Abstract:Representative case to show the identification of leading lesion (a), where two solid tumor foci in right (red line) and left PZ (green line) were identified on axial fusion T2WI and DWI. Because tumor in right PZ had larger size than left PZ lesion, and an infiltration of the peri-prostatic fat (presence of ECE) was suspected, it was selected as leading lesion for imaging evaluation. Representative cases to show the imaging features for ECE (b), SVI (c) and local LN metastasis (d), respectively. Presence of ECE is defined as an infiltration of the peri-prostatic fat, irregular bulging associated with disruption of the capsule, focal thickening, capsular retraction, and peri-capsular spicula (arrow; b). SVI is featured with focal or diffuse low T2 SI and/or abnormal contrast enhancement within and/or along the seminal vesicle, restricted diffusion, obliteration of the angle between the base of the prostate and the seminal vesicle, and demonstration of direct tumor extension from the base of the prostate into and around the seminal vesicle (arrow; c). LN over 8 mm in short axis dimension are regarded as suspicious LN metastasis. Abstract:Abstract Purpose: To investigate whether prebiopsy multi-parametric (mp) MRI can help to improve predictive performance in prostate cancer. Experimental Design: Based on a support vector machine (SVM) analysis, we prospectively modeled clinical data (age, PSA, digital rectal examination, transrectal ultrasound, PSA density, and prostate volume) and mp-MRI findings [Prostate Imaging and Reporting and Data System (PI-RADS) score and tumor–node–metastasis stage] in 985 men to predict the risk of prostate cancer. The new nomogram was validated in 493 patients treated at the same institution. Multivariable Cox regression analyses assessed the association between input variables and risk of prostate cancer, and area under the receiver operating characteristic curve (Az) analyzed the predictive ability. Results: At 5-year follow-up period, 34.3% of patients had systemic progression of prostate cancer. Nomogram (SVM-MRI) predicting 5-year prostate cancer rate trained with clinical and mp-MRI data was accurate and discriminating with an externally validated Az of 0.938, positive predictive value (PPV) of 77.4%, and negative predictive value of 91.5%. The improvement was significant (P < 0.001) compared with the nomogram trained with clinical data. When stratified by PSA, SVM-MRI nomogram had high PPV (93.6%) in patients with PSA > 20 ng/mL, with intermediate to low PPV in PSA 10 to 20 ng/mL (64%), PSA 4 to 10 ng/mL (55.8%), and PSA 0 to 4 ng/mL (29%). PI-RADS score (Cox HR, 2.112; P < 0.001), PSA level (HR, 1.435; P < 0.001), and age (HR, 1.012; P = 0.043) were independent predictors of prostate cancer. Conclusions: Featured with low false positive rate, mp-MRI could be the first investigation of a man with a raised PSA before prostate biopsy. Clin Cancer Res; 23(14); 3692–9. ©2017 AACR.
Can 3D Multiparametric Ultrasound Imaging Predict Prostate Biopsy Outcome?

Abstract:ObjectivesTo assess the value of 3D multiparametric ultrasound imaging, combining hemodynamic and tissue stiffness quantifications by machine learning, for the prediction of prostate biopsy outcomes.MethodsAfter signing informed consent, 54 biopsy-naïve patients underwent a 3D dynamic contrast-enhanced ultrasound (DCE-US) recording, a multi-plane 2D shear-wave elastography (SWE) scan with manual sweeping from base to apex of the prostate, and received 12-core systematic biopsies (SBx). 3D maps of 18 hemodynamic parameters were extracted from the 3D DCE-US quantification and a 3D SWE elasticity map was reconstructed based on the multi-plane 2D SWE acquisitions. Subsequently, all the 3D maps were segmented and subdivided into 12 regions corresponding to the SBx locations. Per region, the set of 19 computed parameters was further extended by derivation of eight radiomic features per parameter. Based on this feature set, a multiparametric ultrasound approach was implemented using five different classifiers together with a sequential floating forward selection method and hyperparameter tuning. The classification accuracy with respect to the biopsy reference was assessed by a group-k-fold cross-validation procedure, and the performance was evaluated by the Area Under the Receiver Operating Characteristics Curve (AUC).ResultsOf the 54 patients, 20 were found with clinically significant prostate cancer (csPCa) based on SBx. The 18 hemodynamic parameters showed mean AUC values varying from 0.63 to 0.75, and SWE elasticity showed an AUC of 0.66. The multiparametric approach using radiomic features derived from hemodynamic parameters only produced an AUC of 0.81, while the combination of hemodynamic and tissue-stiffness quantifications yielded a significantly improved AUC of 0.85 for csPCa detection (p-value < 0.05) using the Gradient Boosting classifier.ConclusionsOur results suggest 3D multiparametric ultrasound imaging combining hemodynamic and tissue-stiffness features to represent a promising diagnostic tool for biopsy outcome prediction, aiding in csPCa localization.

