Abstract:Dry eye is an ocular surface disease caused by multiple factors,which is characterized by imbalance of tear film homeostasis,changes of ocular surface micro-environments and ocular uncomfortable symptoms.Tear film instability,tear hyperosmolarity,ocular surface inflammation and damage and corneal neurosensory abnormalities play etiological roles in the process of dry eye.Dry eye seriously affects work and life quality of the patients.With the progress of science and technology,the treatment of dry eye is becoming more diversified.The therapeutic methods are different for mild,moderate and serous dry eye.The choice of therapies actually depends on the individual state and condition of therapeutic institutions.A systemic,standardized and comprehensive therapy is associated with the improvement of treatment outcome.This article systematically summarizes the traditional and emerging treatments of dry eye,which are clinically feasible and can provide reference for choosing treatment methods.

Abstract:Abundant ion-channels, including various perceptual receptors, chloride channels, purinergic receptor channels, and water channels that exist on the ocular surface, play an important role in the pathogenesis of dry eye. Channel-targeting activators or inhibitor compounds, which have shown positive effects in in vivo and in vitro experiments, have become the focus of the dry eye drug research and development, and individual compounds have been applied in clinical experimental treatment. This review summarized various types of ion-channels on the ocular surface related to dry eye, their basic functions, and spatial distribution, and discussed basic and clinical research results of various channel receptor regulatory compounds. Therefore, further elucidating the relationship between ion-channels and dry eye will warrant research of dry eye targeted drug therapy.

Abstract:Background Dry eye disease (DED) stands as a prominent ocular condition of global prevalence, emerging as a growing concern within public health. However, the underlying mechanisms involved in its pathogenesis remain largely unknown. In recent years, with the development of metabolomics, numerous studies have reported alterations in ocular surface metabolism in DED and offered fresh perspectives on the development of DED. Main text The metabolic changes of the ocular surface of DED patients are closely intertwined with the cellular metabolism process and immune inflammation changes. This article expounds upon the correlation between ocular surface metabolism and immune inflammation alterations in DED in terms of glycolysis, lipid metabolism, amino acid metabolism, cellular signaling pathways, and immune inflammation regulation. Conclusion The alterations in ocular surface metabolism of patients with dry eyes are closely associated with their inflammatory status. Our work contributes novel insights into the pathogenesis of dry eye diseases and offers innovative molecular targets for diagnosing, detecting, and managing DED patients.

Abstract:Dry eye-related ocular surface examination is very important in the diagnosis and treatment of dry eye disease. With the recent advances in science and technology, dry eye examination techniques have progressed rapidly, which has greatly improved dry eye diagnoses and treatment. However, clinically, confusion remains about which examination to choose, how to ensure the repeatability of the examination, and how to accurately interpret the examination results. In this review, we systematically evaluate previous examinations of dry eye, analyze the latest views and research hotspots, and provide a reference for the diagnosis and management of dry eye.

Abstract:Dry eye is one of the most frequently ocular surface diseases. Recent researches found that many reasons caused decrease of ocular surface damage and the quality of tears , such as the change of ocular surface, immuno-inflammatory responses, apoptosis and the reduction of sex hormone. It is reported that the decline of ovarian function and hormone level in postmenopausal women which leads to abnormal structure and function of tear film is more likely to develop dry eye. In this paper, the ocular surface, pathogenesis and progresses of treatment on postmenopausal women with dry eye are reviewed.

Abstract:Tear secretion is controlled by the lacrimal functional unit consisting of the ocular surface(cornea,conjunctiva,accessory lacrimal glands,and meibomian glands),the main lacrimal gland and the interconnecting innervation.Abnormalities at any portion of this functional unit can result in a loss of tear film integrity and normal function.Many factors contribute to dry eye.The tear film will recover if the causes are eliminated before pathophysiological changes happen on the ocular surface.However,the ocular surface will be damaged by unstable tear film.Whatever the initial causes of dry eye,the diseases show the similar pathophysiological process in the progressing phase.Inflammation is the key mechanism and the other factors such as apoptosis,nervous and hormonal regulation are closely involved in dry eye pathogenesis.

Abstract:Dry eye is currently one of the most common ocular surface disease. It can lead to ocular discomfort and even cause visual impairment, which greatly affects the work and quality of life of patients. With the increasing incidence of dry eye disease (DED) in recent years, the disease is receiving more and more attention, and has become one of the hot research fields in ophthalmology research. Recently, with the in-depth research on the etiology, pathogenesis and treatment of DED, it has been shown that defects in immune regulation is one of the main pathological mechanisms of DED. Since the non-specific and specific immune response of the ocular surface are jointly regulated, a variety of immune cells and inflammatory factors are involved in the development of DED. The conventional treatment of DED is the application of artificial tears for lubricating the ocular surface. However, for moderate-to-severe DED, treatment with anti-inflammatory drugs is necessary. In this review, the immunomodulatory mechanisms of DED and the latest research progress of its related treatments including Chinese medicine will be discussed.

