Abstract:Excitation-contraction coupling (E-C coupling) is thought to be based on elementary calcium release units (CRUs) in which clusters of ryanodine receptors (RyRs) localized on the sarcoplasmic reticulum (SR) are in close apposition to L-type Ca2+ channels (LCCs) on the transverse tubules (TTs). However, a fraction of LCC-RyR structure may be uncoupled due to the remodelling of TTs, which would tend to destroy the E-C coupling in the failing heart. Here we proposed a multiscale model of the ventricular myocyte to investigate the relationship between LCC-RyR structure and cardiac electro-mechanical function. The mathematical model consisted of a two-dimensional (2D) subcellular Ca2+ reaction-diffusion sub-model, a cellular electrophysiological sub-model and a cardiomyocyte contraction sub-model. The simulation results showed that the remodelling of CRU microstructure would disturb Ca2+ homeostasis, leading to a dyssynchronous Ca2+ transient, and postpone the generation of isometric force. Our study suggests that structural remodelling is an important mechanism for dysfunction of Ca2+ handling, cellular electrophysiology and contractility in failing heart.
Simulation of the Effect of Rogue Ryanodine Receptors on a Calcium Wave in Ventricular Myocytes with Heart Failure.

Abstract:Calcium homeostasis is considered to be one of the most important factors for the contraction and relaxation of the heart muscle. However, under some pathological conditions, such as heart failure (HF), calcium homeostasis is disordered, and spontaneous waves may occur. In this study, we developed a mathematical model of formation and propagation of a calcium wave based upon a governing system of diffusion-reaction equations presented by Izu et al (2001 Biophys. J. 80 103-20) and integrated non-clustered or 'rogue' ryanodine receptors (rogue RyRs) into a two-dimensional (2D) model of ventricular myocytes isolated from failing hearts in which sarcoplasmic reticulum (SR) Ca(2+) pools are partially unloaded. The model was then used to simulate the effect of rogue RyRs on initiation and propagation of the calcium wave in ventricular myocytes with HF. Our simulation results show that rogue RyRs can amplify the diastolic SR Ca(2+) leak in the form of Ca(2+) quarks, increase the probability of occurrence of spontaneous Ca(2+) waves even with smaller SR Ca(2+) stores, accelerate Ca(2+) wave propagation, and hence lead to delayed afterdepolarizations (DADs) and cardiac arrhythmia in the diseased heart. This investigation suggests that incorporating rogue RyRs in the Ca(2+) wave model under HF conditions provides a new view of Ca(2+) dynamics that could not be mimicked by adjusting traditional parameters involved in Ca(2+) release units and other ion channels, and contributes to understanding the underlying mechanism of HF.

Abstract:Myocytes have been regarded as the main objectives in most cardiac modeling studies and attracted a lot of attention. Connective tissue cells, such as fibroblasts (Fbs), also play crucial role in cardiac function. This study proposed an integrated myocyte-Isac-Fb electromechanical model to investigate the effect of Fbs and stretch activated ion channel current (Isac) on cardiac electrical excitation conduction and mechanical contraction. At the cellular level, an active Fb model was coupled with a human atrial myocyte electrophysiological model (includingIsac) and a mechanical model. At the tissue level, electrical excitation conduction was coupled with an elastic mechanical model, in which finite difference method (FDM) was used to solve the electrical excitation equations, while finite element method (FEM) was used for the mechanics equations. The simulation results showed that Fbs andIsaccoupling caused diverse effects on action potential morphology during repolarization, depolarized the resting membrane potential of the human atrial myocyte, slowed down wave propagation, and decreased strains in fibrotic tissue. This preliminary simulation study indicates that Fbs andIsachave important implications for modulating cardiac electromechanical behavior and should be considered in future cardiac modeling studies.
Role of CaMKII and PKA in Early Afterdepolarization of Human Ventricular Myocardium Cell: A Computational Model Study

