Abstract:Autoimmune hepatitis (AIH) is an immune-mediated inflammatory liver disease for which the pathogenesis remains incompletely understood. The current study aimed to reveal key biological processes and immune cells implicated in AIH by integrated bioinformatic analysis. The global gene expression in livers from wild-type BALB/c mice, mice with Tgfb1 deficiency, and mice with both Tgfb1 and Ifng deficiency was assessed by microarray data analysis. Differentially expressed genes were identified and subjected to functional enrichment analysis. AIH mice with Tgfb1 deletion showed significantly enhanced immune responses but impaired metabolic processes, whereas increased T cell activation and cytokine production, but weakened organic acid and lipid metabolic processes were observed in mice with deletion of both Tgfb1 and Ifng. In addition, infiltration of immune cells was evaluated by CIBERSORT. Increased infiltration of T cells, macrophages, and natural killer cells, and decreased infiltration of neutrophils, eosinophils, plasma cells, and B cells were observed in AIH mice. In conclusion, we identified potential biological processes and immune cells that contributed to AIH; further investigations are needed to confirm these findings and thus provide a potential novel therapeutic target for AIH treatment.

Identification of Effective Diagnostic Biomarker and Immune Cell Infiltration Characteristics in Acute Liver Failure by Integrating Bioinformatics Analysis and Machine-Learning Strategies

Abstract:Background: To determine effective biomarkers for the diagnosis of acute liver failure (ALF) and explore the characteristics of the immune cell infiltration of ALF.Methods: We analyzed the differentially expressed genes (DEGs) between ALF and control samples in GSE38941, GSE62029, GSE96851, GSE120652, and merged datasets. Co-expressed DEGs (co-DEGs) identified from the five datasets were analyzed for enrichment analysis. We further constructed a PPI network of co-DEGs using the STRING database. Then, we integrated the two kinds of machine-learning strategies to identify diagnostic biomarkers of top hub genes screened based on MCC and Degree methods. And the potential diagnostic performance of the biomarkers for ALF was estimated using the AUC values. Data from GSE14668, GSE74000, and GSE96851 databases was performed as external verification sets to validate the expression level of potential diagnostic biomarkers. Furthermore, we analyzed the difference in the protein level of diagnostic biomarkers between normal and ALF mice models. Finally, we used CIBERSORT to estimate relative infiltration levels of 22 immune cell subsets in ALF samples and further analyzed the relationships between the diagnostic biomarkers and infiltrated immune cells.Results: A total of 200 co-DEGs were screened. Enrichment analyses depicted that they are highly enriched in metabolism and matrix collagen production-associated processes. The top 28 hub genes were obtained by integrating MCC and Degree methods. Then, the collagen type IV alpha 2 chain (COL4A2) was regarded as the diagnostic biomarker and showed excellent specificity and sensitivity. COL4A2 also showed a statistically significant difference and excellent diagnostic effectiveness in the verification set. In addition, there was a significant upregulation in the COL4A2 protein level in ALF mice models compared with the normal group. CIBERSORT analysis showed that activated CD4 T cells, plasma cells, macrophages, and monocytes may be implicated in the progress of ALF. In addition, COL4A2 showed different degrees of correlation with immune cells.Conclusion: In conclusion, COL4A2 may be a diagnostic biomarker for ALF, and immune cell infiltration may have important implications for the occurrence and progression of ALF.

Abstract:In the dynamic field of immunology, the discovery of innate lymphoid cells (ILCs) in 2008 marked a transformative milestone, as highlighted by Vivier et al. (2018). These cells, present in both humans and mice, have become central to research due to their crucial role in maintaining barrier immunity and their involvement in a range of microbiota-driven disorders.ILCs are a diverse group of cells categorized into three main subsets based on their expression of transcription factors and cytokines (Vivier et al., 2018). Group 1 ILCs (including ILC1 and NK cells) are characterized by the expression of T-bet and IFN-γ, mirroring the function of Th1 cells.Group 2 ILCs express GATA3 and cytokines such as IL-4, IL-5, and IL-13, resembling Th2 cells.Group 3 ILCs, defined by the expression of RORγt and production of are analogous to Th17 cells. Although ILCs perform functions similar to adaptive T cells, they are distinguished by the absence of antigen-specific receptors. Instead, their activation and responses are mediated through cytokine and alarmin signaling. These cells are primarily located at mucosal surfaces and other barrier sites and act as a crucial first line of defense by rapidly producing cytokines that orchestrate immune responses and maintain tissue homeostasis. Their role in health and disease is increasingly recognized, particularly in conditions such as inflammatory bowel diseases, cancer, autoimmunity, and metabolic disorders. A key aspect of their function is their interaction with the microbiota-a complex and diverse community of microorganisms essential for shaping host immunity. In particular, the gut microbiota influences immune cell development, modulates inflammatory responses, and impacts systemic immunity (Zheng et al., 2020). Positioned strategically at these barrier sites, ILCs are well-equipped to sense microbial signals and respond accordingly, helping to maintain a balanced immune environment. Disruptions in this crosstalk can lead to dysbiosis, driving the onset and progression of various diseases. Understanding these interactions holds significant potential for developing microbiota-targeted therapies that could restore immune balance and offer new approaches to disease treatment and prevention. In addition to exploring these biological interactions, the integration of multi-omics technologies and artificial intelligence (AI) has opened new avenues for investigating the complex relationships between immune cells and their environment. Xu et al. contribution within this Research Topic utilizes these advanced approaches to uncover new neutrophil clusters and potential biomarkers in sepsis, offering valuable insights into the immune landscape during severe infection. Abstract:Immune checkpoint molecules (ICM) are critical in maintaining immunologic homeostasis and participate in preventing or promoting autoimmune disease development. Exploring a large panel of intrahepatic inhibitory and stimulatory ICM is necessary for drawing a general picture of the immune alterations in autoimmune hepatitis (AIH). Here, we performed a multiparametric analysis of ICM, including PD-1, TIM3, LAG3, CTLA-4, OX40 and 4-1BB, and we determined their expression on intrahepatic lymphocyte subsets in untreated and in treated patients with AIH in comparison to normal liver tissue. AIH patient-derived liver tissue revealed the overexpression of ICM, mainly PD-1 and 4-1BB, as well as the strong correlation between PD-1+ CD8+ T-cell abundance and severity of AIH (alanine transaminase and aspartate transaminase levels). Our results show that the ICM play an important role in the loss of immune homeostasis in the liver, providing an attractive approach to investigate their role as targets for effective therapeutic interventions.

Abstract:Clinical routine in hepatology involves the diagnosis and treatment of a wide spectrum of metabolic, infectious, autoimmune and neoplastic diseases. Clinicians integrate qualitative and quantitative information from multiple data sources to make a diagnosis, prognosticate the disease course, and recommend a treatment. In the last 5 years, advances in artificial intelligence (AI), particularly in deep learning, have made it possible to extract clinically relevant information from complex and diverse clinical datasets. In particular, histopathology and radiology image data contain diagnostic, prognostic and predictive information which AI can extract. Ultimately, such AI systems could be implemented in clinical routine as decision support tools. However, in the context of hepatology, this requires further large-scale clinical validation and regulatory approval. Herein, we summarise the state of the art in AI in hepatology with a particular focus on histopathology and radiology data. We present a roadmap for the further development of novel biomarkers in hepatology and outline critical obstacles which need to be overcome.
