Abstract:Objective To investigate the value of mesh in laparoscopic repair of parastomal hernia. Methods Laparoscopic repair with mesh was performed in 6 patients with parasromal hernia from Decem- ber 2004 to April 2006 in this hospital.Results Laparoscopic mesh herniorrhaphy was successfully per- formed in 6 patients.The operative time was 65 ～ 130 minites (average 90 min).Postoperative hospital stay was 6 ～ 9 days (average 7 days).Two patients complained of persistent pain for three months and two pa- tients had seroma,which was cured by needle aspiration and pressure bandage.No death and intestinal fis- tula nor infection occurred postoperatively.No recurrence of hernia was observed in all patients in 7 to 26 months(average 13 months) during follow-up.Conclusions Laparoscopie parastomal hernia repair using mesh is a safe and effective procedure of less injury to tissues.
Improved Patient Outcomes in Paraesophageal Hernia Repair Using a Laparoscopic Approach: A Study of the National Surgical Quality Improvement Program Data

Abstract:A consensus on the optimal surgical approach for repair of a paraesophageal hernia has not been reached. The aim of this study was to examine the outcomes of open and laparoscopic paraesophageal hernia repairs (PHR), both with and without mesh. A review of the National Surgical Quality Improvement Program (NSQIP) database from 2007 to 2011 was conducted. Patients who underwent an open or laparoscopic PHR were included. The primary outcome was 30-day mortality. Secondary outcomes included infections, respiratory and cardiac complications, intraoperative or perioperative transfusions, sepsis, and septic shock. Statistical analyses using odds ratios were performed comparing the open and laparoscopic approaches. A total of 4470 patients were identified using NSQIP; 2834 patients had a laparoscopic repair and the remaining 1636 patients underwent an open PHR. Compared with the laparoscopic approach, the open repair group had significantly higher 30-day mortality (odds ratio, 4.75; 95% confidence interval, 2.67 to 8.47; P < 0.0001). The laparoscopic approach had a statistically significant decrease in infections, respiratory and cardiac events/complications, transfusion requirements, episodes of sepsis, and septic shock ( P < 0.05). Our data suggest increased perioperative morbidity associated with an open PHR compared with laparoscopic. There was no statistically significant difference in any of the primary or secondary outcomes in patients repaired with mesh compared with those without. The overall use of mesh in paraesophageal hernia repairs has increased. The NSQIP data show significantly increased 30-day mortality in open repair compared with laparoscopic as well as a significantly higher perioperative complication rate.
[Laparoscopic Anti-Reflux Surgery with Biological Mesh in Treatment of Gastroesophageal Reflux Disease].

Abstract:OBJECTIVE:To evaluate the application of biological mesh in laparoscopic anti-reflux procedure for gastroesophageal reflux disease (GERD).METHODS:The clinical data of 20 consecutive GERD patients underwent anti-reflux surgery in Sir Run Run Shaw Hospital from December 2012 to April 2014 were retrospectively analyzed. The laparoscopic hiatal repair with 360 fundoplicaiton was performed and the biological mesh (BiodesignTM, Surgsis) was implanted for reinforcement of hiatal repair.RESULTS:All laparoscopic procedures were successful, no conversion and no intra-operative complications occurred. The pre-operative complains were relieved in all patients, and no recurrence was observed during 3-18 month of follow-up. Six patients got dysphagia after operation; 5 of them were controlled through medication and psychological induction; 1 received esophageal dilatation by bougie.CONCLUSION:The application of biological mesh in laparoscopic anti-reflux procedure for gastroesophageal reflux disease is satisfactory.
The Primary Experiences on Laparoscopic Paraileostomal Herniorraphy with Meshes

Abstract:Objective To evaluate the eflicacy and safety of laparoscopic paraileostomal herniorrha- phy with meshes.Methods A laparoscopic tension-free repair operation has been performed on 18 patients with parastomal hernias from September 2004 to December 2006 in our hospital.Among the patients,3 cases underwent by ileal conduits had got paraileostomal hernias.Results Laparoscopic paraileostomal hernior- rhaphy with meshes was all successfully performed on the 3 patients.The operating time was ranged from 45 to 90 rain (mean,63.3 min);the hernia defect was ranged from 4 to 5 cm (mean,4.5 cm);the postoper- ative hospital stay was ranged from 3 to 5 days (mean,4 days);and the postoperative follow-up of 17 to 25 months (mean,21.3 months) revealed that there was no wound infection or early recurrence in our cases. And one patient felt temporary postoperative flatulence;one patient complained about the severe pain in re- pair area temporary and was then released in the next 2 weeks.The postoperative complication also included the seroma (n = 1),and the patient was recovered after twice punctures and local pressure wrapping-up. Conclusions Laparoscopic paraileostomal herinorrhaphy with meshes can be considered as a safe,feasible and effective operation method.