Abstract:BACKGROUND:Although European Society of Urogenital Radiology proposed the potential of multiparametric magnetic resonance imaging (MP-MRI) as a tool in the diagnostic pathway for prostate cancer (PCa) and published a unified scoring system named Prostate Imaging Reporting and Data System (PI-RADS version 1), these still need to be validated by real-life studies.OBJECTIVE:To evaluate the role of MP-MRI in detection and prediction of PCa.METHODS:Patients with clinical suspicion of PCa who underwent prebiopsy MP-MRI from 2002 to 2009 were recruited. MP-MRI results were retrospectively assigned as overall scores using PI-RADS by two radiologists. Patients were followed and the end point was the diagnosis of PCa. Receiver operating characteristics (ROC) curve was performed to test diagnostic efficacy of MP-MRI, under results of biopsy within three months. The cox proportional hazards model was used to identify independent variables for the detection of PCa.RESULTS:Finally, 1113 of the 1806 enrolled patients were included for analysis. The median follow-up was 56.0 months (1-137 mo). For 582 patients biopsied within three months, area under the curve for the detection of PCa with MP-MRI was 0.88 (95% confidence interval [CI], 0.75-1.00) in group of baseline prostate specific antigen (PSA) 0.01-4.00 ng/ml (n = 31), 0.90 (95% CI, 0.84-0.95) in PSA 4.01-10.00 ng/ml (n = 142), and 0.91 (95% CI, 0.87-0.94) in PSA >10.00 ng/ml (n = 409), respectively. In the cox model adjusted for age and baseline PSA level, for the detection rate of PCa, compared with PI-RADS 1-2 (reference), the hazard ratio was 6.43 (95% CI, 4.29-9.65) for PI-RADS 3, 18.58 (95% CI, 13.36-25.84) for PI-RADS 4-5 (p < 0.001).CONCLUSIONS:Prebiopsy MP-MRI with PI-RADS is demonstrated as a valuable diagnostic and predictive tool for PCa.

Abstract:Objective MR perfusion-weighted imaging(PWI) has been widely applied in the research of cerebral tumor, benign and malignant musculoskeletal neoplasms and so on. The aim of this study is to explore the application of MR perfusion-weighted imaging in prostatic cancer (Pca), and evaluate the correlation of PWI features with vascular endothelial growth factor (VEGF) and microvessel density (MVD). Methods Twenty-eight consecutive patients who were diagnosed clinically as prostatic cancer and thirty healthy volunteers were examined by PWI. MVD and VEGF were stained with immunohistochemical methods. Some parameters of PWI, including the steepest slope of signal intensity-time curve (SS max ) and the change in relaxation rate (ΔR 2 peak *) at lesions, were analyzed. Correlation analysis was used to determine the relationship between the results of PWI and that of immunohistochemistry. Result (1)In the healthy volunteers, the steepest slope of signal intensity-time curve (SS max ) and ΔR 2 peak * of perfusion curve were: 0.430±0.011, 2.01±0.7 respectively; however, in the prostatic cancer, they were 57.8±5.0, 3.0±0.6 respectively; with significant difference ( t =4.11, 3.28, P <0.01). (2) The VEGF and MVD expression of twenty-eight Pca patients were significantly higher. Conclusion On MR perfusion-weighted imaging, SS max and ΔR 2*peak can reflect MVD and VEGF expression levels in prostatic cancer, suggesting information on tumor angiogenesis. Thus they are beneficial to the diagnosis and treatment of prostatic cancer.

