Abstract:Lichen-planus-like keratosis is usually diagnosed pathologically; rarely, a definitive diagnosis can be made grossly in the clinic. Only a few cases of lichen-planus-like keratosis have been reported in China. The purpose of this study was to investigate the clinical and pathological features of lichen-planus-like keratosis in China. Fifty cases of lichen-planus-like keratosis patients diagnosed pathologically during a 5-year period in our clinic were analyzed. Clinical features were recorded. Sectioned specimens were subjected to hematoxylin and eosin staining to observe pathological changes. Results showed that there were 34 males and 16 females (ratio 2:1) with an average age of 61.2 years. Most of the lesions were single papules or plaques with rough surfaces. They were distributed on the face, larger than 1 cm, and dark red to brown in color. Only one case (2%) was considered to be lichen-planus-like keratosis clinically. By hematoxylin and eosin staining, solar lentigo and solar elastosis could be found in 68% and 32% of lichen-planus-like keratosis lesions, respectively. Eosinophil (42%) and plasma cell (36%) infiltration was also found frequently. Exoerythrocytes could be detected in 50% of the cases. Lichen-planus-like keratosis is not uncommon in clinical practice in China, the diagnosis of lichen-planus-like keratosis should be made by a combination of clinical manifestations and pathological changes. It is better to classify lichen-planus-like keratosis as a benign skin tumor. More attention should be paid to lichen-planus-like keratosis in China.
Clinical Features and Skin Microbiome of Tinea Scrotum: an Observational Study of 113 Cases in China

Abstract:Background The scrotum is considered as an uncommon site for tinea, hence there is a lack of knowledge about the clinical characteristics, pathogenic agents and the skin microbiome changes of tinea scrotum. Objective We sought to analyze the clinical features, pathogenic agents and skin microbiome of tinea scrotum. Methods A two-center prospective observational study was carried out in outpatient dermatology clinics in Zhejiang, China, from September 2017 to September 2019. The diagnosis of tinea scrotum was confirmed by direct microscopy. Clinical and mycological data were collected. The composition of microbial communities of patients with tinea scrotum was analyzed and compared with healthy controls. Results A total of 113 patients with tinea scrotum were included. Tinea scrotum was either presented with isolated lesions (9/113, 8.0%) or accompanied by tinea of other sites (104/113, 92.0%). Tinea cruris was detected in 101 cases (89.38%). Fungal culture was positive in 63 cases, among which Trichophyton rubrum was grown in 60 cases (95.2%) and Nannizzia gypsea was cultured in 3 cases (4.8%). The skin microbiome in scrotum lesions from 18 patients showed increased abundance of Trichophyton compared with 18 healthy individuals, while Malassezia was decreased. No significant difference in bacterial diversity was found. Conclusions Tinea scrotum was often companied by superficial fungal infections of other skin sites, with tinea cruris being the most common condition. Instead of N. gypsea , T. rubrum was the most frequently identified pathogen for tinea scrotum. In general, tinea scrotum exhibited changes in the fungal communities of the skin with increased Trichophyton and decreased Malassezia abundance.

Abstract:Objective To understand the clinic features of Chinese patients with lichen planus pigmentosus (LPP). Methods We report 2 patients with LPP,and select 10 other LPP cases which reported in recent years,than accomplish the meta-analyze among these cases. Results There are 6 males and 4 females among 10 LPP patients,with different clinic feature and dissatisfied curative effect. Conclusions There were no significant differences between males and females in incidence rate,with most onset in adult and chronic process; there were significant heterogeneity in phenotype of this disease and many clinic subgroups; the typical histopathology characteristics of the disease is liquation in basal cell (generally or local),infiltrated by melanocytes,lymphocytes (surrounding or along with basement membrane zone),and showed difference in epidermal layer,which potentially caused by the different lesion of biopsy we got and presented earlier stage and advanced stage.

