The association between mtDNA content as well as polymorphisms in peripheral blood leukocytes and risk of bladder cancer has not been reported. METHODS We identified 926 bladder cancer patients and compared these to 926 healthy-controls frequency matched on age, gender, and ethnicity enrolled in an ongoing bladder cancer study at the MD Anderson Cancer Center. Multivariable logistic regression analysis was used to examine the association between mtDNA content in peripheral blood lymphocytes and the risk of bladder cancer. RESULTS Patients diagnosed with bladder cancer had significantly decreased mtDNA content when compared to control subjects (median: 0.98 vs. 1.04, p<0.001). Low mtDNA content (ie, less than the median in control subjects) was associated with a statistically significant increased risk of bladder cancer, when compared with high mtDNA content (Odds Ratio (OR) = 1.37, 95% CI = 1.13 to 1.66, p<0.001). In a trend analysis, a statistically significant dose–response relationship was detected between lower mtDNA content and increasing risk of bladder cancer (P for trend <0.001). When stratified by host characteristics, advanced age (>65 years), male/female sex and positive smoking history were all significantly associated with low mtDNA content and increased risk of bladder cancer. We identified two unique mtDNA polymorphisms significantly associated with risk of bladder cancer: mitot10464c (OR=1.39 95%CI: 1.00-1.93, p=0.048) and mitoa4918g (OR=1.40, 95% CI: 1.00-1.95, p=0.049). Analysis of the joint effect of low mtDNA content and unfavorable mtDNA polymorphisms revealed a 2.5 fold increased risk of bladder cancer (OR=2.50, 95% CI: 1.60-3.94, p<0.001). Significant interaction was observed between mitoa4918g and mtDNA content (p for interaction = 0.028). CONCLUSIONS Low mtDNA content appears to be associated with increased risk of bladder cancer. Abstract:Proceedings: AACR 103rd Annual Meeting 2012‐‐ Mar 31‐Apr 4, 2012; Chicago, IL Mitochondrion plays essential role in energy production and apoptosis process from the initiation to regulation of apoptosis. Many studies have showed the association of dys-regulated mitochondrial DNA (mtDNA) with the development and progression of cancer, aging and longevity, and neurodegenerative diseases. In this study, we will test the hypothesis that variation in mtDNA content (also called mtDNA copy number) may be associated with individual's bladder cancer risk and clinical outcome in a case-control study of 1026 bladder cancer patients and 980 healthy-controls. We will also assess the association of mtDNA content with progression/recurrence in non-muscle invasive bladder cancer (NMIBC) patients and overall survival in muscle invasive and metastatic bladder cancer (MIMBC) patients. Quantitative real-time polymerase chain reaction in peripheral blood lymphocytes was used to measure mtDNA content. We observed significantly lower mtDNA content in bladder cancer patients (mean = 1.05, SD = 0.36) than controls (mean = 1.11, SD = 0.39) with P value < 0.001. When we categorized the mtDNA content using the quartile distribution in controls, Lower mtDNA content (less than 1st quantile) was associated with 1.69-fold increased risk of bladder cancer (95% confidence interval [CI] = 1.17 to 1.68), compared to those who have higher mtDNA content (greater than 4th quartile). In patients with MIMBC, we also observed the significant association of lower mtDNA content with poor survival. After categorizing the mtDNA content using quartile distribution in patients who were alive, there is a significantly increased risk of death for patients with lower mtDNA content (hazard ratio = 1.75, 95% CI = 1.11 to 2.77). There is also a significantly reduced median survival time (44.5 months vs 25.4 months, log-rank P = 0.004). However, there is no significant association of mtDNA content with progression/recurrence in NMIBC patients. In conclusion, low mtDNA content is associated with significantly increased risk of bladder cancer risk and poor survival. Abstract:Abstract Mitochondria play an important role in cellular energy metabolism, free radical generation, and apoptosis. Cumulative evidence suggests that mtDNA copy number may be a biomarker for cancer risk. Using a nested case-control design, we evaluated mtDNA copy number in peripheral blood cells in relation to colorectal cancer risk. Included in the study were 436 colorectal cancer cases and 881 matched controls identified from the Shanghai Woman's Health Study, a population-based, prospective cohort study conducted among Chinese women. Relative mtDNA copy number was determined in triplicate by a quantitative real-time PCR assay using peripheral blood cell DNA samples collected at the time of study enrollment, prior