Abstract:Warthin's tumor is alike cystadenoma lymphomatosum, adenolymphoma, and cystadenolymphoma. The tumor is a benign neoplasm predominantly found in the parotid gland. This tumor usually causes minimal pain, but the patient may complain of earaches. The tumor is removed surgically and is easily performed due to its superficial location. This is a case we are presenting: a 72-year-old man visited our clinic primarily complaining of swelling at the right parotid gland area. MRI revealed well-defined margins of the tumor in the superficial lobe of the right parotid gland and heterogeneous contrast enhancement. We performed a superficial parotidectomy followed up with a pathological examination confirming Warthin's tumor. We report a typical case of Warthin's tumor and a literature review.

Abstract:OBJECTIVE:To investigate the value of the signal intensity on T2-weighted magnetic resonance (MR) imaging using quantitative analysis in the differentiation of parotid tumors.MATERIALS AND METHODS:MR data of 80 pleomorphic adenomas (PAs), 68 Warthin tumors (WTs), and 34 malignant tumors (MTs) confirmed by surgery and histology were retrospectively analyzed. The signal intensities of tumor, normal parotid gland, spinal cord, and buccal subcutaneous fat were measured, and the signal intensity ratios (SIRs) between the tumor and the three references were calculated. Receiver operating characteristic curve was used to determine the optimal threshold and diagnostic efficiency of SIR for differentiating PAs, WTs, and MTs.RESULTS:The area under the curve (AUC) of tumor to parotid gland SIR (SIRP), tumor to spinal cord SIR (SIRC), and tumor to buccal subcutaneous fat SIR (SIRF) for differentiating PAs and WTs was 0.922, 0.918, and 0.934, respectively. The sensitivity and specificity at an optimal SIR threshold were 86.3% and 91.2%, 80.0% and 97.1%, and 85.0% and 94.1%, respectively. The AUC of SIRP, SIRC, and SIRF for distinguishing PAs from MTs was 0.793, 0.802, and 0.774, respectively. The sensitivity and specificity at an optimal SIR threshold was 86.3% and 61.8%, 80.0% and 73.5%, and 82.5% and 73.5%, respectively. The AUC of SIRP, SIRC, and SIRF for distinguishing WTs from MTs was 0.716, 0.709, and 0.759, respectively. The sensitivity and specificity at an optimal SIR threshold were 61.8% and 82.4%, 55.9% and 82.4%, and 64.7% and 86.8%, respectively.CONCLUSION:SIRP, SIRC, and SIRF on T2-weighted MR images had high diagnostic efficiency for differentiating between PAs and WTs, while SIRP and SIRC for differentiating between PAs and MTs, and SIRF for differentiating between WTs and MTs had relatively high diagnostic efficiency.
Imaging Features Of Parotid Gland Oncocytoma: A Case Series Study

Abstract:Background: Reports regarding the imaging findings of parotid gland oncocytoma are limited and prospective definitive diagnosis is difficult. This case series is aimed at furthering the knowledge of imaging for parotid gland oncocytoma.Methods: Seven patients with pathologically confirmed parotid gland oncocytoma were identified between January, 2016 and August, 2020. They were subjected to computed tomography, magnetic resonance imaging or ultrasound. Imaging features of lesions were retrospectively analyzed and documented by a senior radiologist.Results: Among the 7 recruited parotid gland oncocytoma patients, 4 were males and 3 were females. They were aged between 47 and 87 years with a median age of 63 years. The range of maximum diameter of the mass was about 1.6-4.5 cm, with the median size being 3.5 cm. All cases were unilateral single masses. Three patients had their parotid gland oncocytoma located in the deep lobe of the parotid gland and 4 patients had their parotid gland oncocytoma located in the superficial lobe. Most lesions exhibited a clear boundary and irregular shapes or lobulated, and contain cystic components. Local pain was the most common clinical manifestation. Most of the lesions exhibited a hypoechoic mass on ultrasound, soft tissue density on computed tomography, iso to hypointense signal on T1 weighted imaging, slightly hypersignal on T2 weighted imaging, while contrast-enhanced computed tomography, magnetic resonance imaging, and ultrasound showed avid enhancement.Conclusions: Parotid gland oncocytomas usually exhibit a clear boundary to surrounding parotid tissue, lobulated or irregular shape and avid enhancement, sometimes accompanied with cystic components. Diagnosis of parotid gland oncocytoma should be considered when these features are encountered in clinical practice.
Bilateral Parotid Gland Oncocytoma Mimicking Oropharyngeal Cancer Metastasis: A Case Report from a Radiation Oncologist's Perspective