Abstract:Objectives This study aims to develop and evaluate multiparametric MRI (MP-MRI)-based radiomic models as a noninvasive diagnostic method to predict several biological characteristics of prostate cancer. Methods A total of 252 patients were retrospectively included who underwent radical prostatectomy and MP-MRI examinations. The prediction characteristics of this study were as follows: Ki67, S100, extracapsular extension (ECE), perineural invasion (PNI), and surgical margin (SM). Patients were divided into training cohorts and validation cohorts in the ratio of 4:1 for each group. After lesion segmentation manually, radiomic features were extracted from MP-MRI images and some clinical factors were also included. Max relevance min redundancy (mRMR) and recursive feature elimination (RFE) based on random forest (RF) were adopted to select features. Six classifiers were included (SVM, KNN, RF, decision tree, logistic regression, XGBOOST) to find the best diagnostic performance among them. The diagnostic efficiency of the construction models was evaluated by ROC curves and quantified by AUC. Results RF performed best among the six classifiers for the four groups according to AUC values (Ki67 = 0.87, S100 = 0.80, ECE = 0.85, PNI = 0.82). The performance of SVM was relatively the best for SM (AUC = 0.77). The number and importance of DCE features ranked first in the models of each group. The combined models of MP-MRI and clinical characteristics showed no significant difference compared with MP-MRI models according to Delong’s tests. Conclusions Radiomics models based on MP-MRI have the potential to predict biological characteristics and are expected to be a noninvasive method to evaluate the risk stratification of prostate cancer.

Abstract:Purpose: To evaluate the diagnostic performance of multiparametric (MP) magnetic resonance (MR) imaging for prostate cancer detection by using transperineal template prostate mapping (TTPM) biopsies as the reference standard and to determine the potential ability of MP MR imaging to identify clinically significant prostate cancer. Materials and methods: Institutional review board exemption was granted by the local research ethics committee for this retrospective study. Included were 64 men (mean age, 62 years [range, 40-76]; mean prostate-specific antigen, 8.2 ng/mL [8.2 μg/L] [range, 2.1-43 ng/mL]), 51 with biopsy-proved cancer and 13 suspected of having clinically significant cancer that was biopsy negative or without prior biopsy. MP MR imaging included T2-weighted, dynamic contrast-enhanced and diffusion-weighted imaging (1.5 T, pelvic phased-array coil). Three radiologists independently reviewed images and were blinded to results of biopsy. Two-by-two tables were derived by using sectors of analysis of four quadrants, two lobes, and one whole prostate. Primary target definition for clinically significant disease necessary to be present within a sector of analysis on TTPM for that sector to be deemed positive was set at Gleason score of 3+4 or more and/or cancer core length involvement of 4 mm or more. Sensitivity, negative predictive value, and negative likelihood ratio were calculated to determine ability of MP MR imaging to rule out cancer. Specificity, positive predictive value, positive likelihood ratio, accuracy (overall fraction correct), and area under receiver operating characteristic curves were also calculated. Results: Twenty-eight percent (71 of 256) of sectors had clinically significant cancer by primary endpoint definition. For primary endpoint definition (≥ 4 mm and/or Gleason score ≥ 3+4), sensitivity, negative predictive value, and negative likelihood ratios were 58%-73%, 84%-89%, and 0.3-0.5, respectively. Specificity, positive predictive value, and positive likelihood ratios were 71%-84%, 49%-63%, and 2.-3.44, respectively. Area under the curve values were 0.73-0.84. Conclusion: Results of this study indicate that MP MR imaging has a high negative predictive value to rule out clinically significant prostate cancer and may potentially have clinical use in diagnostic pathways of men at risk.

Abstract:Introduction: Prostate cancer (PCa) risk stratification is essential in guiding therapeutic decision. Multiparametric magnetic resonance tomography (mpMRI) holds promise in the prediction of adverse pathologies (AP) after prostatectomy (RP). This study aims to identify clinical and imaging markers in the prediction of adverse pathology. Methods: Patients with PCa, diagnosed by targeted biopsy after mpMRI and undergoing RP, were included. The predictive accuracy of mpMRI for extraprostatic extension (ECE), seminal vesicle infiltration (SVI), and lymph node positivity was calculated from the final histopathology. Results: 846 patients were involved. Independent risk parameters include imaging findings such as ECE (OR 3.12), SVI (OR 2.55), and PI-RADS scoring (4: OR 2.01 and 5: OR 4.34). mpMRI parameters such as ECE, SVI, and lymph node metastases showed a high prognostic accuracy (73.28% vs. 95.35% vs. 93.38%) with moderate sensitivity compared to the final histopathology. The ROC analysis of our combined scoring system (D’Amico classification, PSA density, and MRI risk factors) improves the prediction of adverse pathology (AUC: 0.73 vs. 0.69). Conclusion: Our study supports the use of mpMRI for comprehensive pretreatment risk assessment in PCa. Due to the high accuracy of factors like ECE, SVI, and PI-RADS scoring, utilizing mpMRI data enabled accurate prediction of unfavorable pathology after RP.

