Abstract:The molecular mechanisms underlying fetal growth restriction (FGR) remain poorly understood. We investigated a role for H19, a long noncoding RNA (lncRNA) that is highly expressed in placenta, in the pathogenesis of FGR. We used RT-qPCR to compare levels of H19 in placentas from pregnancies with idiopathic preterm FGR (n = 12) and gestational age-matched controls with spontaneous preterm delivery (n = 12). We then used siRNA-mediated H19 knockdown on first trimester extravillous trophoblast (EVT) cells to evaluate the effect of decreased H19 on cell function, via trans-well migration and invasion studies. Subsequently, we performed genome-scale transcriptome profiling, coupled with pathway analysis, to determine a mechanism for our observed changes. Parametric and nonparametric statistical analysis was performed as appropriate, with significance set at a p value of <0.05. H19 level is approximately 2-fold lower in placentas with FGR as compared to controls (p=0.02). H19 knockdown in EVT cells leads to impaired migration (p=0.02) and invasion (p<0.01). Genome-scale transcriptome profiling and pathway analysis reveal decreased expression of betaglycan (TGFβ type III receptor), a direct regulator of ligand access to the TGFβ signaling receptors, in both H19 knockdown EVT cells (p<0.01) and FGR placentas (p=0.01). Additionally, the increased bioavailability of microRNA let-7, as a result of decreased H19, contributes to reduced expression of betaglycan, a novel target of let-7-mediated posttranscriptional regulation. Depressed H19 level leads to impaired migration and invasion of a human trophoblast cell line, consistent with failure of trophoblast invasion associated with FGR. Dysregulation of this newly identified H19/TGFβ signaling regulatory pathway may contribute to the underlying molecular mechanism of idiopathic FGR. Our finding represents the first example of a lncRNA-based mechanism in FGR, thus holding promise in the development of novel diagnostic and therapeutic modalities for this detrimental disorder.