to cancer diagnosis. Baseline levels of mtDNA copy number were lower among women who subsequently developed colorectal cancer (geometric mean = 0.271, 95% CI: 0.263-0.279) than those who remained cancer-free (geometric mean = 0.281, 95% CI: 0.275-0.288) (P=0.0274). Multivariate adjusted odds ratios were 1.15 (95% CI: 0.80-1.65), 1.12 (95% CI: 0.78-1.60) and 1.58 (95% CI: 1.10-2.26) for third to the first quartile of mtDNA copy numbers, respectively, comparing with the highest quartile (P for trend = 0.0135). The association varied little by the interval between blood draw and cancer diagnosis. To our knowledge, this is the first large prospective study evaluating the association of mtDNA copy number with subsequent risk of developing colorectal cancer. Our data suggests that decreased mtDNA copy number may be associated with increased colorectal cancer risk, and mtDNA copy number measured in peripheral blood cells may be a potential biomarker useful for colorectal cancer risk assessment. Abstract:Mitochondrial DNA plays a critical role in the pathophysiology of cancer. However, the associations between mitochondrial DNA copy number (mtDNA-CN) and cancer risk are controversial. Mendelian randomization (MR) analyses were performed using three independent instrumental variables (IVs) to explore potential associations between mtDNA-CN and 20 types of cancer. The three sets of IVs were primarily obtained from participants in the UK Biobank and the Cohorts for Heart and Aging Research in Genomic Epidemiology consortium using different methods. The outcome data of cancers were investigated using summary statistics from the FinnGen cohort. The potential causal associations were evaluated using the MR-Egger regression, weighted median, inverse-variance weighted (IVW), and weighted mode methods. The robustness of IVW estimates was validated using leave-one-out sensitivity analysis. Additionally, a meta-analysis was conducted to pool results from three sets of IVs. The results revealed that genetically predicted mtDNA-CN was not associated with cancer risk (odds ratio = 1.02; 95% confidence interval: 0.95-1.10). Subgroup analyses indicated no causal association between mtDNA-CN and breast, lung, prostate, skin, colorectal, gastric, liver, cervical uteri, esophageal, thyroid, bladder, pancreas, kidney, corpus uteri, ovary, brain, larynx, and anus cancers. It was observed that mtDNA-CN was associated with lip, oral cavity, and testis cancers. However, these results should be interpreted with caution because a small number of patients with lip and oral cavity or testis cancers were included. The comprehensive MR analysis demonstrated that mtDNA-CN is not a suitable biomarker for tumor risk assessment.

Abstract:Abstract Mitochondrion is an essential cellular organelle generating energy, and also plays a key role in the initiation and regulation of apoptosis. Impaired mitochondria function may inhibit apoptosis; enhance reactive oxygen species and generate cancer related proteins. The difference in mitochondrial DNA (mtDNA) content (also termed mtDNA copy number) may contribute to cancer predisposition as well as variations in clinical outcomes. To test this hypothesis, we conducted a large-scale case-control study to investigate the role of mtDNA content in lung cancer etiology and clinical outcomes. MtDNA content in 2,442 newly diagnosed lung cancer patients and 2,417 matched controls were measured by quantitative real-time polymerase chain reaction. Clinical outcome data were collected for non-small cell lung cancer (NSCLC) patients (2,020 subjects). We analyzed the association of MtDNA copy number with the risk of developing lung cancer and also the clinical outcomes in NSCLC patients. MtDNA content was significantly lower in patients than controls (mean=1.17 vs. 1.23, P=4.28×10−4). Compared to higher mtDNA content (above the median value in the controls), lower mtDNA content was associated with a significantly increased risk for lung cancer (OR=1.25, 95%CI=1.11-1.41, P=1.52×10−4). In the trend analysis, we detected a significant dose-response relationship between mtDNA content and risk, as lower mtDNA content was associated with a progressively increased risk for lung cancer (P-trend=1.40×10−4). In the analysis of clinical outcomes, a significant difference in mtDNA content was observed between death and survival groups (mean=1.12 vs. 1.27, P=6.62×10−4) in early stage (stage I and II) NSCLC patients. Compared to those who have higher mtDNA content, patients who have lower mtDNA content had a 45% higher risk for death (HR=1.34, 95%CI=1.05-1.71, P=0.019), and a dramatically shorter survival time (58 months vs. 96 months, log-rank P=5.68×10−4). A dose-response effect of increased risk for death was found