Abstract:Abstract Purpose: mTOR is a validated target in cancer. It remains to be determined whether melanoma patients bearing mTOR mutation could be selected for treatment with PI3K–AKT–mTOR pathway inhibitors. Experimental Design: A total of 412 melanoma samples were included. Gene aberrations in all exons of mTOR were detected by Sanger sequencing and confirmed by using Agilent's SureSelect Target Enrichment System. HEK293T cells stably expressing mTOR mutants were constructed by using transcription activator-like effector nucleases technique. Function of mTOR mutants and in vitro sensitivity of gain-of-function mTOR mutations to PI3K–AKT–mTOR pathway inhibitors were analyzed. Results: The overall incidence of somatic nonsynonymous mutations of mTOR was 10.4% (43/412). mTOR nonsynonymous mutations were relatively more frequent in acral (11.0%) and mucosal (14.3%) melanomas than in chronic sun-induced damage (CSD; 6.7%) and non-CSD (3.4%) melanomas. Of the 43 cases with mTOR mutations, 41 different mutations were detected, affecting 25 different exons. The median survival time for melanoma patients with mTOR nonsynonymous mutation was significantly shorter than that for patients without mTOR nonsynonymous mutation (P = 0.028). Transient expression of mTOR mutants in HEK293T cells strongly activated the mTOR–p70S6K pathway. In HEK293T cells with stable expression of H1968Y or P2213S mTOR mutants, LY294002 and AZD5363 showed higher potency than temsirolimus or BYL719 in inhibiting the PI3K–AKT–mTOR pathway and cell proliferation. Conclusions: mTOR nonsynonymous mutations are frequent in melanoma patients. mTOR nonsynonymous mutation may predict a worse prognosis of melanoma. Abstract:9049 Background: mTOR is a validated target in cancer treatment. mTOR mutations are estimated to be 2.46% of all cancers and 5.76% of cutaneous melanoma. However, mTOR mutations in other types of melanoma have not been reported. Methods: This study involved tumor samples from 412 Chinese melanoma patients, including 210 acral melanomas, 105 mucosal melanomas, 30 melanomas on skin with chronic sun-induced damage and 58 melanomas on skin without chronic sun-induced damage. Tissue samples were analyzed for mutations in all exons of mTOR gene in genomic DNA by Sanger sequencing. Mutation status was confirmed using Agilent’s sureselect target enrichment system. HEK293T cells stably expressing mutant mTOR were constructed by TALEN (Transcription Activator-like Effector Nucleases). Function of mTOR mutants in these cells and in vitro sensitivity of mTOR gain-of-function mutations to PI3K-AKT-mTOR pathway inhibitors were analyzed. Results: The overall incidence of somatic mutations within the mTOR gene was 10.4% (43/412). Increased mTOR mutations were relatively more frequent in acral (11.0%) and mucosal (14.4%) melanomas than in CSD (6.7%) and Non-CSD (3.4%) melanomas. Of the 43 cases with mTOR mutations, 41 different mutations were detected, affecting 25 different exons. Point mutations resulting in single amino acid substitutions (totaling 40 mutations detected in 43 patients) were the most common type of mTOR mutation. The median survival time for melanoma patients with mTOR mutation was significantly shorter than that for patients without mTOR mutation (P = 0.028). Transient expression of several mTOR mutants in HEK293 cells strongly activated the mTOR/p70S6K pathway. In HEK293 cells with stable expression of H1968Yor P2213S mTOR mutants, LY294002 and AZD5363 showed higher potency than Temsirolimus or BYL719 in inhibiting the mTOR/p70S6K pathway. Conclusions: These data demonstrate that mTOR mutations are more frequent in Chinese melanoma patients than in Caucasian melanoma patients. mTOR mutations is a worse prognostic factor in Chinese melanoma patients. Clinical trials with PI3K-AKT-mTOR pathway inhibitors may be beneficial for melanoma patients with mTOR mutations.