Abstract:Early afterdepolarization (EAD) plays an important role in arrhythmogenesis. Many experimental studies have reported that Ca2+/calmodulin-dependent protein kinase II (CaMKII) and β-adrenergic signaling pathway are two important regulators. In this study, we developed a modified computational model of human ventricular myocyte to investigate the combined role of CaMKII and β-adrenergic signaling pathway on the occurrence of EADs. Our simulation results showed that (1) CaMKII overexpression facilitates EADs through the prolongation of late sodium current’s (INaL) deactivation progress; (2) the combined effect of CaMKII overexpression and activation of β-adrenergic signaling pathway further increases the risk of EADs, where EADs could occur at shorter cycle length (2000 ms versus 4000 ms) and lower rapid delayed rectifier K+ current (IKr) blockage (77% versus 85%). In summary, this study computationally demonstrated the combined role of CaMKII and β-adrenergic signaling pathway on the occurrence of EADs, which could be useful for searching for therapy strategies to treat EADs related arrhythmogenesis.
Activation of Extracellular-Signal Regulated Kinase and Protein Phosphatases in Human Atria During Atrial Fibrillation

Abstract:AIM: The purpose of this study was to determine whether the signal transduction systems were activated at the molecular atrial tissue level in patients with atrial fibrillation (AF) and whether atrial expression of extracellular-signal regulated kinase (ERK) and protein phosphatases is altered. METHODS: Atrial tissue sample of 30 patients undergoing cardiac surgery were examined. 20 patients had AF, 10 patients had no history of AF. The mRNA expression of calcineurin B and MKP-1 were detected by semi-quantitative RT-PCR. ERK1 and phospho-ERK1 were analyzed at the protein level by Western blot. RESULTS: Western blot analysis showed that atrial fibrillation did not induce significant change in ERK1 expression level in the left atrium. In contrast , phospho-ERK1 content was increased in the patients with AF in comparison with those who had sinus rhythm (SR). The mRNA expression of calcineurin B and MKP-1 in the patients with AF were significantly higher than that in patients with SR. CONCLUSION: The activation of extracellular-signal regulated kinase and protein phosphatases may have correlation with the initiation or maintenance of atrial fibrillation.
[Expression of Connexin in Atrium of Patients with Atrial Fibrillation and Its Signal Transduction Pathway].

Abstract:OBJECTIVE:To explore the expression of connexin (Cx) and its signal transduction pathway in the atrium of patients with atrial fibrillation (AF).METHODS:Atrial tissue samples of 63 patients undergoing cardiac surgery, including patients with chronic AF or paroxysmal AF (PAF), and those with sinus rhythm, were collected during operation. The mRNA expressions of calcineurin B and mitogen-activated protein kinase-1 (MKP-1) were detected by semi-quantitative RT-PCR. The protein expressions of extracellular-signal regulated kinase 1 (ERK1), phospho-ERK1 (P-ERK1), Cx40 and Cx43 were analyzed by Western blotting. HE staining and immunohistochemistry were used to examine the distribution of Cx40 and Cx 43.RESULTS:Increased amounts of Cx40 protein (left atrium: 2.2 +/- 0.8, 2.2 +/- 0.6; right appendages: 2.1 +/- 0.5, 2.0 +/- 0.8) were found in the left atrium and right appendages of patients with Chronic persistent AF (CAF) or paroxysmal AF (PAF) (P < 0.05). The expression of Cx43 was only increased in the left atrium of patients with CAF and PAF (3.1 +/- 0.6, 2.8 +/- 0.7 vs 1.0 +/- 0.2, both P < 0.05). The amounts of Calcineurin B mRNA, MKP-1 mRNA and P-ERK1 of patients with CAF or PAF were significantly increased compared with patients in sinus rhythm (P < 0.05). Immunohistochemistry revealed that Cx40 and Cx43 of CAF patients and PAF patients acculmated in the intracellular site, and at the lateral member of atrial cells, both connexins redistributed.CONCLUSION:The increased expression and disorderly distribution of Cx 40 and Cx 43 protein in the atrium of AF patients may be related with the abnormal activation and disequilibria of regulation of ERK1, MKP-1 and calcineurin.