Abstract:Dry eye is a multifactorial disease of the tears and ocular surface that results in symptoms of discomfort,visual disturbance,and tear film instability with potential damage to ocular surface.It is accompanied by increased osmolarity of tear film and inflammation of the ocular surface.Current studies have shown that inflammation may play an important role in the pathological damage of dry eye.This article reviews how inflammation in dry eye leads to potential damage to the tears and ocular surface.Evidence for inflammatory mechanisms accounting for potential damage to the tears and ocular surface will be provided from the following aspects:tear hyperosmolarity,ahered expression of mucins,disruption of the structure and function on keratoconjunctival epithelium,lacrimal gland and meibomian gland.

Abstract:Dry eye, a prevalent ocular surface disease, is significantly influenced by modern lifestyle factors such as night-shift work and extended screen time. Emerging evidence suggests a strong correlation between disturbances in circadian rhythm, sleep disorders, and dry eye. However, the precise underlying mechanisms remain unclear. Recent studies have underscored the crucial role of circadian rhythms and neuroendocrine regulation in maintaining ocular surface health. Advances in treatment strategies targeting neuroendocrine pathways have shown promising developments. This review explores the interplay between circadian rhythms, neuroendocrine regulation, and the ocular surface, examines the impact of circadian disruption on the pathophysiology of dry eye, and proposes intervention strategies to alleviate dry eye associated with disturbances in circadian rhythms.

Abstract:Homeostasis of the lacrimal functional unit is needed to ensure a well-regulated ocular immune response comprising innate and adaptive phases. When the ocular immune system is excessively stimulated and/or immunoregulatory mechanisms are disrupted, the balance between innate and adaptive phases is dysregulated and chronic ocular surface inflammation can result, leading to chronic dry eye disease (DED). According to the Tear Film and Ocular Surface Society Dry Eye Workshop II definition, DED is a multifactorial disorder of the ocular surface characterized by impairment and loss of tear homeostasis (hyperosmolarity), ocular discomfort or pain, and neurosensory abnormalities. Dysregulated ocular immune responses result in ocular surface damage, which is a further contributing factor to DED pathology. Several therapeutics are available to break the vicious circle of DED and prevent chronic disease and progression, including immunosuppressive agents (steroids) and immunomodulators (cyclosporine and lifitegrast). Given the chronic inflammatory nature of DED, each of these agents is commonly used in clinical practice. In this study, we review the immunopathology of DED and the molecular and cellular actions of current topical DED therapeutics to inform clinical decision making.

Abstract:Dry eye can damage the ocular surface and result in mild corneal epithelial defect to blinding corneal pannus formation and squamous metaplasia. Significant progress in the treatment of dry eye has been made in the last two decades; progressing from lubricating and hydrating the ocular surface with artificial tear to stimulating tear secretion; anti-inflammation and immune regulation. With the increase in knowledge regarding the pathophysiology of dry eye, we propose in this review the concept of ocular surface microenvironment. Various components of the microenvironment contribute to the homeostasis of ocular surface. Compromise in one or more components can result in homeostasis disruption of ocular surface leading to dry eye disease. Complete evaluation of the microenvironment component changes in dry eye patients will not only lead to appropriate diagnosis, but also guide in timely and effective clinical management. Successful treatment of dry eye should be aimed to restore the homeostasis of the ocular surface microenvironment.

Abstract:Dry eye disease is a common clinical problem encountered by ophthalmologists worldwide. Interest in this entity has increased in recent years due to the consequences it has on the ocular surface after any surface procedure. With changing times, several new factors have come to light that can influence this disease. The effect of the COVID-19 pandemic has also been greatly felt, with a range of causes, starting from increased screen work to inflammatory processes, exacerbating the condition in many. With changes in the concepts of the etiopathogenesis of the disease, a paradigm shift has taken place in the approaches to treatment. More researchers are in favor of a new tear film-oriented approach that tries to localize the disease to a single component in the tear film. Innovation of newer techniques for the treatment of meibomian gland disease has also made its foray into clinical ophthalmology. Newer drug formulations and molecules are underway to better treat the inflammatory component of the disease. Many other receptors and targets for the treatment of dry eyes are being researched. This review hopes to provide a succinct, narrative summary of the relevant research on dry eye disease to date to increase awareness about the nature and future course of this disease and its management.

Abstract:Background: Millions of people worldwide suffer from dry eye disease. Dry eye, a multifunctional condition of the ocular surface, typically occurs in conjunction with an unbalanced tear film. With increasing age, the dry eye problem becomes worse. Aqueous-deficit dry eye and evaporative dry eye are the two traditional classifications for dry eye. Various examination tools are used to diagnose dry eye. Clinical trials are conducted in four phases to check the safety and efficacy of drugs. The quick clearance from the precorneal space is ensured by the eye's advanced defense mechanism. It restricts the integrated medicine's entry into the eyes, resulting in a usually low bioavailability for topical eyedrops. In this study, we focus on recently developed formulations for curing dry eye. Objective: This review's goal was to outline the etiology, clinical discovery and development, patents, and recent advancements for dry eye disease. Results: The current study has described the widespread incidence of dry eye, which was found to be more common as people aged and recently developed formulations are treating dry eyes. According to research, novel formulations are enhancing ocular drug delivery. Conclusion: In this review, etiology, clinical data, dry eye formulation patents, and recent advancements are all included.