Tumor suspicion on MRI incorporating T 2 and Diffusion-weighted imaging predicts adverse pathology on radical prostatectomy

Abstract:Objective: The objective of this study was to determine whether suspicion for tumor on pretreatment prostate magnetic resonance imaging (MRI) incorporating T2 and diffusion-weighted imaging (DWI) predicts more adverse pathology following radical prostatectomy (RP) in prostate cancer patients. Methods: A single-institutional retrospective analysis was performed on 745 patients who underwent 3.0 Tesla pelvic-phased-array MRI of the prostate that included T2 and DWI before RP between July 2009 and December 2015. Patients were grouped into no suspicion for tumor (NST), equivocal suspicion for tumor (EST), or strong suspicion for tumor (SST) based on MRI findings. Preoperative variables and post-operative pathology, including primary Gleason score (≥ 4 vs < 3), pathological stage (≥ pT3 vs < T3), surgical margin and lymph node positivity, were compared between the three groups. A univariate logistic regression analysis was used to assess the associations between preoperative parameters and surgical pathology. Multivariable logistic regression analyses were performed to determine whether MRI finding was an independent predictor of adverse pathology after RP. Results: The preoperative PSA level, Gleason score and clinical stage showed significant variations between the NST, EST, and SST groups. Tumor suspicion on MRI was significantly associated with primary Gleason pattern, pathologic stage, and surgical margin. On multivariate analysis, MRI findings were independently predictive of primary Gleason score ≥ 4, pathologic stage ≥ T3, and positive margin. Conclusion: Prostate cancer patients with increased tumor suspicion on MRI incorporating T2 and DWI seemed to have worse pathologic features after RP. MRI may have potential use as a surrogate biomarker for adverse pathology.

Abstract:Aims/Background Prostate cancer stands out as one of the most prevalent malignant tumours among males. The non-invasive identification of clinically significant prostate cancer via magnetic resonance imaging plays a critical role in circumventing unnecessary biopsies and determining suitable treatment strategies for patients. Our study aimed to evaluate the potential improvement in predictive accuracy for clinically significant prostate cancer by incorporating perfusion data obtained from dynamic contrast-enhanced magnetic resonance imaging acquisition protocols into multiparametric magnetic resonance imaging parameters. Methods Radiomics extracted from perfusion parameters (K trans , K ep , V e ) of dynamic contrast-enhanced magnetic resonance imaging were analysed in patients suspected of prostate cancer who underwent 3T multiparametric magnetic resonance imaging between January 2017 and June 2023 in this retrospective study. The pathological findings obtained from biopsy or therapy were categorised into groups based on the Gleason sum score as either clinically significant prostate cancer (Gleason sum score > 7) or non-clinically significant prostate cancer (Gleason sum score ≤ 6). Diagnostic models were constructed using logistic regression analysis, incorporating prostate imaging reporting and data system V2.1 scores and clinical data, with or without radiomics extracted from dynamic contrast-enhanced. The area under curve (AUC) values were compared using the DeLong test. Results Overall, 214 men (clinically significant prostate cancer [n=78] and non-clinically significant prostate cancer [n=136]) were included. The clinical-prostate imaging reporting and data system model demonstrated an AUC of 0.89 (95% confidence interval: 0.84–0.95) in the training cohort and 0.91 (95% confidence interval: 0.84–0.98) in the test cohort. For the clinical-prostate imaging reporting and data system-radscore model, the AUC values were 0.97 (95% confidence interval: 0.95–0.99) for K trans , 0.98 (95% confidence interval: 0.96–1.00) for V e , and 0.96 (95% confidence interval: 0.93–0.98) for K ep in the training cohort, and 0.97 (95% confidence interval: 0.94–1.00) for K trans , 0.95 (95% confidence interval: 0.91–1.00) for V e , and 0.97 (95% confidence interval: 0.94–1.00) for K ep in the test cohort. Radiomics based on perfusion parameters exhibited good diagnostic performance in predicting clinically significant prostate cancer. The clinical-prostate imaging reporting and data system-radscore model demonstrated superior diagnostic capability compared to perfusion-based radiomics or clinical-prostate imaging reporting and data system models alone. Conclusion The application of radiomics, which involves extracting perfusion parameters from dynamic contrast-enhanced imaging, has the potential to enhance diagnostic accuracy for clinically significant prostate cancer.