Abstract:Objective Inquire into the molecular biological mechanism of the occurrence and maintenance of atrial fibrillation (AF) by researching adjustment and control of the L type the passage and muscles sarcoplasmic reticulum (SR) Ca2+- the ATPase expression by main calcium ion (Ca2+) of AF and the sinus rhythm. Methods We took 200 mg right auricle tissues and (or) left atrium tissues from each of the 63 patients undergoing cardiac surgery (including three groups: chronic AF, paroxysmal AF and sinus rhythm), extracted the protein and measured the density of overall sample protein, using Western-blot method to analyse the expression level of atria muscle L-type Ca2+ Passage and SR Ca2+- the ATPase. Isolated the total RNA of the artria muscle tissue by Trizol Method, measured the expression amount of atria muscle L-type Ca2+ Passage and SR Ca2+-ATPase mRNA with RT-PCR method. Detailed clinical data were obtained before and after operation. Results Firstly, left atrial diameter was obviously higher in patients with chronic AF or paroxysmal AF than in patients with sinus rhythm. Secondly, the patients with chronic AF have lower ratio of L-type Ca2+/GAPDH protein and SR Ca2+-ATPase/GAPDH protein than the group of sinus rhythm both in left atrium and right atrium with significant difference. Compared with sinus rhythm group, ratio of L-type Ca2+/GAPDH protein and SR Ca2+-ATPase/GAPDH protein in paroxysmal AF group decreased with no statistical significance. 3. Level of L-type Ca2+ and SR Ca2+-ATPase mRNA of the chronic AF group declined than sinus rhythm group both in left and right atrium with statistical differences. Conclusion L-type calcium channel and SR Ca2+ -ATPase mRNA and the amount of protein changed significantly in the patients with chronic AF and not significantly in paroxysmal AF group. There was statistical significance between left and right atrium.
An approach to calcium signal transmission between L - type calcium channel and sarcoplasmic reticulum of atrial myocytes during atrial fibrillation

Abstract:AIM: To inquire into the Ca2+ signal transmission from L- type calcium channel to the sarcoplasma reticu-lum in atrial fibrillation. METHODS: Ten adult cross- bred dogs were used in the experiment. Five dogs underwent continuous rapid atrial pacing (500+++20 beats/min) for twenty - four weeks to create persistent atrial fibrillation. Another group of size -matched dogs ( n = 5) without pacemaker implantation was used as a control group. Canine atrial myocytes were isolated by enzymatic dissociation. The Ca2+ cytosolic transient in atrial myocytes was analyzed by confocal imaging after loading myocytes with the acetoxymethyl ester of fluo- 3 (Fluo- 3/AM) . Ca2+ signal transmission from L- type Ca2+ channels in the plasma membrane to the sarcoplasma reticular IP3R and RyR in atrial myocytes during atria fibrillation were measured. RESULTS: (1) Ca2 + signal transmission from L-type calcium channel to IP3R in the sarcoplasma reticulum: intracellular Ca2+ concentration was slightly increased in two groups after blocking T- type calcium channel and RyR, but showed no statistic significance (P > 0.05) between them. (2) Ca2+ signal transmission from L- type calcium channel to RyR in the sarcoplasma reticulum: intracellular Ca2+ concentration was risen (1.5576±0.1989) in control groups after blocking T - type calcium channel and IP3R, and no significance was observed ( P > 0.05) compared with that in atrial fibrillation group (1.5372±0.2952). CONCLUSIONS: Ca2 + signal transmission possibly exists from L- type calcium channel to RyR and IP3R in the sarcoplasma reticulum, but it does not play an important role in intracellular Ca2+- overload and abnormal Ca2+ signal transmission during atrial fibrillation.

Abstract:Objective:To investigate calcium channel gene mRNA expression effects in atrial fibrillation mechanisms.Method:Fifteen dogs were randomly individed into three groups-atrial fibrillation group (n=5), atrial fibrillation and mibefradil group (n=5) and control group (without pacemaker implantation) (n=5). Atrial fibrillation dogs underwent continuous rapid atrial pacing (520 bpm) for twenty-four weeks to create persistent atrial fibrillation. L-type calcium channel α1 subunit, T-type calcium channel α1H subunit, Na+/Ca2+ exchanger and Kv4.3 mRNA were analysed by reverse transcription and polymerase chain reaction (RT-PCR).Result:In the atrial fibrillation group, L-type calcium channel α1 subunit mRNA slightly decreased (P>0.05), T-type calcium channel α1H subunit mRNA remarkably increased (P<0.05), Na+/Ca2+ exchanger mRNA did not change significantly (P>0.05), and Kv4.3 mRNA significantly decreased (P<0.05), compared with in the control group.Conclusion:T-type calcium channel α1H subunit mRNA significantly increased and Kv4.3 mRNA significantly decreased, which may be a major cause of calcium overload in persistent atrial fibrillation for six months. L-type calcium channel α1 subunit and Na+/Ca2+ exchanger mRNA did not change remarkably, suggesting that they did not play an important role in atrial fibrillation mechanisms.