Abstract:The pathogenesis of striae distensae (SD) is complicated and has not yet been fully elucidated. Hormonal changes, overstretched skin, and structural and functional changes in the skin may be crucial factors in the development of SD. Therapy aims to stimulate dermal collagen synthesis and improve skin texture. Mainstream treatments include topical medications, chemical peeling, laser and radiofrequency therapy, microdermabrasion, microneedle therapy, and filler injection therapy. In the present review, we summarize current perspectives on the pathogenesis and clinical therapy of SD.

Abstract:Objective: To promote the understanding of atrophie blanche and improve the diagnosis and treatment. Methods: Thirteen cases of atrophie blanche were retrospectively studied, by reviewing the clinical manifestations, laboratory examination results, therapeutic regimens and the follow-up data. Results: The symptoms of all the patients, most of them were young female, were more severe in the summer than those in the winter. The main skin lesions were red or mauve macules, patches, papules, or vesicles on both lower limbs, especially on the ankles and dorsa of the feet, which gradually became ulcerated, exudative and crusted. These lesions eventually became ivory atrophic scars, surrounded by telangiectasia or small glomus tumor-like appearance. Livedo reticularis on the lower extremities were seen in some patients. There were no specific laboratory findings. The histopathology of the skin specimens were consistent with the changes of livedo vasculitis. The patients were effectively treated with oral danazol, dipyridamole, low dose of aspirin, or DANSHEN dripping pills, and topical application of sodium heparin cream or mucopolysaccharide polysulfate cream. But the relapses were seen in most cases after stopping of the treatment. Conclusion: The diagnosis of atrophie blanche is mainly based on clinical manifestations and histopathologic findings. The remission could be successfully achieved by the medications but the relapses are often seen.

Abstract:Lichen sclerosus (LS) was first described in women by a researcher in 1887.1 It was recognized in men by another investigator in 1928.2 Lichen sclerosus is a chronic, inflammatory lymphocytic dermatosis that occurs in anywhere from 1:30 to 1:1,000 adults.3,4 There is a slight predominance of women, with a bimodal age distribution in pre-pubertal individuals and again in life's sixth-seventh decades. Studies have established that the majority with pre-pubertal onset continue to have adulthood disease.5 Psychosocial implications of this disease, specifically self-image, anxiety, and sexual function, can be debilitating for patients.6-9 As no cure has been described for lichen sclerosus. Treatment is aimed at symptomatic relief and preventing additional effacement. Unfortunately, the scarring that occurs is usually permanent.10,11 As it is unclear whether treatment alters the theoretic risk of malignant degeneration, estimated at 4%-5%,12,13 frequent clinical examinations are indicated.14-17.

Abstract:Background The clinical similarity of lupus miliaris disseminatus faciei (LMDF) and other papular granulomatous facial disorders often makes its correct diagnosis challenging. Diagnosis often requires the assistance of pathological examination, and dermoscopy can be used as an auxiliary and non-invasive examination method, however, the current findings remain incomplete. Objectives This study aimed to summarize the clinical, histopathological and dermoscopic features of LMDF in the Chinese Han population and aiming to provide practical significance to correct diagnosis. Methods 109 patients of LMDF were collected in the Department of Dermatology, the Second Affiliated Hospital of Xi’an Jiaotong University from August 2015 to August 2023. The clinical and histopathological manifestations of all patients, as well as the dermoscopic image features of 44 cases, including background, follicular findings, vessels, and other structures, were summarized and evaluated. Results The most significant histopathological features of LMDF in 109 cases is epithelioid granulomatous infiltrate in the superficial dermis, with or without caseation. The most significant dermoscopic features of LMDF in all 44 cases were orange structureless background (30/44), follicular plug (32/44), follicular white scar-like area (32/44), unspecific linear vessels (24/44), linear vessels with branch (24/44) and white streaks (18/44). Conclusion Histopathologically, LMDF is characterized by the presence of epithelioid granulomatous infiltrate in the superficial dermis, with or without caseation. Dermoscopically, it exhibits a distinctive orange structureless background, follicular plug, follicular white scar-like area, nonspecific linear vessels, linear vessels with branches, and white streaks.

