Influences of Formation of Lymphcyst Following Ligation of Lymphatic Vessels in Pelvic Lymph Nodes Dissection

Abstract:【Objective】 The purpose of this randomized study was to evaluate the clinical outcome and postoperative morbidity of patients following lymph vessels ligation during pelvic lymphadenectomy.【Methods】 The trial was performed on 32 patients with gynecological malignancy.They had pelvic lymphadenectomy during surgical treatment which was carried out by the same operator from July 2006 to January 2007.Patients with heart disease,hepatopathy,nephronia,pneumonopathy,hypoproteinemia and a history of radiotherapy were excluded from this study.They had unilateral lymph vessels ligation randomly.All the patients had clinical follow-up with sonographic evaluation and physical examination for lymphocysts and other postoperative complications at 1 week,4 weeks,12 weeks,and 24 weeks after surgery.【Result】 The incidence of postoperative lymphocysts in the ligation side was lower than that in the non-ligation side 1 week after surgery(40.63% and 15.63%);it was statistically significant(P 0.05).The incidence of postoperative lymphocysts in the ligation side was lower than that in the non-ligation side respectively 4 weeks and 12 weeks after surgery(43.75%: 21.88%;26.67%: 16.67%),but was not statistically significant(P 0.05).The incidence of postoperative lymphocysts in the ligation side was higher than that in the non-ligation side 24 weeks after surgery(10.35%: 13.79%),but was not statistically significant(P 0.05).【Conclusion】 Ligation of the deep inguinal lymph vessels,obturator lymph vessels,common iliac lymph vessels,and the lymph vessels at the crossing of the external iliac and the inter iliac vein can decrease the incidence of postoperative lymphocysts in short-term period.