Abstract:Abstract Objective: Melanoma is a highly heterogeneous disease. A certain subgroup of patients with primary melanomas exhibits a greater potential to develop metastatic disease and thus has worse survival. The present study attempts to identify the molecular features shared by metastatic melanomas and the subgroup of primary melanomas, and use the information to assist clinicians to design personalized therapy for primary melanoma patients. Patients and methods: Seven previously reported biomarkers, including BRAF, Dicer, Fbw7, KAI1, MMP2, p27 and Tip60, were investigated. A training cohort with 250 melanoma patients (145 primary melanomas and 105 metastatic melanomas) and an independent cohort with 92 primary melanoma patients were used for the study. Logistic regression analysis was used to identify discriminate biomarkers of metastatic melanoma from primary melanoma. Kaplan-Meier survival and multivariate Cox proportional hazard regression analysis were performed to assess the significance of the molecular signature in the prognosis of melanoma. Results: In the training cohort, we found the loss expression of KAI1 and p27 to be most significant between metastatic and primary melanoma. The primary melanoma patients with loss expression of both KAI1 and p27 had poor 5-year survival in both training cohort (P = 0.002) and independent cohort (P = 0.03). Multivariate Cox regression analysis showed that the KAI1-/p27- signature was an independent factor for disease- specific survival (P = 0.004). More important, compared to KAI1 and p27 as an individual prognostic marker, the KAI1-/p27- signature is more closely associated with melanoma patient survival: the P value is 0.004 for KAI1-/p27-, and 0.044, 0.181 for KAI1 and p27, respectively. Conclusion: Loss of both KAI1 and p27 defines a subgroup of primary melanoma patients with poor prognosis, and the combination signature is better than individual biomarker. The clinical significance of KAI1 and p27 warrants further assessment in prospective clinical trials. Abstract:8561 Background: mTOR is a ubiquitously expressed protein kinase and a validated target in the treatment of cancer. However, the characterization of mTOR pathway in Asian melanoma populations has not been reported. Methods: This study involved primary tumor samples from 173 melanoma patients (79 acral melanomas, 57 mucosal melanomas, 17 melanomas on skin with chronic sun-induced damage, 20 melanomas on skin without chronic sun-induced damage) in Peking University Cancer Hospital & Institute. Clinical data were collected. Immunohistochemistry assays using antibodies against pmTOR, pS6RP, p4E-BP1 and pAKT were performed on formalin-fixed, paraffin-embedded specimens. Somatic mutations within the hotspot regions of frequently mutated genes in mTOR pathway (AKT1, TSC1, PI3K genes, etc) were analyzed by PCR amplification and Sanger sequencing. Results: Among the 173 samples, 26% of the cases (45/173) demonstrated positive staining with pmTOR. Expression of pS6RP was observed in 37% (64/173) of cases. Expression of p4E-BP1 was observed in 27% (46/173) of cases. pAKT was expressed by 22% (38/173) of cases. Somatic mutations in examined genes were detected. The median survival time for patients with expression of pmTOR was significantly shorter than patients with absence of pmTOR expression (25.3 vs 62.9 months, P =0.048). In addition, statistical differences were found for ulceration rates between melanomas with or without pS6RP (64.1%vs 35.9%, P= 0.049). Moreover, the median survival time for mucosal melanoma patients with pAKT expression was significantly shorter than for patients with absence of pAKT expression (20.4 vs 52.1 months, P =0.022). Conclusions: These data demonstrate that activation of the mTOR signaling pathway carries prognostic significance in Asia patients with melanoma. Targeted therapies to mTOR pathway may offer a therapeutic benefit for these melanoma patients.

Abstract:8562 Background: c-Kit mutation is a common genetic abnormality in certain melanoma subtypes, and is a target for treatment. However, the mechanisms of secondary (acquired) resistance and how to treat patients in the second line are unknown. Methods: Tissuesamples were obtained from c-Kit-mutated melanoma patients who failed of imatinib after having PR or SD for at least 6 months pre- and post-imatinib therapy. Somatic mutations in genes in MAP kinase (c-Kit, BRAF, NRAS, etc) and PI3K-AKT-mTOR (AKT1, TSC1, PTEN, etc) pathways were detected by DNA sequencing. The expression of pS6RP, p4E-BP1, pAKT and pERK1/2 were examined by immunohistochemistry (IHC) assays. Results: Four paired samples (formalin-fixed, paraffin-embedded) were analyzed. 1) Laboratory results: No secondary mutations in addition to the c-Kit mutations identified at baseline were identified. However, IHC showed increased pS6RP and p4E-BP1 (two targets of mTOR activation) as well as increased pAKT and pERK1/2 in imatinib-resistant samples, suggested that mTOR signaling pathway was selectively activated in these secondary imatinib-resistant melanomas. 2) Treatment: Everolimus is an orally administered inhibitor of mTOR. The four patients received everolimus (10 mg/day) by continuous daily dosing. The tumor response revealed 1 PR and 3 SD (all >3 months) with grade 1-2 toxicities consisting of myelosupression, hyperglycemia, hypercholesterolemia and hypertriglyceridemia, consistent with previously reported toxicities for everolimus. Conclusions: The case series provides an important clue that activation of the mTOR signal pathway may be one of the mechanisms for secondary imatinib resistance in c-Kit-mutated melanomas. The results also show that everolimus may be effective and safe for melanoma patients who develop resistance to imatinib and could be investigated in combination with c-Kit inhibitors in treatment-naive patients.

