Abstract:As a result of the growing demand for health services, China's large city hospitals have become markedly overstretched, resulting in delicate and complex operating room scheduling problems. While the operating rooms are struggling to meet demand, they face idle times because of (human) resources being pulled away for other urgent demands, and cancellations for economic and health reasons. In this research we analyze the resulting stochastic operating room scheduling problems, and the improvements attainable by scheduled cancellations to accommodate the large demand while avoiding the negative consequences of excessive overtime work. We present a three-stage recourse model which formalizes the scheduled cancellations and is anticipative to further uncertainty. We develop a solution method for this three-stage model which relies on the sample average approximation and the L-shaped method. The method exploits the structure of optimal solutions to speed up the optimization. Scheduled cancellations can significantly and substantially improve the operating room schedule when the costs of cancellations are close to the costs of overtime work. Moreover, the proposed methods illustrate how the adverse impact of cancellations (by patients) for economic and health reasons can be largely controlled. The (human) resource unavailability however is shown to cause a more than proportional loss of solution value for the surgery scheduling problems occurring in China's large city hospitals, even when applying the proposed solution techniques, and requires different management measures.

Abstract:BACKGROUND:Operating Room (OR) is the core sector in hospital expenditure, the operation management of which involves a complete three-stage surgery flow, multiple resources, prioritization of the various surgeries, and several real-life OR constraints. As such reasonable surgery scheduling is crucial to OR management.OBJECTIVE:To optimize OR management and reduce operation cost, a short-term surgery scheduling problem is proposed and defined based on the survey of the OR operation in a typical hospital in China.METHOD:The comprehensive operation cost is clearly defined considering both under-utilization and overutilization. A nested Ant Colony Optimization (nested-ACO) incorporated with several real-life OR constraints is proposed to solve such a combinatorial optimization problem.RESULTS:The 10-day manual surgery schedules from a hospital in China are compared with the optimized schedules solved by the nested-ACO. Comparison results show the advantage using the nested-ACO in several measurements: OR-related time, nurse-related time, variation in resources' working time, and the end time.CONCLUSIONS:The nested-ACO considering real-life operation constraints such as the difference between first and following case, surgeries priority, and fixed nurses in pre/post-operative stage is proposed to solve the surgery scheduling optimization problem. The results clearly show the benefit of using the nested-ACO in enhancing the OR management efficiency and minimizing the comprehensive overall operation cost.

Abstract:The complex optimisation problems arising in the scheduling of operating rooms have received considerable attention in recent scientific literature because of their impact on costs, revenues and patient health. For an important part, the complexity stems from the stochastic nature of the problem. In practice, this stochastic nature often leads to schedule adaptations on the day of schedule execution. While operating room performance is thus importantly affected by such adaptations, decision-making on adaptations is hardly addressed in scientific literature. Building on previous literature on adaptive scheduling, we develop adaptive operating room scheduling models and problems, and analyse the performance of corresponding adaptive scheduling policies. As previously proposed (fully) adaptive scheduling models and policies are infeasible in operating room scheduling practice, we extend adaptive scheduling theory by introducing the novel concept of committing. Moreover, the core of the proposed adaptive policies with committing is formed by a new, exact, pseudo-polynomial algorithm to solve a general class of stochastic knapsack problems. Using these theoretical advances, we present performance analysis on practical problems, using data from existing literature as well as real-life data from the largest academic medical centre in The Netherlands. The analysis shows that the practically feasible, basic, 1-level policy already brings substantial and statistically significant improvement over static policies. Moreover, as a rule of thumb, scheduling surgeries with large mean duration or standard deviation early appears good practice.

Abstract:Operating rooms are considered as the most valuable resource in hospital, which is important for the service quality to patients and the efficiency improvement in hospital operations management. This paper studies the week scheduling of operating rooms integrating other limited key resources including ICU beds and the workload of surgeons. A mixed integer program model has been proposed to optimize the operating room schedule. The results showed the utilization of each operating room has been balanced and maximized. Along with the sensitivity analysis, this model can be used to assist hospital managers in operating room decision making.