Abstract:Objective To summarize the effectiveness of laparoscopic parastomal herniorrhaphy with meshes.Methods A laparoscopic mesh repair was performed for 16 patients with parasromal hernias from September 2004 to November 2006 in Hua Shan Hospital,Shanghai Medical College of Fu Dan University.Results Laparoscopic parastomal herniorrhaphy with meshes was successfully performed for 14 patients,Two cases were converted to an open procedure because of extensive adhesion (n=1) and intestine damaged (n=1).The average operating time was 116 min (range,45-180 min).The mean hernia defect was 5.4 cm (range,4-6cm) and postoperative hospital stay averaged 5.4 days(range,3-8 days).Twelve patients complained the temporary pain in operative area and 7 patients were occurred in seroma. One patient felt dyspnea, which ameliorate by the assistant of VPAP.There was no wound infection or early recurrence in the cases.The local appearance was better in patients with small hernia sac or who accept herniaraphy earlier than in those with larger hernia sacs or who accept operation later.Conclusion Laparoscopic parastomal herinorrhaphy with meshes is a security, feasibility and effective method.
Laparoscopic mesh repair of incisional hernias

Abstract:Objective To investigate the operative techniques and clinical outcome of laparoscopic incisional hernia repair with mesh.Methods The clinical data of 23 patients with abdominal incisional hernia treated by laparoscopic mesh repair from Jan.2006 to Dec.2006 were analyzed retrospectively.Results The operation was successfully performed on 22 patients and one case was converted to open mesh repair because of extensive intra-abdominal adhesion.Prolonged pain occurred in 4 patients,and seroma occurred in 3 patients.No recurrence occurred in any of cases during a follow-up period of 4 to 24 months(average 13.8 months).Conclusion The laparoscopic mesh repair could be performed on most incisional hernia cases safely,and unexpected defects could be discovered intraoperatively.If extensive adhesion interferes with dissection,the procedure should be converted to open repair.

Abstract:[Summary] This paper reported one case of rectocele treated with laparoscopic artificial mesh repair in July 2010. Along with domestic and foreign literature review, the clinical value of laparoscopic artificial mesh repair for rectocele was investigated. After treatment the rectocele symptoms disappeared, without postoperative complications. A follow-up for 56 months showed good result. The operation has advantages of minimal invasion, simple performance, fast recovery, and few complications, being worthy of clinical application.
Laparoscopic repair of paraesophageal hiatus hernia in infants and children: a review of 66 cases

Abstract:Objective To explore the efficacy and safety of laparoscopic repair of paraesophageal hiatus hernia in infants and children.Methods From July 2012 to August 2015,41 boys and 25 girls underwent laparoscopic repair of paraesophageal hiatus hernias.The mean age was (26 ± 10)(1-39) months and mean weigh (9.5 ± 6.1) (3.2-16) kg.Upper gastrointestinal contrast radiography was performed.The clinical types were Ⅰ (n=25),Ⅱ (n =37),Ⅲ (n =2) and recurrent hernia (n =1).And there were Toupet (270-degree) fundoplication (n =32) and Nissen fundoplication (n =34).Results All repairs were successful without recurrence or serious complications.The mean operative duration was 70 (48-230) min,the mean volume of blood loss ＜10 ml and the mean hospital stay of 11 days.An unrestricted diet resumed at 24-72 h The postoperative follow-up period was 1-37 months.Conclusions Both safe and feasible,laparoscopic repair of paraesophageal hiatus hernia is worthy of wider clinical applications.With greater experiences,the number of open hole may decrease from 4 to 3 holes.And its operative duration has no obvious difference with that of conventional laparotomy
Laparoscopic versus open mesh repair for the treatment of recurrent inguinal hernia: a systematic review and meta-analysis.