Abstract:Objective To report a group of cases characterized by pigmented symmetrical acrokeratoderma.Methods Sixteen patients characterized by pigmented syrnmetrical acrokeratoderma were collected from outpatients from May 1998 to September 2007 in the Department of Dermatology,Second Affiliated Hospital,Sun Yat-Sen University.General information on the sex,age,profession,family history of patients were documented.All patients underwent clinical and histopathological examinations.and were followed up for the assessment of treatment efficiency.Results Among these patients,14 were males and 2 were females,ranging in age from 9 to 54 years,with duration of disease varying from 2 months to 8 years(mean course 2.0±1.5 years).Characteristic manifestations were rough,scaling,brownish aubum or black maculopapular eruptions with mild pruritus.The lesions were symmetrically distributed on palms,dorsal digits and wrists of 16(100%)patients,elbows and knees of 2(12.5%)patients,and ankles of 3(18.8%)patients.Histopathologic examination revealed epidermal hyperkeratosis,acanthosis and papillomatous hyperplasia as well as dermal infiltration with a few lymphocytes.No heredity tendency was observed in any of the patients.The patients showed a poor response to,or experienced a relapse after treatment with oral tretinoins,nicotinamide,vitamin B6 as well as topical tretinoin ointment,glucocorticoid ointment,and so on.Condusion These cases may represent a new entity clinically characterized by pigmented symmetrical acrokeratoderma,and pathologically characterized by epidermal hyperkeratosis,acanthosis and papillomatous hyperplasia with unknown etiology.
Lichen planopilaris: A retrospective study of 32 cases in an Australian tertiary referral hair clinic

Abstract:Background/objectives: Lichen planopilaris is a primary lymphocytic cicatricial alopecia. Management of patients with lichen planopilaris is difficult due to a paucity of high-quality data on its epidemiology and pathogenesis and the efficacy of therapies. The purpose of this study was to report the characteristics and treatment outcomes of patients with lichen planopilaris in a tertiary referral centre. Methods: A retrospective review of medical records in patients with lichen planopilaris seen in the Hair Clinic at the Skin & Cancer Foundation Inc., Melbourne, from 2012 to 2016. Results: Altogether 32 patients with lichen planopilaris (29 women) were included. The onset age ranged from 17 to 77 years with a mean age of 55.2 ± 13.5 years. Scalp pruritus (84%) and perifollicular erythema (72%) were the most common presenting symptoms and signs, respectively. Lichen planopilaris involved the frontal scalp in 66% of patients, the parietal in 56%, and vertex scalp in 50%. There were wide variations in treatment response. Conclusion: Lichen planopilaris is characterised by a marked female predominance and clinically with pruritus, perifollicular erythema and perifollicular scale. The current range of treatments used produced mixed and often unsatisfactory results. Multicentre, prospective, randomised controlled trials are warranted to provide clearer data on efficacious treatment options.
Clinical Features and Histopathological Analysis of Oral Lichen Planus: an Analysis of 105 Chinese Patients.

