Abstract:Objectives Cognitive dysfunction has been identified as a major symptom of a series of psychiatric disorders. Multidisciplinary studies have shown that cognitive dysfunction is monitored by a two-way interaction between the neural and immune systems. However, the specific mechanisms of cognitive dysfunction in immune response and brain immune remain unclear. Materials and methods In this review, we summarized the relevant research to uncover our comprehension of the brain-immune interaction mechanisms underlying cognitive decline. Results The pathophysiological mechanisms of brain-immune interactions in psychiatric-based cognitive dysfunction involve several specific immune molecules and their associated signaling pathways, impairments in neural and synaptic plasticity, and the potential neuro-immunological mechanism of stress. Conclusions Therefore, this review may provide a better theoretical basis for integrative therapeutic considerations for psychiatric disorders associated with cognitive dysfunction.

Abstract:Neuropsychiatric diseases have traditionally been studied from brain, and mind-centric perspectives. However, mounting epidemiological and clinical evidence shows a strong correlation of neuropsychiatric manifestations with immune system activation, suggesting a likely mechanistic interaction between the immune and nervous systems in mediating neuropsychiatric disease. Indeed, immune mediators such as cytokines, antibodies, and complement proteins have been shown to affect various cellular members of the central nervous system in multitudinous ways, such as by modulating neuronal firing rates, inducing cellular apoptosis, or triggering synaptic pruning. These observations have in turn led to the exciting development of clinical therapies aiming to harness this neuro-immune interaction for the treatment of neuropsychiatric disease and symptoms. Besides the clinic, important theoretical fundamentals can be drawn from the immune system and applied to our understanding of the brain and neuropsychiatric disease. These new frameworks could lead to novel insights in the field and further potentiate the development of future therapies to treat neuropsychiatric disease.

Abstract:"We are all in this together" - we often hear this phrase when we want to flag up a problem that is not for a single individual but concerns us all. A similar reflection has been recently made in the field of mental disorders where brain-centric scientists have started to zoom out their brain-focused graphical representations of the mechanisms regulating psychiatric diseases to include other organs or mediators that did not belong historically to the world of neuroscience. The brain itself - that has long been seen as a master in command secluded in its fortress (the blood brain barrier), has now become a collection of Airbnb(s) where all sorts of cells come in and out and sometimes even rearrange the furniture! Under this new framework of reference, mental disorders have become multisystem pathologies where different biological systems - not just the CNS -contribute 'all together' to the development and severity of the disease. In this narrative review article, we will focus on one of the most popular biological systems that has been shown to influence the functioning of the CNS: the immune system. We will specifically highlight the two main features of the immune system and the CNS that we think are important in the context of mental disorders: plasticity and memory.

Abstract:A growing number of studies are pointing out the need for a conceptual shift from a brain-centered to a body-inclusive approach in mental health research. In this perspective, the link between the immune and the nervous system, which are deeply interconnected and continuously interacting, is one of the most important novel theoretical framework to investigate the biological bases of major depressive disorder and, more in general, mental illness. Indeed, depressed patients show high levels of inflammatory markers, administration of pro-inflammatory drugs triggers a depressive symptomatology and antidepressant efficacy is reduced by excessive immune system activation. A number of molecular and cellular mechanisms have been hypothesized to act as a link between the immune and brain function, thus representing potential pharmacologically targetable processes for the development of novel and effective therapeutic strategies. These include the modulation of the kynurenine pathway, the crosstalk between metabolic and inflammatory processes, the imbalance in acquired immune responses, in particular T cell responses, and the interplay between neural plasticity and immune system activation. In the personalized medicine approach, the assessment and regulation of these processes have the potential to lead, respectively, to novel diagnostic approaches for the prediction of treatment outcome according to the patient's immunological profile, and to improved efficacy of antidepressant compounds through immune modulation.