Abstract:Background: Inguinal hernia repair is a common surgical procedure; however, the effects of laparoscopic and open mesh repair in the treatment of recurrent inguinal hernia are unclear. This study aimed to evaluate the effects of laparoscopic and open mesh repair in the treatment of recurrent inguinal hernia by conducting a meta-analysis of randomized controlled trials (RCTs). Methods: A comprehensive, meta-analysis of RCTs on the effects of laparoscopic and open mesh repair in the treatment of recurrent inguinal hernia was performed. English- and Chinese-language studies published up to December 30, 2017 were identified via a systematic search of the PubMed, Embase, Science Direct, Cochrane Central Register of Controlled Trials, China National Knowledge Infrastructure (CNKI), and Wanfang databases. Summary odd ratios or weighted mean differences (MDs) with 95% confidence intervals (CIs) were adopted to calculate each outcome using a fixed or random effects model. Results: Ten RCTs involving a total of 1,017 patients were included. There was no statistically significant difference in the rate of recurrence (P=0.23; OR: 0.74, 95% CI: 0.45-1.21), hematoma (P=0.47; OR: 0.71, 95% CI: 0.28-1.79), urinary retention (P=0.94; OR: 0.97, 95% CI: 0.46-2.07) and acute pain (P=0.71; OR: 0.74, 95% CI: 0.14-3.76) between the laparoscopic and open mesh repair groups. The incision infection rate (P=0.02; OR: 0.28, 95% CI: 0.10-0.81) of the laparoscopic group was lower and the length of hospital stay (P<0.0001; MD: -3.65, 95% CI: -4.76 to -2.53) was significantly shorter than those of the open repair group. However, the laparoscopic group had a longer operative time (P=0.0002; MD: 20.30, 95% CI: 9.60-31.01). Conclusions: The laparoscopic approach is superior to the open mesh approach for the repair of recurrent inguinal hernia in some aspects, including the incision infection rate and length of hospital stay. However, more high-quality studies on the effects of laparoscopic and open mesh repair for the treatment of recurrent inguinal hernia are warranted.

Abstract:This study was to appraise safety and feasibility of laparoscopic approach and investigate the clinical effects of laparoscopic tension-free repair of esophageal hiatal hernia using mesh. From August 2006 to July 2009, 24 patients with esophageal hiatal hernia underwent laparoscopic repair. Twenty-three patients received laparoscopic tension-free repair using mesh, at the same time, Toupet or Dor partial fundoplication was performed. One patient was converted to open surgery. The average operating time was 90 min (70-210 min) and the blood loss was between 10-110 mL. There was no death. The mean postoperative hospital stay was 5 days (3-30 days). During a follow-up period of 12-20 months (mean 15 months), there was no recurrence of the hernia, and no complication with use of mesh. The present study suggested that laparoscopic approach was secure and minimally invasive operation for esophageal hiatal hernia and the use of mesh could reduce recurrence rate.
Clinical effects of prosthetic mesh in the treatment of incarcerated groin hernias

Abstract:Background: Incarcerated groin hernias are a common presentation as surgical emergencies. Other surgical treatment modalities are becoming more widely accepted including the use of prosthetic mesh and laparoscopy for assessment of hernia content viability and for repair. The aim of this study was to report our current experience with the use of prosthetic mesh in the treatment of incarcerated hernias and to compare the effects of different operative approaches. Methods: This retrospective study included 219 consecutive patients who underwent herniorrhaphy for incarcerated hernia between January 2013 and December 2017 in Beijing Chao-Yang Hospital. Twenty patients who were not used mesh were excluded. Patients who died in the postoperative period due to systemic complications, as well as those who were lost during the follow-up period, were excluded from the study. Demographics, surgical details, short term and long-term outcomes were collected. Mann-Whitney U-test and χ2 test were used for statistical analysis. Results: A total of 156 patients (78.4%) presented with inguinal hernias, 42 with femoral hernia (21.1%), and one with mixed hernia (0.5%), respectively. Mesh was placed in 199 patients (100%), including 15 patients with concomitant bowel resection. Four patients (2.0%) developed surgical site infections (SSI), four patients (2.0%) had foreign body sensation, one patient (0.5%) had hernia recurrence, two patients (1.0%) had chronic pain, 22 patients (11.1%) had seroma, and the mortality was 2.0%. No significant difference was noted concerning the development of surgical site infection, postoperative recurrence, chronic pain, foreign body sensation, and mortality rates between the transabdominal preperitoneal (TAPP) repair and open mesh repair. There was statistically significant difference in the postoperative incidence of seroma between two groups (12 of 49 [24.5%] vs. 10 of 150 [6.7%]; P=0.001). Conclusions: Our experience demonstrates that acutely incarcerated or strangulated groin hernia in adults is a serious neglected problem. The use of mesh could become current practice even in case of bowel resection. There was no statistically significant difference in the postoperative complications between TAPP repair and open mesh repairs except seroma.
Controversies and challenges of laparoscopic surgery for incarcerated inguinal hernia