Abstract:PURPOSE:To study the clinical and pathological characteristics of oral lichen planus (OLP) in a large sample. MATERIALS AND METHODS:A comprehensive analysis was conducted on 105 patients with oral lichen planus (OLP), considering various factors including sex, age, disease site, lesion type, lesion area, morphological characteristics, self-reported symptoms, and history of systemic diseases. Histopathological examination was performed for each patient, and the pathology results were analysed according to sex and age group. RESULTS:70.5% of the OLP patients were female, and OLP was most likely to occur in the cheek, followed by the tongue, lips, gums and palate. The patients with moderate pain according to the VAS score accounted for 60%. Thirty-nine percent of the OLP patients had a systemic disease, and the most common clinical type of OLP was nonerosive. Most of the pathological results showed liquefaction degeneration of basal cells and infiltration of lamina propria lymphocytes. There was no statistically significant difference in pathological manifestations between male and female patients, and there were statistically significant differences in pathological manifestations among different ages patients. CONCLUSION:This study analysed the sociodemographic data and clinical manifestations of 105 OLP patients to guide follow-up treatment planning and disease monitoring. Moreover, pathological manifestations should be analysed to avoid delayed treatment and to monitor for carcinogenesis. Furthermore, the correlation of pathological manifestations among OLP patients with different sexes and ages is conducive to further research on the specific differential manifestations and possible underlying mechanisms involved.

Abstract:Objective To analyze clinicopathological features of parapsoriasis.Methods Clinical and pathological data were collected from 81 patients with parapsoriasis in Department of Dermatology,Peking University First Hospital between January 2016 and May 2018,and analyzed retrospectively.Results Among the 81 patients with parapsoriasis,44 were male and 37 were female,with age ranging from 6 to 77 years.Their disease course ranged from 7 days to 30 years,and the median disease course was 12 months.Moreover,61 (75.3％) patients were aged less than 40 years,and 20 (24.7％) were aged 41 years and older.Of the 81 patients,16 (19.8％) were diagnosed with small plaque parapsoriasis,20 (24.7％) with large plaque parapsoriasis,37 (45.7％) with pityriasis lichenoides chroniea,and 8 (9.9％) with pityriasis lichenoides et varioliformis acuta.Additionally,distribution patterns of lesions included diffuse type in 65 cases (80.2％),central type in 6 cases (7.4％),and peripheral type in 10 cases (12.3％).Histopathological examination of skin lesions revealed liquefaction degeneration of basal cells in 69 cases (85.2％),migration of lymphocytes into the epidermis in 67 cases (82.7％),focal parakeratosis in 42 cases (51.9％),keratinocyte necrosis in 29 cases (35.8％),extravasation of erythrocytes in 23 cases (28.4％),epidermal spongiosis in 21 cases (25.9％),and dermal perivascular focal infiltration in 61 cases (75.3％).Conclusion Parapsoriasis has characteristic clinical and pathological manifestations,and a close combination of clinical manifestations with pathological features is necessary for its accurate diagnosis.
Oral Lichen Sclerosus Et Atrophicus - Literature Review and Two Clinical Cases.

Abstract:Lichen sclerosus et atrophicus (LSA), also called Lichen sclerosus (LS) is a chronic, benign, depigmenting disease of the skin and mucous membranes, most frequently affecting genital mucosa and skin. Involvement of the oral mucosa without concurrent genital or skin lesions has very rarely been reported in the literature. Here we report on two Chinese women with LSA limited to the dorsum of the tongue, and describe the clinical manifestations and histopathological findings of these two patients.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Lichen planus (LP) is considered to be an immune‐mediated disease of a not fully understood etiology. There are scarce data on the immune cells forming the band‐like infiltration in cutaneous LP (CLP). The objective of the current study was to investigate the immunohistochemical pattern of cells forming the infiltrate in CLP by assessing the immunoexpression of selected cell lineage markers.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>The immunohistochemical analysis of the expression of CD4, CD8, CD20, CD56, CD68, c‐Kit, and Foxp3 was performed in formalin‐fixed paraffin‐embedded (FFPE) biopsy specimens from 14 cases of CLP and 11 healthy volunteers.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The expression of CD4 (<i>P</i> < 0.001), CD8 (<i>P</i> < 0.001), CD68 (<i>P</i> < 0.001), Foxp3 (<i>P</i> < 0.001), CD56 (v = 0.019), and CD20 (<i>P</i> < 0.001) was significantly higher in lesional skin in CLP compared to healthy controls. The ratio of CD4+ to CD8+ cells in the infiltrate was 1.75:1. The expression of Foxp3, CD56, and CD20 was markedly lower than the expression of CD4 and CD8. There was no statistically significant difference in c‐Kit expression between CLP lesions and healthy skin (<i>P</i> = 0.57). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>We found a wide variety of immune cells in the inflammatory infiltrate in CLP. The expression of CD4, CD8, CD68, Foxp3, CD56, and CD20 was significantly increased in CLP, while the expression of c‐Kit was comparable in lesional skin and controls. The presence of various cell populations, including T regulatory cells, NK cells, and B cells, may indicate a complex pathogenesis of CLP.</p></section>

