Abstract:Obstruction of the upper airway in infants may be associated with continued respiratory efforts without air entering the lungs.1-4 Our clinical experience has led us to suspect that some apnea monitors in current use are unable to differentiate between these ineffective respiratory efforts and true breathing. To document this deficiency of apnea monitors, we examined the response of two types of apnea monitors to respiratory movements during upper airway obstruction. SUBJECTS AND METHODS Five healthy full-term infants were studied during the first three days of life, and two premature infants in stable condition (birth weight, 1.32 kg and 1.30 kg; gestational age, 32 weeks) were studied at 25 days of age when they no longer had apnea associated with bradycardia in the nursery.

Abstract:Sun glare is one of the major environmental issues contributing to traffic crashes. Every year, many traffic crashes in the United States are attributed to sun glare. However, quantitative analysis of the influence of sun glare on traffic crashes has not been widely undertaken. This study used traffic crash narrative data for 7 years (2010–2016) from Louisiana to identify crash reports that provided evidence of drivers indicating sun glare as the primary contributing factor of the crashes. Additional geometry and traffic information was collected to identify the list of key crash-contributing factors. This study used cluster correspondence analysis to perform the data analysis. After performing several iterations, six clusters were identified that provided additional insight in relation to sun glare-related crashes. The six clusters are associated with mixed (business and residential) localities, intersection-related crashes on U.S. roadways, single-vehicle crashes on residential two-lane undivided roadways, curve-related crashes on parish roadways in residential localities, interstate-related crashes in open country localities, and curve-related crashes in open country localities. The findings of the current study can add insights to the ongoing safety analysis on sun glare-related crashes.

Effect of Technological Distractions on Pedestrian Safe-Crossing Performance During Mixed Pedestrian–Bicycle Flow Overlapping with Turning Vehicles: A Case Study of Hangzhou, China

Abstract:Crashes involving pedestrians are most likely to occur when the pedestrian is crossing the road. In the particular context of road traffic systems in China, with frequent on-crosswalk cycling and pedestrian green signals overlapping with vehicle right- and left-turn green signals, walking on a signalized intersection crosswalk seems much more challenging. As such, pedestrians need to be more cautious and perform properly. The present study designed tasks for safe crossing at this form of intersection and, using video cameras, explored the performance of pedestrians. Taking smartphones as source of distraction, comparisons were made between distracted and nondistracted pedestrians. Of the phone use rate of 20.3% while crossing, 13.4% were visual-manual interactions, 3.5% were hand-free phones, and 3.4% were hand-held phones. For both genders, pedestrians who crossed while visually-manually interacting with a mobile or talking/listening on a mobile were less likely to glance before initiating crossing, less likely to look at traffic while crossing, less likely to avoid on-crosswalk opposite pedestrians and bicycles, and less likely to make eye contact with right/left-turning drivers. Surprisingly, female pedestrians who were texting took longer to react to the pedestrian green signal than males who were texting. Findings of this study suggest that distraction while crossing roads exposes pedestrians to a high risk of accident. Hence, to improve pedestrian safety, the authors advise relevant actions such as unsafe-crossing warning technologies; pedestrian, bicyclist, and driver education; and law enforcement. The built environment and the determination of fault in urban pedestrian crashes: Toward a systems-oriented crash investigation

Abstract:This study identifies built environmental factors that influence the determination of fault in urban pedestrian crashes in the United States, with implications for both safety and equity. Using data from Columbus, Ohio, we apply regression modeling, spatial analysis, and case studies, and find pedestrians are more likely to be found at fault on fast, high-volume arterial roads with bus stops. We also observe that better provision of crossings leads to more marked intersection crashes, which are less likely to be blamed on pedestrians. In addition, large differences in both the provision of crossings and fault exist between neighborhoods. We interpret findings through the lenses of the systems-oriented safety approaches Safe Systems and Vision Zero. The conclusion argues that the designation of individual responsibility for crashes preempts collective responsibility, preventing wider adoption of design interventions as well as systemic changes to the processes that determine the built environment of US roadways.

