Abstract:Cervical, ovarian and endometrial cancer are the three most common types of gynecologic cancer. As a hub, the vagina connects the site of gynecological cancer with the external environment. Lactobacilli participate in the formation of a healthy vaginal microenvironment as the first line of defense against pathogen invasion; a dysbiotic vaginal microenvironment loses its original protective function and is associated with the onset, metastasis, poor efficacy and poor prognosis of gynecological cancer. The early diagnosis of cancer is the key to improve the survival time of patients with cancer. The screening of Porphyromonas, Sneathia and Atopobium vaginae , and other microbial markers, can assist the diagnosis of gynecological cancer, and screen out the high‐risk population as early as possible. With the in‐depth study of the microbes in tumor tissues, reasearchers have analyzed the immunological associations of microorganisms in tumor tissues. Due to the structural‐functional interconnection between the organ of gynecological tumorigenesis and the vagina, the present study aims to review the relationship between vaginal and tumor microorganisms and gynecological cancer in terms of occurrence, screening, treatment and prognosis.

The Vaginal Microecology in Gynecological Outpatients: a Report from China

Abstract:Objective To study the vaginal microecology of the patients in the outpatient department of Obstetrics and gynecology in China.Methods A multicenter cross-sectional study was conducted in gynecologic clinic of 9 collaborative hospitals in China.200 consecutive patients were collected in each hospital and their vaginal microecology combined with related factors were analyzed.Results A total of 2 093 specimens were collected in this study.The detection rate of Trichomonas was 5.5%(115/2 093). The detection rate of Candida mycelia was 15.9%(333/2 093), with germinal spores was 4.1%(86/2093).The detection rate of bacterial vaginosis was 18.8%(394/2 093).The distribution results of vaginal flora in patients showed that the normal flora accounted for only 27.3%(571/2 093).The normal flora with the insufficiency of H2O2 accounted for 23%(480/2 093).The bacteria inhibiting flora accounted for 3.8%(79/2 093).The abnormal microflora(non BV type)accounted for 14.9%(312/2 093).The abnormal microflora(BV intermediate type)accounted for 13.4%(280/2 093).The abnormal microflora(BV type)accounted for 17.6%(369/2 093).The average pH of vaginal discharge was 4.58 ±0.495.There was no significant difference of the incidence of trichomonas and bacterial vaginosis between north and south of the Yangtze river, while the detection rate of fungal hyphae and the fungal spores is significantly higher in the south than that in the north.The analysis results of factors affecting the microecology showed that age and contraception methods were two important factors.The patients′age from bacteria inhibition group was 49.64 +16.68 which was significantly higher than that of the other microecology groups.The proportion of abnormal microflora of patients from the oral contraceptive group was 40%(20/50).The proportion of abnormal microflora of patients from IUD group was 36.6%(63/172).Compared with these two contraception methods, the proportion of abnormal microflora of patients from condoms usage group was 27.8%(91/327)which was significantly lower.The incidence of abnormal leucorrhea in the normal group was 37.7%, which was significantly lower than that of other abnormal groups.Conclusion This study showed the vaginal microecology status of the Chinese outpatient ′s clinic and found that the vagina microecology was related to age, region and contraceptive methods.The typical manifestation of microecological abnormality is the increase of leucorrhea.(Chin J Lab Med,2018, 41:287-291)

Abstract:Objective To investigate vaginal micro-ecological status in gynecological outpatients with different symptoms.Methods Retrospective analysis the results of the vaginal micro-ecological evaluation in gynecological outpatients with different symptoms from November 2011 to September 2012(The index include intensity and diversity of bacteria group,dominant flora,pathogens,AV score,Nugent score and five bacteria function and inflammation index).Results The average age of 4019 patients was 33.56.The normal vaginal micro-ecological patients was 0.70%(28/4019);the patients with vaginal micro-ecological disorder was 99.30%(3991/4019);the latter included ill-defined infected vaginal micro-ecological disorder(66.33%,2666/4019),and defined infected vaginal micro-ecological disorder(32.97%,1324/4019).In patients with defined infected vaginal micro-ecological disorder,VVC was 24.33%(978/4019);AV was 3.11%(125/4019);BV was 3.14%(126/4019);TV was 5.15%(207/4019),and mixed infection was 8.31%(110/1324).Abnormal dominant flora was 54.32%(2183/4019).Conclusion In gynecological outpatients,the constituent ratio of patients with ill-defined infected vaginal micro-ecological disorder is highest;in defined vaginal infections patients,the constituent ratio of vulvovaginal candidiasis is highest;becoming the preferred diagnosis method of vaginal infection disease,the vaginal micro-ecological evaluation is of great significance in the clinical diagnosis and treatment practice.