Abstract:Ca2+ is a fundamental second messenger in all cell types and is required for numerous essential cellular functions, including cardiac and skeletal muscle contraction. The intracellular concentration of free Ca2+ ([Ca2+]) is regulated primarily by ion channels, pumps (ATPases), exchangers and Ca2+-binding proteins. Defective regulation of [Ca2+] is found in a diverse spectrum of pathological states that affect all the major organs. In the heart, abnormalities in the regulation of cytosolic and mitochondrial [Ca2+] occur in heart failure (HF) and atrial fibrillation (AF), two common forms of heart disease and leading contributors to morbidity and mortality. In this Review, we focus on the mechanisms that regulate ryanodine receptor 2 (RYR2), the major sarcoplasmic reticulum (SR) Ca2+-release channel in the heart, how RYR2 becomes dysfunctional in HF and AF, and its potential as a therapeutic target. Inherited RYR2 mutations and/or stress-induced phosphorylation and oxidation of the protein destabilize the closed state of the channel, resulting in a pathological diastolic Ca2+ leak from the SR that both triggers arrhythmias and impairs contractility. On the basis of our increased understanding of SR Ca2+ leak as a shared Ca2+-dependent pathological mechanism in HF and AF, a new class of drugs developed in our laboratory, known as rycals, which stabilize RYR2 channels and prevent Ca2+ leak from the SR, are undergoing investigation in clinical trials.

Calcium homeostatic feedback control predicts atrial fibrillation initiation, remodeling, and progression

Abstract:Atrial fibrillation (AF) is a progressive disorder, with arrhythmia episodes increasingly longer and ultimately permanent. The chaotic electrical activity by itself is well-known to drive progression, a process classically summarized as "AF begets AF." However, the mechanisms underlying this progression are not yet well defined. We hypothesize that calcium homeostatic feedback regulating ion channel expression is a critical mechanistic component of this pathological process. We propose a modeling framework that tracks both short-term beat-to-beat electrical and calcium activity and long-term tissue substrate remodeling as a single coupled dynamical system. Importantly, the full AF progression from healthy to pathological remodeled tissue is reproduced, in contrast with prior studies that consider "snapshots" of various AF stages. Simulations predicts that single cells respond to fast pacing by maintaining intracellular calcium concentrations through dynamic ion channel expression and electrical phenotype changes. In two-dimensional (2D) homogeneous tissue, spontaneous spiral waves stabilize into permanent re-entry. In 2D heterogeneous tissue, we observe the initiation of re-entrant activity in response to fast pacing, followed by increasingly longer intermittent, and then permanent, arrhythmic activity. Simulations predict critical properties of re-entrant wave locations, leading to a novel hypothesis: spiral wave activity itself drives substrate remodeling and the emergence of remodeled tissue "niches" that support the initiation and ultimately stabilization of fast re-entrant activity. Thus, the model joins multiple lines of inquiry (i.e., long-term calcium regulation, ion channel co-expression and remodeling, and tissue-scale arrhythmia spatiotemporal organization) into a single coherent framework, and for the first time, captures the dynamics of the long-term natural history of AF.

