Abstract:Background: The aim of this study was to evaluate comparatively the effectiveness of computed tomography-guided percutaneous radiofrequency ablation (CT-PRFA) for primary non-small cell lung cancer (NSCLC) and lung metastases from hepatocellular carcinoma (HCC) and to explore the potential miRNA mechanisms for the efficacy of CT-PRFA. Methods: 14 patients pathologically diagnosed with NSCLC and 12 patients with lung metastases from HCC were enrolled in the study and underwent CT-PRFA. Clinical outcomes were compiled on the basis of review of medical records, imaging follow-up reports, and any biopsy-proved residual or recurrent disease. Real-time RT-PCR was used to quantify the selected miRNAs known to be play key roles in lung cancer. Results: A total of 21 tumors were treated with umbrella-tip electrodes and spiral-tip electrodes were used for the remaining 8 tumors. The median follow-up was 13.5 months (range, 3-30 months) and no patient was lost to follow-up. The rate of technique efficacy for primary tumors was ∼93% (13 of 14). Treatment was successful in 11 out of 12 (91.7%) lung metastases patients. Overall survival rate was 80.8% at 2 years, and cancer-specific survival rate was 100% at 2 years. The tumor-free survival was 69.2% at 1 year and 26.9% at 2 years. Before PRFA, tumor suppressor let-7a and miR-34a were downregulated whereas oncomiR miR-21 was upregulated in primary tumors, and let-7a and miR-126 levels were downregulated whereas oncomiRs miR-21, miR-155 and miR-17-5p/miR-20b levels were upregulated in secondary tumors. This abnormal expression was normalized by CT-PRFA. Most notably, CT-PRFA failed to normalize the deregulated miRNAs in the non-survivors. Conclusions: CT-PRFA is a effective treatment for primary NSCLCs and secondary lung tumors from HCC and the efficacy may be related to its ability to normalize deregulated expression of miRNAs: upregulating tumor suppressor miRNAs and downregulating oncomiRs.

Abstract:Radiofrequency ablation (RFA) is a well-established technique for the treatment of soft tissue tumors of the liver and kidney that has increasingly been used for both benign and malignant bone tumors, such as giant cell tumor of bone (GCT). Reports in the literature pertaining to RFA for GCT have involved computed tomography (CT)-guided RFA, which presents several drawbacks such as radiation, lack of real-time monitoring, and the fact that it may be time-consuming. We report a case of GCT treated with percutaneous RFA using ultrasound guidance with CT fusion, which offers a simple, real-time guidance solution without radiation.

Abstract:Imaging-guided radiofrequency ablation(RFA) is a safe and minimally-invasive interven-tional therapy with satisfactory short-term efficacy and less complications.Nowadays,RFA has been widely and successfully applied in the clinical practice for the treatment of hepatocellular carcinomas,lung cancers,adrenal malignancies,bone tumors and hypersplenism.For recent years,a combination of RFA with transcatheter arterial chemoembolization,chemical ablation or percutaneous vertebroplasty has become a new interventional therapeutic pattern for treating the tumors comprehensively,and it can be anticipated that by combining RFA with other therapeutic programs the effectiveness for tumors will be further improved.

Abstract:Radiofrequency ablation(RFA) is a local and minimally-invasive therapy.In recent years,RFA has been increasingly practiced in the clinical treatment of bone tumors.RFA is very safe and effective in relieving pain caused by inoperable metastatic bone lesions and in reinforcing the ablated vertebra when combined with bone cement injection,which is helpful in preventing pathologic fracture.RFA offers an ideal and effective alternative for patients with inoperable metastatic bone tumors.