Abstract:Purpose This prospective study aimed to evaluate the difference between 99m Tc-PSMA single-photon emission computed tomography (SPECT)/CT and multiparametric magnetic resonance imaging (mpMRI) in the detection of primary prostate cancer (PCa). Materials and methods Fifty-six men with suspected PCa between October 2019 and November 2022 were prospectively enrolled in this study. The median age of the patients was 70 years (range, 29-87 years). Patients were divided into high-(Gleason score>7, n=31), medium- (Gleason score=7, n=6) and low-risk groups (Gleason score < 7, n=6). All patients underwent 99m Tc-PSMA SPECT/CT and mpMRI at an average interval of 3 days (range, 1-7 days). The maximum standardized uptake value (SUV max ), the minimum apparent diffusion coefficient (ADC min ), and their ratio (SUV max /ADC min ) were used as imaging parameters to distinguish benign from malignant prostatic lesions. Results Of the 56 patients, 12 were pathologically diagnosed with a benign disease, and 44 were diagnosed with PCa. 99m Tc-PSMA SPECT/CT and mpMRI showed no significant difference in the detection of primary PCa (kappa =0.401, P =0.002), with sensitivities of 97.7% (43/44) and 90.9% (40/44), specificities of 75.0% (9/12) and 75.0% (9/12), and AUC of 97.4% and 95.1%, respectively. The AUC of SUV max /ADC min was better than those of SUV max or ADC min alone. When SUV max /ADC min in the prostatic lesion was >7.0×10 3 , the lesion was more likely to be malignant. When SUV max /ADC min in the prostatic lesion is >27.0×10 3 , the PCa patient may have lymph node and bone metastases. SUV max was positively correlated with the Gleason score ( r =0.61, P=0.008), whereas ADC min was negatively correlated with the Gleason score ( r =-0.35, P =0.023). SUV max /ADC min was positively correlated with the Gleason score ( r =0.59, P =0.023). SUV max /ADC min was the main predictor of the high-risk group, with an optimal cut-off value of 15.0×10 3 . Conclusions The combination of 99m Tc-PSMA SPECT/CT and mpMRI can improve the diagnostic efficacy for PCa compared with either modality alone; SUV max /ADC min is a valuable differential diagnostic imaging parameter.

Abstract:PurposeTo compare the performance of radiomics to that of the Prostate Imaging Reporting and Data System (PI-RADS) v2.1 scoring system in the detection of clinically significant prostate cancer (csPCa) based on biparametric magnetic resonance imaging (bpMRI) vs. multiparametric MRI (mpMRI). MethodsA total of 204 patients with pathological results were enrolled between January 2018 and December 2019, with 142 patients in the training cohort and 62 patients in the testing cohort. The radiomics model was compared with the PI-RADS v2.1 for the diagnosis of csPCa based on bpMRI and mpMRI by using receiver operating characteristic (ROC) curve analysis. ResultsThe radiomics model based on bpMRI and mpMRI signatures showed high predictive efficiency but with no significant differences (AUC = 0.975 vs 0.981, p=0.687 in the training cohort, and 0.953 vs 0.968, p=0.287 in the testing cohort, respectively). In addition, the radiomics model outperformed the PI-RADS v2.1 in the diagnosis of csPCa regardless of whether bpMRI (AUC = 0.975 vs. 0.871, p= 0.030 for the training cohort and AUC = 0.953 vs. 0.853, P = 0.024 for the testing cohort) or mpMRI (AUC = 0.981 vs. 0.880, p= 0.030 for the training cohort and AUC = 0.968 vs. 0.863, P = 0.016 for the testing cohort) was incorporated. ConclusionsOur study suggests the performance of bpMRI- and mpMRI-based radiomics models show no significant difference, which indicates that omitting DCE imaging in radiomics can simplify the process of analysis. Adding radiomics to PI-RADS v2.1 may improve the performance to predict csPCa.

