Abstract:Virtual patients and digital patients/twins are two similar concepts gaining increasing attention in health care with goals to accelerate drug development and improve patients' survival, but with their own limitations. Although methods have been proposed to generate virtual patient populations using mechanistic models, there are limited number of applications in immuno-oncology research. Furthermore, due to the stricter requirements of digital twins, they are often generated in a study-specific manner with models customized to particular clinical settings (e.g., treatment, cancer, and data types). Here, we discuss the challenges for virtual patient generation in immuno-oncology with our most recent experiences, initiatives to develop digital twins, and how research on these two concepts can inform each other.

Abstract:The concept of Digital Twins (DTs), software models that mimic the behavior and interactions of physical or conceptual objects within their environments, has gained traction in recent years, particularly in medicine and healthcare research. DTs technology emerges as a pivotal tool in disease modeling, integrating diverse data sources to computationally model dynamic biological systems. This narrative review explores potential DT applications in medicine, from defining DTs and their history to constructing DTs, modeling biologically relevant systems, as well as discussing the benefits, risks, and challenges in their application. The influence of DTs extends beyond healthcare and can revolutionize healthcare management, drug development, clinical trials, and various biomedical research fields.

Abstract:The adoption of digital twins (DTs) in precision medicine is increasingly viable, propelled by extensive data collection and advancements in artificial intelligence (AI), alongside traditional biomedical methodologies. However, the reliance on black-box predictive models, which utilize large datasets, presents limitations that could impede the broader application of DTs in clinical settings. We argue that hypothesis-driven generative models, particularly multiscale modeling, are essential for boosting the clinical accuracy and relevance of DTs, thereby making a significant impact on healthcare innovation. This paper explores the transformative potential of DTs in healthcare, emphasizing their capability to simulate complex, interdependent biological processes across multiple scales. By integrating generative models with extensive datasets, we propose a scenario-based modeling approach that enables the exploration of diverse therapeutic strategies, thus supporting dynamic clinical decision-making. This method not only leverages advancements in data science and big data for improving disease treatment and prevention but also incorporates insights from complex systems and network science, quantitative biology, and digital medicine, promising substantial advancements in patient care.

Abstract:UNSTRUCTURED With the development of the world economy and improvements in living standards, people are paying more attention to their healthcare. As simulation is playing an increasingly important role in the field of medicine, providing the individual patient with a customised diagnosis and treatment is envisage as part of future precision medicine. Such customisation will become possible through the emergence of digital twin (DT) technology. As an engineering concept that reflects the true mirror image of a physical entity, the DT is increasingly being used in the field of precision health and personalised medicine. This review summarises the progress of prominent research on DT technology in the medical field and forecasts potential applications and future opportunities and discusses several challenges remaining in digital healthcare. The author hopes that the review encourages additional efforts towards the development and acceptance of DT techniques.

Abstract:Digital twins in biomedical research, i.e. virtual replicas of biological entities such as cells, organs, or entire organisms, hold great potential to advance personalized healthcare. As all biological processes happen in space, there is a growing interest in modeling biological entities within their native context. Leveraging generative artificial intelligence (AI) and high-volume biomedical data profiled with spatial technologies, researchers can recreate spatially-resolved digital representations of a physical entity with high fidelity. In application to biomedical fields such as computational pathology, oncology, and cardiology, these generative digital twins (GDT) thus enable compelling in silico modeling for simulated interventions, facilitating the exploration of 'what if' causal scenarios for clinical diagnostics and treatments tailored to individual patients. Here, we outline recent advancements in this novel field and discuss the challenges and future research directions.

Abstract:Digital twins are virtual replicas of physical objects or systems. This new technology is increasingly being adopted in industry to improve the monitoring and efficiency of products and organizations. In healthcare, digital human twins (DHTs) represent virtual copies of patients, including tissues, organs, and physiological processes. Their application has the potential to transform patient care in the direction of increasingly personalized data-driven medicine. The use of DHTs can be integrated with digital twins of healthcare institutions to improve organizational management processes and resource allocation. By modeling the complex multi-omics interactions between genetic and environmental factors, DHTs help monitor disease progression and optimize treatment plans. Through digital simulation, DHT models enable the selection of the most appropriate molecular therapy and accurate 3D representation for precision surgical planning, together with augmented reality tools. Furthermore, they allow for the development of tailored early diagnosis protocols and new targeted drugs. Furthermore, digital twins can facilitate medical training and education. By creating virtual anatomy and physiology models, medical students can practice procedures, enhance their skills, and improve their understanding of the human body. Overall, digital twins have immense potential to revolutionize healthcare, improving patient care and outcomes, reducing costs, and enhancing medical research and education. However, challenges such as data security, data quality, and data interoperability must be addressed before the widespread adoption of digital twins in healthcare. We aim to propose a narrative review on this hot topic to provide an overview of the potential applications of digital twins to improve treatment and diagnostics, but also of the challenges related to their development and widespread diffusion.

