Abstract:Objective:To investigate the clinical pathological and immunohistochemistrical features of primary malignant fibrous histiocytoma(MFH) of breast and its prognosis.Methods:To analyze the date of 1 case of MFH of breast and with literature review.Results:The histocytoma tissue consists of fibrous cells,fibroblastoma and xanthoma cells.Immunohistochemistry results showed that the expression of CD68 and Vimentin were both positive,the proliferation index of Ki67 was 38.38%.Conclusion:Prognosis of MFH is related to tumor size,position and depth.

Abstract:Abstract Background Malignant fibrous neoplasms (MFN) of long bones are rare lesions. Moreover, the prognostic determinants of MFN of long bones have not been reported. This study aimed to present epidemiological data and analyse the prognostic factors for survival in patients with MFN. Materials and methods The Surveillance, Epidemiology, and End Results (SEER) programme database was used to screen patients with malignant fibrous neoplasms (MFN) of long bones from 1973 to 2015, with attention to fibrosarcoma, fibromyxosarcoma, periosteal fibrosarcoma and malignant fibrous histiocytoma. The prognostic values of overall survival (OS) and cancer-specific survival (CSS) were assessed using the Cox proportional hazards regression model with univariate and multivariate analyses. The Kaplan–Meier method was used to obtain OS and CSS curves. Results A total of 237 cases were selected from the SEER database. Malignant fibrous histiocytoma was the most common form of lesion in long bones. Multivariate analysis revealed that independent predictors of OS included age, stage, tumour size and surgery. Age, stage, tumour size and surgery were also independent predictors of CSS. Additionally, the most significant prognostic factor was whether metastasis had occurred at the time of initial diagnosis. Conclusion Among patients with MFN of long bones, age (> 60 years), tumour size (> 10 cm), distant stage, and non-surgical treatment are factors for poor survival.

Abstract:Objective:To discuss the clinical features and diagnosis of malignant fibrous histiocytoma(MFH).Methods:A case was reported that we had treated one patient who suffered primary malignant fibrous histiocytoma of distal femur with cerebral ventricle transference and review of literature.Results:The clinical features of MFH are not characteristic.It's difficult to diagnos with clinic and imageoloy.The diagnosis of MFH mostly depends on the pathologic histology.The local recurrence and beyond transference will occur after treatment.Conclusion:If patients have bone ache,mero-lump and the iconography features such as erose or macro-osteolytic lesion or macro-lump of parenchyma which have no periosteal reaction,they should be considered to this disease.

Abstract:Purpose To study the clinicopathologic features of inflammatory myofibroblastic tumor (IMT) of long bone.Methods HE and immunohistochemistry of EnVision two-step were used to observe the clinical,radiological,histological and immunophenotype features of IMT of bone.The literatures were reviewed.Results 4 cases of IMT of bone were respectively located in the tibia (2 cases) and femur (2 cases).Histologically,the lesions were characterized by collagen-rich and spindled to plump myofibroblast-like cells and a variable admixture of inflammatory cells.Immunohistochemical study showed that the vimentin,SMA,actin,H-caldesmon and CD34 were positive.Conclusion The IMT is a rare and locally aggressive tumor.The diagnosis should combine the histological characters with immunohistochemical results and should be differentiated from the other tumors and tumor-like lesions.

Abstract:Background.Malignant fibrous histiocytoma (MFH) of the bone is a rare tumor.Most studies comparing limb salvage and amputation have reported that limb salvage had no adverse effect on the long-term survival of patients.This study evaluates the oncological outcomes of limb salvage procedures that were used for 15 patients with MFH of the humerus.Objectives.The aim of this study was to assess the functional and oncological outcomes of patients with malignant fibrous histiocytoma of the humerus after en bloc resection and prosthesis implantation. Material and methods.A retrospective review of the charts of 15 patients who had undergone resection of malignant fibrous histiocytoma of the humerus followed by reconstruction with prosthesis was used in this study.A functional evaluation was based on Enneking's modified system of the functional evaluation of surgical management for musculoskeletal tumors.Complications of the procedures were also analyzed.Results.Eight men and 7 women at an average age of 52.9 years were included in the study.The tumor involved the distal humerus in 3 patients, the proximal humerus in 8 patients and the mid-shaft humerus in 4 patients.Excellent results were achieved in 4 patients, good to fair in 10 and poor in 1.One patient had local recurrence.Pulmonary metastases occurred in 6 patients.Conclusions.Limb salvage surgery with chemotherapy is a viable treatment option for patients with malignant fibrous histiocytoma of the humerus.