Abstract:The problem studied in this paper is elective surgery scheduling, with resource constraints in each of the three following stages: preoperative, perioperative, and postoperative stages. With the integrated availability of hospital beds in wards and operating rooms, the aim is to determine operation start times of surgeries and allocate the hospital beds to patients while getting patients treated as soon as possible. This task is crucial in providing timely treatments for the patients while ensuring the hospital's resource utilization balance. For the problem, we first formulate it as mixed-integer programming, which is NP-complete. Then, we propose several heuristics to overcome the long computation time. To make the solution better, we also propose improved algorithms. Finally, we conduct a series of numerical studies to illustrate the efficiency of our proposed algorithms and examine the impact of the number of jobs, beds, and surgery blocks on the performance measure. Computational experiments showed the superior performance of our heuristics in makespan.

Abstract:This paper describes a stochastic model for Operating Room (OR) planning with two types of demand for surgery: elective surgery and emergency surgery. Elective cases can be planned ahead and have a patient-related cost depending on the surgery date. Emergency cases arrive randomly and have to be performed on the day of arrival. The planning problem consists in assigning elective cases to different periods over a planning horizon in order to minimize the sum of elective patient related costs and overtime costs of operating rooms. A new stochastic mathematical programming model is first proposed. We then propose a Monte Carlo optimization method combining Monte Carlo simulation and Mixed Integer Programming. The solution of this method is proved to converge to a real optimum as the computation budget increases. Numerical results show that important gains can be realized by using a stochastic OR planning model.

Abstract:The purpose of this paper is to propose and compare several optimization methods for elective surgery planning when operating room (OR) capacity is shared by elective and emergency surgery. The planning problem is considered as a stochastic optimization problem in order to minimize expected overtime costs and patients’ related costs. An “almost” exact method combining Monte Carlo simulation and mixed integer programming is presented, and its convergence properties are investigated. Several heuristic and meta-heuristic methods are then proposed. Numerical experimentations are conducted to compare the performance of different optimization methods.

Abstract:In this paper, we address the need for distributed surgical scheduling across multiple hospitals and multiple days. We develop an effective ex-ante distributed schedule to determine elective service operation and elective patient assignment plan in a hospital alliance with the goal of minimizing the total expected cost considering possible realizations of treatment durations and emergency demands. The problem is formulated into a two-stage stochastic integer programming where uncertainty is characterized by a scenario set. Cancellations of scheduled elective patients and inter-hospital transfers of materialized emergency patients are explicitly modeled as second-stage decisions. To solve the problem, we propose an integer decomposition algorithm that incorporates a novel logic-based optimality cut along with a validity proof. Additionally, we introduce several enhancements to improve the algorithm. Through numerical experiments, we demonstrate that our proposed algorithm outperforms both the commercial solver and the existing integer decomposition algorithm for the proposed problem. It provides effective solutions that closely approach the best upper bounds achieved by the solver in less time, or even surpass the solver's best upper bounds. Furthermore, we provide operational insights into other commonly-used scheduling approaches through numerical comparisons, including the strengths and limitations of the most common way of scheduling elective patients and two strategies for handling emergencies within a hospital alliance.

Abstract:With the advancement of surgery and anesthesiology in recent years, surgical clinical pathways have changed significantly, with an increase in outpatient surgeries. However, the surgical scheduling problem is particularly challenging when inpatients and outpatients share the same operating room blocks, due to their different characteristics in terms of variability and preferences. In this paper, we present a two-phase stochastic optimization approach that takes into account such characteristics, considering multiple objectives and dealing with uncertainty in surgery duration, arrival of emergency patients, and no-shows. Chance Constrained Integer Programming and Stochastic Mixed Integer Programming are used to deal with the advance scheduling and the allocation scheduling, respectively. Since Monte Carlo sampling is inefficient for solving the allocation scheduling problem for large size instances, a genetic algorithm is proposed for sequencing and timing procedures. Finally, a quantitative analysis is performed to analyze the trade-off between schedule robustness and average performance under the selection of different patient mixes, providing general insights for operating room scheduling when dealing with inpatients, outpatient, and emergencies.