Abstract:Objectives: To study the vaginal microecological regime in infertility women and discuss the clinical significance of microecological evaluation.Method: We selected 300 infertility women,took vaginal discharge for microecologic evaluation.389 reproductive age women who had routine physical examination were taken as control.Results: In infertility group,the incidence of candida was 7.7%(23 examples),trichomonaswas 0%(0 examples);Nugent grading: normal in 89.1%(267 examples),BV middle type in 4.7%(14 examples),BV in 6.2%(19 examples);The dominant bacterium was gram-positive big bacillus,accounted for 83.2%(249 examples),gram-positive coccus accounted for 0.7%(2 examples),gram-negative small bacillus accounted for 16.1%(49 examples);The microecological disturbance rate was 34.0%(102 examples).Conclusion: In infertility woman,the incidence of BV,VVC and the vaginal flora disturbance were not higher than that of healthy female.There were no significant differences in microecological regime between the primary and secondary infertility,as well as in terms of tube factor and the male factor.

Abstract:OBJECTIVE:To investigate the microecological status of vaginal microflora in the women with different vaginal symptoms.METHODS:From March 2006 to October 2007, 6982 cases with varying degree vaginal symptoms including pruritus, increaseed leucorrhea, the leucorrhea having unusual smell, in the gynecology outpatient department were studied. The vagina secretions were examined in terms of the pH value, the hydrogen peroxide test, and Gram dyeing inspection of vaginal bacteria and microecology appraisal for colony's density, the multiplicity, the superiority fungus, and the inflammatory response.RESULTS:Among 6982 patients, normal vaginal microecology was identified in 750 (10.74%, 750/6982); abnormal microecology was found in 6232 (89.26%, 6232/6982); bacterial vaginosis (BV) was detected in 729 (10.44%, 729/6982); vulvovaginal candidiasis (VVC) was in 1527 (21.87%, 1527/6982). Ninety five patients (1.36%, 95/6982) were with both BV and VVC. Abnormal bacteria colonies were found in 1229 (17.60%, 1229/6982), and others were found in 2652 (37.98%, 2652/6982).CONCLUSIONS:The vaginal microecology in the women with different vaginal symptoms can be either normal or abnormal. Microecology clinical evaluation system can assess the vaginal microecosystem.
Molecular Analysis of the Diversity of Vaginal Microbiota Associated with Bacterial Vaginosis

Abstract:Background Bacterial vaginosis (BV) is an ecological disorder of the vaginal microbiota that affects millions of women annually, and is associated with numerous adverse health outcomes including pre-term birth and the acquisition of sexually transmitted infections. However, little is known about the overall structure and composition of vaginal microbial communities; most of the earlier studies focused on predominant vaginal bacteria in the process of BV. In the present study, the diversity and richness of vaginal microbiota in 50 BV positive and 50 healthy women from China were investigated using culture-independent PCR-denaturing gradient gel electrophoresis (DGGE) and barcoded 454 pyrosequencing methods, and validated by quantitative PCR. Results Our data demonstrated that there was a profound shift in the absolute and relative abundances of bacterial species present in the vagina when comparing populations associated with healthy and diseased conditions. In spite of significant interpersonal variations, the diversity of vaginal microbiota in the two groups could be clearly divided into two clusters. A total of 246,359 high quality pyrosequencing reads was obtained for evaluating bacterial diversity and 24,298 unique sequences represented all phylotypes. The most predominant phyla of bacteria identified in the vagina belonged to Firmicutes , Bacteroidetes , Actinobacteria and Fusobacteria . The higher number of phylotypes in BV positive women over healthy is consistent with the results of previous studies and a large number of low-abundance taxa which were missed in previous studies were revealed. Although no single bacterium could be identified as a specific marker for healthy over diseased conditions, three phyla - Bacteroidetes , Actinobacteria and Fusobacteria , and eight genera including Gardnerella , Atopobium , Megasphaera , Eggerthella , Aerococcus , Leptotrichia / Sneathia , Prevotella and Papillibacter were strongly associated with BV ( p < 0.05). These genera are potentially excellent markers and could be used as targets for clinical BV diagnosis by molecular approaches. Conclusions The data presented here have clearly profiled the overall structure of vaginal communities and clearly demonstrated that BV is associated with a dramatic increase in the taxonomic richness and diversity of vaginal microbiota. The study also provides the most comprehensive picture of the vaginal community structure and the bacterial ecosystem, and significantly contributes to the current understanding of the etiology of BV.