Abstract:Many pedestrians cross out of crosswalks (i.e., unmarked roadway) in developing countries, but researches about their safety are under reported. This study explored safety related factors and their casual relations at unmarked roadway. Videos of 254 pedestrians' crossing process were analyzed objectively on safety and evaluated subjectively on perceived safety. The two safety indexes are consistent on important factors, with higher running frequency reduce safety while bigger group size increase safety. The two factors had contrary effect on pedestrian speed, which is positively related with safety. Higher looking frequency before crossing also enhance safety, partly by reducing running frequency and increasing going backwards with its planning nature. Longer waiting time before crossing can facilitate this planning behavior while at the same time leads to bigger group size. Buses are safer than cars, but they are not perceived as safer. In situations where only some vehicles yield, yielding ones bring danger due to sight blocking of unyielding ones in adjacent lanes. These findings can be applied to the design of intelligent transportation systems and the education of drivers and pedestrians to improve safety. (C) 2011 Elsevier Ltd. All rights reserved.
Comparative analysis of middle ear immune response and micronutrient level between mucoid and purulent otitis media.

Abstract:OBJECTIVE To determine the differences in the outcome of treatment, the middle ear immune response, and micronutrient status between patients with purulent (POM) and mucoid (MOM) otitis media. DESIGN Children with acute otitis media were recruited, treated, and followed up for between 6 and 10 months. The sera and middle ear secretion (MES) were analyzed for immunoglobulins and zinc using enzyme-linked immunoassay and flame atomic absorption spectrophotometry, respectively. The parameters were compared with selected healthy controls. SUBJECTS There were 399 participants: 228 children with acute otitis media selected using the American Academy of Otolaryngology-Head and Neck Surgery criteria and 171 healthy controls, with ages ranging between 6 months and 9 years (mean 7 years; SD 2.32 years). RESULT POM accounted for 126 of 228 patients, and MOM accounted for 102. The chronicity of otitis media (using the 3-month duration cutoff) was seen in 87 subjects (46%): 31 of 126 POM subjects and 56 of 102 MOM subjects (p = .002). The serum IgA to IgG ratios were 0.03 and 0.07 among POM and MOM, respectively; however, the MES IgA to IgG ratios were higher, 0.3 and 0.2, for POM and MOM, respectively. This gives a MES to serum ratio of IgA to IgG value of 0.3 to 0.03 (10) for POM and 0.2 to 0.07 (2.67) for MOM, which showed a significant statistical difference (p = .001). The MES to serum ratios of IgE were 0.89 and 1.3 among POM and MOM, respectively (p = .03). There was a significant difference in the serum IgG (p = .019), serum zinc (p = .009), and MES IgA (p = .028) and IgG (p = .01). CONCLUSION Chronicity was greater in MOM than in POM; POM had a higher middle ear immune response (measured by the MES to serum ratio of IgA to IgG) and serum zinc than MOM but a lower MES IgE.

Abstract:Encouraging the use of nonmotorized modes of transportation is a goal at the intersection of public health, environment, and transportation, given active transportation's numerous benefits to individuals and communities. However, increased use of nonmotorized modes can also bring increased pedestrian–vehicle and bicycle–vehicle crashes. About 4,000 fatal and serious injury crashes involving a motor vehicle and a pedestrian or bicyclist occurred between 2007 and 2014 in the Texas Department of Transportation Austin District. This study contributed to the field of research by developing four crash frequency and severity models for pedestrians and bicyclists to identify the factors associated with the frequency of crashes and the presence of more severe crashes across the census block groups in the Austin District. The models showed the important role of different variables in crash frequency and severity, including travel demand, commute behaviors, network characteristics, and sociodemographic features. Using the model results, areas of concern with the greatest potential for safety improvements were identified. The study results can help develop safety improvement interventions for vulnerable road users in the Austin District and exemplify how other transportation agencies can estimate pedestrian and bicycle crashes using their existing database and crowdsourced data.

Abstract:Road traffic safety is an essential component of public safety and a globally significant issue. Pedestrians, as crucial participants in traffic activities, have always been a primary focus with regard to traffic safety. In the context of the rapid advancement of intelligent transportation systems (ITS), it is crucial to explore effective strategies for preventing pedestrian fatalities in pedestrian–vehicle crashes. This paper aims to investigate the factors that influence pedestrian injury severity based on pedestrian-involved crash data collected from several sensor-based sources. To achieve this, a hybrid approach of a random parameters logit model and random forest based on the SHAP method is proposed. Specifically, the random parameters logit model is utilized to uncover significant factors and the random variability of parameters, while the random forest based on SHAP is employed to identify important influencing factors and feature contributions. The results indicate that the hybrid approach can not only verify itself but also complement more conclusions. Eight significant influencing factors were identified, with seven of the factors identified as important by the random forest analysis. However, it was found that the factors "Workday or not" (Not), "Signal control mode" (No signal and Other security facilities), and "Road safety attribute" (Normal Road) are not considered significant. It is important to note that focusing solely on either significant or important factors may lead to overlooking certain conclusions. The proposed strategies for ITS have the potential to significantly improve pedestrian safety levels.