Abstract:Objective:Investigate DWI(diffusion-weighted MR imaging) combined MRS (spectroscopy MR imaging) the diagnosis of prostate cancer patients.Methods:A retrospective analysis of 64 cases of prostate cancer patients in our hospital from November 2008 to November 2011,my department to check clinical data.Results:ADC value comparison of patients with prostate cancer District,with the non-cancerous area was a significant difference(P<0.05).The MRS of patients with prostate cancer,its Cit peak was lower performance(Cre+Cho) peak was significantly higher performance(Cre+Cho),and Cit ratio of the mean (1.83±1.54);patients with prostate cancer District MRS,cit peak was increased performance,reduce its(Cre+Cho) peak(Cre+Cho),and cit ratio of the mean (0.82 soil 0.39),showed a significant difference(P<0.05).Patients with conventional MRI has a sensitivity of 71.9%,its combination of MRI and MRS sensitivity was 87.5%,and their joint application of DWI and MRS and MRI sensitivity was 93.7%.Conclusion:DWI(diffusion weighted MR imaging) and MRS (Magnetic Resonance MR imaging) for diagnosis of prostate cancer patients,compared with a separate application can significantly improve the detection sensitivity,reasonable promotion should be based on the condition of the patients.

Abstract:PURPOSE:To evaluate the feasibility of using mpMRI image features predicted by AI algorithms in the prediction of clinically significant prostate cancer (csPCa). MATERIALS AND METHODS:This study analyzed patients who underwent prostate mpMRI and radical prostatectomy (RP) at the Affiliated Hospital of Jiaxing University between November 2017 and December 2022. The clinical data collected included age, serum prostate-specific antigen (PSA), and biopsy pathology. The reference standard was the prostatectomy pathology, and a Gleason Score (GS) of 3 + 3 = 6 was considered non-clinically significant prostate cancer (non-csPCa), while a GS ≥ 3 + 4 was considered csPCa. A pre-trained AI algorithm was used to extract the lesion on mpMRI, and the image features of the lesion and the prostate gland were analyzed. Two logistic regression models were developed to predict csPCa: an MR model and a combined model. The MR model used age, PSA, PSA density (PSAD), and the AI-predicted MR image features as predictor variables. The combined model used biopsy pathology and the aforementioned variables as predictor variables. The model's effectiveness was evaluated by comparing it to biopsy pathology using the area under the curve (AUC) of receiver operation characteristic (ROC) analysis. RESULTS:A total of 315 eligible patients were enrolled with an average age of 70.8 ± 5.9. Based on RP pathology, 18 had non-csPCa, and 297 had csPCa. PSA, PSAD, biopsy pathology, and ADC value of the prostate outside the lesion (ADCprostate) varied significantly across different ISUP grade groups of RP pathology (P < 0.001). Other clinical variables and image features did not vary significantly across different ISUP grade groups (P > 0.05). The MR model included PSAD, the ratio of ADC value between the lesion and the prostate outside the lesion (ADClesion/prostate), the signal intensity ratio of DWI between the lesion and the prostate outside the lesion (DWIlesion/prostate), and the ratio of DWIlesion/prostate to ADClesion/prostate. The combined model included biopsy pathology, ADClesion/prostate, mean signal intensity of the lesion on DWI (DWIlesion), DWI signal intensity of the prostate outside the lesion (DWIprostate), and signal intensity ratio of DWI between the lesion and the prostate outside the lesion (DWIlesion/prostate). The AUC of the MR model (0.830, 95% CI 0.743, 0.916) was not significantly different from that of biopsy pathology (0.820, 95% CI 0.728, 0.912, P = 0.884). The AUC of the combined model (0.915, 95% CI 0.849, 0.980) was higher than that of the biopsy pathology (P = 0.042) and MR model (P = 0.031). CONCLUSION:The aggressiveness of prostate cancer can be effectively predicted using AI-extracted image features from mpMRI images, similar to biopsy pathology. The prediction accuracy was improved by combining the AI-extracted mpMRI image features with biopsy pathology, surpassing the performance of biopsy pathology alone.