Abstract:Rationale: The mechanisms underlying atrial fibrillation (AF), the most common clinical arrhythmia, are poorly understood. Nucleoplasmic Ca 2+ regulates gene-expression, but the nature and significance of nuclear Ca 2+ -changes in AF are largely unknown. Objective: To elucidate mechanisms by which AF alters atrial cardiomyocyte (CM) nuclear Ca 2+ ([Ca 2+ ] Nuc ) and Ca 2+ /calmodulin-dependent protein kinase-II (CaMKII)-related signaling. Methods and Results: Atrial CMs were isolated from control and AF-dogs (kept in AF by atrial tachypacing [600 bpm x 1 week]). [Ca 2+ ] Nuc and cytosolic [Ca 2+ ] (Ca 2+ ] Cyto ) were recorded via confocal microscopy. Diastolic [Ca 2+ ] Nuc was greater than [Ca 2+ ] Cyto under control conditions, while resting [Ca 2+ ] Nuc was similar to [Ca 2+ ] Cyto ; both diastolic and resting [Ca 2+ ] Nuc increased with AF. Inositol-trisphosphate-receptor (IP 3 R) stimulation produced larger [Ca 2+ ] Nuc increases in AF versus control CMs, and IP 3 R-blockade suppressed the AF-related [Ca 2+ ] Nuc -differences. AF upregulated nuclear protein-expression of IP 3 R-type 1 (IP 3 R1) and of phosphorylated CaMKII (immunohistochemistry and immunoblot), while decreasing the nuclear/cytosolic expression-ratio for histone deacetylase type-4 (HDAC4). Isolated atrial CMs tachypaced at 3 Hz for 24 hours mimicked AF-type [Ca 2+ ] Nuc changes and L-type calcium current (ICaL) decreases versus 1-Hz-paced CMs; these changes were prevented by IP3R knockdown with short-interfering RNA directed against IP 3 R1. Nuclear/cytosolic HDAC4 expression-ratio was decreased by 3-Hz pacing, while nuclear CaMKII and HDAC4 phosphorylation were increased. Either CaMKII-inhibition (by autocamtide-2-related peptide) or IP 3 R-knockdown prevented the CaMKII-hyperphosphorylation and nuclear-to-cytosolic HDAC4 shift caused by 3-Hz pacing. In human atrial CMs from AF patients, nuclear IP 3 R1-expression was significantly increased, with decreased nuclear/non-nuclear HDAC4 ratio. MicroRNA-26a was predicted to target ITPR1 (confirmed by Luciferase assay) and was downregulated in AF atrial CMs; microRNA-26a silencing reproduced AF-induced IP3R1 upregulation and nuclear diastolic Ca 2+ -loading. Conclusions: AF increases atrial CM nucleoplasmic Ca 2+ -handling by IP 3 R1-upregulation involving miR-26a, leading to enhanced IP 3 R1-CaMKII-HDAC4 signaling and I CaL -downregulation.

Abstract:Objective To inquire into T-type calcium channel effects on atrial electrical remodelling and its mechanisms during atrial fibrillation.Methods Animal experiment was performed in Peking University Pepole's Hospital from Feb.2002 to Oct.2006.Fifteen adult cross-bred dogs were used in the experiment.Ten dogs underwent continuous rapid atrial pacing(500 beats/min)for twenty-four weeks to create persistent atrial fibrillation.In five rapidly-paced dogs,50mg pure powder/day of mibefradil dihydrochloride was given from the second day after pacemaker implantation and continued until the twenty-fourth week.A group of size-matched dogs(n=5)without being given mibefradil was used as a pure atrial fibrillation group.Another group of size-matched dogs(n=5)without pacemaker implantation was used as a control group.Atrial fibrillation duration was determined by electrophysiological study.Canine atrial myocytes were isolated by enzymatic dissociation and intracellular Ca2+ cytosolic transient was studied with confocal imaging.Results (1)The preoperation atrial effective refractory period was 280/90~110 ms.In the twenty-fourth week after rapid atrial pacing,atrial effective refractory period was obviously extended(2000/1400~1700 ms)in the T-type Ca2+ channel blocker group compared with the preoperation one.In the twenty-fourth week,the induced rate of persistent atrial fibrillation was 75% in atrial fibrillation group,whereas the persistent atrial fibrillation occurred in only one case(20%)in the T-type Ca2+ channel blocker group.(2)Intracellular Ca2+ concentration of atrial myocytes was unremarkably changed in control group after blocking L-type Ca2+ channel(1.17±0.09 OD ratio),whereas the intracellular Ca2+ concentration was obviously enhanced(2.35±1.05 OD ratio)(P0.05)in atrial fibrillation group compared with that of control group,and the Ca2+ concentration in the T-type Ca2+ channel blocker group was reduced by 30%(2.05±0.90 OD ratio)compared with that of atrial fibrillation group.(3)Intracellular Ca2+ concentrations of atrial myocytes in three groups were slightly increased after blocking T-type Ca2+ channel,and no difference between them(1.49±0.17 OD ratio)in control group,1.53±0.33 OD ratio in atrial fibrillation group and 1.38±0.35 OD ratio in T-type Ca2+ channel blocker group).Conclusion (1)T-type Ca2+channel blocker can obviously prolong atrial effective refractory period and inhibit the perpetuation of atrial fibrillation,but it can not prevent the occurrence of atrial fibrillation.(2)T-type Ca2+current is increased and L-type Ca2+ current is reduced in atrial myocytes during atrial fibrillation,suggesting that T-type Ca2+channel may play an important role in the mechanisms of intracellular Ca2+-overload during atrial fibrillation.
Exploring the Impact of BKCa Channel Function in Cellular Membranes on Cardiac Electrical Activity

