Although such condition is not extremely rare, it is an challenge in the differential diagnosis of pulmonary problems such as hypoxemia and pulmonary lesions.Case presentation: We report a meaningful case of a 23-year-old male presented with elevated hemoglobin (23.0 g/dl) on admission. Physical examination revealed cyanosis, digital clubbing and low oxygen saturation on room air. The patient was initially diagnosed as polycythemia vera while the subsequent result of bone marrow aspiration was negative. During further assessment, pulmonary arteriovenous malformations were detected by computed tomographic angiography of pulmonary artery. Lobectomy was successfully performed with significant increase of oxygen saturation from 86% to 98%. The patient received an uneventful postoperative recovery and was followed up for nearly 5 years without recurrence.Conclusions: Pulmonary arteriovenous malformations should be suspected in the patients with central cyanosis, digital clubbing, polycythemia and pulmonary lesion and without cardiac malformations. Embolization or surgery is highly recommended to reduce the risks caused by pulmonary arteriovenous malformations.

Abstract:BACKGROUND:New materials and devices have been used in the management of cardiac malformations. In this paper, we present our experience with interventional occlusion of congenital vascular malformations.METHODS:Between January 1997 and December 2005, 139 patients with congenital vascular malformations who had undergone interventional occlusion in the Children's Hospital, Zhejiang University School of Medicine were studied. The clinical data of the patients were retrospectively reviewed including pre-operative evaluation, surgical procedures, immediate complete closure rate, short-term complications, and short-term outcome.RESULTS:Of the 139 patients, 126 had patent ductus arteriosus, and successful deployment was achieved in 121 of the 126 patients (96%, 121/126). Six patients had coronary artery fistula and 14 different coils were used for embolization; the immediate complete closure rate was 83.3%, and the complete closure rate after one month was 100%. The abnormal vessels of 3 patients with pulmonary sequestration were completely occluded using four 0.038-inch Gianturco coils. In 3 patients with aortopulmonary collaterals, 14 abnormal vessel branches were occluded with sixteen 0.038-inch Gianturco coils, reaching a closure rate of 100%. One patient with pulmonary arteriovenous fistula was occluded successfully with two 0.038-inch Gianturco coils.CONCLUSIONS:Transcatheter closure using coils is a safe and effective alternative to surgical ligation in the management of congenital vascular malformations in children. Selection of appropriate coils is important to achieve a better outcome.
Transcatheter arterial embolization for treatment of pulmonary sequestration in pediatric patients

Abstract:Objective To introduce a new interventional embolization technique for the treatment of pulmonary sequestration in pediatric patients. Methods Between 1995 and 2004, 10 patients (8 males and 2 females, aged 1 to 13 years with average of 9.9±5.8 years)received the treatment.All patients had repeated respiratory tract infection and hemoptysis.The diagnosis of pulmonary sequestration was confirmed by chest X_ray,thoracic CT and MRI scans.Selective angiography revealed vascular conglomeration and the origin,the diameter and the number of hypertrophic feeding arteries,as well as concomitant venous vessels.The blood_supplying arterial vessel was occluded by placing an Appleatzer or metal spring coil which was 30%～40% larger in diameter than the vessel. Results All the interventional treatments were successful and no fistula remained.The symptom of hemoptysis was immediately relieved and respiratory infections disappeared within 3 to 5 days after the treatment.One patient treated with an Appleatzer complained chest pain for about two weeks,while temperature,heart rate,respiratory rate and blood pressure remained normal.One to six months after the interventional therapy,the chest X_ray examination was performed in all cases.Pre_interventional opacities had disappeared completely in 8 patients,whereas slight densities remained in 2 patients without clinical symptoms. Conclusions The interventional treatment is a new option with safe and effective cure,and quick recovery without leaving obvious scars, thoracic surgery for lobe excision can be hence avoided in pediatric patients with pulmonary sequestration.

Abstract:Arteriovenous malformation (AVM) is a kind of life threatened disease. Especially AVM in the posterior mediastinum is a rare, painful and it is difficult for treatment. We report a 44-year-old male patient who developed arteriovenous malformation (AVM) in the posterior mediastinum. The patient complained 3 years of mild left back pain and chest congestion, and his pain increased over the last 3 months. Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) examinations showed an enhancing vascular lesion in the left posterior mediastinum which in contact with the descending thoracic aorta. It was initially misdiagnosed as lung mass. It was considered to be vascular malformations. His pain was completely resolved after steel coil embolization was successfully administered.