Abstract:Objective:To evaluate the efficacy of magnetic resonance imaging/ultrasound (MRI/US) imaging fusion guided transperineal prostate biopsy for the diagnosis of prostate cancer.Method:From September 2014 to March 2016,121 cases were hospitalized for MRI/US imaging fusion guided transperineal prostate biopsy and their data were analyzed retrospectively.Each case underwent systematic 12-core biopsy (SB) add 2-core targeting biopsy (TB) per region of interest (ROI).Pathological findings of each biopsy core was analyzed for Gleason grade and the single-core length of prostate cancer.We recorded the clinical,imaging and pathological data of all cases.We analyzed various data by t test,rank sum test and chi-square test.Result:TB had an obvious higher single-core positive rate for prostate cancer (20.0 ％) and an obvious higher single-core positive rate for high-risk prostate cancer (10.3 ％) than those of SB (12.7 ％ and 5.5 ％),with the statistically significant difference (P =0.001 and P=0.002).TB had a higher single-core length of prostate cancer than that of SB,with the statistically significant difference (P=0.046).Positive cores of TB had a higher primary Gleason grade,secondary Gleason grade and Gleason score compared with positive cores of SB,without statistically significant difference (P＞0.05).Conclusion:When taking MRI/US imaging fusion guided transperineal prostate biopsy,TB can detect high-risk prostate cancer more effectively than SB can.MRI/US imaging fusion guided transperineal prostate biopsy is worth promoting if it is permitted.

Abstract:Objectives: To compare prostate-specific membrane antigen (PSMA) PET with multiparametric MRI (mpMRI) in the diagnosis of pretreatment prostate cancer (PCa). Methods: Pubmed, Embase, Medline, Web of Science, and Cochrane Library were searched for eligible studies published before June 22, 2022. We assessed risk of bias and applicability by using QUADAS-2 tool. Data synthesis was performed with Stata 17.0 software, using the "midas" and "meqrlogit" packages. Results: We included 29 articles focusing on primary cancer detection, 18 articles about primary staging, and two articles containing them both. For PSMA PET versus mpMRI in primary PCa detection, sensitivities and specificities in the per-patient analysis were 0.90 and 0.84 (p<0.0001), and 0.66 and 0.60 (p <0.0001), and in the per-lesion analysis they were 0.79 and 0.78 (p <0.0001), and 0.84 and 0.82 (p <0.0001). For the per-patient analysis of PSMA PET versus mpMRI in primary staging, sensitivities and specificities in extracapsular extension detection were 0.59 and 0.66 (p =0.005), and 0.79 and 0.76 (p =0.0074), and in seminal vesicle infiltration (SVI) detection they were 0.51 and 0.60 (p =0.0008), and 0.93 and 0.96 (p =0.0092). For PSMA PET versus mpMRI in lymph node metastasis (LNM) detection, sensitivities and specificities in the per-patient analysis were 0.68 and 0.46 (p <0.0001), and 0.91 and 0.90 (p =0.81), and in the per-lesion analysis they were 0.67 and 0.36 (p <0.0001), and 0.99 and 0.99 (p =0.18). Conclusion: PSMA PET has higher diagnostic value than mpMRI in the detection of primary PCa. Regarding the primary staging, mpMRI has potential advantages in SVI detection, while PSMA PET has relative advantages in LNM detection. Clinical relevance statement: The integration of prostate-specific membrane antigen (PSMA) PET into the diagnostic pathway may be helpful for improving the accuracy of prostate cancer detection. However, further studies are needed to address the cost implications and evaluate its utility in specific patient populations or clinical scenarios. Moreover, we recommend the combination of PSMA PET and mpMRI for cancer staging. Key points: • Prostate-specific membrane antigen PET has higher sensitivity and specificity for primary tumor detection in prostate cancer compared to multiparametric MRI. • Prostate-specific membrane antigen PET also has significantly better sensitivity and specificity for lymph node metastases of prostate cancer compared to multiparametric MRI. • Multiparametric MRI has better accuracy for extracapsular extension and seminal vesicle infiltration compared to ate-specific membrane antigen PET.

The integrated models combining the PTR radiomics signature with clinical characteristics performed better than corresponding radiomics signatures in the internal validation set (eg. AUC: 0.718 vs. 0.671, <i>P</i> < 0.05 on mpMRI) but performed similar in the external validation set (eg. Abstract:Background Data on the utilization and effects of prebiopsy prostate multiparametric magnetic resonance imaging (mpMRI) to support its routine use in real‐world setting are still scarce. Objective To evaluate the change of clinical practice of prebiopsy mpMRI over time, and assess its diagnostic accuracy. Design, Setting, and Participants We retrospectively analyzed data from 6168 patients who underwent primary prostate biopsy (PBx) between January 2011 and December 2021 and had prostate‐specific antigen (PSA) values ranging from 3 to 100 ng/mL.