Abstract:The vaginal microbiota, the host endocrine system, the vaginal anatomy, and the local mucosal immunity comprise the vaginal microbiota, which interacts with each other to maintain the balance of the vaginal microbiota, which maintains female reproductive health. Puberty, menstruation, pregnancy, and menopause are four phases women go through during their reproductive and post-reproductive years. Vaginal microbiota composition and abundance are heavily influenced by estrogen and progesterone, which start at puberty and continue during the reproductive years in a dynamic balance with some fluctuations. Estrogen promotes proliferation of vaginal epithelial cells and increases glycogen storage, while progesterone lyses vaginal epithelial cells, facilitating the release of glycogen to maintain normal pH. This review summarizes the latest national and international evidence on the composition and distribution of vaginal microecology in women during different physiological and pathological periods and proposes a hormone-driven microbial diversity hypothesis to explain the temporal patterns of vaginal microbial diversity during the female reproductive cycle and menopause. A relatively balanced vaginal microecological system has a positive effect on the maintenance of female health. An imbalance in the ratio of flora can lead to susceptibility to infections or reproductive complications. The study of human microecology and its role in the development and progression of human disease is essential for the prevention, diagnosis, and treatment of related obstetric and gynecologic conditions.

Abstract:The cervicovaginal microbiota plays a key role in the health and reproductive outcomes of women. In reality epidemiological studies have demonstrated that there is an association between the structure of cervicovaginal microbiota and reproductive health, although key mechanistic questions regarding these effects remain unanswered and understanding the interplay between the immune system and the structure of the cervicovaginal microbiota. Here, we review existing literature relating to the potential mechanisms underlying the interaction between vaginal microbes and the immune system; we also describe the composition and function of the microbiome and explain the mechanisms underlying the interactions between these microbial communities and various aspects of the immune system. Finally, we also discuss the diseases that are caused by disorders of the reproductive tract and how the immune system is involved. Finally, based on the data presented in this review, the future perspectives in research directions and therapeutic opportunities are explored.
Diversity of Cervicovaginal Microbiota Associated with Female Lower Genital Tract Infections

