Abstract:( Anesth Analg . 2020;130:187–193) When performing a cesarean delivery, anesthesiologists are most likely to use spinal anesthesia or combined spinal-epidural anesthesia, although these methods are associated with high rates of hypotension. Phenylephrine is considered to be an acceptable vasopressor for hypotension prevention, but not much research has been done to determine the optimal dose for prophylactic phenylephrine infusions. The few studies that have examined phenylephrine infusions did not use infusion rates that had been properly adjusted for patient body weight, so more research is warranted. These investigators conducted their study to examine the dose-response relationship for a fixed-rate, weight-based phenylephrine infusion under combined spinal-epidural anesthesia during cesarean delivery. In addition, the authors aimed to determine the infusion dose that would adequately prevent hypotension for 50% (ED50) and 90% (ED90) of patients.
