Abstract:Patients with MSI colorectal tumor have good prognosis and cannot benefit from 5-fluorouracil (5-Fu)-based chemotherapy reported by previous studies. While, single nucleotide polymorphisms (SNP) of ERCC1 and XRCC1 have be proved to influence clinical outcome of colorectal cancer patients treated with oxaliplatin-based chemotherapy. We aim to study the correlation between molecular status and clinical-pathological features, and their effect on CRC patients' clinical outcome treated with mFOLFOX6 adjuvant chemotherapy. In this study, MSI status was determined in tumors and paired normal tissues from 101 colorectal cancer (CRC) patients. We also examined SNP of ERCC1 (c. C354T) and XRCC1 (c. G1196A) in 83 and 85 patients' blood. MSI was found to be significantly correlated with well/moderate histopathological differentiation (p = 0.038) and CRC family history (p = 0.003). CEA and CA19-9 levels was positive correlated significantly by Spearman correlation analysis (Pearson's r = 0.823, p < 0.0001). No significant correlation was found between MSI status and ERCC1/XRCC1 polymorphisms. We found greater drop of CEA level in mFOLFOX6 sensitive group than low sensitive group after mFOLFOX6 adjuvant chemotherapy according to MSI status, ERCC1 and XRCC1 SNP. MSI was significantly correlated with well/moderate histopathological differentiation (p = 0.038) and CRC family history (p = 0.003). The MSI status, ERCC1 and XRCC1 polymorphisms may influence the clinical outcome of colorectal cancer patients treated with mFOLFOX6 adjuvant chemotherapy.

Abstract:What's new? Current guidelines recommend performing mismatch repair (MMR) protein or microsatellite instability (MSI) detection to stratify colorectal cancer patients for diagnosis and treatment. However, the MMR protein immunohistochemistry (IHC) and MSI‐PCR methods have been widely reported to yield inconsistent results. Here, the authors developed an MSI predictive nomogram for identifying colorectal cancer patients with mismatch repair deficiency by IHC but microsatellite stability by MSI‐PCR (dMMR&MSS). They found that dMMR&MSS patients had significantly worse overall survival and immunotherapy efficacy than dMMR&MSI‐H patients. The study highlights the importance of identifying dMMR&MSS colorectal cancer patients and provides an affordable screening strategy. The inconsistency between mismatch repair (MMR) protein immunohistochemistry (IHC) and microsatellite instability PCR (MSI‐PCR) methods has been widely reported. We aim to investigate the prognosis and the effect of immunotherapy in dMMR by IHC but MSS by MSI‐PCR (dMMR&MSS) colorectal cancer (CRC) patients. A microsatellite instability (MSI) predicting model was established to help find dMMR&MSS patients. MMR and MSI states were detected by the IHC and MSI‐PCR in 1622 CRC patients (ZS6Y‐1 cohort). Logistic regression analysis was used to screen clinical features to construct an MSI‐predicting nomogram. We propose a new nomogram‐based assay to find patients with dMMR&MSS, in which the MSI‐PCR assay only detects dMMR patients with MSS predictive results. We applied the new strategy to a random cohort of 248 CRC patients (ZS6Y‐2 cohort). The consistency of MMR IHC and MSI‐PCR in the ZS6Y‐1 cohort was 95.7% (1553/1622). Abstract:Adjuvant chemotherapy (ACT) is usually used to reduce the risk of disease relapse and improve survival for stage II/III colorectal cancer (CRC). However, only a subset of patients could benefit from ACT. Thus, there is an urgent need to identify improved biomarkers to predict survival and stratify patients to refine the selection of ACT. We used high-throughput proteomics to analyze tumor and adjacent normal tissues of stage II/III CRC patients with /without relapse to identify potential markers for predicting prognosis and benefit from ACT. The machine learning approach was applied to identify relapse-specific markers. Then the artificial intelligence (AI)-assisted multiplex IHC was performed to validate the prognostic value of the relapse-specific markers and construct a proteomic-derived classifier for stage II/III CRC using 3 markers, including FHL3, GGA1, TGFBI. The proteomics profiling-derived signature for stage II/III CRC (PS) not only shows good accuracy to classify patients into high and low risk of relapse and mortality in all three cohorts, but also works independently of clinicopathologic features. ACT was associated with improved disease-free survival (DFS) and overall survival (OS) in stage II (pN0) patients with high PS and pN2 patients with high PS. This study demonstrated the clinical significance of proteomic features, which serve as a valuable source for potential biomarkers. The PS classifier provides prognostic value for identifying patients at high risk of relapse and mortality and optimizes individualized treatment strategy by detecting patients who may benefit from ACT for survival.