Vulvar lichen sclerosus in girls and adult females: A single-center retrospective study of 744 patients in China

Abstract:Vulvar lichen sclerosus (VLS) is a chronic, inflammatory disease which is accompanied by itching and pain, affecting the patient's daily life and sexual activity. However, the disease characteristics of children and adults are not completely the same. Currently, there are few studies in China that compare the characteristics of VLS between girls and adult female patients. The aim of this study was to compare the epidemiology, clinical features, and combined autoimmune diseases of VLS patients between girls and adult females, and to help clinicians better understand VLS in different age groups. We enrolled 744 female patients for analysis, divided by age into a child group (<18 years) and an adult group (≥18 years). Among girl patients, 94.6% had preadolescent onset, while among adult female patients, only 4.6% had preadolescent onset, which was a statistically significant difference. The highest percentage of adult female patients had onset during their child-bearing period (75.4%), while 20% had postmenopausal onset, with a significant difference when the three onset states were compared. White patches were equally common in both girl and adult female patients' external genital area, while mossy lesions and labia minora atrophy were more common in adult female patients. Involvement of the clitoris, labia minora, and vaginal opening area were more common in adult patients. The perianal area was more commonly involved in girl patients. We found eight cases (1.2%) of secondary squamous cell carcinoma in adult female patients. We also found that 13 patients had concurrent lichen sclerosus lesions on the vulva and extragenital region, including two girls and 11 adult females. Extragenital lichen sclerosus (EGLS) occurred mostly in the torso. Clinicians should be aware of these differences so that early diagnosis and treatment of the disease can be achieved, to avoid irreversible anatomical alterations and the risk of cancer.

Abstract:Background Lichen striatus (LS) is an acquired skin disorder with a linear pattern along Blaschko's lines. It commonly occurs in childhood, and the lesions spontaneously regress within several months. Objectives Although up to 50% of LS cases exhibit hypopigmentation that can persist for several months to years, it is unknown why LS is associated with such a high incidence of hypopigmentation compared to other inflammatory skin diseases. Therefore, this study aimed to analyse the differences in the skin microbiome between LS patients with and without hypopigmentation. Methods Differences in skin microbiome were analysed using whole genome sequencing of skin biopsies and subsequent bioinformatics analyses. Results Some microbes commonly found in hypopigmented skin disorders, including Cutibacterium acnes, were more abundant in patients with LS showing hypopigmentation than in those not showing hypopigmentation. Conclusions The skin microbiota may be involved in the development of hypopigmentation in LS and may be considered a treatment target to reduce LS duration and hypopigmentation.

Abstract:Women are primarily affected by lichen planus, a chronic autoimmune skin and mucosal disorder in their 40s to 60s. Medication, systemic disorders, and mental stress are some of the factors that can cause it, though the precise cause is still unknown. Middle-aged females are the main victims of the disease; children are rarely affected. Oral lichen planus (OLP) is less common in atrophic and erosive forms. There is disagreement over whether OLP causes cancer; therefore physicians need to keep a close watch for any intraoral lichenoid lesions, and patients with OLP are advised to follow up regularly. This case report details a 52-year-old woman's right buccal mucosa and tongue affected by reticular lichen planus.