Abstract:Both controversies and challenges exist in the application of laparoscopic techniques for incarcerated inguinal hernias. The main controversy is whether the mesh could be used where pollution might exist. Studies have revealed that the rates of incision infection and mesh infection doesn’t increase significantly in laparoscopic treatment of incarcerated hernia. Moreover,bowel viability and blood supply can be better observed during the whole time of procedure in such method. Careful preoperative evaluation, reasonable choice of surgical approach,correct mesh use and skilled laparoscopic hernia repair skills are the guarantee for the success of laparoscopic operation. Further evaluation is still needed and should be supported by more large data and evidence-based studies.
Evaluation of Mesh Repair for Parastomal Hernias: A Report of 38 Cases

Abstract:Objective To summarize the mesh repair with open Onlay or laparoscopic keyhole mesh for the treatment of parastomal hernia. Methods Thirty-eight cases with parastomal hernia were treated in Sixth People's Hospital Affiliated to Shanghai Jiao tong University from January 2005 to April 2018, and the clinical data were analyzed retrospectively. Results All of the thirty-eight patients had hernioplasty with keyhole meshor Ultrapro mesh: 15 underwent open hernia repair plus stoma reconstruction, 6 were laparoscopic IPOM hernia repair, 5 were hybrid repair and 12 were Lap-re-Do repair. The mean operating time was (150±72) minutes (of 40 to 300 minutes). The total time of follow-up was 2 to 46 months. The postoperative complications included 2 cases of hernia recurrence, 4 cases of seroma, 3 cases of wound infection and 1 case of slight intestinal circulation disorder. All were cured after therapy. Conclusion The parastomal hernia repair with meshis safe and effective, which should be individualized by careful preoperative evaluation and actual intraoperative conditions. Comparative Review of Outcomes: Single-Incision Laparoscopic Total Extra-Peritoneal Sub-Lay (SIL-TES) Mesh Repair Versus Laparoscopic Intraperitoneal Onlay Mesh (IPOM) Repair for Ventral Hernia

Abstract:AbstractTo compare outcomes between single-incision laparoscopic totally extra-peritoneal sub-lay (SIL-TES) mesh repair and laparoscopic intraperitoneal onlay mesh (IPOM) repair of ventral hernia (VH). A retrospective selection of 104 patients who underwent VH repair (50 and 54 in the SIL-TES and IPOM groups, respectively) was made. Patient data were collected, and quality of life was evaluated using Carolinas Comfort Scale (CCS) 1 month and 3 months after surgery. There were no significant differences in sex, American Society of Anesthesiologists class, defect size, mesh area, estimated blood loss, and complication rate between the groups. Age was lower, body mass index was higher, prevalence of primary VH was significantly higher (p < 0.0001), and pain was less at 24 and 48 h post procedure (p < 0.0001) in the SIL-TES group. Drainage placement was more (p < 0.0001), operation time was shorter (p = 0.012), and hospitalization duration and total hospitalization cost were greater in the IPOM group than that in SIL-TES group (8.3 ± 0.3 vs 4.3 ± 0.4 days, p < 0.0001; $7126.9 ± 141.4 vs $2937.3 ± 58.3, p < 0.0001, respectively). Pain and movement limitation scores evaluated by CCS were significantly worse at 1 month (4.93 ± 0.28 vs 1.75 ± 0.28: p < 0.0001; 2.52 ± 0.24 vs 1.15 ± 0.18: p < 0.0001, respectively) and 3 months (4.32 ± 0.37 vs 0.9 ± 0.29: p < 0.0001; 2.06 ± 0.25 vs 0.69 ± 0.11: p < 0.0001, respectively) in IPOM group, compared with the according scores in SIL-TES group. There was no readmission within 30 days and no hernia recurrence at mean follow-up of 12 months. SIL-TES mesh repair is safe and effective and is superior to IPOM repair.
Prospective randomized controlled trial comparing partially absorbable lightweight mesh and multifilament polyester anatomical mesh in laparoscopic inguinal hernia repair