Abstract:Day surgery scheduling allocates hospital resources to day surgical cases and decides on the time to perform the surgeries in the day surgery center (DSC). Based on the day surgery service process of large public hospitals in China, we found that the service efficiency of the process depends on the utilization of hospital resources efficiently and could be optimized through day surgery scheduling. We described it as a flexible flow shop owing to the three-station nature of surgery. Allocating all types of hospital resources to the three stations and determining the length of time for each stage during surgery are crucial to improving the efficiency of DSC. This paper integrates a three-station job shop scheduling problem (JSSP) into the day surgery scheduling and optimization problem. The JSSP was formulated as a mixed-integer linear programming model, and the elicitation of the model for scheduling surgeries with different priorities in the DSC is discussed. The model illustrated a case study of the DSC within West China Hospital (WCH). Numerical experiments based on the genetic algorithm design were conducted. Compared to the other optimization strategies, we proposed that the three-station job shop scheduling strategy (TSJS) could not only improve the efficiency and reduce the waiting time of the patients of the DSC in large public hospitals in China but also allow for timely scheduling adjustments during the advent of emergency surgeries.

Abstract:This paper provides an overview of our 10-year research on the application of stochastic and dynamic programming techniques to address health care operational deficiencies in a demand-driven way. We first describe the main operational deficiencies motivating our research in the capacity allocation and scheduling of diagnostic equipment and operating rooms. We then present main findings of extensive field studies to show current practices and key features of the problems under consideration. Applications of stochastic and dynamic programming to these problems are discussed by giving key assumptions, mathematical models, properties of the optimal solution, solution approaches and main numerical findings. The relaxation of the key assumptions is shown to lead to various future research directions that have drawn significant interests of the operations research and industrial engineering communities. We conclude by identifying barriers and potential solutions on the path from theories to applications.

Abstract:Surgery scheduling is of critical importance, because an operating room (OR) is the major cost generating unit in the hospital. However, schedulers face tremendous challenges brought by case cancellation, which have been observed in most departments. On the other hand, the randomness of surgery duration also has a significant impact on an OR schedule. In this paper, we develop a stochastic integer programming model for multiple ORs that simultaneously considers the uncertainties of case cancellation and surgery duration. We aim at minimizing the costs from the perspectives of both health care providers and patients. The Benders decomposition is used to address the computational complexity. A series of experiments is conducted to show the effectiveness of the proposed model and solution approaches. A case study based on two departments at West China Hospital is carried out, where the total cost can be reduced by approximately 27%. A sensitivity analysis is conducted in the case study, from which we gain managerial insights.

Abstract:This paper addresses the dynamic assignment of a given set of surgeries to multiple identical operating rooms (ORs). Surgeries have random durations and planned surgeon arrival times. Surgeries are assigned dynamically to ORs at surgery completion events. The goal is to minimize the total expected cost incurred by surgeon waiting, OR idling, and OR overtime. We first formulate the problem as a multistage stochastic programming model. An efficient algorithm is then proposed by combining a two-stage stochastic programming approximation and some look-ahead strategies. A perfect information-based lower bound of the optimal expected cost is given to evaluate the optimality gap of the dynamic assignment strategy. Numerical results show that the dynamic scheduling and optimization with the proposed approach significantly improve the performance of static scheduling and First Come First Serve (FCFS) strategy.Note to Practitioners-Hospitals usually consider ORs as the most critical resources and are used to manage patients and human resources to avoid OR idle time. As surgeons are becoming another critical resource, providing surgeons with reliable surgery starting times becomes more and more important. Unfortunately, uncertainties of surgery durations make the problem of setting reliable surgery starting times challenging. We propose an efficient algorithm for dynamic surgery assignment in a multi-OR system by taking into account given surgeon arrival times while best balancing surgeon waiting time and OR idle and overtime. The proposed approach significantly outperforms benchmark strategies. Various sensitivity analyses provide valuable managerial insights to hospital decision makers.