A Study on the Coupling Influence of Impact Speed and Vehicle Type on the Movement and Injury of Pedestrian

Abstract:Three typical pedestrian-vehicle accident cases with different types of vehicle involved are select-ed according to the statistic results of pedestrian’s injury features. The software MADYMO is utilized to conduct a re-construction simulation, which is verified with real traffic accident data acquired and a vehicle multi-body dynamic model is built. Then on this basis a simulation is performed to study the effects of vehicles types and impact speed on the relative speed of pedestrian’s head over vehicle, throw distance and the injury severities of pedestrian’s head, thorax and lower leg. The results show that, besides the strong correlation between impact speed and pedestrian’s movement pattern and their injuries in different parts of body, the types of vehicle also have certain degrees of influ-ences, for the same collision speed:1) the throw distance in van collisions is about 1. 25 times as long as that in mini-car and sedan collisions;2 ) the HIC value in van collisions is around 1. 3~2. 2 times as high as that in mini-car and sedan collisions in a speed range of 38 ~ 60 km/h;3 ) the thoracic injury in van collisions is some 1. 7~4. 5 times as severe as that in mini-car and sedan collisions;4 ) the injury severity of lower leg in van collisions is a-bout 20% ~50% lower than that in mini-car and sedan collisions.

Abstract:In this study, our goal was to determine the impact of various risk factors on traffic accidents in the city of Shenyang, China, and to discuss the various common factors that influence pedestrian and non-motor vehicle accidents. A total of 1227 traffic accidents from 2015 to 2017 were analyzed, of which, 733 were accidents involving pedestrians and 494 were non-motor vehicle accidents. Among these traffic accidents, pedestrians and non-motor vehicle users had either minor or no responsibility. Sixteen influencing factors, including main responsible party attributes, pedestrian/non-motor vehicle user attributes, time attributes, space attributes, and environmental attributes were analyzed with regards to their impact on accidents using the binary logistic regression model (BLR) and the classification and regression tree analysis model (CART). Age, administrative division, and time of year were the three most common factors for pedestrian and non-motor vehicle accidents. For pedestrian accidents, the personal influencing factors of the main responsible party included illegal acts while driving and hit-and-run behavior. Factors affecting pedestrian and non-motor vehicle accidents also had different orders of importance.

Abstract:The pattern of pedestrian crashes varies greatly depending on lighting circumstances, emphasizing the need of examining pedestrian crashes in various lighting conditions. Using Louisiana pedestrian fatal and injury crash data (2010-2019), this study applied Association Rules Mining (ARM) to identify the hidden pattern of crash risk factors according to three different lighting conditions (daylight, dark-with-streetlight, and dark-no-streetlight). Based on the generated rules, the results show that daylight pedestrian crashes are associated with children (less than 15 years), senior pedestrians (greater than 64 years), older drivers (>64 years), and other driving behaviors such as failure to yield, inattentive/distracted, illness/fatigue/asleep. Additionally, young drivers (15-24 years) are involved in severe pedestrian crashes in daylight conditions. This study also found pedestrian alcohol/drug involvement as the most frequent item in the dark-with-streetlight condition. This crash type is particularly associated with pedestrian action (crossing intersection/midblock), driver age (55-64 years), speed limit (30-35 mph), and specific area type (business with mixed residential area). Fatal pedestrian crashes are found to be associated with roadways with high-speed limits (>50 mph) during the dark without streetlight condition. Some other risk factors linked with high-speed limit related crashes are pedestrians walking with/against the traffic, presence of pedestrian dark clothing, pedestrian alcohol/drug involvement. The research findings are expected to provide an improved understanding of the underlying relationships between pedestrian crash risk factors and specific lighting conditions. Highway safety experts can utilize these findings to conduct a decision-making process for selecting effective countermeasures to reduce pedestrian crashes strategically.