with decreased mtDNA content (P-trend=0.048) in this group of patients. No significant results were found in advance stage (stage III and IV) patients. To our knowledge, this is the first study to evaluate the association of MtDNA copy number with lung cancer risk and clinical outcomes. Abstract:Background: We previously reported that higher levels of mitochondrial DNA copy number (mtDNA CN) were associated with lung cancer risk among male heavy smokers (i.e., >= 20 cigarettes per day) in the Alpha-Tocopherol Beta-Carotene (ATBC) study. Here, we present two additional prospective investigations nested in the Prostate, Lung, Colorectal, and Ovarian (PLCO) cancer screening trial and the Shanghai Women's Health Study (SWHS), and pooled with previously published data from ATBC.Materials: All DNA were extracted from peripheral whole blood samples using the phenol-chloroform method, and mtDNA CN was assayed by fluorescence-based qPCR. Multivariate unconditional logistic regression models were used to estimate ORs and 95% confidence intervals for the association of mtDNA CN and lung cancer risk.Results: Overall, mtDNA CN was not associated with lung cancer risk in the PLCO, SWHS, or pooled populations (all P trends > 0.42, P heterogeneity = 0.0001), and mtDNA CN was inversely associated with lung cancer risk among male smokers in PLCO, the opposite direction observed in ATBC. In addition, the mtDNA CN association observed among male heavy smokers in ATBC was the opposite direction in PLCO.Conclusions: mtDNA CN was not consistently associated with lung cancer risk across three prospective study populations from Europe, Asia, and the United States.Impact: This pooled study suggests no consistent association between prediagnostic mtDNA CN levels and lung cancer risk across several populations. Abstract:BACKGROUND: Compelling epidemiological evidence indicated that alterations of mitochondria! DNA (mtDNA), including mutations and abnormal content of mtDNA, were implicated in the tumorigenesis of several malignancies in a tumor-specific manner, such as lung cancer, breast cancer, and non-Hodgkin lymphoma. This study was undertaken to investigate whether mtDNA content in peripheral blood lymphocytes (PBLs) could be used as a risk predictor for colorectal cancer (CRC). METHODS: The mtDNA content was measured by using quantitative real-time polymerase chain reaction in PBLs from 320 CRC patients and 320 matched controls. RESULTS: The authors found that CRC patients exhibited statistically significantly higher mtDNA content than matched controls (median, 1.03 vs.86; P < .001). They further assessed the association between mtDNA content and CRC risk using multivariate logistic regression. By using the median value in controls as the cutoff point, they found that, compared with low mtDNA content, high mtDNA content was associated with a significantly increased CRC risk (adjusted odds ratio, 2.03; 95% confidence interval, 1.41-2.81). In a trend analysis, they found a statistically significant dose-response relationship between higher mtDNA content and increased CRC risk (P for trend <.001). Stratified analysis showed that the association between mtDNA content and CRC risk was not modulated by major host characteristics. Abstract:The relationship between blood mitochondrial DNA (mtDNA) copy number and subsequent cancer risk has been investigated previously. However, such association has never been examined in Mexican Americans. In the current study, we examined association between social-demographic factors and blood mtDNA copy number, as well as longitudinal relationship between cancer and mtDNA copy number, among 10,802 Mexican Americans in the Mano-A-Mano Mexican American Cohort. Overall, mtDNA copy number was statistically significantly higher among participants who developed cancer during the study period than among cancer-free controls (0.17 vs 0.13, P = 0.007). Among cancer-free control participants, mtDNA copy number significantly differed by social-demographic characteristics. However, there was a large degree of heterogeneity in these effects across the mtDNA copy number distribution. In the longitudinal analysis, we observed that higher mtDNA copy number was positively associated with increased risk of all cancer types (adjusted hazard ratio [HR], 1.13; 95% confidence interval [CI], 1.09-1.17). Participants with mtDNA copy number in the fourth (highest) quartile had a higher risk of all cancer (adjusted HR, 2.12; 95% CI, 1.65-2.73) than did participants in the first (lowest) quartile. In summary, our results in Mexican Americans support an association between increased mtDNA copy number and cancer risk. Our results also suggest that mtDNA copy number may be influenced by social and demographic factors.