Abstract:Blood centers in large hospitals in China are facing serious problems, including complex patient queues and inflexible nursing schedules. This study is aimed at developing a flexible scheduling method for blood center nurses. By systematically analyzing the constraints that affect scheduling, a flexible scheduling model is established based on queuing theory and mixed integer programming. This combined model can reasonably determine the number of nurses required during a given working period and flexibly arrange nursing schedules while ensuring sufficient rest periods for individual nurses. Results of numerical studies conducted using data from a large hospital in China show a significant improvement in patient waiting time performance metrics over the hospital's current practice. In addition, the nurses' workloads and rest periods are well balanced, indicating that the proposed method can effectively and flexibly arrange nursing shifts in blood centers.
A Stochastic Programming Approach to Surgery Scheduling under Parallel Processing Principle

Abstract:Parallel processing is a principle which enables simultaneous implementation of anesthesia induction and operating room (OR) turnover with the aim of improving OR utilization. In this article, we study the problem of scheduling surgeries for multiple ORs and induction rooms (IR) that function based on the parallel processing principle under uncertainty. We propose a two-stage stochastic mixed-integer programming model considering the uncertainty in induction, surgery and turnover durations. We sequence patients and set appointment times for surgeries in the first stage and assign patients to IRs at the second stage of the model. We show that an optimal myopic policy can be used for IR assignment decisions due to the special structure of the model. We minimize the expected total cost of patient waiting time, OR idle time and IR idle time in the objective function. We enhance the model formulation using bounds on variables and symmetry-breaking constraints. We implement a novel progressive hedging algorithm by proposing a penalty update method and a variable fixing mechanism. Based on real data of a large academic hospital, we compare our solution approach with several scheduling heuristics from the literature. We assess the additional benefits and costs associated with the implementation of parallel processing using near-optimal schedules. We examine how the benefits are inflated by increasing the number of IRs. Finally, we estimate the value of stochastic solution to underline the importance of considering uncertainty in durations.

Stochastic programming for outpatient scheduling with flexible inpatient exam accommodation

Abstract:This study is concerned with the determination of an optimal appointment schedule in an outpatient-inpatient hospital system where the inpatient exams can be cancelled based on certain rules while the outpatient exams cannot be cancelled. Stochastic programming models were formulated and solved to tackle the stochasticity in the procedure durations and patient arrival patterns. The first model, a two-stage stochastic programming model, is formulated to optimize the slot size. The second model further optimizes the inpatient block (IPB) placement and slot size simultaneously. A computational method is developed to solve the second optimization problem. A case study is conducted using the data from Magnetic Resonance Imaging (MRI) centers of Lahey Hospital and Medical Center (LHMC). The current schedule and the schedules obtained from the optimization models are evaluated and compared using simulation based on FlexSim Healthcare. Results indicate that the overall weighted cost can be reduced by 11.6% by optimizing the slot size and can be further reduced by an additional 12.6% by optimizing slot size and IPB placement simultaneously. Three commonly used sequencing rules (IPBEG, OPBEG, and a variant of ALTER rule) were also evaluated. The results showed that when optimization tools are not available, ALTER variant which evenly distributes the IPBs across the day has the best performance. Sensitivity analysis of weights for patient waiting time, machine idle time and exam cancellations further supports the superiority of ALTER variant sequencing rules compared to the other sequencing methods. A Pareto frontier was also developed and presented between patient waiting time and machine idle time to enable medical centers with different priorities to obtain solutions that accurately reflect their respective optimal tradeoffs. An extended optimization model was also developed to incorporate the emergency patient arrivals. The optimal schedules from the extended model show only minor differences compared to those from the original model, thus proving the robustness of the scheduling solutions obtained from our optimal models against the impacts of emergency patient arrivals.