Abstract:This review paper delves into the current body of evidence, offering a thorough analysis of the impact of large-conductance Ca2+-activated K+ (BKCa or BK) channels on the electrical dynamics of the heart. Alterations in the activity of BKCa channels, responsible for the generation of the overall magnitude of Ca2+-activated K+ current at the whole-cell level, occur through allosteric mechanisms. The collaborative interplay between membrane depolarization and heightened intracellular Ca2+ ion concentrations collectively contribute to the activation of BKCa channels. Although fully developed mammalian cardiac cells do not exhibit functional expression of these ion channels, evidence suggests their presence in cardiac fibroblasts that surround and potentially establish close connections with neighboring cardiac cells. When cardiac cells form close associations with fibroblasts, the high single-ion conductance of these channels, approximately ranging from 150 to 250 pS, can result in the random depolarization of the adjacent cardiac cell membranes. While cardiac fibroblasts are typically electrically non-excitable, their prevalence within heart tissue increases, particularly in the context of aging myocardial infarction or atrial fibrillation. This augmented presence of BKCa channels' conductance holds the potential to amplify the excitability of cardiac cell membranes through effective electrical coupling between fibroblasts and cardiomyocytes. In this scenario, this heightened excitability may contribute to the onset of cardiac arrhythmias. Moreover, it is worth noting that the substances influencing the activity of these BKCa channels might influence cardiac electrical activity as well. Taken together, the BKCa channel activity residing in cardiac fibroblasts may contribute to cardiac electrical function occurring in vivo.

Abstract:Our results clearly indicated that the spatiotemporal properties but not the frequency of Ca(2+) sparks were affected in AF atrial myocytes. In addition, the frequency of Ca(2+) waves increased. This profile of the alterations in RyR-mediated Ca(2+) events in AF atrial myocytes was different from previous studies. The underlying mechanisms, as well as possible reasons for this discrepancy, were discussed.

Abstract:The complexity of the heart makes an intuitive understanding of the relative contribution of ion channels, transporters and signalling pathways to cardiac electrophysiology challenging. Computational modelling of cardiac cellular electrophysiology has proven useful to integrate experimental findings, extrapolate results obtained in expression systems or animal models to other systems, test quantitatively ideas based on experimental data and provide novel hypotheses that are experimentally testable. While the bulk of computational modelling has traditionally been directed towards ventricular bioelectricity, increasing recognition of the clinical importance of atrial arrhythmias, particularly atrial fibrillation, has led to widespread efforts to apply computational approaches to understanding atrial electrical function. The increasing availability of detailed, atrial-specific experimental data has stimulated the development of novel computational models of atrial-cellular electrophysiology and Ca(2+) handling. To date, more than 300 studies have employed mathematical simulations to enhance our understanding of atrial electrophysiology, arrhythmogenesis and therapeutic responses. Future modelling studies are likely to move beyond current whole-cell models by incorporating new data on subcellular architecture, macromolecular protein complexes, and localized ion-channel regulation by signalling pathways. At the same time, more integrative multicellular models that take into account regional electrophysiological and Ca(2+) handling properties, mechano-electrical feedback and/or autonomic regulation will be needed to investigate the mechanisms governing atrial arrhythmias. A combined experimental and computational approach is expected to provide the more comprehensive understanding of atrial arrhythmogenesis that is required to develop improved diagnostic and therapeutic options. Here, we review this rapidly expanding area, with a particular focus on Ca(2+) handling, and provide ideas about potential future directions.
Atrial fibrillation in the setting of cardiac amyloidosis - A review of the literature