Abstract:Coils are typically utilized as one of the most popular embolization agents. Coil migration to the pulmonary artery rarely occurs and is associated with a high tendency of severe pulmonary or cardiovascular complications. We herein present a 25-year-old man with arteriovenous malformations in the right ear. Two coils 4-mm in diameter and 3-mm in length migrated to the pulmonary artery during embolization. No further damage related to the coil migration was found during a 2-year follow-up period. In addition, the possible mechanisms of coil migration are discussed.

Abstract:Although there have been a few case reports of pulmonary sequestration(PS), it is primarily located in the lower lobe and left lung, rarely in the right upper lobe. Here, we report a case presented with haemoptysis. Computed tomography (CT) images revealed flake ground-glass shadows in the right upper lobe. Computed tomography angiography (CTA) demonstrated an artery supplied the affected lesions stemmed from the aortic arch. We diagnosed and treated her with bronchial artery angiography with coil embolization. No complications were found after operation until now. Thus, CTA could help identify the abnormal blood vessels, and interventional therapy may be an effective alternative to surgery of pulmonary sequestration.

Abstract:Abstract Background Hereditary haemorrhagic telangiectasia (HHT), or Rendu–Osler–Weber syndrome, is a rare genetic disorder characterized by the development of telangiectasias and arteriovenous malformations (AVMs) throughout the body. We present a case of percutaneous embolization of pulmonary AVMs in an adult patient. Case summary A 26-year-old male patient with polycythaemia of unknown origin and a family history of secundum atrial septal defect underwent cardiac evaluation which revealed clubbing as a sign of peripheral cyanosis. Transthoracic echocardiography showed no intracardiac shunting, but further imaging revealed pulmonary AVMs in the lower lobe of the left lung. Magnetic resonance imaging of the brain detected vascular-ischaemic lesions, likely due to embolization through the pulmonary malformations. Right heart catheterization and pulmonary angiography confirmed the presence of large AVMs in the left lower pulmonary lobe. Percutaneous closure using Amplatzer devices was performed, followed by temporary anticoagulation therapy. Oxygen saturation improved and follow-up imaging confirmed successful closure of the AVMs. Genetic testing using whole exome sequencing identified a mutation in the ENG gene, confirming the diagnosis of HHT. Discussion Our case highlights the importance of investigating both intra- and extracardiac shunting in patients with clinical features of right-to-left shunting. Arteriovenous malformations can serve as a pathway for paradoxical emboli, potentially leading to ischaemic brain events, and might cause pulmonary arterial hypertension. Screening for arteriovenous malformations in various organs and embolization of significant shunts are essential aspects of managing HHT. Genetic testing aids in confirming the diagnosis and guides family testing.
Transcatheter Coil Occluder for Closure of Ventricular Septal Defect (A Report of 4 Cases)

Abstract:Objective To explore the indication, methodology and complication of transcatheter coil closure of ventricular septal defect (VSD) in children. Methods Transcatheter closure of perimembranous VSD with coils was performed in 4 cases from 2003 to 2005. The Duct-Occlud (pfm) and detachable coil(Cook) were chosen for embolization depending on the results of the left ventricular angiogram. The coil size was generally about 1～4 mm larger than the diameter of VSD. Follow up was carried out with echocardiography, ultrasound and clinical examination. Results The defect diameters of the four cases were 2.0 mm, 2.7 mm, 2.5 mm and 1.5 mm respectively. The Duct-Occlud were successfully implanted in 3 cases of perimembranous VSD with the same type coil (OD〔mm〕7-3-6,windings 5-3-4) for each. One detachable coil(Cook)(5×5) was implanted in the remaining case. All cases had trivial residual shunt immediately after implantation which disappeared 24 hours later. Follow-up for 2 months to one year showed no coil displacement and secondary bacterial arteritis. No tricuspid and aortic regurgitation, no emboli, no endocarditis, and no arrhythmia were found.Conclusions Coil closure of some small VSD with membranate part pseudo-ventricular aneurysm has good efficacy with the advantages of simple operation, less metal content and mini-invasion also applicable for infants.
Pulmonary Arteriovenous Malformation Detected by Three-dimensional Computed Tomographic Angiography.