Abstract:mTOR is an important therapeutic target in many types of cancers. In melanoma, the mTOR nonsynonymous mutation rate is up to 10.4%. However, mTOR inhibitors have shown limited effects in clinical trials of melanoma. Because mTOR mutations are distributed, not selecting patients with specific mTOR mutations may be the main reason for therapeutic failures. Our previous research found that mutations in the mTOR P2213S and S2215Y kinase domains resulted in gain-of-function and were sensitive to specific inhibitors. The purpose of this study was to test the effect of heterozygous/homozygous H2189Y mutations on downstream pathways and sensitivity to inhibitors. mTOR kinase activity was analyzed by western blot and a K-LISA mTOR activity kit. The sensitivity of melanoma cells to inhibitors was tested by a proliferation assay. The expression of downstream pathway proteins was also analyzed by western blot. The results showed that heterozygous/homozygous H2189Y mutations were gain-of-function. The heterozygous H2189Y mutation was sensitive to the AKT inhibitor, AZD5363, and the phosphoinositide 3-kinase inhibitor, LY294002. The homozygous H2189Y mutation was sensitive to the mTOR inhibitor, everolimus, and the AKT inhibitors AZD5363 and MK-2206 2HCL,and the phosphoinositide 3-kinase inhibitor, LY294002. These results indicated that homozygous mutations in the kinase domain have a greater effect on protein function than heterozygous mutations. The mTOR kinase domain may play an important role in mTOR kinase activity and may be the target of selective inhibitors. Our study can facilitate the selection of appropriate inhibitors for patients in clinical trials.

Abstract:e13508 Background: Murine double minute-2 (MDM2) has increasing clinic relevance as preclinical and early clinical evidences have showed that the MDM2 inhibitors are promising alternative treatments for patients with MDM2-amplified (-amp) and TP53-wild-type (-WT) tumors by preventing TP53 protein degradation. However, exploration into genomic landscape of MDM2/TP53 across various solid tumors in Chinese patients is still limited. Methods: Formalin-fixed, paraffin-embedded (FFPE) tumor samples were collected from 10010 Chinese patients with solid tumors and subjected to a clinical-grade next-generation sequencing (NGS)-based 450 gene panel testing carried out by a College of American Pathologists (CAP) accredited and Clinical Laboratory Improvement Amendments (CLIA) certified laboratory. Genomic alterations (GAs) including single base substitutions, short and long insertion/deletions (Indels), copy number variations, gene fusions, and rearrangements were analyzed. Tumor mutational burden (TMB) was measured by a NGS algorithm. Results: Amplification of MDM2 occurred in 3.3% of Chinese patients. MDM2-amp was detected most frequently in soft tissue sarcoma (STS, 11.4 %), melanoma (10.3 %) and osteosarcoma (6.3 %). Notably, MDM2-amp was revealed to be mutually exclusive with TP53-mut in most tumor types other than esophageal carcinoma (93.9 %), head and neck carcinoma (68.4%) and ovarian carcinoma (50 %). Tumors with highest frequencies of MDM2-amp/ TP53-WT were melanoma (100%), osteosarcoma (100%) and urothelial carcinoma (100 %). The most common co-altered genes accompanied with MDM2-amp were FRS2 (84%), CDK4 (38%), EGFR (26%), TP53 (17%) and GLI1 (14%). 82.8% patients who had one or more co-alterations potentially targetable with either FDA-approved or investigational agent. Moreover, TMB of MDM2-amp tumors were significantly lower than that of MDM2-WT and MDM2-mutation (-mut) tumors (median TMB: 3.1 vs. 4.6 vs. 10.8, p < 0.001 respectively). Conclusions: MDM2 amplification was found in 3.3% of 10010 Chinese cancer patients, 83.4% of whom harbored wild-type TP53 that were potentially responsive to MDM2 inhibitors. In addition, 82.8% of the patients with MDM2-amp harbored genomic co-alterations and were potentially therapy targetable. This study provided insights into identifying patients who might potentially benefit from anti-MDM2 therapies.