Abstract:Road traffic crashes cause fatalities and injuries of both drivers/passengers in vehicles and pedestrians outside, thus challenge public health especially in big cities in developing countries like China. Previous efforts mainly focus on a specific crash type or causation to examine the crash characteristics in China while lacking the characteristics of various crash types, factors, and the interplay between them. This study investigated the crash characteristics in Shenzhen, one of the biggest four cities in China, based on the police-reported crashes from 2014 to 2016. The descriptive characteristics were reported in detail with respect to each of the crash attributes. Based on the recorded crash locations, the land-use pattern was obtained as one of the attributes for each crash. Then, the relationship between the attributes in motor-vehicle-involved crashes was examined using the Bayesian network analysis. We revealed the distinct crash characteristics observed between the examined levels of each attribute, as well the interplay between the attributes. This study provides an insight into the crash characteristics in Shenzhen, which would help understand the driving behavior of Chinese drivers, identify the traffic safety problems, guide the research focuses on advanced driver assistance systems (ADASs) and traffic management countermeasures in China.

Abstract:Real-world road accident data involving pedestrians were obtained from police records in Beijing. In total, 158 vehicle-pedestrian accidents were included in this study, from which the accident characteristics were identified. The data were analysed and the overall epidemiology established. In addition representative cases were studied in depth using MADYMO reconstructions. For accuracy, a MADYMO pedestrian model based on Chinese anthropometric values was developed. A comparable European-size MADYMO model has also been used to demonstrate the differences between the two anthropometric types. Using the Chinese pedestrian model, a complete accident event is reconstructed, in terms of pre-impact behaviour, causation, injury characteristics and post-impact kinematics.

Abstract:Pedestrian safety remains a significant concern, with the growing number of severe pedestrian crashes resulting in substantial human and economic costs. Previous research into pedestrian crashes has extensively analyzed the influences of weather, lighting, and pedestrian demographics. However, these studies often overlook the critical spatial variables that contribute to pedestrian crashes. Our study aims to explore these overlooked spatial variables by examining the distance pedestrians travel before encountering a severe crash. This approach provides a supplementary perspective in safety analysis, emphasizing the importance of pedestrian movement patterns. The model considers various factors that may influence pedestrian traveled distance before being involved in a severe crash, such as weather conditions, lighting conditions, and pedestrian demographics. Ohio's pedestrian-involved crashes were gathered and analyzed as a case study. The results indicated that 50 % of fatal pedestrian crashes occurred within 0.84 miles of the pedestrians' residences. Moreover, it was shown that factors including lighting condition, pedestrian age, drug toxication, and the location at impact significantly influence the pedestrians traveled distance. These findings provide valuable insights into the spatial distribution of pedestrian crashes and shed light on the factors contributing to their severity. By understanding these relationships, policymakers and urban planners can design targeted interventions such as improving street lighting, implementing traffic calming measures, and developing safety awareness campaigns for specific age groups, to enhance pedestrian safety and reduce the incidence of severe crashes.

Pedestrians’ crossing behaviors and safety at unmarked roadway in China

Abstract:Pedestrians’ crossing out of crosswalks (unmarked roadway) contributed to many traffic accidents, but existing pedestrian studies mainly focus on crosswalk crossing in developed countries specifically. Field observation of 254 pedestrians at unmarked roadway in China showed that 65.7% of them did not look for vehicles after arriving at the curb. Those who did look and pay attention to the traffic did so for duration of time that followed an exponential distribution. Pedestrians preferred crossing actively in tentative ways rather than waiting passively. The waiting time at the curb, at the median, and at the roadway all followed exponential distributions. During crossing, all pedestrians looked at the oncoming vehicles. When interacting with these vehicles, 31.9% of them ran and 11.4% stepped backwards. Running pedestrians usually began running at the borderline rather than within the lanes. Pedestrians preferred safe to short paths and they crossed second half of the road with significantly higher speed. These behavioral patterns were rechecked at an additional site with 105 pedestrians and the results showed much accordance. In terms of safety, pedestrians who were middle aged, involved in bigger groups, looked at vehicles more often before crossing or interacted with buses rather than cars were safer while those running were more dangerous. Potential applications of these findings, including building accurate simulation models of pedestrians and education of drivers and pedestrians in developing countries were also discussed.

