Abstract:Objective To investigate the clinical characteristics and treatment of loose bodies in the ulnocarpal joint. Methods Six patients(7 wrists,5 on the left side and 2 on the right side) who were diagnosed with loose bodies in the ulnocarpal joint were treated from January 2006 to January 2011.The main complaint was chronic ulnar-sided wrist pain,with limitation of wrist extension,ulnar deviation and rotation.Radiographic examinations revealed single or multiple round or oval calcifications at the radial-palmar side of the ulnar styloid,with synovium proliferation.All the patients were treated with removal of loose bodies and synovectomy by arthrotomy.Ulna of one patient who was also diagnosed with ulnocarpal impaction syndrome was shortened simultaneously.Postoperatively the wrists were immobilized by plaster at the supination and ulnar deviation position for four weeks.The pathologic findings showed chondral metaplasia in the involved synovium.Results All patients were follow-up with a mean follow-up time of 25 months.The pain was relieved in all cases without any surgical complications.The range of motion of the wrist returned to normal.According to Cooney's modified wrist scoring system,excellent results were recorded in six wrists and good in one. Conclusion Formation of loose bodies in the ulnocarpal joints was the result of synovial chondrometaplasia,and calcification and ossification of the proliferated chondrocytes.The symptom of ulnar-sided wrist pain was mainly caused by the impingement of loose bodies between the ulnar styloid and triquetrum during wrist extension and ulnar deviation,and also by invasion of the lesional synovium to surrounding tissues.Removal of loose bodies and synovectomy by arthrotomy was the right choice of treatment to achieve satisfactory effect.
Floating Wrist: A Rare Case Report and Review of Anatomical, Diagnostic, and Therapeutic Aspects

Abstract:Introduction: Joint injuries in the upper limb are uncommon and have received limited attention in the literature. This case report presents the first described case in the literature of a "floating wrist" and aims to highlight the diagnostic, anatomical, and therapeutic aspects of this injury. Case report: A 27-year-old male was involved in a severe car accident, resulting in a closed deformity of the right wrist. The patient suffered fractures of the forearm bones and dislocation fracture of all five wrist rays. Surgical intervention was performed to repair the fractures and stabilize the wrist. Despite post-operative complications, the patient demonstrated positive functional outcomes after a 12-month follow-up. Discussion: The term "floating wrist" refers to a rare condition characterized by fractures and/or dislocations of all five wrist rays, accompanied by fractures of the forearm bones. Diagnosis can be complicated due to polytrauma and wrist edema. Early surgical management and rehabilitation are essential for achieving favorable outcomes. Further research is necessary to improve our understanding of this uncommon injury. Conclusion: This case report highlights a unique instance of a "floating wrist" and emphasizes the significance of early diagnosis, appropriate surgical intervention, and prompt rehabilitation in a comprehensive management of these complex injuries. Enhanced comprehension of this rare condition is crucial for improving patient outcomes in similar traumatic scenarios.
The Effect Analysis of Internal Fixation with Absorbable Screw for Unstable Fracture of Scaphoid Bone of Wrist(17 Cases Report)

Abstract:Objective To evaluate the effect of internal fixation of absorbable screw for treating unstable fracture of scaphoid bone of wrist.Methods 17 patients with unstable fracture of scaphoid bone of wrist were performed open reduction and internal fixation with absorbable screw.All the fractures were displaced without deossification.Results All 17 cases were followed up for 8 to 12 months(average 10 months).All fractures were healed within 6.4 months averagely,and the mean extension-flexion arc of the injured wrists was between 106°and 128°.The results revealed 11 of excellence,5 of good,and 1 of false.Conclusion The internal fixation of absorbable screw is an effective therapy for treating unstable scaphoid fracture of wrist with advantage as reliable fixation,slight injury to wrist and avoidance of second operation to remove the internal fixator.