Abstract:Objective: To discuss the clinical efficacy, safety and adverse reaction of CT-guided percutaneous radiofrequency ablation (RFA) for the treatment of metastatic adrenal malignancies. Methods: CT-guided RFA was performed in 19 patients with metastatic adrenal malignancies, the sum total of the lesions was 24. The diameter of the lesions ranged from 1.5 cm to 7.8 cm, with a mean diameter of 3.5 cm. The original malignancies included pulmonary carcinoma (n = 6), hepatic carcinoma (n = 5), renal cell carcinoma (n = 5) and colonic cancer (n = 3). Three months after RFA treatment, enhanced CT scanning was carried out to evaluate the therapeutic results. Results: Of 24 lesions, complete ablation of the tumor revealed on the follow-up enhanced CT scans after RFA was obtained in 20. Residual tumor enhancement was demostrated in the remaining 4 lesions, indicating that the ablation of the tumor was incomplete. The complications included fluctuation of blood pressure (re = 5), mild pain (re = 6) and transient hematuria (re = 2). No serious complications occurred. Conclusion: CT-guided RFA is a safe, effective and minimally-invasive therapy for metastatic adrenal carcinoma with less complications.
Safety and Effectiveness Analysis of Percutaneous Microwave Thermal Ablation Therapy Combined Brachytherapy for Lung Malignant Lesion

Abstract:BACKGROUND:Microwave ablation and radioactive seed implantation are therapeutic options for patients with advanced lung cancer and lung metastases lesion who are not surgical candidates. However, reports on therapeutic effectiveness and safety of the two combination treatments methods have been limited to small case series. This research was to assess the clinical outcome and safety of radioactive seed implantation combined with computed tomography (CT)-guided microwave ablation for the treatment of advanced lung cancer and lung metastases lesion.METHODS:The clinical data of 21 advanced lung cancer and lung metastases patients who received treatment at author hospital during the period from May 2018 to December 2018 were retrospectively analyzed. The patients were divided into two groups by whether received extra percutaneous microwave thermal ablation therapy. The short-term effectiveness and safety were analyzed.RESULTS:The efficacy of control group was 28.75%, and the efficacy of experiment group was 14.28%. The rate of adverse reaction between two groups has no significantly difference (P>0.05).CONCLUSIONS:Seed implantation combined with CT-guided microwave ablation for advanced lung cancer and lung metastases is safety and effective.

Abstract:Objective To investigate the safety and effectiveness of radiofrequency ablation ( RFA ) in the treatment of bone metastases. Methods From November 2008 to December 2012, 19 patients with bone metastases were treated with RFA in our department. There were 10 males and 9 females with an average age of 57 years old ( range;41-76 years ). Lung carcinoma was the most common primary tumor ( n=11 ), followed by kidney carcinoma ( n=2 ), breast carcinoma ( n=1 ), thyroid carcinoma ( n=1 ), rectal carcinoma ( n=1 ), gastric carcinoma ( n=1 ), hepatocellular carcinoma ( n=1 ) and unknown carcinoma ( n=1 ). There were totally 21 lesions in the 19 patients, including 9 lesions in the femur, 6 lesions in the pelvis, 5 lesions in the humerus and 1 lesion in the tibia. Curettage and reconstruction after the RFA were performed for 16 lesions. Percutaneous RFA was performed for 5 lesions, and 3 of them in the femoral shaft were ifxed with intramedullary nails. During the follow-up, the pain scores and function scores were used to evaluate the effects and check the recurrence and the safety of the surgery. There were 2 kinds of evaluation methods. ( 1 ) The preoperative and postoperative pain scores were compared ( numerical rating scale ). ( 2 ) The function evaluation of the patients whose survival time was more than 6 months was performed ( the Musculoskeletal Tumor Society MSTS rating scale ). Results The mean follow-up period was 10 months (range;1-32 months ) and the mean survival time was 9.7 months ( range;1-32 months ). Fifteen patients were dead. The mean pain score was 8.1 points before the surgery and 2.2 points and 2.7 points at 1 week and 3 months after the surgery respectively, and there were statistically signiifcant differences before and after the surgery ( P<0.01 ). Local pain recurred in 1 patient at 6 months after the ablation with the pain score of 7 points, who received oral analgesic but died 11 months later due to the progress of primary tumors. One patient had thermal damage around the skin adjacent to the tourniquet during the surgery and 1 patient developed wound necrosis after the surgery. The function evaluation was performed in 12 patients whose survival time was more than 6 months, and the excellent and good rate was 83.3%. Conclusions The RFA is a safe and effective approach in the palliative treatment of bone metastases. The local pain can be relieved and the development of bone metastases can be controlled.