Abstract:OBJECTIVE:To study the risk factors related to the survival rate, recurrence and metastasis of malignant fibrous histiocytoma of bone.METHODS:From July 1997 and July 2010, 56 patients with malignant fibrous histiocytoma of bone were treated. Univariate and multivariate analysis were performed to determine the probable risk factors including gender, age, tumor location, tumor size and so on.RESULTS:Forty-four cases were followed up ranged from 2 weeks to 78 months (medium 33.3). The 5-year overall survival rate was 50.1%, local recurrence rate 40.9% with a median time of 12 months (3 to 60 months) and metastatic rate 27.5% (11/40) with a median time of 6.5 months (2 to 23 months). Univariate analysis indicated that gender, condition of presentation (primary case or recurrence case), tumor location, surgical margin and surgical stage were significantly related to survival rate (P < 0.05), and tumor location and surgical margin were related to local recurrence rate (P < 0.05), and important vessel or nerve invasion was related to metastatic rate (P < 0.05). Multivariate analysis showed that surgical margin and surgical stage were independent risk factors for survival rate, of which surgical margin was the independent risk factor for recurrence rate.CONCLUSIONS:Surgical margin and surgical stage are independent risk factors for survival rate, of which surgical margin is the independent risk factor for recurrence rate.

Abstract:We present a case of malignant fibrous histiocytoma located in the region of the temple that corresponded to the main site of a traumatic hematoma. There are reports of malignant transformation triggered by different kinds of wounds, but the malignant transformation in a subcutaneous hematoma is rare. For chronic expanding hematoma, magnetic resonance imaging or biopsy is recommended in determining whether it is malignant, especially when the mass is growing rapidly.

Abstract:Abstract： Malignant fibrous histiocytoma (MFH) is a malignant tumor composed of a mixture of fibroblastic and histiocytic cells. A rare case of primary MFH of the breast was reported and thirty-eight Chinese cases in recent twenty-two years were reviewed. Their clinical features and survival information were described and compared with those in English literatures. The 5-year survival rate of the Chinese MFH patients was 57.1%. Large tumor and skin invasion were related with bad prognosis. Simple mastectomy is recommended especially for orient patients.

Abstract:OBJECTIVE:To probe into the diagnosis and treatment of malignant fibrous histiocytoma (MFH) of the prostate.METHODS:One case of MFH of the prostate was diagnosed and treated surgically by radical remocal of the lesion, resection of the rectum and sigmoidoctomy. The clinical features, diagnosis and treatment of MFH of the prostate were reviewed and presented.RESULTS:The patient experienced recurrence 3 weeks after operation, and then underwent radiotherapy. Two months after operation the patient died of metastasis.CONCLUSION:MFH of the prostate has been rare clinically, with poor prognosis. Early and aggressive surgical removal of the primary lesion followed by adjuvant therapy is recommended for the treatment of the disease.