Development of a Nomogram for Predicting Survival in Microsatellite Stable Patients with Resected Colorectal Cancer

Abstract:Background: The benefits of adjuvant chemotherapy remain controversial in microsatellite stable (MSS) stage II resected colorectal cancer (CRC) patients. In this study, we constructed an overall survival (OS) prediction model for this subgroup, integrating a combination of molecular and clinical predictors. Methods: Variables with p-values < 0.05 were entered into multivariate analyses using Cox stepwise regression model to select independent predictors as input for OS prediction nomogram. A final model was selected using a backward step-down process, which used Akaike Information Criterion as a stopping rule. The probability of 3-year and 5-year survival can be obtained by summing up the total score and locating it on the total point scale. Results: We performed targeted sequencing on surgically resected tumor tissue of 122 stage II MSS patients, defined as having less than 15% length-instable loci, using a panel which allows for simultaneous detection of MSI status and mutation in 41 CRC-related genes. Among them, 23, 30 and 69 patients were diagnosed with proximal, distal and rectal cancer, respectively. To predict the probability of 3-year and 5-year survival, we constructed a nomogram incorporating the significant prognostic factors, including APC, ATM, BRAF, PTEN, TP53 (LOF), mutation count (high: >3, low: < =3), age, CEA, and the location of the tumor. The actual and predicted survivals were in an excellent agreement, reflected by a C-index of 0.887 (95% CI: 0.816-0.957). Furthermore, Kaplan-Meier curves for survival outcomes showed significant distinction (p < 0.001) after stratifying our cohort into low, median and high risk groups according to total score obtained from our nomogram. Our data also demonstrated high risk patients who received adjuvant chemotherapy are associated with a better OS (p = 0.004). Low and median risk patients did not benefit from adjuvant chemotherapy, reflected by comparable OS. Conclusions: We developed a nomogram model for predicting survival of MSS patients with stage II resected CRC. It can potentially serve as complementary method for clinicians to identify subgroups necessitating adjuvant therapy. Legal entity responsible for the study: Fudan University Shanghai Cancer Center. Abstract:Background: The role of microsatellite instability (MSI) and prognosis for stage II–III colorectal cancer (CRC) has been described, but the role of MSI in stage I and IV CRC is controversial. Methods: A total of 2,540 CRC patients were collected from Huzhou Central Hospital, China, from January 2006 to 2016, and 783 cases were excluded. This retrospective study illustrates the correlation between MMR status and prognosis for 1,757 CRC patients as well as the correlation between MSI and prognosis for CRC patients. Two groups were classified as MSI-H and MSI-L&MSS. If the expression of one or more mismatch repair (MMR) proteins was negative, it was considered as microsatellite instability high expression (MSI-H), whereas positive expression was considered as microsatellite instability low expression and microsatellite stability (MSI-L&MSS), as assessed by correlation analyses. Overall and disease-free survival were analyzed using the Kaplan–Meier method. Univariable and multivariable analyses were conducted using Cox regression. Results: Preoperative serum S-CEA, positive lymph, tumor size, pathologic tumor (Pt) status, node (N) stage, differentiation, chemotherapy, and the 8th Edition of the American Joint Committee on Cancer (AJCC-8) were significantly correlated with MSI (P=0.028, 0.037, 0.019, 0.007, 0.002, <0.001, <0.001, and <0.001, respectively), whereas tumor location was not associated with MSI. Univariable and multivariable analyses showed that MSI was an independent factor for CRC. The 5-year overall survival (OS) and 5-year disease-free survival (DFS, P 0.05). Conclusion: MSI-H was associated with a good prognosis for stages II to IV, whereas stage I did not show any significant correlation. Moreover, MSI expression was an independent prognostic factor.