Abstract:Introduction: Tension-free mesh repair is currently the gold standard treatment for inguinal hernia. Recent evidence has shown that both open and laparoscopic approaches to inguinal hernia repair can achieve good results. Lots of meshes with different properties are available on the market, but direct comparisons between them are scare. We conducted a prospective randomized controlled trial comparing a partially absorbable lightweight mesh (ULTRAPRO™) and a multifilament polyester anatomical mesh (Parietex™) in laparoscopic total extraperitoneal inguinal hernia repair. Methods: This study was a single-center, prospective randomized controlled trial to compare the surgical handling and clinical outcomes between two different types of meshes. All operations were performed using a standardized operative protocol. This study was approved by the Institutional Review Board of the Hong Kong East Cluster Health Service in 2009 (reference number: 2009-087). The study was registered in the Australian New Zealand Clinical Trial Registry (ACTRN12610000031066). Results: From October 2009 to August 2011, 85 laparoscopic total extraperitoneal inguinal hernia repairs were performed. The mean mesh handling time was 152 s for the ULTRAPRO group and 206 s for the Parietex group (P = 0.001). There were three cases of seroma formation in the ULTRAPRO group and nine in the Parietex group (P = 0.02). The overall recurrence rate was 2.5%. Conclusion: It took less time to manipulate the flat mesh (ULTRAPRO) than the anatomical mesh (Parietex) in laparoscopic total extraperitoneal inguinal hernia repair, but the time difference was small. Lightweight mesh and heavyweight mesh offered similar clinical outcomes in terms of discomfort sensation and foreign body sensation during long-term follow-up.

METHODS: To retrospectively analyze clinical data of 57 patients with esophageal hiatal hernia, all of whom were accepted surgical treatment at Huashan Hospital Affiliated to Fudan University from November 2006 to June 2009, including 24 cases with simple diaphragm feet suture and Nissen fundoplication (control group), 33 cases with simple diaphragm feet suture and Nissen fundoplication were undergoing mesh reforcement simultaneously (experimental group). The therapeutic results of laparoscopic hiatal hernia repair were compared and analyzed with or without mesh. RESULTS AND CONCLUSION: During 1 year follow-up, there was no significant difference between the control and experimental groups, including recurrence rate, patient satisfactory and symptom control (P > 0.05). Compared with the control group, the intraoperative and postoperative complications were not increased in experimental group; meanwhile, operation interval and hospital stay were not significantly longer either (P > 0.05). The short-term follow-up results showed that laparoscopic hiatal hernia repair with mesh was safe and effective, but indications should be strictly controlled.
Evaluation of the effectiveness and safety of the laparoscopic parastomal hernia repair and re-ostomy technique

Abstract:Objective To evaluate the effectiveness and safety of laparoscopic parastomal hernia repair and re-ostomy with Dynamesh-IPST mesh.Methods Laparoscopic parastomal hernia repair and re-ostomy with Dynamesh-IPST mesh were performed on 12 patients from May 2009 to August 2010,including 10 laparoscopic operations and 2 SILS procedures.The post-operative follow-up includes telephone inquiry which involves diet,abdominal symptoms,defecation,stoma condition,physical examination and CT scanning for the 12 patients.Results All the 12 operations were successful without conversion to open herniorraphy.The operating time ranged from 80 to 170 min(mean 112 min).The length of postoperative hospital stay ranged from 3 to 8 d(mean 6 d).The post-operative follow-ups ranged from 1 to 16 months(mean 6 months).There was no recurrence,no post-operative persistent painfulness,no obvious seroma and no mesh-related infection in the series,while 3 patients complained of painfulness around the operative field;4 patients had temporary abdominal distention;4 patients had concomitant defection disorders;the patients were treated according to the symptoms and had well recovered.Conclusions Laparoscopic parastomal hernia repair and re-ostomy technique is feasible,safe,and effective.