Abstract:Dry eye is a multifactorial disease of the ocular surface characterized by a loss of homeostasis of the tear film, and accompanied by ocular symptoms. In recent years, the incidence of dry eye has been increasing year by year, and the diagnosis and treatment of dry eye are constantly evolving and innovating. However, due to the corresponding drawbacks of traditional examination methods and the lack of a large number of clinical trial studies on new examination methods, there is still no unified standard for the diagnosis and treatment of dry eye. In this review, we have performed a broad search for articles discussing different examination methods for dry eye, including promising diagnostic tools and technique, the latest advances, and contradictions, in order to provide a review of dry eye examination methods including the tear volume, the tear film, the eyelid and meibomian gland, and the degree of damage to the epithelial cells of the ocular surface, and provide a reference for the diagnosis and treatment of dry eye.
[Progression of research for the association between sex hormones and dry eye]

Abstract:Researches of the correlation between sex hormones and dry eye have been concerned early.Ocular surface inflammation, epithelial apoptosis and abnormal expression of tear lipocalin and lipid may be important pathogenetic factors of various types of dry eye, and variedness of sex hormone level may have a certain correlation with these factors. The alteration of structure and function of tear film is the direct reason for dry eye. The change of sex hormone level likely gives influence to tear composition and pathological variation of related ocular surface tissues, then affects the structure and function of tear film, and finally lead to dry eye. Different sex hormones give certain different moderating effects to related ocular surface organization, so the recent correlational study of sex hormones and dry eye can be summarized in terms of androgen and estrogen, as these two sex hormones have certain impacts on tear composition and ocular surface organization respectively.

Abstract:The study of new intervention methods on dry eye mainly focuses on maintaining the balance of ocular surface microenvironment,primarily including the following three aspects.(1) Anti-inflammation such as reducing the expression of inflammation factors and damage-associated molecular patterns (DAMPs).(2)Anti-oxidation such as maintaining the balance between oxidase and antioxidant enzymes and activing endoplasmic reticulum stress.(3) Maintaining stability of tear film by increasing the content of mucin and lipid.

Abstract:Clinical management of ocular surface disease, and particularly dry eye disease, remains challenging, and research plays a vital role in guiding the field by providing scientific evidence to support recommendations for optimal patient care. A significant milestone in the history of dry eye research was the publication of the National Eye Institute/Industry Workshop in 1995, 1 followed by the further impactful consensus workshops of the Tear Film and Ocular Surface Society (TFOS) that consolidated the latest peer-reviewed evidence to offer clinical guidance and help set future directions for research and development within the field. It has been 17 years since the publication of the first TFOS Dry Eye Workshop (DEWS) 2 and 7 years since the publication of the second workshop, TFOS DEWS II, 3 and it is clear that the dry eye field continues to witness growing interest in the management of dry eye (Fig. 1).

Abstract:Dry eye disease is a multifactorial condition characterised by tear film instability, hyperosmolarity and ocular surface inflammation. Understanding the epidemiology of dry eye disease and recognising both modifiable and non-modifiable risk factors can assist eye care practitioners in assessing, treating, and managing patients with the condition. This review considers current knowledge surrounding its incidence and prevalence, as well as associated demographic, systemic, ocular, and iatrogenic, and lifestyle-related modifiable risk factors. Population-based prevalence estimates vary according to the diagnostic criteria used to define dry eye disease, as well as severity and demographic characteristics of the population. Considering recent data and variable population demographics, conservative prevalence estimates suggest that 10-20% of the population over 40 years of age report moderate to severe symptoms and/or seek treatment for dry eye disease. Individuals with specific non-modifiable demographic risk factors may be at increased risk of developing dry eye disease. Advanced age, female sex and East Asian ethnicity have been identified as key non-modifiable demographic features predisposing individuals to dry eye disease. Systemic conditions that have been associated with an increased risk of dry eye disease include migraine, Sjögren syndrome, connective tissue disorders, mental health disorders, diabetes mellitus and androgen deficiency. Medications that may contribute to this risk include antidepressants, antihistamines, and hormone replacement therapy. Ocular and iatrogenic risk factors of dry eye disease include blepharitis, Demodex infestation, ocular surgery, blink completeness, contact lens wear, and topical ophthalmic medications. A range of modifiable lifestyle factors that can increase the risk of dry eye disease have also been identified, including low humidity environments, digital screen use, quality of sleep, diet, and eye cosmetic wear. Dry eye is a common disease affecting millions globally. Increasing knowledge regarding its associated risk factors can better prepare the eye care practitioner to successfully manage patients with this ocular surface disease.