Abstract:Microwave ablation has been used to treat bone tumors in extremities for more than 30 years. With improved recognition, updated microwave equipment, and expanded clinical application, microwave ablation has recently been widely used to treat bone tumors. To standardize the application of microwave ablation in the clinical treatment of bone tumors in the limbs, research results and clinical experience involving the use of microwave ablation to treat bone tumors in the limbs have been summarized, and a clinical guideline has been designed. This guideline is aimed at providing a reliable clinical basis for indications, preoperative evaluation and decision‐making, perioperative treatment, complications, and other issues via evidence‐based medicine. Two aspects are considered—percutaneous microwave ablation and intraoperative microwave ablation of bone tumors in extremities. Ultimately, the guideline is intended to standardize treatment and improve the clinical efficacy of microwave ablation of bone tumors in extremities.

Abstract:Objective To investigate the therapeutic effects of ultrasound-guided radiofrequency ab- lation(RFA)in the treatment of hepatic metastases and to assess its clinical significance.Methods A total of 136 patients with 347 hepatic metastases underwent RFA treatment(tumor size,1.1-7.7cm in grea- test dimension).Of these,99 patients had hepatic metastases from digestive tract carcinomas,17 from the breast,12 from the lung and 8 from other primary carcinomas.Before initial RFA procedure,57 patients had single lesion,42 had more than 3 lesions.The tumors more than 3.5 cm were treated with multiple overlap- ping ablations.All the patients underwent echanced CT scan 1 month after RFA.Follow-up ranged from 3 to 51 months.Results The tumor necrosis rate and local residue rate were 95.1% and 4.9%,respectively, according to enhanced CT examination 24 h or 1 month after RFA.Local recurrence or enlargement occurred in 39 tumors(11.2%)2～19 months after treatment.The local tumor necrosis rate and recurrence rate after RFA were closely associated with the size of the focuses.New metastases in other sites of the liver were found in 55 patients(40.4%)1～16 months after RFA.The less the focuses at initial RFA,the less the new metasteses of the liver.The mean survival time was(26.22±1.85)months.The 1-,2-and 3-year survival rates were 80.3%,43.8% and 28.8%,respectively.Three cases developed severe complications,and all recovered by conservative therapy.Conclusions Ultrasound-guided RFA is an effective treatment option for hepatic metastases with high local necrosis and low complication rates.It can prolong survival time, especially for patient with single lesion.In addition,it can be repeatedly used for intrahepatic recurrence and metastases and therefore has good prospect in treating hepatic metastases.
Clinical Research on Advanced Liver Cancer Treated with Percutaneous RFA Cool-Tip Electrode under Ultrasound Guidance

Abstract:Objective To evaluate the clinical efficacy and complications in the treatment of advance liver cancer under ultrasound guidance with percutaneous RFTI-1 TM-cold cycle RF tumor ablation apparatus produced by Nangjing TianMa high-tech Company limited. Methods 25 patients of advance liver cancer, including 8 cases of metastatic liver cancer, 17 of primary hepatic carcinoma, were treated with percutaneous RFA cool-tip electrode under Ultrasound guidance 1 to 2 times. Eight of the patients prior to RFA were treated with TACE of individually three times for each. The postoperative efficacy was evaluated by enhanced CT. Results 10 lesions were completely necrotized and the majority parts of another 15 lesions were also under necrosis. Follow-up of six months, all patients are still alive with marked improvement of life quality. Conclusion The short-term efficacy of percutaneous Ultrasound-guided RFA with cool-tip electrode for treating advance liver cancer is quite satisfied, worthy to be recommended.

Abstract:Tumor ablation therapies provide a minimally invasive approach to treat cancer. However, inhibition of cancer metastasis and recurrence after ablation is still a challenge in clinical trials. Here, we propose a strategy using combinatorial thermal ablation, adjuvants and immune checkpoint blockade (ICB) to inhibit metastatic tumor and recurrence via antitumor immune responses post tumor thermal ablation, which are frequently used in the clinic. Furthermore, a strong immune memory against cancer was observed 80 days after the primary tumor was ablated. Considering that all components in our design are approved by Food and Drug Administration (FDA), we provide a strategy based on clinically used cancer treatment technique that is promising in clinical translation.