Abstract:The use of Digital Twins (DTs) or the digital replicas of physical entities has provided benefits to several industry sectors, most notably manufacturing. To date, the application of DTs in the healthcare sector has been minimal, however. But, as pressure increases for more precise and personalized treatments, it behooves us to investigate the potential for DTs in the healthcare context. As a proof-of-concept demonstration prior to working with real patients, we attempt in this paper, to explore the potential for creating and using DTs. We do this in a synthetic environment at this stage, making use of data that is all computer-generated. DTs of synthetic present patients are created making use of data of synthetic past patients. In the real world, the clinical objective for creating such DTs of real patients would be to enable enhanced real-time clinical decision support to enable more precise and personalized care. The objective of the numerical experiment reported in this paper, is to envisage the possibilities and challenges of such an approach. We attempt to better understand the strengths and weaknesses of applying DTs in the healthcare context to support more precise and personalized treatments.
Patient digital twins: an introduction based on a scoping review

Abstract:The concept of digital twins, widely adopted in industry, is entering healthcare. In this scoping review, we analysed definitions and characteristics of patient digital twins being developed for clinical use. Searching for studies claiming digital twin development/evaluation until August 2023, we identified 86 articles representing 80 unique claimed digital twins, nearly all (98%) in preclinical phases. From the analysis of definitions and characteristics, we propose to define patient digital twin as “a viewable digital replica of a patient, organ, or biological system that contains multidimensional, patient-specific information”. Two main forms were found: simulation digital twins using computational modelling of patient anatomy/physiology to run personalised outcome predictions and therapy evaluations, mostly for one-time assessments; and monitoring digital twins harnessing aggregated patient data for continuous risk/outcome forecasting over time and care optimisation. As patient digital twins rapidly emerge, the proposed definitions and subtypes offer a framework to guide research into realising the potential of these personalised, integrative technologies to advance clinical care.

Abstract:There has been a great deal of interest in the concept, development and implementation of medical digital twins. This interest has led to wide ranging perceptions of what constitutes a medical digital twin. This Perspectives article will provide 1) a description of fundamental features of industrial digital twins, the source of the digital twin concept, 2) aspects of biology that challenge the implementation of medical digital twins, 3) a schematic program of how a specific medical digital twin project could be defined, and 4) an example description within that schematic program for a specific type of medical digital twin intended for drug discovery, testing and repurposing, the Drug Development Digital Twin (DDDT).

Abstract:A digital health twin can be defined as a virtual model of a physical person, in this specific case, a patient. This virtual model is constituted by multidimensional data that can host from clinical, molecular and therapeutic parameters to sensor data and living conditions. Given that in computational pathology, it is very important to have the information from image donors to create computational models, the integration of digital twins in this field could be crucial. However, since these virtual entities collect sensitive data from physical people, privacy safeguards must also be considered and implemented. With these data safeguards in place, health digital twins could integrate digital clinical trials and be necessary participants in the generation of real-world evidence, which could positively change both fields.

Abstract:With many advancements in in silico biology in recent years, the paramount challenge is to translate the accumulated knowledge into exciting industry partnerships and clinical applications. Achieving models that characterize the link of molecular interactions to the activity and structure of a whole organ are termed multiscale biophysics. Historically, the pharmaceutical industry has worked well with in silico models by leveraging their prediction capabilities for drug testing. However, the needed higher fidelity and higher resolution of models for efficient prediction of pharmacological phenomenon dictates that in silico approaches must account for the verifiable multiscale biophysical phenomena, as a spatial and temporal dimension variation for different processes and models. The collection of different multiscale models for different tissues and organs can compose digital twin solutions towards becoming a service for researchers, clinicians, and drug developers. Our paper has two main goals: 1) To clarify to what extent detailed single- and multiscale modeling has been accomplished thus far, we provide a review on this topic focusing on the biophysics of epithelial, cardiac, and brain tissues; 2) To discuss the present and future role of multiscale biophysics in in silico pharmacology as a digital twin solution by defining a roadmap from simple biophysical models to powerful prediction tools. Digital twins have the potential to pave the way for extensive clinical and pharmaceutical usage of multiscale models and our paper shows the basic fundamentals and opportunities towards their accurate development enabling the quantum leaps of future precise and personalized medical software.

Abstract:Health digital twins (HDTs) are virtual representations of real individuals that can be used to simulate human physiology, disease, and drug effects. HDTs can be used to improve drug discovery and development by providing a data-driven approach to inform target selection, drug delivery, and design of clinical trials. HDTs also offer new applications into precision therapies and clinical decision making. The deployment of HDTs at scale could bring a precision approach to public health monitoring and intervention. Next steps include challenges such as addressing socioeconomic barriers and ensuring the representativeness of the technology based on the training and validation data sets. Governance and regulation of HDT technology are still in the early stages.