Abstract:Background: Amputation has been regarded as the standard surgical treatment for distal fibula osteosarcoma. With the advances in surgery and adjuvant chemotherapy, it have made limb salvage possible. However, the choice of a specific reconstruction procedure is frequently based on the surgeon's preference and it lacks of guidelines and high quality studies with the objective result on the subject. Materials and methods: Six patients with the distal fibular osteosarcoma which were received biological reconstruction were retrospectively reviewed at our bone tumor center from November 2003 to November 2015. There were 6 male with a mean age of 24.2 years (range, 12-47 years). The minimum follow-up duration was 53.3 months (median, 96.3 months; average, 108.4 months; range, 53.3-204.1 months). No patient was lost at the last follow-up. All data were obtained from the clinical and radiograph records. Furthermore, the literature review was based on the Google Scholar, Medline, EMBASE and Pubmed databases. The search was performed using the terms "distal fibula", "lower limb tumour", "sarcoma", "fibular metastasis" and "limb-salvage surgery" for the literature review from 1979 to 2017. Results: Of the six patients with the final follow-up in the present study, four cases (83.3%, 4/6) achieved excellent prognosis without oncologic complications. A second surgical procedure was performed in two patients, one to treat local recurrence and one to receive metastasectomy due to the pulmonary metastasis. Case 6 received the below knee amputation due to recurrence. All the cases available for functional evaluation at the final follow-up had a mean functional MSTS score of 29.6 (range, 28-30) except case 6 receiving the amputation below the knee. Meanwhile, the VAS evaluation had a mean functional score of 0.2 points (range, 0-1 points). The results of our cohort and literature review illustrated that the patient with the malignant tumor of lateral malleolus could obtain excellent oncological prognosis and ankle function. Conclusions: We have provided treatment recommendations depending on the tumor volume and associated extent and proposed the primary ankle arthrodesis was performed after en bloc resection of Type II and III lateral malleolus osteosarcoma, based on the proposed classification. Furthermore, the patient with the malignant tumor of lateral malleolus could obtain excellent oncological prognosis. (C) 2019 European Foot and Ankle Society. Published by Elsevier Ltd. Abstract:Objective To report 2 cases of special types of amputation in patients with foot malignant soft tissue tumors.Methods From January 2008 to December 2012,2 cases of special types of amputation in patients with foot malignant soft tissue tumors were conducted.Case 1 of rhabdomyosarcoma in the right forefoot was performed with Chopart amputation.Case 2 of squamous cell carcinoma of the right plantar was performed with Syme amputation.Results Two surgeries went successfully.Case 1 was followed up for 23 months.Case 2 was followed up for 22 months.No recurrence occurred after the surgery.Patients could walk well by prosthesis,with good recovery of lower limb functions and no significant adverse complications.Conclusions In the premise of extensive tumor resection,Chopart and Syme amputation are good options in the treatment of foot malignant soft tissue tumors,which can preserve the limb length and functions to the greatest degree and further improve the life quality of patients.
Malignant transformation of primary ameloblastoma of skull: case report and review of current literature

Abstract:Background: Since 1964, there has been a scarcity of reported cases of primary ameloblastoma (AM) or ameloblastic carcinoma (AMCa) of the skull. The clinical presentation and distinctive features of this uncommon condition at specific anatomical sites remain unclear. We report a case of malignant transformation of a primary AM of the skull situated in the frontal-temporal-parietal region and highlight its similarities to other cases reported in the literature. Clinical presentation: A 53-year-old female patient presented with a 20-day history of headaches and bilateral lower limb weakness for 10 days. Physical examination revealed slow and unsteady gait. An occupying lesion was observed in the right frontal-temporal-parietal region of the skull on the Cranial imaging. A right cranial bone tumor margin expansion resection was performed. The patient's motor functions recovered normally after surgery. Postoperative imaging examinations showed10 tumor resection. Follow-up imaging examinations showed tumor recurrence. The patient underwent resection of the recurrent tumor. Postoperative pathological analysis revealed malignant transformation of the AM.Follow-up imaging examinations showed tumor recurrence again. The patient was admitted for stereotactic radiotherapy. Follow-up imaging examinations demonstrated no evidence of tumor recurrence and subsequent chest CT revealed no signs of metastasis. Conclusion: Primary AM or AMCa of the skull is increasingly being described in the literature, but detailed reports on the malignant transformation of primary AM of the skull are lacking. The pathogenesis of this condition remains unclear. Aggressive treatment and close follow-up may be crucial for preventing disease recurrence and malignant transformation.

