Abstract:The use of disinfectants, particularly those containing quaternary ammonium compounds (QUACs), has dramatically escalated globally since the coronavirus disease 2019 pandemic. We report a case that highlights the risks associated with ingesting low-concentration QUAC solutions and emphasize the importance of effective management in resolving severe lesions without sequelae. A 17-month-old boy experienced severe respiratory failure after ingesting a disinfectant containing benzalkonium chloride (BAC). The child was initially treated at a local emergency department and was subsequently transferred to a pediatric poison center. Upon evaluation, the child was found to have grade III-A corrosive esophageal lesions and chemical pneumonitis. Several complications, including massive pneumothorax and candidemia, occurred during the clinical course of the disease. However, with timely medical intervention and appropriate supportive care, the patient completely recovered without any long-term sequelae. The properties of BAC and the comprehensive management approach may have been responsible for the patient's full recovery, despite the potentially life-threatening effects of ingesting disinfectants.

What problem does this paper attempt to address?
The paper attempts to address the issue of severe clinical symptoms and management methods resulting from children accidentally ingesting disinfectants, particularly those containing quaternary ammonium compounds. Specifically, the paper highlights the risks associated with the ingestion of low-concentration quaternary ammonium compound solutions by reporting a case of a 17-month-old boy who suffered severe respiratory failure after ingesting Benzalkonium Chloride (BAC) disinfectant, and explores effective management and treatment measures. ### Main Issues: 1. **Risk of Ingesting Disinfectants**: The paper focuses on the serious health problems that can arise when children accidentally ingest disinfectants containing quaternary ammonium compounds, particularly the toxic effects of Benzalkonium Chloride (BAC). 2. **Importance of Clinical Management**: The paper emphasizes the importance of timely and appropriate medical intervention in preventing long-term sequelae in such poisoning incidents. 3. **Necessity of Preventive Measures**: The paper also discusses how raising parental awareness of the dangers of corrosive substances and taking preventive measures can reduce the occurrence of such incidents. ### Background: - **Increased Use of Disinfectants Globally**: Since the COVID-19 pandemic, the use of disinfectants has significantly increased worldwide, with about half of the products containing quaternary ammonium compounds (QUACs). - **Rise in Poisoning Incidents**: With the increased use of disinfectants, the number of poisoning incidents has also risen, especially among children. - **Lack of Detailed Research**: Most studies do not explore the chemical structure and uses of these disinfectants in detail, making it difficult to accurately assess their potential harm to children. ### Research Objectives: - **Case Report**: By reporting a severe case of Benzalkonium Chloride poisoning in a child, the paper aims to demonstrate its clinical manifestations and treatment process. - **Literature Review**: Reviewing relevant literature to summarize the clinical features and management strategies of quaternary ammonium compound poisoning. - **Propose Recommendations**: Proposing preventive measures and management recommendations to reduce the occurrence of similar incidents and improve treatment outcomes. ### Conclusion: - **Risk Identification**: Even low-concentration Benzalkonium Chloride solutions can cause severe clinical symptoms, including respiratory failure and lung injury. - **Effective Management**: Timely medical intervention and appropriate treatment measures can significantly improve patient prognosis and prevent long-term sequelae. - **Preventive Measures**: Raising parental awareness of the dangers of corrosive substances and taking appropriate storage and usage measures can effectively reduce the risk of children accidentally ingesting disinfectants.
