Duration of Diabetes, Glycemic Control and Risk of Heart Failure among Adults with Diabetes: A Cohort Study

Abstract:Background: To investigate the influences of diabetes duration and glycemic control and their potential interplays on the development of heart failure (HF).Methods: We included 24,962 adults who had diabetes but without HF at baseline recruitment of the UK Biobank (2006 to 2010). The duration of clinically confirmed diabetes was self-reported, and the status of glycemic control was determined by glycated hemoglobin (HbA1c). Findings: There were 2,193 incident HF cases during a median follow-up of 11.7 years. Both duration of diabetes and HbA1c levels were positively associated with risk of HF (both P-trend <0.001). The HRs (95% CI) for diabetes durations of 5 to <10, 10 to <15, and ≥15 years were 1.19 (1.07, 1.33), 1.36 (1.20, 1.55), and 1.78 (1.58, 1.99), respectively (vs. duration <5 years); and the HRs for HbA1c of 53.0 to <58.5 mmol/mol (7.0% to <7.5%), 58.5 to <63.9 mmol/mol (7.5% to <8.0%), and ≥63.9 mmol/mol (8.0%) were 1.18 (1.05, 1.33), 1.14 (0.98, 1.32), and 1.57 (1.41, 1.75), respectively [vs. HbA1c <53.0 mmol/mol (7.0%)]. Individuals with both a long disease duration (≥15 years) and a poorer glycemic control [HbA1c ≥63.9 mmol/mol (8.0%)] had a particularly higher risk of HF (P for interaction = 0.01).Interpretation: The risk of HF among individuals with diabetes varied by both duration of diabetes and glycemic control, with the highest risk among those with long-standing duration and poorer glycemic control, which needs to be considered when developing relevant preventive strategies.Funding Information: This work was supported by grants from the Shenzhen Science and Technology Innovation Committee (ZDSYS20200810171403013).Declaration of Interests: The authors have no relevant financial or non-financial interests to disclose.Ethics Approval Statement: The investigation conforms to the principles outlined in the Declaration of Helsinki.