Abstract:This symposium, organized by the ECNP Network for Immuno-NeuroPsychiatry, aims to show where immunoneuropsychiatry stands on the road to clinical application and to identify remaining obstacles. Immune processes contribute to the functioning of the central nervous system, but the risk of developing mental illness is greater in patients with dysfunctions of the immune mechanisms. Some brain diseases, such as autoimmune encephalitis, may even be causally induced by immune dysfunction, for example, when autoantibodies bind to brain receptors and disturb their function. The detection of such processes in psychiatry is highly significant for clinicians, as patients with autoimmune-mediated diseases can potentially be treated by immunomodulatory drugs (Pollak et al., 2020, Lancet Psychiatry). Prof. Marion Leboyer will act as discussant to moderate and integrate the three presentations: In the first presentation, Livia de Picker (from Duffel/Belgium) provides insights into the patterns of microglial activation across different brain disorders. In particular, she presents the results from a transdiagnostic meta- analysis of 156 human CNS TSPO PET case-control studies (De Picker et al., 2023, in review) and the findings from a human TSPO PET study and post-mortem study of patients with psychosis (De Picker et al., 2019 and 2021, Brain, Behavior, and Immunity). Next, Dominique Endres (from Freiburg/Germany) will present exemplary cases and the first case series and clinical experiences with immunotherapies from patients with suspected autoimmune encephalitis and isolated psychiatric manifestations (Endres et al., 2022, Molecular Psychiatry). He will also show that the spectrum of autoantibodies against brain structures may be broader in psychotic patients than initially believed (Endres et al., 2022, Biological Psychiatry). In the last presentation, Michael E. Benros (from Copenhagen, Denmark) will provide an overview of biomarkers for autoimmune psychiatric syndromes, with particular reference to recent results from cerebrospinal fluid (CSF) studies. Abstract:Cognitive impairment is a devastating condition commonly observed with normal aging and neurodegenerative disorders such as Alzheimer's Disease (AD). Although major efforts to prevent or slow down cognitive decline are largely focused within the central nervous system (CNS), it has become clear that signals from the systemic milieu are closely associated with the dysfunctional brain. In particular, the bidirectional crosstalk between the CNS and peripheral immune system plays a decisive role in shaping neuronal survival and function via neuroimmune, neuroendocrinal and bioenergetic mechanisms. Importantly, it is emerging that some neuroprotective and cognition-strengthening drugs may work by targeting the brain-periphery interactions, which could be intriguingly achieved without entering the CNS. We describe here how recent advances in dissecting cognitive deficits from a systems-perspective have contributed to a non-neurocentric understanding of its pathogenesis and treatment strategy. We also discuss the therapeutic and diagnostic implications of these exciting progresses and consider some key issues in the clinical translation. Abstract:From ancient time, scientists are striving to improve physical health which in turn means the state of psychological health is often overlooked in term of close connection with CNS and immunology. The importance of healthy emotional state is vital from individual self to global community. A challenging question that scientists face pertains to “how to keep healthy status from birth to death of each person in term of physiology and mind”? What is the vital key factor of the one’s inner soul that is influential enough to be connected with our biological brain? This study can be explored further by utilizing technology commonly used for physiological diagnostic purposes. One physiological area to understand the emotional mind can be through exploring the biochemistry occurring in the brain that is the linkage between CNS and immune system combined with physical and mental health. However, the field of human psychology is a complex phenomenon that brings new challenges as to how to objectively observes, measure and manipulate towards positivity. Nevertheless, the importance to pursue such study does not diminish in its value; because the state of psychological conditions have been perceived by psychiatrist, philosophers and clinical psychologist to be of a global health issues until the last day of this world.