Abstract:The female genital tract (FGT) harbors very large numbers of bacterial species that are known to play an important role on vaginal health. Previous studies have focused on bacterial diversity in the vagina, but little is known about the ectocervical microbiota associated with FGT infections. In our study, vaginal swabs and ectocervical swabs were collected from 100 participants in China, including 30 women with bacterial vaginosis (BV; BV group), 22 women with cervicitis (Cer group), 18 women with BV in combination with cervicitis (BC group) and 30 healthy control women (CN group). The diversity and richness of cervicovaginal microbiota were investigated with culture-independent polymerase chain reaction (PCR)-denaturing gradient gel electrophoresis (DGGE) and quantitative PCR (qPCR) targeting 11 microorganisms that have been associated with FGT infections. Despite significant interpersonal variations, the PCR-DGGE profiles revealed that vaginal microbiota and ectocervical microbiota were clearly much more complex in the BV group, while the ectocervical microbiota showed no significant difference between healthy and diseased participants. Using species-specific qPCR, BV and cervicitis were significantly associated with a dramatic decrease in Lactobacillus species ( p < 0.05), and potential pathogenic species such as Gardnerella , Atopobium , Eggerthella , Leptotrichia / Sneathia , and Prevotella were more common and in higher copy numbers in BV than in CN samples ( p values ranged from 0.000 to 0.021). No significant differences were observed between healthy and cervicitis samples ( p > 0.05) in ectocervical microbiota. The total numbers of bacteria were significantly lower in the ectocervix as compared in the vagina ( p < 0.05). Intriguingly, vaginal microbiota from participants with BV in combination with cervicitis was quite different from that of participants with BV or cervicitis alone. Our study demonstrated that the cervicovaginal microbiota was actively involved in the process of FGT infections. The predominant bacteria of the cervicovaginal communities were clearly associated with BV; however, there was not sufficient evidence that the ectocervical microbiota is directly involved in the development of cervicitis.

Abstract:OBJECTIVE The Vaginal Microecology Evaluation System (VMES) is a technical and economically feasible tool to assess the vaginal microbiomes of gynecological outpatients. The aim of this review was to introduce knowledge on the potential changes that can occur in the vaginal microbiome and how to use the Evaluation System to diagnose and manage a broad range of vaginal infections. DATA SOURCES This review was based on data in articles published in PubMed up to December 10, 2018, with the following keywords: "Vaginal microecosystem", "vaginal microbiome", "vaginal microbiota", and "vaginal flora". STUDY SELECTION Original articles and critical reviews on VMES and vaginal microbiota selected for this review. References of the retrieved articles were also screened to search for potentially relevant papers. RESULTS Any imbalance in the naturally occurring vaginal bacterial flora may result in infections such as bacterial vaginitis, aerobic vaginitis, vulvovaginal candidiasis, or Trichomonas vaginalis. VMES is mainly composed of morphological and functional microecological indicators. The former diagnostic modality includes bacterial density, flora diversity, dominant bacterial flora, indicators of inflammation, pathogenic microorganisms, Nugent score, and AV score. Functional indicators consist of two main components: metabolites and microbial enzymes. CONCLUSIONS The establishment of VMES has a high clinical value in the treatment of vaginal infections. Abstract:The vaginal microbiome is a unique and dynamic system,any disruption of which will lead to a series of problems,such as vaginitis.Vaginal microenvironment varies in response to numerous factors,including anatomical structure,microbial flora,local immunity and endocrine regulation.At present,the treatment strategy of vaginal infections is changing gradually from simply effectively killing various pathogenic microorganisms to treat vaginitis,to restoring the vaginal defense function and reconstructing vaginal microecology.Reconstruction therapy for abnormal vaginal microenvironment mainly consists of five aspects,including the recovery of vaginal pH value,the establishment of dominant flora in the vagina,the repair of vaginal mucosa and endocrine modulation,and the restoration of vaginal local immunity.Reconsidering vaginal infectious diseases,comprehensively evaluating the infectious and posttreatment status of vaginal microbiome,and recovering vaginal microenvironment using a variety of means and managements are the important treatment strategy of vaginal infections in the future.

Abstract:Objective The female reproductive tract is a significant microecological region, and its micro-environment can directly affect women’s cervical health. This research aimed to investigate the effect of vaginal microecology on human papillomavirus (HPV) infection and cervical intraepithelial neoplasia(CIN). Methods A retrospective cohort study enrolling 2,147 women who underwent a colposcopic examination between August 2021 and August 2022 was conducted. The relationship between vaginal microecology and HPV infection as well as cervical lesions were assessed using the chi-square test, univariate and multivariate logistic regression analyses, and Cochran-Armitage trend test. Results HPV infection was linked to the imbalance of vaginal microecology [odds ratio (OR)=3.00, 95% confidence interval (CI)=1.66–5.43; P<0.001]. Clue cell (OR=1.59, 95% CI=0.99–2.54; P=0.054) and sialidase (OR=1.54, 95% CI=1.01–2.35; P<0.046) were considered as significant risk factors for HPV infection. Further analysis showed that vaginal microecological disorder was more likely to be detected in patients infected with HPV 16/18 subtypes (OR=9.86, 95% CI=2.37–41.80; P=0.002). Although there was no significant correlation between the incidence of vaginal microecological disorder and the severity of cervical lesions (P > 0.05), the proportions of abnormal PH value (OR=2.6, 95% CI=1.63–10.42; P=0.001) and abnormal vaginal cleanliness (OR=2.6, 95% CI=1.36–4.0; P= 0.004) increased as the histological stage progressed. Conclusion Vaginal microecology associates with HPV infection and the progression of cervical lesions. Detection of vaginal secretion may contribute to the development of targets for micro-environmental modulation with probiotics and the reduction of the incidence of cervical cancer.