Abstract:Abstract Benign fibrous histiocytoma of the bone (BFHB) is a rare mesenchymal tumor, representing less than 1% of all benign bone tumors. This controversial entity is characterized by a mixture of fibroblasts arranged in a storiform pattern, varying amounts of osteoclast-type giant cells and foamy macrophages. Curettage or simple resection is usually curative. However, it was reported that up to 11% of the patients suffer from relapse. Here, we report a case of malignant transformation of BFHB after long-lasting disease stabilization under denosumab therapy.

Surgical Options and Reconstruction Strategies for Primary Bone Tumors of Distal Tibia: A Systematic Review of Complications and Functional Outcome.

Abstract:BACKGROUND:Primary malignant or aggressive benign bone tumors rarely occur in distal tibia, and limb salvage remains the mainstay of surgical options. However, reconstruction methods for large bone defect after wide tumor resection in this location are debatable. The purpose of this systematical review is to critically evaluate each reconstruction method regarding the postoperative complications and functional outcome.METHODS:A systematic review of the 33 studies including 337 cases with tumors affecting distal tibia was performed after searching the PubMed and EMBASE databases. Pooled descriptive statistics with separate analyses for postoperative complications and functional outcome of different reconstruction options were performed.RESULTS:290 (86.1%) patients received limb salvage procedures. Reconstruction strategies including biological reconstruction, such as autograft, allograft, distraction osteogenesis and non-biological prosthetic replacement. The patients received limb salvage procedures tended to have a higher MSTS score (77.1% vs 70.9%, P = .055) and a higher incidence of local relapse (28/290 vs 0/47, P = .052) than those amputated. Biological reconstruction methods provided better functional outcome (78.4% vs 72.2%, P = .017) compared with non-biological prosthetic reconstruction, although similarity of incidence of major complications (51/253 vs 12/37, P = .091). With respect to the comparison between autograft and allograft reconstruction, the autograft seemed to have less major postoperative complications occurrence (27/165 vs 22/78, P = .032), and consequently better functional outcome (MSTS score, 80.2% vs 74.3%, P = .025) than allograft reconstruction.CONCLUSIONS:Limb salvage results in better functional outcome compared with amputation. Biological reconstruction is more advocated than prosthetics replacement, and furthermore, autograft might be suggested to be the optimal reconstructive method with regard to better postoperative functional outcome and less major complications.

Abstract:Background and aim: Hemosiderotic fibrolipomatous tumor (HFLT) is a locally invasive tumor composed of mature adipocytes accompanied by spindle cells containing hemosiderin deposition. In 2013, it was categorized by WHO as a soft tissue tumor with uncertain differentiation. So far, the literature has reported 60 cases but primary HFLT in bone has never before been reported. We set out to investigate the clinicopathological features of primary HFLT in bone. Methods: We retrospectively reviewed the clinical, imaging, histological, and immunophenotypic features and treatment of 1 case of primary HFLT in bone, and combined this with literature discussion. Results: HFLT occurred in the lateral femoral condyle of a 50-year-old male patient, which might have been overlooked were it not for the knee-joint pain and dysfunction. CT and MRI showed osteolytic bone destruction with a clear 4.0-cm diameter boundary, diagnosed as cystic damage of the lateral condyle of the left femur. SPECT metabolism was not active. Histologically, the lesion was composed of different proportions of mature fat cells, spindle cells, and hemosiderin. Immunohistochemistry revealed spindle cells expressing vimentin, p63, but not CD34, calponin, and others. The tumor tissue was thoroughly removed by curetting, and a bone graft was carried out after immersion in anhydrous ethanol. At the 11-month postoperative follow-up, the patient was recovering well. Conclusions: Primary HFLT in bone is extremely rare. In imaging, it can easily be misdiagnosed as a bone cyst. Histological morphology of the current case is similar to that of soft tissue HFLT.
Myositis ossificans mimicking bone surface osteosarcoma: case report with literature review

