Impact of total bilirubin on the hepatic function, renal function and myocardial enzyme among neonate

Abstract:Objective To explore the effect of neonatal total bilirubin (TBIL) on hepatic function, renal function and myocardial enzyme. Methods From August 2013 to July 2014, 1 303 neonates were selected from the obstetrical department. TBIL, alanineaminotransferase (ALT), aspartateaminotransferase (AST), aspartate transaminase (ALP), glutamyl transpeptidase (GGT), albumin (ALB), blood urea nitrogen (BUN), uric acid (UA), creatinine (Cr), lactate dehydrogenase (LDH), creatine kinase (CK) and hydroxybutyrate dehydrogenase (HBDH) were measured. The total of 1 303 neonate were divided into 3 groups: control group (n=606), mild jaundice group (n=428) and moderate/severe jaundice group (n=269), and the indexes of hepatic function, renal function and myocardial enzyme between groups were compared. Results Statistically insignificant difference of ALT, AST, BUN and UA between groups were detected (F=0.58, P=0.56; F=0.73, P=0.33; F=2.00, P=0.14; F=0.23, P=0.80), while the difference of ALP, GGT, ALB, Cr, CK, HBDH and LDH between groups were statistically significant (F=3.88, P=0.02; F=10.03, P<0.01; F=15.20, P<0.01; F=4.94, P<0.01; F=5.84, P<0.01; F=14.67, P<0.01; F=12.98, P<0.01). The measurements of ALP, GGT and ALB among moderate/severe jaundice group was statistically higher than those among control group (P<0.01), and the measurements of HBDH and LDH among mild and moderate/severe jaundice group were statistically higher than those among control group (P<0.01), and the measurements of GGT, HBDH and LDH among moderate/severe jaundice group were statistically higher than those among mild jaundice group (P<0.01); while the measurements of Cr and CK among mild and moderate/severe jaundice group were statistically lower than those among control group (P<0.01). Conclusions The neonatal hyperbilirubinemia has impairment effect on hepatic function, renal function and myocardium; earlier therapy and intervention should be taken in order to protect hepatic function, renal function and myocardium from impairment.