Abstract:Introduction: dislocations of carpal bones without associated fractures are considered a rare injury, the most common mechanism of injury being axial loading with wrist in extension plus ulnar deviation. The literature reports a wide variety of complex carpal injuries, even so, it is possible to identify previously undescribed injuries. Objective: to present an atypical case of a patient with an injury to the midcarpal stabilizing mechanism and the stabilizing mechanism of the proximal row of the wrist following trauma to the hand that required carpectomy as definitive surgical treatment. Presentation of case: a 48 year old male patient is presented who is admitted to our hospital unit after presenting injury to the left hand after being run over by a motor vehicle, with axial load mechanism, presenting deformity in the left thoracic extremity, fracture of the proximal metaphysis of the second phalanx of the third finger as well as dislocation of the proximal interphalangeal joint, with traumatic amputation of the second phalanx of the fourth finger plus extensor injury in zone V of the fifth finger with loss of skin coverage of the fourth and fifth finger, attending our hospital unit 24 hours after the injury. Conclusions: carpal bone dislocations are an orthopedic emergency, with 20% going unnoticed in trauma centers. Early closed reduction is the initial treatment to avoid severe complications, however, surgical treatment is the gold standard for fixation. Carpectomy is considered a mostly adequate sequelae management treatment, however it is well accepted for complex injuries to the wrist stabilization mechanisms, as it can be performed in a short surgical time and early rehabilitation can be initiated and functional ranges of motion can be achieved with low sequelae.

Abstract:We present an unusual case involving the simultaneous dislocation of the trans-scapho-retro-lunate in the right wrist and a posterolateral dislocation in the right elbow joint with a distal radius fracture in a 23-year-old male with no notable medical history. These injuries occurred when he fell from a height of around 12 meters onto his outstretched right hand. The patient presented to the hospital in a normal upper limb trauma position with no discomfort to critical functions. Predominant symptoms at the emergency department were discomfort in the right wrist, hand, and ipsilateral elbow, as well as the entire upper limb functional impairment. Both the wrist and the elbow seemed distorted on examination, with considerable edema and loss of bone landmarks. Passive mobilization was hampered by pain, but peripheral pulses were detectable. The radial, ulnar, and median nerves' autonomous sensory-motor domains were intact, with a cutaneous opening classified as stage 2 by the Cauchoix-Duparc classification. The elbow dislocation was successfully treated using a closed reduction method. External manipulation was employed to reduce the trans-scaphoid perilunate dislocation, which was subsequently stabilized through percutaneous screw fixation of the scaphoid using a triquetrum-lunate pin. Additionally, a styloid pin was utilized to address and manage a distal radius fracture, followed by the implementation of a radiometacarpal external fixator. After one year and three months, the patient reported no pain in his elbow and minimal wrist discomfort during heavy lifting.

Abstract:Case: This case report details the rare presentation of a scaphoid waist fracture in a 27-year-old right-hand-dominant man, with an atypical displacement of the proximal fracture fragment into the flexor compartment of the forearm. Urgent open reduction and internal fixation using a Herbert screw were performed, resulting in fracture union and favorable wrist function at the 1-year follow-up. Abstract:BACKGROUND:Isolated palmar dislocation of distal radioulnar joint is a rare injury. It can easily lead to misdiagnosis. Previous literature reports were all rotation violence. We reported a patient with direct impact violence.CASE PRESENTATION:We report a 31-year-old male laborer presented to our hospital with an acute trauma. Severe tenderness and limited mobility were seen in his right wrist. He received an x-ray film examination and diagnosed as the isolated palmar dislocation of distal radioulnar joint. The treatment was closed reduction and splint fixation. After half a year, the patient gained a functional recovery of his previously afflicted wrist.CONCLUSIONS:To the best of our knowledge, this is the first case of isolated palmar dislocation of distal radioulnar joint caused by a direct impact violence. Patients and physicians should be aware of the properties of this mechanism of injury so that early diagnosis and treatment can be achieved.

Abstract:Wrist pain is a common presentation in primary care clinics. Chronic pain after trauma with non-acute radiographs requires careful physical examination and a case-specific workup. We present a case of a 32-year-old female evaluated at the primary care clinic with two months of left wrist pain after a hypersupination injury that was found to be secondary to avascular necrosis of the lunate on the left wrist with no radiographic signs of fracture or focal sclerosis on plain films. This case demonstrates the importance of identifying less common chronic wrist pain etiologies.

