731 individuals were under 8 exercise regimens. Stata12.0 statistical software was used for statistical analysis. Results: Heat pools significantly reduced systolic blood pressure (SBP) by 15.62 mmHg (95% confidence interval [CI]: –23.83, –7.41), and cycling reduced SBP by 14.76 mmHg (–17.04, –12.48). Two to three types of aerobic exercise performed at the same time also significantly reduced diastolic blood pressure (DBP) by 5.61 mmHg (–7.71, –3.52), and isometric handgrip training exercise reduced DBP by 5.57 mmHg (–7.48, –3.66). Cycling also significantly reduced heart rate (HR) by 9.57 beats/minute (–11.25, –7.90). Conclusions: The existing literature suggests that different types of exercise can effectively reduce the levels of SBP, DBP and HR in individuals with prehypertension or hypertension.