Abstract:Though filled pauses and gestures frequently accompany speech, their function is not well understood. We suggest that it may be helpful in furthering our knowledge of these phenomena to examine their relationship to each other. To this end, we carried out two studies examining whether they tend to occur together, or to occur at separate times. Both faculty colloquium speakers" and undergraduate subjects used filled pauses less frequently when they were gesturing than when they were not gesturing. This effect held for 30 out of 31 subjects. We suggest that detailed theories may be premature, but speculate that gestures may be an indication that the speech production apparatus has completed its search for the next word, phrase or idea and is ready to continue.
Injury and fatality risks for child pedestrians and cyclists on public roads

Abstract:Background: Pedestrians and cyclists are often referred to as "vulnerable road users," yet most research is focused on fatal crashes. We used fatal and nonfatal crash data to examine risk factors (i.e., relationship to an intersection, urbanicity, crash circumstances, and vehicle type) for police-reported pedestrian and cyclist injuries on public roads among children aged 0-9 and aged 10-19. We also compared risk factors among these two age groups with adults aged 20-29 and aged 30-39. Methods: Crash data were obtained for 2016-2020 from the National Highway Traffic Safety Administration's Fatality Analysis Reporting System for fatal crash injuries and Crash Report Sampling System for nonfatal crash injuries. We collected data on victim demographics, roadway, and vehicle- and driver-related factors. Descriptive analyses were conducted between and within pedestrian and cyclist victims. Results: We analyzed 206,429 pedestrian injuries (36% in children aged 0-19) and 148,828 cyclist injuries (41% in children aged 0-19) from 2016 to 2020. Overall, child pedestrians had lower injury rates than adults, but children aged 10-19 had greater cycling crash rates than adults. Almost half of the pedestrian injuries in children aged 0-9 were "dart-out" injuries (43%). In the majority of the cyclist injuries, children in both age groups failed to yield to vehicles (aged 0-9 = 40% and aged 10-19 = 24%). For children and all ages included in the study, the fatality risk ratio was highest when pedestrians and cyclists were struck by larger vehicles, such as trucks and buses. Further exploration of roadway factors is presented across ages and transportation mode. Conclusion: Our findings on child, driver, vehicle, and roadway factors related to fatal and nonfatal pedestrian and cyclist injuries may help to tailor prevention efforts for younger and older children.

Abstract:This study examines the impact of urban form and street infrastructure on pedestrian safety in Atlanta, Georgia, and Boston, Massachusetts. With a significant rise in pedestrian fatalities over the past decade, understanding how cities' built environments influence safety is critical. We conducted geospatial analyses and statistical tests, revealing unique patterns in each city. Atlanta's sprawling, motorist-oriented layout is associated with increased pedestrian accidents, particularly at crosswalks, due to limited land use diversity, arterial roads, and streets with high speed limits and multiple lanes. In contrast, Boston's compact, pedestrian-oriented design leads to improved safety, featuring safer pedestrian crossings, greater land use diversity, reduced arterial roads and lower speed limits on single-lane streets. This study also highlights the importance of diverse urban forms and pedestrian infrastructure in shaping pedestrian safety. While population density and land use diversity impact accident rates, the presence of crosswalks and street configurations play crucial roles. Our findings underscore the urgency for urban planners to prioritize pedestrian safety through targeted interventions, such as enhancing crosswalks, reducing speed limits and promoting mixed land use. Future research should explore additional variables, alternative modelling techniques and non-linear approaches to gain a more comprehensive understanding of these complex relationships.

Differential influences of road physical design attributes on pedestrian versus driver traffic accident frequencies

Abstract:Traffic accidents involving pedestrians and drivers pose significant public health and safety concerns. Understanding the differential influences of road physical design attributes on crash frequencies for these two groups is critical for developing targeted safety interventions. Considering that the zero-truncated characteristic of the data is uncertain, the results of the zero-truncated negative binomial models and traditional negative binomial models are calculated to seek the better model. The result revealed that the road surface conditions and vertical and horizontal curvature have greater influence on both pedestrian and driver compared to number of lanes and speed limit. And speed limits were more pronounced for pedestrian crash frequency than driver group. Conversely, the effect of different types of intersections was stronger for driver crash frequency. The differential influences of road physical design attributes on traffic crash frequencies for pedestrians versus drivers highlight the importance of adopting a user-centric approach to transportation safety planning and infrastructure design. Tailoring interventions to address the unique needs and vulnerabilities of different road user groups can lead to more effective safety improvements and better overall traffic safety outcomes.