Abstract:Carpal instability is a set of complex and important wrist pathology, which is usually secondary to different injury severities and patterns to the wrist joint or pathological changes. Scapholunate instability is the most common type of carpal instability, and other prevalent patterns include lunotriquetral instability, midcarpal instability, radiocarpal instability and perilunate injuries. The diagnosis and treatment of carpal instability is relatively difficult, which has drawn much attention in the field of wrist surgery. With deeper understanding of pathological mechanism of wrist instability, there reported new surgical procedures with better suitability to anatomical character and kinematics to repair or reconstruct ligaments. Also, with the rapid development of wrist arthroscopy techniques, many traditional open procedures can now be accomplished in a minimally invasive approach, under assistance of wrist arthroscopy. The authors review the progress in diagnosis and treatment of carpal instability in recent years and discuss the existed confusion so as to provide a reference for clinical work.
Acute Hand Injury in Surfing: A Case Report

Abstract:Hand injuries from surfing tend to be severe and require medical attention. Follow-up of a surfing injury is difficult because many patients are visiting and go home after treatment. We report a case of a surfing hand injury sustained abroad, which was treated upon the patient's return, allowing for follow-up. The mechanism of injury was traction and torsion from the surfboard leash while surfing. The patient was initially treated for nailbed injury but presented later back home after persistent pain, for which an unstable distal phalanx fracture in their right ring finger was found by x-ray. This was surgically reduced with K-wire insertion and nailbed repair. Postoperatively, the injured finger was kept in a splint, and the patient had physiotherapy. Pain was significantly reduced, and the patient regained sufficient function. Considering a fracture as a differential for finger injury caused by the surfboard leash may prevent management delays. Injury may be prevented through education and redesign of the surfboard leash.

Abstract:Purpose: To study the effects of surgical and nonoperative treatment on wrist function in patients with distal radius fracture. Methods: In total, 97 patients treated for distal radius fracture in the Department of Orthopedic Trauma at the People's Hospital of Peking University from Jan. 2010 to Jun. 2016 were selected for outpatient follow-up, including manipulative reduction and dorsal splint fixation in 24 cases, bivalve cast fixation in 19 cases and open reduction and internal fixation in 54 cases. Evaluation was based on Sartiento's modification of the Gartland and Werley score. Efficacy was assessed with wrist pain as the focus. Results: The wrist function scores of the surgical group were better than nonoperative groups. There was no significant difference in wrist function scores between the dorsal splint group and the bivalve cast group. The ulnar wrist pain incidence had no significant difference in surgical and nonoperative groups. The displace rate in dorsal splint group was higher than other groups. Conclusion: The overall effect of surgical treatment of distal radius fracture is better than nonoperative treatment. The ulnar wrist pain incidence has no significant difference in these groups. Dorsal splint fixation is more prone to displace than bivalve cast fixation.

Abstract:Introduction: Perilunate dislocation of the carpus is an unusual injury, requiring high-energy trauma. The simultaneous bilateral form is so rare that is worthy of report, with only a few cases mentioned in the literature. We present the case of a patient with an open volar dislocation of the right wrist associated with a closed dorsal dislocation of the left wrist and distal radioulnar subluxation. Case report: A 30-year-old, right hand dominant male, suffered injuries to both wrists after being involved in a high-speed car accident. Examination of the patient revealed diffuse swelling and tenderness to both wrists with a lacerated wound over the dorsum of right wrist. Anteroposterior and lateral X-rays of both wrists were obtained, which showed bilateral perilunate pure dislocations: volar in the right side and dorsal the left side combined with a dorsal subluxation of the distal radioulnar joint. Discussion: Perilunate dislocations are relatively rare injuries involving approximately only 7% of all injuries of the carpus. It is often overlooked in primary evaluations, approximately 25% of perilunate injuries were missed. A better comprehension of the injury's mechanism and its full extent has led to better management strategies. It is recommended in the literature, that the best treatment of these injuries is open reduction, restoration of carpal bones relationship, repair of the ligaments and a proper rehabilitation and physiotherapy program.
Arthroscopic Management for Juvenile Elbow Loose Bodies: A Retrospective Review of Fifteen Cases