Abstract:9544 Background: A number of studies have investigated the relationship between MSI and CRC OS. However, results have been inconsistent. This may have been influenced by design issues; specifically, underpowered datasets, retrospective analyses, potential patient selection, and non-blinded investigators. We investigated MSI as a marker of OS in the context of a multi-center randomized phase III trial of 10,000 CRC patients randomized to receive adjuvant portal fluorouracil infusion. Results were pooled with published work from adjuvant clinical trials. Methods: A random subset of 1,000 tissue blocks were centrally collected and anonymized. Tissue was microdissected and MSI status determined by genotyping BAT26 prospective to patient follow-up. Laboratory analyses were performed blinded to all clinical data. OS data was pooled with that from studies investigating MSI status in adjuvant clinical trials by standard meta-analysis techniques. Results: Efficacy data from the clinical trial has yet to report. 543 samples have been genotyped to date. 15% of tumors are MSI+. Median follow-up was 55 months. CRCs with MSI tended to have a better OS (HR=0.77, 95% CI:0.48–1.24). This was more pronounced for colonic tumors (HR=0.60, 95% CI:0.30–1.19; rectal tumors HR=1.12, 95% CI:0.58–2.13). Pooling our results with OS data from 3 adjuvant trials (n=1,684, MSI= 326) confirmed a significant survival advantage for MSI+ tumors (HR=0.76, 95% CI:0.61–0.94, phet=0.84, I2=0%). Conclusions: MSI is a marker for improved OS in stage II/III CRC. Additional prospective studies are required to define the role of adjuvant chemotherapy in these tumors. Abstract:Colorectal cancers (CRCs) exhibiting microsatellite instability (MSI) have special biological behavior. The clinical predictors for MSI and its survival relevance for the Chinese population were still unclear. Seven hundred ninety-five CRC patients were retrospectively assessed. Mismatch repair (MMR) proteins (MSH2, MSH6, PMS1, and MLH1) expression was detected by immunohistochemistry using tumor tissues of all patients. DNA MSI status was analyzed by polymerase chain reaction in 182 samples randomly selected from the 795 cases. Among all CRC tumor tissues, 97 cases (12.2%) were with an MMR protein-deficient (MMR-D) phenotype, whereas 698 cases (87.8%) were with an MMR proteins intact (MMR-I) phenotype. A total of 21 (11.5%) CRCs were identified as having high microsatellite instability, 156 (85.7%) tumors were having microsatellite stability (MSS), and five (2.7%) were having low microsatellite instability. Importantly, MMR status was demonstrated to be moderately consistent with MSI status (κ=0.845, 95% confidence interval [CI] 0.721, 0.969). Unconditional logistic regression analysis revealed age, number of lymph node, tumor diameter, and tumor site as predictors for MSI with a substantial ability to discriminate different MSI status by area under curve of 80.62% using receiver operation curve. Compared with MMR-I, MMR-D was an independent prognostic factor for longer overall survival (hazard ratio =0.340, 95% CI 0.126, 0.919; P=0.034). MMR-D is an independent prognostic factor for better outcome. Our results may provide evidence for individualized diagnosis and treatment of CRC, but this will require further validation in larger sample studies.