Abstract:Although surgical resection is the primary means of curing both primary and metastatic lung cancers, about 80% of lung cancers cannot be removed by surgery. As most patients with unresectable lung cancer receive only limited benefits from traditional radiotherapy and chemotherapy, many new local treatment methods have emerged, including local ablation therapy. The Minimally Invasive and Comprehensive Treatment of Lung Cancer Branch, Professional Committee of Minimally Invasive Treatment of Cancer of the Chinese Anti-Cancer Association has organized multidisciplinary experts to develop guidelines for this treatment modality. These guidelines aim at standardizing thermal ablation procedures and criteria for selecting treatment candidates and assessing outcomes; and for preventing and managing post-ablation complications.

Abstract:Objective To evaluate the effect of CT-guided percutaneous coaxial needle biopsy combined with microwave ablation (MWA) for the treatment of lung tumors, and to discuss its technique, safety, curative effect and clinical application value. Methods A total of 20 patients with lung tumor, who were admitted to the Department of Interventional Radiology, Shanghai Ruijin Hospital, China, during the period from August 2014 to June 2016, were collected. Among the 20 patients, primary pulmonary cancer was diagnosed in 9 and metastatic tumor in 11. A total of 23 tumor lesions were detected, and CT-guided percutaneous coaxial needle biopsy combined with MWA was performed for all lesions. Based on the size and shape of the tumor, the corresponding ablation power and time of duration were set up. After the treatment, the patients were followed up for 4-26 months, and chest enhanced CT examination was regularly reexamined to assess the curative effect. Results Percutaneous coaxial needle biopsy was carried out for 23 lesions, the tumor diameters ranged from 0.80 cm to 2.40 cm, with a mean of 1.39 cm. MWA was employed for 23 lesions. CT scan performed immediately after MWA showed that the density of tumor lesion became decreased, the diagnostic positive rate of needle biopsy was 83.3％. After the treatment, complete remission was obtained in 15 patients, partial remission in 2 patients, stable disease in one patient, and progression.

Abstract:Objective To compare the outcomes of cone beam computed tomography (CBCT)-guided radiofrequency ablation (RFA) for hepatocellular carcinoma (HCC) with those of traditional spiral computed tomography (s-CT)-guided RFA. Methods This retrospective study analysed data from patients with HCC that underwent RFA guided by either CBCT or s-CT. A number of preoperative and postoperative characteristics, including operation time, ablation time, radiation dose and hospital stay were recorded for all patients. The incidence of intraoperative and postoperative complications was recorded. The therapeutic effect was evaluated at 1, 3 and 6 months after RFA. Results A total of 47 patients with HCC (12 females and 35 males) underwent successful RFA: 21 underwent CBCT-guided RFA and 26 underwent s-CT-guided RFA. Except for one case of pneumothorax in the s-CT group, no serious complications occurred. The objective response rate and disease control rate at 1, 3 and 6 months after RFA showed no significant differences between the two groups. Throughout the 6-month follow-up period, the complete ablation rate was 19 of 21 patients (90.5%) in the CBCT group and 19 of 26 patients (73.1%) in the s-CT group. Conclusions CBCT was a safe and effective guiding modality for RFA in patients with HCC.

Abstract:Designing high accuracy in the diagnosing and fault-freely eliminating lymphatic metastasis of breast malignancy, to avoid the invasiveness and complications caused by traditional assays, is of great therapeutic importance. To this end, theranostic W18O49 nanoparticles targeting to human epidermal growth receptor 2 (HER-2) over-expressed breast malignancy were synthesized via polyol method. By taking advantage of their high X-ray attenuating and photothermotherapy potency, lymph nodes in the mice bearing HER-2 positive metastasis could be clearly distinguished under CT guidance and selectively eliminated by laser ablation. The therapeutic efficacy was further confirmed by the significantly extended survival period. These finding evidenced the potential of these nanoparticles for imaging guided photothermal ablation of HER-2 positive breast malignancy.

Clinical Application of CT-guided Radiofrequency Ablation for the Treatment of Metastatic Bone Neoplasms