Abstract:Background/aim: Parotid oncocytomas typically present as benign, unilateral, slow-growing, painless, and solitary masses that are histologically firm and multilobulated. They are often misdiagnosed as pleomorphic adenomas, hemangiomas, or other forms of oncocytosis. However, in our case, the parotid oncocytomas initially mimicked bilateral parotid gland metastasis of advanced oropharyngeal cancer. Here, we present a case of oropharyngeal cancer with bilateral parotid oncocytomas treated with chemoradiotherapy (CCRT). Case report: We report the case of a 74-year-old man with a sore throat, neck pain, right earache, oropharyngeal cancer, and bilateral parotid gland oncocytoma. Head and neck computed tomography and magnetic resonance imaging (MRI) showed soft-tissue swelling in the right tonsillar fossa and several enlarged level II neck lymph nodes. MRI revealed enhancing masses in both parotid glands, initially suspected to be metastatic lymph nodes. A biopsy of the right palatine tonsil confirmed squamous cell carcinoma with human papilloma virus-16 positivity. A positron emission tomography scan was performed, and biopsy-proven malignant lesions were observed in the right tonsillar region with metastatic lymph nodes in the right and left neck. Focal hypermetabolism was observed in the parotid glands, suspected to be pathological lesions such as metastatic intra-parotid lymph node or Warthin's tumor. An ultrasonography-guided biopsy of the left parotid gland confirmed an oncocytoma. Based on these results, the patient was scheduled for CCRT. After pathological confirmation of parotid oncocytoma, CCRT was administered, excluding the parotid glands within the radiotherapy field. Conclusion: This is a case of bilateral parotid gland oncocytoma mimicking oropharyngeal cancer.
Findings of rare focal lesions in the salivary glands by high frequency ultrasound

Abstract:Objective To investigate the findings for rare focal lesions of the salivary glands by ultrasound.Methods The ultrasound images of 20 rare focal lesions in the parotid and submandibular glands included in 20 cases with pathological confirmation were retrospectively reviewed.Results Most of 14 benign lesions(including 4 basal cell adenomas,3 hemangiomas,5 benign lymphoepithelial lesions,myoepthelioma and an oncocytoma)appeared round or oval(78.6%,11/14),hypoechogenic(74.1%,10/14)and well-circumscribed(78.6%,11/14).The CDFI of 14 lesions showed no-(n=1),rare-(n=4),or rich-blood flow(n=9).The 6 malignant tumors(including 2 mucoepidermoid carcinomas,1 adenoid cystic carcinoma,1 cystadenocarcinoma,1 adenocarcinoma,and 1 non-Hodgkin's lymphoma)were irregular(n=2,33.3%)or oval(n=4,66.7%).Most of them were heterogeneously echoic(83.8%,5/6),ill-circumscribed(66.7%,4/6),and 3(50.0%)showed intralesional fine calcification.The CDFI of 5 lesions showed no-(n=1),rare-(n=1),or rich-blood flow(n=3).Conclusions The rare focal lesions in the salivary glands is helpful for diagnosis of salivary gland lesions by ultrasound,but ultrasonic appearances may be overlapped and misdiagnosed.