Abstract:A 9-year-old boy from a remote village in southern China, who experienced moderate exercise-induced shortness of breath and has received anti-tuberculosis treatment for about half a year without improving, presented to our department. Physical examination revealed a reduced arterial oxygen saturation level (79% at room temperature, not corrected by oxygen support) and mild central cyanosis with clubbed fingers and toes. Chest radiography showed a huge abnormal shadow in the left upper lung field (Figure 1A and B, arrows). Echocardiography revealed an enormous nascystic area beside the main pulmonary artery without pulmonary hypertension. Computed tomography was performed and revealed complex vascular structures with two large aneurysmal sacs (diameter 37 mm and 15 mm, respectively) in the left upper lobe (Figure 2A and B, arrows) and the segmental arteries of two anterior segment artery branches were defined as the feeding arteries of the complex malformation (Figure 2C and D, arrows), and three-dimensional computed tomography angiography (Figure 2E and F, arrow) revealed an enhanced lung nodule that was connected with linear structures suggestive of feeding arteries and two huge drainage veins (see video). The patient’s family medical history, clinical examination and imaging studies did not reveal any evidence of hereditary haemorrhagic telangiectasia. Following communication with the patient’s parents, the embolisation and video-assisted thoracoscopic surgery (VATS) were refused and surgical resection was requested due to insufficient financial support. Subsequently, the left upper pulmonary lobe was surgically removed. Intraoperative finding was that the connective part of the malformation between the arterial and venous vasculature is fragile and tends to rupture (Figure 3A). The postoperative histological examination confirmed the diagnosis of pulmonary arteriovenous malformations (Figure 3B). The patient was discharged uneventfully on postoperative day 7 with the arterial oxygen saturation level returned to normal (99%). He is in good condition and free of recurrence in the follow-up visit at nine months. Evaluation of percutaneous transcatheter embolization for pulmonary arteriovenous malformations

Abstract:Abstract Background The purpose of this study was to assess the safety and efficacy of percutaneous transcatheter embolization (TCE) for the treatment of pulmonary arteriovenous malformations (PAVMs). Methods Forty-three consecutive patients (n = 17 males; n = 26 females) with 72 untreated PAVMs underwent coil and/or plug embolization between January 2010 and February 2018. The mean patient age was 42 ± 14 years (range 19–71 years). The median size of the feeding artery was 7.9 ± 2.9 mm (range 3.5–14.0 mm). The arterial blood gas level and cardiac function of all patients were analysed. The technical success rate, recanalization rate, and complications were evaluated. Computed tomography angiography (CTA) examinations were scheduled for 12 months after treatment and every 2–4 years thereafter. Results Twenty-five PAVMs were treated with coils alone, twenty-one were treated with plugs alone, and twenty-six were treated with both coils and plugs. The technical success rate was 100%. There were no complications during operation. However, one patient (2.3%) had pulmonary thrombosis and embolism post-operation. The patients’ pre-operative and post-operative PaO 2 and SaO 2 levels were significantly different ( p < 0.01). A comparison of the New York Heart Association (NYHA) grade before and after embolization in all patients showed a significant decrease in the post-operative grade ( p < 0.01). The 72 PAVMs were divided into three groups (coils only group [n = 25], plugs only group [n = 21], and coils/plugs combined group [n = 26]). After 12 months of follow-up, there were seven reperfusion PAVMs in the coil group, seven reperfusion PAVMs in the plug group, and 1 reperfusion PAVM in the combined group. There were significant differences between the two groups and the combined group. Conclusion Percutaneous TCE is safe and effective for the treatment of PAVMs. A combination of coils and vascular plugs may be useful for preventing recanalization after the embolization of PAVMs.

Heart-lung Transplantation in Eisenmenger Syndrome with Pulmonary Embolisms

Abstract:A 37-year-old man with a known history of an atrial septal defect was admitted to the hospital with shortness of breath, hemoptysis, chest pain, and tightness. On admission, the patient had a heart rate of 120 beats/min, respiratory rate of 21 breaths/min, basal resting oxygen saturation of 81%, and blood pressure of 99/60 mmHg. Transthoracic echocardiography showed the presence of a secundum type of atrial septal defect with a diameter of 21 mm, right ventricular (RV) enlargement, interventricular septal flattening, and severely impaired RV systolic function (fractional area change = 27%) (Fig. A). Color Doppler images showed a sizeable bidirectional shunting across the interatrial septum, confirming ongoing interatrial communication and significant pulmonary hypertension (PH) (Fig.B, video 1). In addition, there was severe tricuspid (Video 1) and pulmonary regurgitation. Continuous-wave Doppler imaging showed severe PH with RV systolic pressure of 103 mmHg and mean pulmonary artery (PA) pressure of 61 mmHg. Furthermore, echocardiography showed a mixed echogenicity intravascular mass that expands the PA lumen, accompanied by aneurysmal dilatation of the main PA and the obstruction of the PA branches (Fig. C, video 2). Color Doppler imaging showed no discernable flow of PA branches.