Inflammation-Related Functional and Structural Dysconnectivity as a Pathway to Psychopathology

Abstract:Findings from numerous laboratories and across neuroimaging modalities have consistently shown that exogenous administration of cytokines or inflammatory stimuli that induce cytokines disrupts circuits and networks involved in motivation and motor activity, threat detection, anxiety, and interoceptive and emotional processing. While inflammatory effects on neural circuits and relevant behaviors may represent adaptive responses promoting conservation of energy and heightened vigilance during immune activation, chronically elevated inflammation may contribute to symptoms of psychiatric illnesses. Indeed, biomarkers of inflammation such as cytokines and acute phase reactants are reliably elevated in a subset of patients with unipolar or bipolar depression, anxiety-related disorders, and schizophrenia and have been associated with differential treatment responses and poor clinical outcomes. A growing body of literature also describes higher levels of endogenous inflammatory markers and altered, typically lower functional or structural connectivity within these circuits in association with transdiagnostic symptoms such as anhedonia and anxiety in psychiatric and at-risk populations. This review presents recent evidence that inflammation and its effects on the brain may serve as one molecular and cellular mechanism of dysconnectivity within anatomically and/or functionally connected cortical and subcortical regions in association with transdiagnostic symptoms. We also discuss the need to establish reproducible methods to assess inflammation-associated dysconnectivity in relation to behavior for use in translational studies or biomarker-driven clinical trials for novel pharmacological or behavioral interventions targeting inflammation or its effects on the brain.
Inflammatory Cytokines Mediate Brain Dysconnectivity and Cognitive Deficits Across Major Psychiatric Disorders

Abstract:Background: Cognitive impairment influenced by brain dysconnectivity and inflammatory cytokines has consistently been the most prominent feature across major psychiatric disorders (MPDs) including schizophrenia, bipolar disorder, and major depressive disorder. The commonalities call upon a trans-diagnostic and dimensional approach in delineating the neural mechanisms underlying the relationship between brain connectivity, cognition, and inflammation among MPDs.Methods: 1543 participants, including 1058 with MPDs and 485 healthy controls, from a single site were enrolled in this study. Participants underwent diffusion tensor imaging and resting-state functional magnetic resonance imaging, with the majority of participants (n = 1169, 765 participants with MPDs and 404 healthy controls) also completing the Wisconsin Card Sorting Test (WCST). Blood samples were obtained from 325 participants within 24 h of imaging. Fractional anisotropy (FA) and regional functional connectivity (FC) were used to measure structural and functional connectivity, respectively.Findings: Differences in inflammatory cytokine indices were assessed using IL-1β, IL-6 and TNF-α. WCST deficits were associated with decreased FA; increased regional FC; and elevated IL-1β, IL-6 and TNF-α among MPDs. Overlapping structural and functional dysconnectivity within the frontolimbic-striatal regions contributed to cognitive deficits among MPDs. Mediation analyses revealed that IL-1β mediated the association between FA and WCST and that IL-6 mediated the relationship between regional FC and WCST.Interpretation: Our findings implicate an intriguing mechanism for the effects of inflammation on cognitive function through different molecular-brain-cognition pathways among MPDs. Abstract:Current views on immunity support the idea that immunity extends beyond defense functions and is tightly intertwined with several other fields of biology such as virology, microbiology, physiology and ecology. It is also critical for our understanding of autoimmunity and cancer, two topics of great biological relevance and for critical public health considerations such as disease prevention and treatment. Central to this review, the immune system is known to interact intimately with the nervous system and has been recently hypothesized to be involved not only in autonomic and limbic bio-behaviors but also in cognitive function. Herein we review the structural architecture of the brain network involved in immune response. Furthermore, we elaborate upon the implications of inflammatory processes affecting brain-immune interactions as reported recently in pathological conditions due to SARS-Cov-2 virus infection, namely in acute and post-acute COVID-19. Moreover, we discuss how current neuroimaging techniques combined with ad hoc clinical autopsies and histopathological analyses could critically affect the validity of clinical translation in studies of human brain-immune interactions using neuroimaging. Advances in our understanding of brain-immune interactions are expected to translate into novel therapeutic avenues in a vast array of domains including cancer, autoimmune diseases or viral infections such as in acute and post-acute or Long COVID-19.