Abstract:Objective To further improve the recognition of image manifestation in the parotid disease.Materials and Methods 29 cases of parotid disease were selected and their CT or (and) MRI findings were analyzed in details.Results The image manifestation of parotid disease could be divided into three types: (1)localized mass (n=23);(2)diffuse mass (n=3),all of them were malignant tumor; (3)diffuse swell without mass (n=3), include 2 cases of parotitis and 1 case of angiocavernoma of parotid and occlusal surface. Among the localized mass, 15 were benign tumor in which only 1 lesion was ill defined, while 4 were malignant tumor in which 3 lesions were ill defined, there was significant difference between them (P=0.01). As for the size of mass, relatively, mixed tumor of parotid gland was larger than that of Warthin's tumor. The average diameter of mixed tumor of parotid gland was 4.3cm while the diameter of Warthin's tumor was 2.3cm, deviation between them was significant (t=2.737,P=0.02).Conclusion According to the image findings combined with clinical manifestation, most parotid disease can be diagnosed correctly.
Fine needle aspiration diagnosis of benign oncocytic lesions of the head and neck associated with false positive 18F-fluorodeoxyglucose uptake on positron emission tomography scan

Abstract:Introduction: 18F-fluorodeoxyglucose (FDG) uptake on positron emission tomography/computed tomography (PET/CT) has become the mainstay for staging and post-therapy surveillance of cancer as malignant neoplasms generally demonstrate higher FDG uptake that benign entities. However, there are certain benign lesions, most notably oncocytic tumors, that can display very high uptake and fine needle aspiration (FNA) is usually done to confirm malignancy. Therefore, it is important to recognize that benign oncocytic lesions of the head and neck may also present as FDG-avid lesions to avoid a diagnostic pitfall. Methods: Electronic search of institutional surgical and cytopathology archives was conducted to identify cases of benign oncocytic lesions involving the head and neck region diagnosed by FNA from January 2012 to April 2022. Chart review was used to assess whether lesions were initially discovered via PET scanning. Results: One hundred and twenty-five cases of oncocytic lesions were identified; 12 (9%) PET positive lesions were identified in the head and neck region from patients being evaluated for metastasis or for suspicion of malignancy. Cytopathology of all 12 cases demonstrated benign oncocytic lesions; eight (67%) of these cases were consistent with Warthin tumor, one (8.3%) was a benign oncocytic lesion, and one (8.3%) was consistent wit a parathyroid adenoma. Most (58%) of the PET-positive lesions were in parotid region, two from thyroid gland (17%), one from submandibular gland (8%), one from paratracheal area (8%). The PET scan SUVs ranged from 3.3 to 19.5 g mL-1. Conclusions: Oncocytic lesions including Warthin tumors can result in false-positive FDG uptake on PET scans. Clinicians and cytopathologists should be aware of PET-positive benign oncocytic head and neck lesions.

Patient concerns: A 62-year-old man presented with a history of a walnut-size mass in left parotid gland when yawned for half-month, and a 48-year-old woman complaint about a grape-size, painless mass behind the right ear for a month. Diagnoses: Radiological examinations suggested that both lesions were cyst-solid mixed lesions with relatively smoothed margins, with or without significant enhancement while the lesion without enhancement had a hemorrhage. Then a diagnosis of benign tumor arising from the parotid gland was made. Final diagnosis of myopericytoma was confirmed by histopathological and immunohistochemical examinations after surgical resection. Interventions: Both patients underwent excision of the tumor and the superficial parotidectomy with facial nerve preservation. Outcomes: Both patients recovered without any intraoperative or postoperative complication and had no signs of recurrence during a 17-month and 5-year follow-up. Lessons: Parotid gland myopericytoma is an exceedingly rare tumor which diagnosis can be challenging, and this is the first published report specifying the magnetic resonance features of the disease.