Robotic right lower lobectomy for a persistent large pulmonary arteriovenous malformation following repeated coil embolization

Abstract:Pulmonary arteriovenous malformations create continuous shunting of unoxygenated blood through the lungs into the systemic circulation. These malformations are asymptomatic if small, but cause serious symptoms as they grow in size. Treatment primarily consists of endovascular embolization; lobectomy is preserved for recurring or endovascularly untreatable cases. We describe a case of a 24-year-old man who was first treated with coil embolization 10 years previously, with complete symptom resolution. However, more recently he noted recurrent exercise intolerance, with shortness of breath and hypoxemia. After repeat re-embolization, a computed tomography scan noted some persistent flow. Given the patient's young age, we considered resection as a definite therapy. The patient underwent an uncomplicated robot-assisted right lower lobectomy. Afterward, his symptoms resolved completely. In selected cases, robotic lobectomy for pulmonary arteriovenous malformation is feasible and safe.
Minimally invasive coil embolization for significant left-to-right shunts due to giant coronary-to-pulmonary artery fistulas: a case report

Abstract:Background: Coronary-to-pulmonary artery fistula (CPF) is a rare disease, and its optimal treatment strategy remains controversial. Herein, we report a rare case of minimally invasive coil embolization of giant CPFs. Case summary: A 78-year-old man with a history of persistent atrial fibrillation and lumbar canal stenosis presented to our hospital with breathlessness. Cardiac computed tomography revealed giant CPFs inducing a significant left-to-right shunt (Qp/Qs 1/2.1) with a coronary artery aneurysm smaller than the size indicated for surgical treatment. To reduce the left-to-right shunt flow, coil embolization procedures for the fistulas were performed twice. Initially, the fistula arising from the right coronary artery was embolized using three Target® XXL (6 × 40 mm, 5 × 20 mm) and two Target® XL SOFT (4 × 12 mm) coils (Stryker Inc., Tokyo, Japan). One month later, the fistulas arising separately from the left coronary artery were embolized. After the procedures, the major shunt flow disappeared angiographically, and Qp/Qs significantly decreased to 1/1.2. Additionally, the fractional flow reserve of the left coronary artery increased from 0.79 to 0.93, and cardiopulmonary exercise testing showed an improvement in his exercise tolerance. Discussion: In similar cases, a surgical procedure with ligation of the CPFs combined with resection of a small aneurysm and coronary artery bypass grafting would normally have been considered the best approach. However, endovascular treatment targeting only the fistulas was a superior strategy considering the patient's age. The coil embolization technique effectively controlled the shunt flow of the CPFs. This technique is considerably less invasive than surgical therapy.

Abstract:Background: The extremity intraosseous arteriovenous malformations (AVMs) are rare high flow vascular lesions for which the treatment remains challenging. The aim of the study wasto assess treatment methods, interim results, and complications of coils and absolute ethanol in managing extremity intraosseous AVMs via direct puncture approach. Methods: From 2009 to 2017, 12 patients (mean age, 27.5 years; range, 3e54 years) with extremity intraosseous AVMs underwent staged coils and absolute ethanol treatment via the direct puncture approach. All patients were symptomatic before the procedure as per the Schobinger staging system. The mechanical detachable coils and undetachable coils were used first followed by the injection of absolute ethanol used as a sclerosant agent via a direct puncture approach. Follow-up evaluation (6 to 72 months; mean, 23.5 months), including imaging and symptoms and signs, was performed in all patients. Results: Twenty-seven absolute alcohol procedures were performed for patients with intravascular sclerosis (range: 1-3, mean: 2) with one procedure required per patient (n = 2), 2 per patient (n = 5), or 3 per patient (n = 5). The average stretched length of the total coils per patient was 843.33 cm. The amount of absolute ethanol used ranged from 10 mL to 45 mL (mean, 25.69 mL) in a single session. Nine of twelve patients (75%) exhibited complete responses, and 3 patients (25%) exhibited partial responses. One patient experienced minor complication of transient motor nerve injury and completely recovered half a month later. No major complications occurred. Conclusions: The study provides strong evidence, suggesting that direct puncture and coils with absolute ethanol approach is an effective and safe treatment in patients with extremity intraosseous AVMs, yielding minor complications and good results.

Abstract:Vein of Galen aneurysmal malformation is a relatively rare disease in which failure of the median prosencephalic vein of Markowski to involute early in gestation leads to a grossly dilated deep cerebral vein with multiple arterial feeders, causing a large arteriovenous shunt which leads to high-output cardiac failure. We describe a case of a term neonate who presented to a tertiary neonatal centre on day one of life with history, symptoms, and signs consistent with perinatal asphyxia; however, in the context of worsening multi-organ dysfunction and cardiomegaly, the infant was found to have a severe vein of Galen aneurysmal dilatation leading to high-output cardiac failure. The patient was transferred to a tertiary paediatric hospital and underwent a total of four coiling procedures to embolise the multiple feeder arteries supplying the aneurysmal malformation. This case highlights the difficulties in diagnosing this relatively uncommon condition, particularly in the context of a possible perinatal insult.
