Abstract:We very much appreciate the comments on our Correspondence in regard to a “new era for organ transplantation in China”.1Sun Q Gao X Wang H et al.A new era for organ transplantation in China.Lancet. 2014; 383: 1971-1972Summary Full Text Full Text PDF PubMed Scopus (15) Google Scholar We are mindful that the qualification as “a new era” is a matter of contention and substantial debate; however, the progresses that have been made in such a short time are indeed verifiable and substantial. Instead of focusing on what has failed—since much has been written and spoken on these issues—we choose to bring to light what China has been trying to positively achieve in the past few years. Substantial changes have taken place in the past years to reform the transplantation system in China, notably the Regulation on Human Organ Transplantation, in 2007. Under the new laws, organ trafficking and transplant tourism are strictly forbidden. In 2010, the Chinese Government implemented a national standard protocol for three categories of deceased organ donation. The Human Organs Acquisition and Distribution Management Regulations subsequently followed in 2013. Furthermore, a national electronic organ allocation system was introduced, which has a medically derived allocation algorithm that helps to allocate the donor organs equitably. Concurrently, the entire structure of the new national transplantation infrastructure was established.2Huang J Wang H Fan ST et al.The national program for deceased organ donation in China.Transplantation. 2013; 96: 5-9Crossref PubMed Scopus (79) Google Scholar These measures, led by former Vice Minister of Health Jiefu Huang, were welcomed by WHO, and generated substantial increases in donations. As of July 7, 2014, according to the China Organ Donation Administrative Centre, 1963 donations enabled 5388 patients to receive organs for transplantation. We deem the new system to be founded on justice, equity, and national consensus. In this sense, this is truly the beginning of a new era for transplantation in China. The new system is still developing and will undergo many changes. It is only a beginning, not a celebratory finish. Much work is to be done and progress is to be made. In a populous nation, such as China, with a long history of civilisation and culture, continued criticisms and pressures without recognition of rapid positive changes can be detrimental to continued progress. A new era for organ transplantation in ChinaAs changes in policies for organ donation take shape in China, it became necessary to develop an ethical and sustainable organ transplantation system. At present, there are no legislations for the declaration of brain death because this notion is still not readily accepted in Chinese society.