Abstract:Background and purpose:Microsatellite instability(MSI) status is commonly applied to predict the prognosis and chemosensitivity in stage Ⅱ and stage Ⅲ colorectal cancer patients.However,researches of its function on metastasis colorectal cancer are limited.This study investigated its value on prognosis and chemosensitivity in metastatic colorectal cancer(CRC) patients.Methods:We retrospectively investigated tumor tissues from metastasis CRC patients who were treated with oxaliplatin and 5-FU-based therapy regimens(FOLFOX and XELOX).Immunostaining of proteins of the mismatch repair gene h MLH1,h MSH2,h MSH6 and h PMS2 was performed.Prognostic value and chemosensitivity in patients with MSI status were also determined.Results:Clinical features from 113 patients were analyzed.No correlation of overall survival(OS) and chemosensitivity with MSI status was found.We further investigated 79 patients with synchronous metastasis and palliatively tumor resection.Median progression free survival(PFS) from 22 MSI patients was significant longer than that in 57 MSS patients(19.9 months vs 7 months,P=0.005).No significant difference was seen in OS comparison(P=0.07).MSI status was also an independent prognostic factors of PFS by Cox multivariate analysis(MSS/MSI,HR=2.079,P=0.043).Moreover,in this group,MSI patients had improved disease control rate(59.1% vs 31.6%,P=0.025) in chemosensitivity analysis than MSS patients.Conclusion:A better PFS in MSI patients with synchronous metastasis and palliative tumor resection was found after treated with oxaliplatin and 5-FU-based therapy and a better chemosensitivity in MSI patients was also found.

The immune cells in the tumor microenvironment were analyzed by the ESTIMATION algorithm. The microsatellite-related gene signature (MSRS) was constructed by the least absolute shrinkage and selection operator (LASSO) Cox regression based on the differentially expressed genes (DEGs) and its prognostic value and predictive value of response to immunotherapy were assessed. The prognostic value of the MSRS was also validated in another cohort. ResultsThe MSI-H cancers cells were clustered differentially in the dimension reduction plot. Most of the immune cells have a higher proportion in the tumor immune microenvironment, except for CD56 bright natural killer cells. A total of 238 DEGs were identified. Based on the 238 DEGs, a neural network was constructed with a Kappa coefficient of 0.706 in the testing cohort. The MSRS is a favorable prognostic factor of overall survival, which was also validated in another cohort (GSE39582). Besides, MSRS is correlated with tumor mutation burden in MSI-H colon cancer. However, the MSRS is a barely satisfactory factor in predicting immunotherapy with the area under the curve (AUC) of 0.624. ConclusionWe developed the MSRS, which is a robust prognostic factor of overall survival in spite of a barely satisfactory immunotherapy predictor. Further studies may need to improve the predictive ability.
Gene mutation profiling in microsatellite instability colorectal cancer and its association with the efficacy of immunotherapy: A retrospective study

Abstract:Background Microsatellite instability‐high (MSI‐H) colorectal cancer (CRC) is known for its heightened responsiveness to immunotherapy. However, establishing robust predictive markers for immunotherapy efficacy remains imperative. This retrospective study aimed to elucidate the genetic landscape of MSI‐H CRC and correlate these genetic alterations with immunotherapy outcomes in a cohort of 121 patients. Methods We analyzed clinical and molecular data from 121 patients with MSI‐H CRC. We conducted a thorough genetic analysis of MSI‐H CRC patients, with a specific emphasis on the APC, TP53, RAS, and MMR genes. We further analyzed the relationship between gene mutations and immunotherapy efficacy. The primary endpoints analyzed were objective response rate (ORR) and progression‐free survival (PFS). All statistical analysis was conducted using SPSS26.0 and R 4.2.0 software. Results Our findings underscored the complexity of the genetic landscape in MSI‐H CRC, shedding light on the intricate interplay of these genes in CRC development. Notably, mutations in MMR genes exhibited a distinctive pattern, providing insights into the underlying mechanisms of MSI‐H. Furthermore, our results revealed correlations between specific genetic alterations and immunotherapy outcomes, with a particular focus on treatment response rates and progression‐free survival. Conclusion This study represents a significant step toward unraveling the genetic nuances of MSI‐H CRC. The distinctive pattern of MMR gene mutations not only adds depth to our understanding of MSI‐H CRC but also hints at potential avenues for targeted therapies. This research sets the stage for future investigations aimed at refining therapeutic strategies and improving outcomes for patients with MSI‐H CRC.