Abstract:Objective:To investigate vaginal micro-ecological status in community population.The objective was to provide a new way for prevent and cure lower reproductive tract infections.Methods:313 women of childbearing age(including 74 vaginitis cases) were selected randomly by cluster sub-sampling.The group case control method was used for data analysis.Results:The pH mean value was 4.4 in disorder group of vaginal micro-ecology,which was significant higher than 4.0 in normal group of vaginal micro-ecology(t=6.3581,P0.01).But it did not exceed the normal extent of the vaginal acidity.Moreover pH mean value among the vaginitis groups had more significant increase(all P0.01),which had exceeded the normal extent of the vaginal acidity,as compared to the normal group.The detection rates of lactobacillus and H2O2(+) in vagina among disorder group of vaginal micro-ecology were significantly lower than normal group(all P0.01).And the detection rates of these two indices among vaginitis group had more significant decrease compared with the normal group(all P0.01).The detection rate of vaginal lactobacillus in patients with vaginatis was also significantly lower than women without vaginitis(27.0% vs 74.9%,P0.01).The lower detection rate of vaginal lactobacillus,the higher risk of common vaginitis(all P0.01).Conclusions:The increasing of vaginal pH value could be considered as an early warning signal of abnormal vaginal micro-ecology.The detection rate of lactobacillus in vagina could be used as an index for monitoring vaginal health state and rehabilitation level after RTIs therapy.
Disturbances of Vaginal Microbiome Composition in Human Papillomavirus Infection and Cervical Carcinogenesis: A Qualitative Systematic Review

Abstract:Background: Emerging evidence has demonstrated a close association between perturbations in vaginal microbiota composition in women and human papillomavirus (HPV) infection, cervical lesions, and cervical cancer (Ca); however, these findings are highly heterogeneous and inconclusive. Aim: To perform a comprehensive systematic review of the global disturbance in the vaginal microbiota, specifically in women with HPV-associated cervical diseases, and to further conduct within- and across-disease comparisons. Method: Twenty-two records were identified in a systematic literature search of PubMed, Web of Science, and Embase up to February 28, 2022. We extracted microbial changes at the community (alpha and beta diversity) and taxonomic (relative abundance) levels. Within- and across-disease findings on the relative abundance of taxonomic assignments were qualitatively synthesized. Results: Generally, significantly higher alpha diversity was observed for HPV infection, cervical lesions, and/or cancer patients than in controls, and significant differences within beta diversity were observed for the overall microbial composition across samples. In within-disease comparisons, the genera Gardnerella , Megasphaera , Prevotella , Peptostreptococcus , and Streptococcus showed the greatest abundances with HPV infection; Sneathia and Atopobium showed inconsistent abundance with HPV infection, and Staphylococcus was observed in Ca. Across diseases, we find increased levels of Streptococcus and varying levels of Gardnerella were shared across HPV infections, high-grade squamous intraepithelial lesions, and Ca, whereas Lactobacillus iners varied depending on the HPV-related disease subtype. Conclusions: This systematic review reports that vaginal microbiome disturbances are correlated to the depletion of Lactobacillus , enrichment of anaerobes, and increased abundance of aerobic bacteria in HPV infection and related cervical diseases. Moreover, L. iners may exert either protective or pathogenic effects on different HPV-related diseases.