Abstract:OBJECTIVE:The aim of this study was to characterize the ultrasound (US) and computed tomography (CT) findings of primary acinic cell carcinoma (AciCC) of the parotid gland.SUBJECTS AND METHODS:Seventy patients (70 lesions) with histopathologically proven AciCC underwent US or CT examination. The following characteristics were assessed on US images: size, shape, border, echogenicity, echotexture, internal structure, distal acoustic enhancement, and vascularity. The following characteristics were evaluated on CT images: size, shape, border, density, CT values on plain and contrast-enhanced scans, enhancement pattern, enhancement degree, and surrounding bone destruction.RESULTS:On US images, lesions were irregular, well-defined, hypoechoic, heterogeneous, and poorly vascularized. On CT images, lesions were regular and well-defined, and showed slight heterogeneous enhancement.CONCLUSION:These findings suggest that most primary AciCCs show benign features on US and CT.

Abstract:Sonography was performed on 104 cases with parotid tumors. The shape, boundary and internal echoes were related to the malignancy of the tumor. The diagnostic criteria of parotid tumors were proposed. The sonograms of 32 surgical specimens of parotid tumors were compared with histopathologic findings to investigate the pathologic bases of various sonograms. The boundary echoes were related to integrity of the capsule of the tumor. The internal echoes were related to internal structures of the tumor.

Abstract:Mucosa-associated lymphoid tissue (MALT) lymphoma commonly arises from chronic inflammation or autoimmune diseases, such as Sjögren syndrome (SjS). Although rare, amyloid deposition in MALT lymphoma has been reported. We present a rare case of parotid gland MALT lymphoma in a 49-year-old woman, in whom preoperative diagnosis was challenging due to atypical imaging findings resulting from amyloid deposits. MRI showed T2-hypointense and T1-iso- to slightly hyperintense masses in the left parotid gland and right sublingual gland, with predominant marginal contrast enhancement and no significant diffusion restriction. Additionally, atrophy and fatty replacement of the parenchyma were noted in bilateral parotid glands, suggesting SjS. Left superficial parotidectomy was performed and pathological findings confirmed MALT lymphoma with extensive amyloid deposition. Histopathological findings of the resected parotid gland parenchyma also suggested SjS. MALT lymphoma should be considered in the differential diagnosis of multiple salivary gland masses in patients with suspected SjS. If MRI reveals atypical imaging findings for malignant lymphoma, particularly T2-hypointensity with no significant diffusion restriction, the possibility of amyloid deposition in MALT lymphoma should be considered.

Abstract:Warthin Tumor, also known as papillary cystadenoma lymphomatosum, is the second most prevalent benign parotid gland tumor, accounting for approximately 2 to 15% of all cases. A 62-year-old male patient presented with a progressively painless swelling in the lower left face region that had been developing for 4 years. During the extraoral examination, a well-defined, firm mass with a diameter of 10 centimeters was identified. As part of the diagnostic process, magnetic resonance imaging and a fine-needle aspiration biopsy (FNAB) were performed, both of which confirmed the diagnosis of a Warthin Tumor in the parotid gland.

22 Cases with Pathologic Correlation Adenoma of the Parotid Gland: A Report of CT and Ultrasound Features of Basal Cell

Abstract:BACKGROUND AND PURPOSE: Parotid gland BCA is a rare benign tumor. Only a few studies describing the imaging features of BCA have been published. This study investigated CT and sonography characteristics of BCA of the parotid gland. MATERIALS AND METHODS: Demographics of patients with BCA were evaluated, and lesion characteristics of CT ( n (cid:1) 22) and sonography ( n (cid:1) 20) were reviewed. These cases were grouped into 3 types: type 1 tumors, located at the superficial region of superficial lobe of the parotid gland; type 2 tumors, located at the deeper region of superficial lobe; and type 3 tumors, located in the deep lobe. Imaging findings were correlated with pathology. RESULTS: Sixteen patients (73%) were female and 6 (27%) were male. The mean age was 51.5 years (SD 10.2; range 32–73). The size of the tumors was less than 30 mm. The sizes of type 1, type 2, and type 3 tumors were 11.4 (cid:2) 3.29 mm, 19.3 (cid:2) 5.44 mm, and 26 (cid:2) 3.6 mm, respectively. The CT attenuation increase was 64.5 (cid:2) 19 HU on contrast CT. The type 1 tumors were solid (11/11), showed homogeneous or slightly heterogeneous enhancement on CT, and were homogeneously or slightly heterogeneously hypoechoic on sonography. Cystic changes tended to occur in type 2 (7/8) or type 3 (2/3) tumors, which showed obvious heterogeneous attenuation on CT and anechoic on sonography. CONCLUSIONS: The BCA tends to be small and shows early intense enhancement. The solid tumor is common in the superficial region of the parotid gland, and cystic lesions occur mostly in the deeper parts of the superficial lobe or in the deep lobe. ABBREVIATIONS