Abstract:Objective: Loose bodies in the elbow can lead to elbow structural damage and seriously affect joint function. Arthroscopy has been used in the treatment of elbow loose bodies. Our goal in this study was to assess the efficacy of arthroscopic treatment of elbow loose bodies in adolescents. Methods: We retrospectively studied fifteen cases of juvenile patients (aged 12 to 25 years, mean 17.6 years) who received arthroscopic surgery to remove elbow loose bodies between July 2009 and October 2013. All patients underwent inspection and removal of elbow loose bodies through arthroscopy. Patients received postoperative follow up for seven to thirty months (mean 15.9 months). We compared symptoms, flexion and extension angles and changes in imaging studies before and after surgery, and evaluated the efficacy with the modified HSS elbow scoring system. Results: Elbow joint pain was resolved in fourteen patients after arthroscopy and slight joint pain remained in one patient. Elbow "locking" symptom disappeared in eleven patients postoperatively. The difference of the maximum initiative extension and flexion angles before surgery and after surgery was statistically significant. Follow-up CT scans were performed in all patients and showed no residual loose bodies in elbow joints. In accordance with the modified HSS scoring system, thirteen patients were scored excellent and two patients were scored satisfactory. There were no complications. Conclusion: Under arthroscopy, a complete inspection could be performed and the loose bodies could be removed completely in juvenile patients with elbow loose bodies. This procedure causes minimal disturbance to elbows, and patients can recover quickly from surgery. It is an effective treatment for elbow loose bodies in adolescents.
Diagnosis and treatment of intraosseous ganglion cysts of the wrist joint

Abstract:Objective To investigate the clinical characteristics and treatment methods of intraosseous ganglion cysts of the wrist joint. Methods From July 2008 to July 2012, 18 patients ( 20 wrists ) with intraosseous ganglion cysts in the wrist joint were adopted. There were 5 males and 13 females, whose average age was 33.5 years old. There were 11 cases in the lunate, 6 cases in the scaphoid, 1 case in the triangular bone, 1 case in the large multangular bone and 1 case in the small multangular bone. The main complaint was chronic wrist pain, decreased grip strength and limitation of wrist extension-lfexion movement. The X-ray showed circular and quasi-circular areas of reduced bone mineral density within the carpal bone with a clear boundary and slightly sclerotic margin. The CT and MRI revealed the liquid content. All the patients were treated by curettage of the ganglion cysts and autologous cancellous bone grafting. The wrists were immobilized by plaster at the functional position for 4 weeks after the operation. Results All the patients were followed up for a mean period of 15 months ( range;3-48 months ). The pathologic findings showed the ganglion cysts in 18 wrists. After the operation, 10 patients were pain-free and 7 patients got pain relieved, with no improvement in 3 patients. The grip strength was improved in 14 patients among the 16 patients with decreased grip strength. The range of motion of the wrist joint decreased in 6 patients. Recurrence was found in 1 case. According to the modiifed Mayo wrist scoring system, excellent results were recorded in 10 wrists, good in 8 and fair in 2. Conclusions Intraossous ganglion cysts in the wrist joint remain a cause of chronic wrist pain. The deifnite diagnosis relies on the clinical symptoms, imaging manifestations, intraoperative ifndings and pathologic examinations, and satisfactory operative results can be achieved.