Abstract:Abstract Background : Oral lichen planus (OLP) is an intractable chronic inflammatory disease associated with dyskeratosis. Several clinical classifications of OLP have been reported. However, only a few detailed studies have investigated the clinical symptoms, classifications, and treatment effect. In this study, we investigated the clinical features of OLP at the time of the initial diagnosis and classified it using the clinical visual examination type classification. Furthermore, we evaluated whether there is a difference in the clinical characteristics and effectiveness of symptomatic treatment between the typical reticular type and other types of OLP. Methods : A total of 132 patients with OLP were included. Patients’ age, sex, chief complaints, clinical morphological forms, lesion sites, cytological and histopathological findings, therapeutic strategies, and therapeutic responsiveness were evaluated. Results : Of the 132 patients with OLP, there were 42 men and 90 women, with a mean age of 66.4±13.0 years. The buccal mucosa was the most affected site (n=95). The reticular form was the most common clinical morphological form, observed in 124 cases. Biopsy was performed in 48 patients. The erosive form was the most common among the biopsied lesions. Regarding therapeutic strategies, topical steroids were prescribed to 85 patients. Among these patients, 47 experienced symptomatic relief and 38 showed no changes in symptoms. Based on the clinical morphological form, the rates of symptomatic relief following topical steroid use were 70.4% for the reticular form and 15.9% for the erosive form. In patients with erosive and plaque-like forms, it was difficult to achieve symptomatic relief with topical steroid use, whereas in those with other forms, a moderate response was observed. Conclusion : Topical steroids are often ineffective in patients with the erosive or mixed form, and these clinical morphological forms are associated with a higher risk of malignant transformation compared with the other forms. Abstract:Lichenoid tissue reaction/interface dermatitis represents a class of mucocutaneous inflammatory diseases which share common histopathological manifestations. One patient presented to our clinic whose oral lesions could not categorized into a definitely clinical or pathological diagnosis, but could be ascribed to lichenoid tissue reaction/interface dermatitis with moderate-to-severe dysplasia. Photodynamic treatment was applied in this case and a satisfactory result was eventually achieved. Signs of recurrence were not revealed at the follow-up of the tenth month.

Abstract:Background: Lichen simplex chronicus (LSC) is an acquired chronic dermatosis that is often accompanied by severe paroxysmal pruritus, and its pathogenesis to date is still unclear. Transient receptor potential channel A1 (TRPA1) is a non-selective cation channel. TRPA1 is widely expressed in skin, sensory neurons, and various other tissues, along with various biological functions and activation mechanisms. This study aimed to explore the changes in TRPA1 expression in human LSC and provide evidence for further research on the role of TRPA1 in chronic pruritus. Methods: A total of 21 skin lesion specimens from LSC patients with skin pruritus lasting more than 6 weeks, and 28 normal skin tissue specimens through the skin biopsy from June 2018 to February 2019 were collected. The expression of TRPA1 in these skin tissues was evaluated through western blotting, quantitative reverse transcription PCR (qRT-PCR), and immunohistochemical analysis. The changes of inflammatory mediators, including interleukin (IL)-6, IL-13, endothelin-1 (ET-1), and thymic stromal lymphopoietin (TSLP), as well as substance P, are also analyzed by qRT-PCR. TRPA1 was detected using immunohistochemistry for all skin layers, the basal layer, and around the blood vessels of the dermis. Results: The expression of TRPA1 in LSC specimens was significantly decreased as compared with that in the normal specimens (P<0.05). Also, TRPA1 protein levels were consistently decreased in LSC specimens (P<0.05). Simultaneously, the mRNA expressions of TRPA1, IL-6, IL-13, ET-1, TSLP, and substance P presented with no significant differences between LSC and normal specimens. Conclusions: TRPA1 expression is proved downregulated in skin lesions of LSC patients with skin pruritus, indicating that TRPA1 serves as a crucial role in the pathogenesis of human LSC.