Abstract:Objective: To investigate the diagnostic value of high frequency ultrasound in parotid tumors. Methods: High frequency ultrasonic manifestations and the diagnostic coincidence rate in 72 patients with pathologically confirmed parotid tumors were retrospectively analyzed. Results: Pleomorphic adenoma, Warthin tumor and parotid cancer accounted for 90.28% (65/72) of all the 72 cases of parotid tumors, and the ultrasonic diagnostic coincidence rate of them was 88.00% (22/25), 56.25% (9/16) and 45.83% (11/24), respectively. Warthin tumor mostly occurred in elder men, which showed rich blood flow signal. Parts of parotid cancers without typical malignant features were difficult to be identified with ultrasonography. Conclusion: The differential diagnosis of parotid tumors mainly focus on pleomorphic adenoma, Warthin tumor and parotid cancer. Combination closely with clinic information is an important way to improve ultrasonic diagnostic coincidence rate.

Abstract:Objective:To analysis the CT and MRI appearances of parotid tumors,and improve the cognition parotid tumors.Methods:The CT/MRI findings of in 90 cases with parotid tumors were retrospectively analyzed.Results:Among 90 cases,there were 74(74/90,82.2%) benign tumors,16(16/90,17.8%) malignant tumors.On CT and MRI,most benign tumors were located in the shallow lobe,round or oval in shape,well-defined,homogeneous or heterogeneous soft tissue mass;and most of malignant tumor were located in the deep lobe or deep-shallow lobe,irregular in shape,ill-defined,heterogeneous soft tissue mass,infiltration of adjacent structure and lymph node metastasis.Conclusion:CT and MRI both can well demonstrate the size,number and location of parotid tumors,and there is still helpful for the diagnosis,differentiation and therapeutic decision.
True Oncocytic Acinic Cell Carcinoma: A Case Image

Abstract:A 67-year-old female with a history of chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) presented with right sided otalgia and a 2-3 cm firm, tender right posterior parotid mass. Fine needle aspiration biopsy (FNAB) established a diagnosis of acinic cell carcinoma (AciCC). Further workup demonstrated lung nodules which were confirmed by FNAB to represent metastatic AciCC. A right radical parotidectomy with sacrifice of the facial nerve, segmental mandibulectomy, and selective neck dissection (levels II-IV) was performed. Microscopically, the tumor displayed an infiltrative border with a solid multinodular growth pattern and fibrosclerotic septation. The tumor was composed mainly of uniform cells with abundant eosinophilic granular cytoplasm with round nuclei with prominent nucleoli. Nuclei were fairly monomorphic, mitotic counts were 3-4 per 2mm2 and there was no necrosis despite the aggressive growth pattern. An anti-mitochondrial immunohistochemical stain showed strong reactivity in the tumor cells, with an internal positive control of adjacent striated ducts. An immunohistochemical stain for NR4A3 demonstrated strong nuclear reactivity in the tumor cells. Electron microscopy highlighted the tumor cells with numerous mitochondria and distinctive electron dense intramitochondrial inclusions. Concurrent CLL/SLL was identified on histologic examination of the lymph nodes, but they were free of AciCC. After eight weeks of follow-up, she tolerated the surgery well and is currently receiving radiation therapy to the parotid and neck. In this illustrative case, we justify the oncocytic designation of AciCC by morphology, immunohistochemistry, and electron microscopy.