Maximizing the Efficiency of Use of Nurses under Uncertain Surgery Durations: A Case Study.

Abstract:This paper studies a nurse scheduling problem of assigning a set of nurses to surgeries scheduled on each workday in an operating room (OR) suite. This problem plays a decisive role in utilizing nurses efficiently, which is of paramount importance for OR suites to provide high-quality service at ever reduced cost. Due to significant uncertainty in surgery durations, designing schedules that achieve high nurse efficiency is complicated by the competing objective of ensuring on-time starts of surgeries. For trading off between the two performance criteria, we formulate the problem as a mixed integer programming (MIP) model with explicit probability modeling of uncertainty. We are concerned about improving nurse efficiency in terms of overtime and idle time of nurses while mastering the risk of delay of surgeries. The MIP model is applied in a large size Chinese hospital, and the results are compared with the actual performance of the OR suite. The comparisons reveal that through examining the trade-off between the performance criteria, important nurse efficiency improvements can be achieved with good on-time start performance. Moreover, the applicability of the MIP model in various problem settings is also investigated.
Surgical scheduling by Fuzzy model considering inpatient beds shortage under uncertain surgery durations

Abstract:Operating Room (OR) management has been among the mainstream of hospital management research, as ORs are commonly considered as one of the most critical and expensive resources. The complicated connection and interplay between ORs and their upstream and downstream units has recently attracted research attention to focus more on allocating medical resources efficiently for the sake of a balanced coordination. As a critical step, surgical scheduling in the presence of uncertain surgery durations is pivotal but rather challenging since a patient cannot be hospitalized if a recovery bed will not be available to accommodate the admission. To tackle the challenge, we propose an overflow strategy that allows patients to be assigned to an undesignated department if the designated one is full. It has been proved that overflow strategy can successfully alleviate the imbalance of capacity utilization. However, some studies indicate that implementation of the overflow strategy exacerbates the readmission rate as well as the length of stay (LOS). To rigorously examine the overflow strategy and explore its optimal solution, we propose a Fuzzy model for surgical scheduling by explicitly considering downstream shortage, as well as the uncertainty of surgery duration and patient LOS. To solve the Fuzzy model, a hybrid algorithm (so-called GA-P) is developed, stemming from Genetic Algorithm (GA). Extensive numerical results demonstrate the plausible efficiency of the GA-P algorithm, especially for large-scale scheduling problems (e.g., comprehensive hospitals). Additionally, it is shown that the overflow cost plays a critical role in determining the efficiency of the overflow strategy; viz., benefits from the overflow strategy can be reduced as the overflow cost increases, and eventually almost vanishes when the cost becomes sufficiently large. Finally, the Fuzzy model is tested to be effective in terms of simplicity and reliability, yet without cannibalizing the patient admission rate.

Abstract:This paper studies operating room planning which is now a very important topic for hospitals due to budget restrictions and aging population. Both block scheduling and open scheduling strategies are considered. Based on the model of Fei et al. which is considered to be close to reality, we develop a heuristic that can be applied to each of these strategies. This heuristic stems from the idea of dynamic programming by aggregating states to avoid the exponential increasing of the number of states. Our objective is to develop a general method based on this idea to solve a series of set partitioning problems. Computational results show that our algorithm is competitive and requires less computation time than existing methods.

Abstract:The operating room is a very important health care resource of the hospital.Effective process optimization and the use of anesthesia recovery room improve the operating efficiency gradually,but still can not meet the growing demand for medical services in the patients and the clinical departments.And the over utilization and underutilization in the operating room still exist.Artificial surgical resource scheduling improves the operative efficiency of resource to a certain degree.The mathematics surgical resource scheduling and collaborative optimization based on computer system compensate for the lack of manual scheduling system resources to match the dynamic real-time optimization,to effectively reduce the surgical inpatient waiting time and to improve surgical efficiency of resource use.