Abstract:Myositis ossificans, a benign tumor composed of spindle cells and osteoblasts, can clinically and radiologically mimic osteosarcoma. While recognition and accurate diagnosis of myositis ossificans can be a challenge, this is critical as it may allow a conservative surgical approach to maximize functional outcomes. Herein, we present a patient with surface myositis ossificans confirmed genetically by the presence of COL1A1::USP6 gene fusion, along with a literature review. Due to the enhanced visualization of the bone matrix, computed tomography (CT) imaging may be a superior imaging modality to magnetic resonance (MR) imaging. Staged biopsies with samples obtained from the periphery and center of the lesions may allow pathologists to discern the zonal distribution histologically. Furthermore, immunohistochemistry fluorescence in situ hybridization and molecular testing can aid in the distinction of myositis ossificans from mimics. Because of their resemblance to other bone tumors, these cases of myositis ossificans highlight the importance of a multidisciplinary approach integrating clinical, radiologic, and pathologic analysis and involving serial imaging, sampling, and judicious use of ancillary immunohistochemical and molecular testing.
Novel “Double-Strut” Fibula Ankle Arthrodesis for Large Tumor-Related Bone Defect of Distal Tibia

Abstract:Reconstruction for large bone defect of distal tibia after wide resection of tumor is difficult, and the best option remains controversial. This study presents a novel “double-strut” fibula ankle arthrodesis for this issue. Nine patients with malignant or aggressive tumors of distal tibia underwent novel “double-strut” fibula ankle arthrodesis after wide tumor resection were retrospectively reviewed. We assessed the bone union time, complications and oncology outcome clinically and radiographically. The Musculoskeletal Tumor Society (MSTS) score and the Foot and Ankle Outcome Score (FAOS) were used to evaluate the functional outcome. The average followup period was 53 ± 46 months. There was no deep infection or graft fracture observed in this series. Internal fixation loosening was found in one case. In these patients, eight achieved union at both proximal and distal junctions, while one achieved union only distally. The mean union time of the proximal junctions and distal junctions was 10.5 ± 1.6 months and 8.7 ± 2.3 months, respectively. The mean postoperative MSTS score was 83% ± 8%. The subscales of FAOS indicating the most problem was Sport and Recreation Function with a mean score of 18 ± 11. At the final follow-up, one of them (1/9, 11%) experienced local recurrence in soft tissue and received another resection surgery, and four (4/9, 44%) patients developed lung metastases. For large bone defect of distal tibia, this novel “double-strut” fibula reconstruction can be a viable alternative, which is capable of achieving durable ankle fusion and functional salvaged limb with low rate of complications.
Surgical Treatment of Primary Malignant Tumours of the Distal Tibia: Clinical Outcome and Reconstructive Strategies.

Abstract:AIMS:We retrospectively report our experience of managing 30 patients with a primary malignant tumour of the distal tibia; 25 were treated by limb salvage surgery and five by amputation. We compared the clinical outcomes of following the use of different methods of reconstruction.PATIENTS AND METHODS:There were 19 male and 11 female patients. The mean age of the patients was 19 years (6 to 59) and the mean follow-up was 5.1 years (1.25 to 12.58). Massive allograft was used in 11 patients, and autograft was used in 14 patients. The time to union, the survival time of the reconstruction, complication rate, and functional outcomes following the different surgical techniques were compared. The overall patient survival was also recorded.RESULTS:Out of 14 patients treated with an autograft, 12 (86%) achieved union at both the proximal and distal junctions. The time to union at both junctions of the autograft was significantly shorter than in those treated with an allograft (11.1 vs 17.2 months, p = 0.02; 9.5 vs 16.2 months, p = 0.04). The complication rate of allograft reconstruction was 55%. The five patients treated with an amputation did not have a complication. Out of the 25 patients who were treated with limb salvage, three (12%) developed local recurrence and underwent amputation. The mean functional Musculoskeletal Tumor Society (MSTS) score after autograft reconstruction was higher than after allograft reconstruction (81% vs 67%; p = 0.06), and similar to that after amputation (81% vs 82%; p = 0.82). The two- and five-year overall rates of survival were 83% and 70%, respectively.CONCLUSIONS:This consecutive case series supports the safety of limb salvage and the effectiveness of biological reconstruction after the resection of a primary tumour of the distal tibia. Autograft might be a preferable option. In some circumstances, below-knee amputation remains a valid option.