Abstract:Abstract Skin cutaneous melanoma (SKCM) is the most serious form of skin cancer and the reliable genetic prognostic biomarkers are not clear. The purpose of the present academic work was to identify reliable prognostic biomarkers and understand the underlying mechanism. Gene expression profiles of GSE31909, GSE104849 and GSE111766 were available from Gene Expression Omnibus (GEO). DEGs were collected from 13 melanoma samples and 12 melanocyte samples which were in the three profile datasets by GEO2R analysis. Gene ontology (GO) analysis and KEGG analysis were performed with the Database for Annotation, Visualization and Integrated Discovery (DAVID).We further established protein-protein interaction (PPI) and identify core genes with Search Tool for the Retrieval of Interacting Genes (STRING) and Molecular Complex Detection (MCODE) app in Cytoscape software. There were total of 63 consistently expressed genes in the three datasets, including eight up-regulated genes enriched in biological processes like osteoblast differentiation and extracellular matrix organization, and cell component like extracellular space, while 55 down-regulated genes enriched in positive regulation of gene expression, intracellular signal transduction, cell adhesion and apoptotic process. With prognostic information and expression data from UALCAN and Gene Expression Profiling Interactive Analysis (GEPIA), we identified three significant genes ( TNC, SPP1 and KIT ). After Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis, all three genes were located in the PI3K-Akt Signaling Pathway. Our results provide additional genetic biomarkers for SKCM patients.

Abstract:Background: Comprehensive analysis of PI3K-AKT-mTOR pathway gene alterations in breast cancer may be helpful for targeted therapy. Methods: We performed targeted sequencing using a panel of 520 cancer-related genes to investigate gene alterations in the PI3K-AKT-mTOR pathway from 589 consecutive Chinese women diagnosed with stage I-III breast cancer. Analyses of overall survival (OS) were performed using the publicly available clinical and genomic data from METABRIC. Results: PI3K-AKT-mTOR pathway gene alterations were detected in 62.6% (369/589) of our cohort. The most commonly altered genes were PIK3CA (45%), PTEN (7.5%), AKT1 (5.9 %), PIK3R1 (2.7%), and PIK3CG (2%). Four PIK3CA mutations (E545K, H1047R, E542K, and H1047L) were detected in all the breast cancer molecular subtypes. Seven PIK3CA mutations (E545G, E418_L422delinsV, E726K, E110del, G1049R, G118D, and D350G) were only detected in HR+ subtypes. Two PIK3CA mutations (C420R and N345K) were only detected in non-triple-negative subtypes. Most cases with PTEN mutation were HR+/HER2- subtype (77.3%), followed by triple-negative subtype (18.2%). In the METABRIC breast cancer dataset, no significant OS difference was observed between the PIK3CA-mutant and wild-type groups. However, patients with multiple PIK3CA mutations (mOS: 131 vs. 159 months, P= 0.029), or PIK3CA mutations located in the C2 domain had significantly shorter OS (mOS, 130 vs. 154 months, P=0.020) than those without the mutations. Conclusions: Our study reveals the heterogeneity in PI3K-AKT-mTOR pathway among the breast cancer molecular subtypes in our cohort. Moreover, the number and specific sites of PIK3CA mutations have distinct prognostic impact.

Abstract:Mammalian Target of Rapamycin (mTOR) is a validated target in cancer. It remains to be determined whether non-small-cell lung cancer patients bearing mTOR mutation could be selected for treatment with PI3K-AKT-mTOR pathway inhibitors. While the genetic spectrum of MTOR mutation NSCLC patients is unclear. The aim of this study is to investigate mutations and prognosis of NSCLC harboring MTOR mutations. A total of 639 patients with non-small-cell lung cancer were recruited between July 2012 and December 2015. The status of MTOR mutation and other genes were detected by next generation sequencing. MTOR gene mutation rate was 3.60% (23/639) in non-small cell lung cancer, including T1830_T1833[2>1] (3 patients), S221N (1 patient), C1483F (1 patient), D1527H (1 patient), I1964V (1 patient), E1799K (1 patient), L88F (1 patient), P2273L (1 patient), E97K (1 patient), L565Q (1 patient), M1038I (1 patient), A1792V (1 patient), R1896Q (1 patient), G1678E (1 patient), M2327I (1 patient), E2419K (1 patient), A2248S (1 patient), T1870S (1 patient), V2198A (1 patient), R1482P (1 patient) and E1362K (1 patient), and median overall survival (OS) for these patients was 13.0 months. Among them, all patients were MTOR gene with co-occurring mutation. Briefly, patients with (n=5) or without (n=18) co-occurring EGFR mutations had a median OS of 17.0 months and 11.0 months respectively (P=0.33); patients with (n=15) or without (n=8) co-occurring TP53 mutations had a median OS of 11.0 months and 13.0 months respectively (P=0.42). MTOR mutation may predict a worse prognosis of NSCLC. MTOR pathway inhibitors everolimus may be beneficial for NSCLC patients with specific MTOR mutations. EGFR and TP53 gene accompanied may have less correlation with MTOR mutation in NSCLC patients. The findings of this study could facilitate both clinical trial design and therapeutic strategies.
