Abstract:Question: A 62-year-old man was referred to our hospital with 2 years of recurrent symptomatic melena requiring multiple blood transfusions and iron therapy. His past medical history was not significant. His physical examination was unremarkable and laboratory tests were significant for mild anemia (hemoglobin, 11 g/dL; mean corpuscular volume, 96.9 fL). Initial bidirectional endoscopy revealed small amount of hematin stain in the hepatic flexure and mild ileum erythema was found on capsule endoscopy (Figure A). Neither of these findings was congruent with his clinical presentation; thus, further evaluations using computed tomography enterography was performed and again this was unrevealing. Finally, on 99Tc-tagged red blood cell scan, an area of hemorrhage was located to the jejunum (Figure B). This procedure was followed with double balloon enteroscopy, which revealed a small bleeding ileum ulcer with some surrounding erythema (Figure C, D). Three-dimensional reconstructions showed no vascular malformations and the patient was discharged. Twenty days later, the patient was readmitted for recurrent melena and his hemoglobin had decreased from 11.5 to 5.4 g/dL. Enteroscopy was repeated and revealed an area of erosion with active oozing located in the upper segment of the ileum (Figure E). This area was successfully treated with argon plasma coagulation. Twenty days later, the patient returned to our hospital again with melena and new-onset right arm pain. A third enteroscopy was performed without signs of bleeding; however, ultrasound examination revealed right cephalic vein thrombosis (Figure F). After repeated admissions for symptomatic melena and lack of a clear diagnosis, attention was turned toward hematologic causes and hematologic workups were initiated. He underwent bone marrow biopsy along with a multitude of hematologic tests, including protein electrophoresis and a coagulation test. Only urea clot solubility test showed a positive result and revealed the diagnosis of factor XIII (FXIII) deficiency. The patient was started on fresh frozen plasma infusion, and at 1 year of follow-up, his hemoglobin has recovered and melena stopped. For patients older than 40 years, vascular lesions, erosions, or ulcer related to nonsteroidal anti-inflammatory drugs are common causes of obscure gastrointestinal bleeding (now called suspected small bowel bleeding).1Raju G.S. The final diagnosis of FXIII deficiency was verified by qualitative measurement of FXIII activity after extensive workups and multiple hospitalizations. Studies have shown that FXIII deficiency had strong association with severe, spontaneous bleeding in gastrointestinal tract, central nervous system, and so on.2Peyvandi F. Palla R.