Abstract:Cardiac amyloidosis (CA) is related to the aggregation of insoluble fibrous deposits of misfolded proteins within the myocardium. Transthyretin amyloidosis (ATTR) and immunoglobulin light-chain amyloidosis are the main forms of CA. Atrial fibrillation (AF) is a common arrhythmia in CA patients, especially in those with ATTR amyloidosis. Increased atrial preload and afterload, atrial enlargement, enhanced atrial wall stress, and autonomic dysfunction are the main mechanisms of AF in CA patients. CA is associated with the formation of endocardial thrombi and systemic embolism. The promoters of thrombogenesis include endomyocardial damage, blood stasis, and hypercoagulability. The prevalence of thrombi in patients with AF remains elevated despite long-term anticoagulation. Consequently, transesophageal ultrasound examinations before cardioversion should be performed to exclude endocardiac thrombi despite anticoagulation. Furthermore, the CHA2DS2-VASc score should not be used to assess the thromboembolic risk in CA patients with AF. Rate control is challenging in patients with CA, while rhythm control is the preferred treatment option, especially in the early stages of the disease process. Although catheter ablation is an effective treatment option, more data are needed to explore the role of the procedure in CA patients.
Influences of Angiotensin Ⅱ on Calcium Channel Current and Intracellular Free Calcium Concentration of Human Atrial Myocytes and the Antagonistic Effect of Amiodarone

Abstract:Objective To observe the influences of angiotensin Ⅱ (AngⅡ) on calcium channel current and intracellular free calcium concentration ([Ca 2+]i) of human atrial myocytes, so as to reveal the electrophysiological mechanisms of atrial arrhythmia induced by AngⅡ, and to investigate the antagonistic effect of amiodarone for providing the theoretical basis in treating atrial arrhythmias.Methods Single human atrial myocyte was isolated. The conventional whole-cell configuration of the patch-clamp technique was used to detect membrane I Ca-L, and confocal microscope was used with Fluo-3/AM as indicator to detect changes of [Ca 2+]i. The experiment comprised of four groups, ie, control group, AngⅡ group, amiodarone group and AngⅡ+amiodarone group.Results Comparing with the control group, AngⅡ of 0.1 μmol/L significantly increased the peak density of I Ca-L in human atrial myocyte [(-12.77±1.61) pA/pF vs (-5.78±0.81) pA/pF, P0.05). Amiodarone of 30 nmol/L had no significant effect on I Ca-L of human atrial myocyte [(-5.58±0.76) pA/pF], but it could antagonize the effects of AngⅡ. In AngⅡ+amiodarone group, the peak density of I Ca-L was [(-7.15±0.82)] pA/pF, which had a significant difference in comparing with the AngⅡ group (P0.05). The intracellular fluoresence intensity of single cardiomyocyte in both control and amiodarone groups was low. The intracellular fluoresence intensity of single cardiomyocyte in AngⅡ group was significantly higher than that in the control group after 15 minutes intervention (P0.05). When amiodarone was added simultaneously with AngⅡ intervention, the values were much lower comparing with AngⅡ group (P0.05).Conclusion AngⅡ of 0.1 μmol/L can significantly increase the peak density of I Ca-L and induce intracellular calcium overload in human atrial myocytes, which may participate in human atrial electrical remodeling and relevant to occurrence and progression of atrial fibrillation. Amiodarone can antagonize the above effects of AngⅡ in human atrial myocytes.

Abstract:The calcium ion(Ca2+)is the simplest and the most versatile intracellular second messenger known to date.Over last decade,the discovery of Ca2+ sparks and a family of local elementary Ca2+ signaling events has unveiled a digital-analog feature of intracellular Ca2+ signaling.Brief,local Ca2+ microdomains and nanodomains are commingled with the continuous global analog Ca2+ signals.Patterned activation of digital Ca2+ signals confers on intracellular Ca2+ signaling a multi-scaled architecture in space,time and magnitude,and is of particular relevance in spatiotemporal regulation of high-threshold Ca2+-dependent processes(e.g.,striated muscle excitation-contraction coupling,exocytosis).Investigation on spark activation,coordination,and termination has revealed new insights as well as perplexing questions about gating of Ca2+ release channels(ryanodine receptor,inositol 1,4,5-trisphosphate receptor)in intact cells,and brought about mechanistic understanding of the diverse and even opposing roles of Ca2+ signaling.Future investigation on digital-analog dichotomy of Ca2+ signaling thus holds the promise to unify the simplicity and complexity of Ca2+ signaling in biology.