Abstract:Background: Heart failure (HF) is a highly heterogeneous and complex condition. Although patient care generates vast amounts of clinical data, robust methods to synthesize available data for individualized management are lacking. Methods: A mechanistic computational model of cardiac and cardiovascular system mechanics was identified for each individual in a cohort of 343 patients with HF. The identified digital twins---comprising optimized sets of parameters and corresponding simulations of cardiovascular system function---for patients with HF in the cohort is used to inform both supervised and unsupervised approaches in identifying phenogroups and novel mechanistic drivers of cardiovascular death risk. Findings: The integration of digital twins into AI-based analyses of patient data enhances the performance and interpretability of prognostics AI models. Prognostics AI models trained with digital twin features are more generalizable than models trained with only clinical variables, as evaluated using an independent prospective cohort. In addition, the digital twin-based approach to phenomapping and predictive AI helps address inconsistencies and inaccuracies in clinical measurements, enables imputation of missing data, and estimates functional parameters that are otherwise unmeasurable directly. This approach provides a more comprehensive and accurate representation of the patient's disease state than raw clinical data alone. Interpretation: The developed and validated digital twin-based AI framework has the potential to simulate patient-specific pathophysiologic parameters, thereby informing prognosis and guiding therapeutic options. Ultimately, this approach has the potential to enhance the ability to focus on the most critical aspects of a patient's condition, leading to individualized care and management.

Abstract:The idea of a systematic digital representation of the entire known human pathophysiology, which we could call the Virtual Human Twin, has been around for decades. To date, most research groups focused instead on developing highly specialised, highly focused patient-specific models able to predict specific quantities of clinical relevance. While it has facilitated harvesting the low-hanging fruits, this narrow focus is, in the long run, leaving some significant challenges that slow the adoption of digital twins in healthcare. This position paper lays the conceptual foundations for developing the Virtual Human Twin (VHT). The VHT is intended as a distributed and collaborative infrastructure, a collection of technologies and resources (data, models) that enable it, and a collection of Standard Operating Procedures (SOP) that regulate its use. The VHT infrastructure aims to facilitate academic researchers, public organisations, and the biomedical industry in developing and validating new digital twins in healthcare solutions with the possibility of integrating multiple resources if required by the specific context of use. Healthcare professionals and patients can also use the VHT infrastructure for clinical decision support or personalised health forecasting. As the European Commission launched the EDITH coordination and support action to develop a roadmap for the development of the Virtual Human Twin, this position paper is intended as a starting point for the consensus process and a call to arms for all stakeholders.

Abstract:A fundamental challenge for personalized medicine is to capture enough of the complexity of an individual patient to determine an optimal way to keep them healthy or restore their health. This will require personalized computational models of sufficient resolution and with enough mechanistic information to provide actionable information to the clinician. Such personalized models are increasingly referred to as medical digital twins. Digital twin technology for health applications is still in its infancy, and extensive research and development is required. This article focuses on several projects in different stages of development that can lead to specific-and practical-medical digital twins or digital twin modeling platforms. It emerged from a two-day forum on problems related to medical digital twins, particularly those involving an immune system component. Open access video recordings of the forum discussions are available.

Abstract:As simulation is playing an increasingly important role in medicine, providing the individual patient with a customised diagnosis and treatment is envisaged as part of future precision medicine. Such customisation will become possible through the emergence of digital twin (DT) technology. The objective of this article is to review the progress of prominent research on DT technology in medicine and discuss the potential applications and future opportunities as well as several challenges remaining in digital healthcare. A review of the literature was conducted using PubMed, Web of Science, Google Scholar, Scopus and related bibliographic resources, in which the following terms and their derivatives were considered during the search: DT, medicine and digital health virtual healthcare. Finally, analyses of the literature yielded 465 pertinent articles, of which we selected 22 for detailed review. We summarised the application examples of DT in medicine and analysed the applications in many fields of medicine. It revealed encouraging results that DT is being increasing applied in medicine. Results from this literature review indicated that DT healthcare, as a key fusion approach of future medicine, will bring the advantages of precision diagnose and personalised treatment into reality.

Abstract:The digital twin (DT) is a concept widely used in industry to create digital replicas of physical objects or systems. The dynamic, bi-directional link between the physical entity and its digital counterpart enables a real-time update of the digital entity. It can predict perturbations related to the physical object's function. The obvious applications of DTs in healthcare and medicine are extremely attractive prospects that have the potential to revolutionize patient diagnosis and treatment. However, challenges including technical obstacles, biological heterogeneity, and ethical considerations make it difficult to achieve the desired goal. Advances in multi-modal deep learning methods, embodied AI agents, and the metaverse may mitigate some difficulties. Here, we discuss the basic concepts underlying DTs, the requirements for implementing DTs in medicine, and their current and potential healthcare uses. We also provide our perspective on five hallmarks for a healthcare DT system to advance research in this field.