Abstract:Background: Mitochondrial DNA (mtDNA) is an approximately 16,000-bp circular double-stranded DNA molecule that is a prime target of oxidative damage. Several somatic mutations in mtDNA have been observed in gastric tumors, suggesting an involvement in gastric cancer risk and progression. mtDNA copy number in leukocyte DNA has also been linked to several other cancers, although the temporal relationship between mtDNA and cancer has not been adequately explored. Methods: Using a nested case-control study design, we examined the association between mtDNA copy number in 162 gastric cancer cases and 299 matched controls within the Shanghai Women's Health Study, a large population-based prospective cohort. Relative mtDNA copy number was measured in triplicate by a quantitative real-time PCR assay in peripheral leukocytes. Results: mtDNA copy number levels were comparable among cases and controls, with a median of 1.04 [interquartile range (IQR), 0.87-1.25] and 1.06 (IQR, 0.88-1.29), respectively. Overall, mtDNA was not associated with gastric cancer risk. However, the association differed when stratified by the time between sample collection and cancer diagnosis. An association between low levels of mtDNA copy number (<median) and gastric cancer risk was apparent among earlier diagnosed cases, in particular, those diagnosed within 2 years of sample collection (OR = 5.32; 95% CI = 1.03-27.60). This association was not present as the time between sample collection and cancer diagnosis increased. Conclusions and Impact: Our findings suggest that there is no association between leukocyte mtDNA copy number and risk of developing gastric cancer; however, we observed a possible early disease effect on mtDNA copy number levels. Abstract:Abstract Mitochondria play an important role in generating energy for vital cellular functions. Alterations in mitochondria function due to mutations or copy number changes can potentially lead to tumorigenesis. Although previous studies have investigated the effect of mitochondrial DNA (mtDNA) depletion on cancer risk, no studies have assessed constitutive mtDNA copy number as a prognostic marker for overall survival in non-small cell lung cancer (NSCLC). Towards this, we measured mtDNA content by a quantitative real-time PCR assay in peripheral blood lymphocytes collected prior to treatment from 1,134 NSCLC cases seen at MD Anderson Cancer Center. A significant inverse dose-response trend was identified for mtDNA content and NSCLC risk (p-value: 0.0076). Those with the highest tertile of mtDNA content had the greatest reduction in risk of dying (HR: 0.81, 95% CI: 0.82-0.97, p-value: 0.014) and a highly significantly enhanced median survival time of 20.20 months compared to only 14.11 months for those with the lowest mtDNA content (p-value: 6.7x10-5). It is known that exposure to tobacco smoke results in the production of reactive oxygen species that can damage mtDNA. Therefore, we also performed stratified analysis based on smoking status. The effect in ever smokers (N=951) mirrored that of the overall population with a significant reduction in risk of death with increasing mtDNA content (p-trend: 0.008) and a dramatic nearly seven month survival advantage for those patients with the highest mtDNA content compared to the lowest (20.73 months compared to 13.98 months, respectively, p-value: 4.1x10-5). Interestingly, mtDNA content was not significant with overall survival in never smokers (N=183), suggesting that the effect in the overall population was driven by the ever smokers and that mtDNA alterations do not play a major role in the course of disease for this unique subgroup of NSCLC patients. Together, these results demonstrate that mtDNA content is a potential prognostic biomarker in NSCLC that can be measured non-invasively prior to treatment to provide needed information regarding course of disease to identify those at high risk for a poor outcome. Abstract:Introduction: The aim of this investigation was to determine whether alterations in mitochondrial DNA ( mtDNA) copy number in colon cancer were associated with clinicopathological parameters and postsurgical outcome.Methods: By quantitative real-time PCR assay, the mtDNA copy number was detected in a cohort of colon cancer and matched adjacent colon tissues (n = 162).Results: The majority of patients had higher mtDNA content in colon cancer tissues than matched adjacent colon tissues. Moreover, high mtDNA content in tumor tissues was associated with larger tumor size, higher serum CEA level, advanced TNM stage, vascular emboli, and liver metastases. Further survival curve analysis showed that high mtDNA content was related to the worst survival in patients with colon cancer at advanced TNM stage.Conclusions: High mtDNA content is a potential effective factor of poor prognosis in patients with advanced stage colon cancer.
