Analysis of the Diagnosis and Treatment Options of Gestational Idiopathic Thrombocytopenic Purpura

Abstract:Objective:To investigate the clinical and laboratory features of Gestational Idiopathic Thrombocytopenic Purpura(GITP),for the improvement of diagnosis and treatment of this severe pregnancy complications.Methods:The clinical and laboratory data of 44 hospitalized patients with GITP were collected retrospectively,these data were compared with that of 148 cases of Gestational Thrombocytopenia(GT),18 cases of Gestational Hypertension associated Thrombocytopenia(GHT) and 42 cases of un-pregnant female ITP patients.Results: The onset time of patients with GITP was earlier than GT and GHT,and the bleeding tendency of GITP patients is obvious in GITP group.The platelet count at presentation and the nadir platelet count during the treatment in GITP group were significantly lower than that of the other two kinds of pregnancy associated thrombocytopenia.45.8% of patients with GITP who tested for platelet associated IgG(PAIgG) were positive,while this number was almost 0 in the GT and GHT group.The rate of cesarean delivery in GITP was higher than that in GT,comparable to that of GHT group.But the volume of hemorrhage during and after the surgery and the abnormal rate of the neonate Apgar scaling were not different among all of the three groups.The neonate body weight from GITP group had no significant difference from that of the other two groups,but the body weight of the GHT group neonate were relatively lower than that from GT group.The incidence of neonatal thrombocytopenia was higher in GITP group,but the difference is not statistically significant due to the low recorded numbers.The platelet count of the majority GT and GHT women recovered to normal level after delivery,but the situation was different in GITP group,in which the platelet count increased much less,further treatments were needed for the GITP patients after delivery.Compared to the un-pregnant patients with ITP,the ones with GITP were not so severe,and the responding rate to corticosteroids was almost the same.Conclusion: The clinical features shown GITP is more sever than GT,but the maternal and fetus outcome of GITP is basically favorable when appropriate treatments were applied.So the earlier detection of GITP and routine monitoring of the platelet count thereafter are important.The severity of ITP would not deteriorate during pregnancy,and the situation could not be alleviated by delivery either.Further treatments were needed for the GITP patients after delivery.