Abstract:Objective To discuss feasibility and clinical result of wrist joint dislocation by movable external fixator and Anchor screw.Methods 16 cases of wrist joint dislocations were treated with movable external fixators,open reduction,and cannulated screws fixation of fracture.Anchor screws were used to repaire intercarpal and radiocarpal ligaments and Kirschner wire fixing fracture temperately.Results All cases were followed up for 3~6 months.16 patients recoveried wrist joint functions basically,and no dislocations and osteonecrosis were found.Based on Cooney′s clinical scoring system,the scores of our series were 7 excellent,7 good and 2 fair.Conclusions Treatment of wrist joint dislocations by movable external fixator and Anchor screw can fix wrist joint effectively,repairing ligaments reliably and allow function movements early.
Treatment of wrist stiffness through posture orthosis and active exercise: A case report

Abstract:Introduction: Wrist fractures, particularly the distal radius, can result in significant stiffness and hand dysfunction if not mobilized early. The variable immobilization period post-fracture depends on fracture type, location, stability, and surgical intervention. Inadequate early mobilization typically leads to structured stiffness, influenced by patient health, injury mechanism, joint surface involvement, associated tissue injuries, and patient motivation. Case presentation: A 64-year-old female in good health suffered a distal radius fracture, treated with open reduction and internal fixation. A modified treatment plan, including custom orthosis and active wrist exercises, was initiated after the standard immobilization phase to enhance the range of motion while accommodating the patient's daily activities. Clinical discussion: The patient underwent 15 evaluations of active range of motion (AROM) using a goniometer, guided by the American Society of Hand Therapists. A Tissue Composition Analysis (TCA) was performed to guide the orthosis-treatment choice. Despite consistent improvement shown in AROM, it was inconclusive whether the modified treatment contributed significantly beyond the standard approach. Conclusions: While the patient's AROM improved, the treatment's effect on this single case cannot definitively confirm the efficacy of the modified approach. A more extensive study is necessary to evaluate the conservative treatment strategy's validity for such fractures in high-demand patients, considering the biomechanical complexity of the injury and the patient's professional needs.
Treating distal radius fracture with radiocarpal subluxation using external fixator

Abstract:Objective To evaluate the efficiency of treating distal radius fracture with radiocarpal subluxation using external fixator.Methods Combined with percutaneous pin fixation,external fixator was used in treating 26 patients with distal radius fracture with radiocarpal subluxation.Wrist movement range and radiographic index were measured and wrist function was evaluated by the Gartland-Werley.Results There was no significant difference in movement range between the treated wrist joint and the normal hand 3 and 12 months after the surgery(P>0.05),and no significant difference in the radial height and radial inclination(P>0.05).Nevertheless,the recovery of the palm tilt was limited(51%and 49% respectively),significantly smaller than the normal(P<0.05).By the Gartland-Werley criteria,the three-month recovery and the twelve-month recovery reached an excellent rate of 80.8% and 84.6% respectively.Conclusion Treating distal radius fracture with radiocarpal subluxation using the external fixator is effective.
Isolated complete dislocation of hamate bone with an ulnar nerve injury: a case report and literature review

Abstract:Background: Isolated complete dislocation of the hamate bone is a rare wrist injury. Only 22 cases have been reported in the literature. Four of them were associated with nerve injury, but it is a pity that the causes of ulnar nerve injury had never been deeply analyzed. In the case report, it was stated that the ulnar nerve injury was caused by both displacement of uncinate bone and change of position of transverse carpal ligament. The complete dislocation of the hamate bone is mostly caused by direct force, often accompanied by tears of the surrounding ligaments.Case Description: A 30-year-old woman was treated because her left hand was crushed by a machine roller. Plain film X-ray showed isolated dislocation of the volar hamate and rotation of the hamate bone to the volar side. Clinical manifestations and computed tomography (CT) signs suggest ulnar nerve injury. The patient was quickly transported to the operating room for open reduction and internal fixation of the hamate bone and repair of the dorsal carpal ligament. The patient fully recovered after 12 months.Conclusions: Isolated complete dislocation of the hamate bone is a rare injury. The most common cause is direct compression force to the wrist. Diagnosis can be made through review of the patient's medical history, physical examination and X-ray, however, CT can accurately evaluate the displacement and rotation of the hamate bone, hamate bone fracture and other concomitant wrist fractures, suggesting compression changes of the median nerve and ulnar nerve to provide a reliable basis for the formulation of a clinical care plan. The case report added the evidence-based practice, and discussed the anatomical mechanism and imaging manifestation of ulnar nerve injury associated with complete dislocation of hamate bone.
