Abstract:Abstract Object: To discuss the application and effect evaluation of glasses-free 3D technology in ICL surgery teaching. Methods: 10 ophthalmologists with no experience in (implantable collamer lens) ICL surgery were selected to randomly watch the videos of 10 cases of ICL surgery on a 2D traditional screen and 10 cases on a glasses-free 3D screen, respectively, and complete the questionnaire on the learning experience after watching. The learning experience includes the incision making process, anterior chamber changes during viscoelastic injection and removal, the lens position judgment after the injection, the adjustment of the lens loop process, the overall three-dimensionality of the surgery, and the situation of dizziness. Results: The glasses-free 3D group showed better learning experience (P<0.05) in the incision making process, the changes of the anterior chamber during viscoelastic injection and removal, the adjustment of the lens loop, and the stereoscopic effect of the surgical video. During the lens injection process, the glasses-free 3D group could more accurately determine the initial position of the foot loop (P<0.001). There was no significant difference in the accuracy between the two groups (P=0.317) on the question of whether the ICL is turned over. Compared with the traditional 2D group, the glasses-free 3D group was more likely to experience dizziness (P<0.001) Conclusions: The glasses-free 3D display videos are more conducive for ophthalmologists to learn the key steps of ICL surgery, but it is still prone to dizziness.
Three-Dimensional Heads-up Cataract Surgery Using Femtosecond Laser: Efficiency, Efficacy, Safety, and Medical Education-A Randomized Clinical Trial

Abstract:Purpose: To compare the efficiency, efficacy, and safety, as well as the educational value, of heads-up (three-dimensional visualization system-assisted) and traditional microscopic cataract surgery. Methods: This randomized noninferiority trial enrolled 242 eyes of 201 patients who received femtosecond laser-assisted cataract surgery. The questionnaire study enrolled 26 medical interns and 39 medical students. Patients received surgery under either a three-dimensional visualization system (3D group, 117 eyes) or traditional microscope (TM group, 125 eyes) after random allocation. The primary outcome was surgical time. The noninferiority margin of surgical time was 60 seconds. Secondary outcomes included ultrasound power, phacoemulsification time, visual acuity, intraocular pressure, endothelial cell density, central corneal thickness, complications, and observer satisfaction scores for surgical procedures. Results: Surgical time was 462.03 +/- 80.36 seconds in the 3D group and 452.13 +/- 76.63 seconds in the TM group (difference 9.90 seconds; 95% CI, -9.98 to 29.78; P = 0.365). Visual acuity and other perioperative parameters were comparable between the 3D group and the TM group (all P > 0.05). Incidences of both intraoperative and postoperative complications were low and not statistically different between groups (all P > 0.05). Across all observers, 3D surgery was superior to TM surgery for improving the degree of satisfaction (all P < 0.001). Conclusions: The surgical efficiency of heads-up cataract surgery is not inferior to traditional microscopic surgery. Both methods achieved similar efficacy and safety outcomes. Moreover, heads-up cataract surgery showed a significant advantage in medical education. Translational Relevance: Our findings show that heads-up cataract surgery has comparable efficiency, efficacy, and safety, as well as superior medical educational value, to TM surgery, which lays the foundation for promoting and popularizing this technology.

Abstract:Objective: Precise minimally invasive surgery (MIS) has significant advantages over traditional open surgery in clinic. Although pre-/intraoperative diagnosis images can provide necessary guidance for therapy, hand-eye discoordination occurs when guidance information is displayed away from the surgical area. In this study, we introduce a real three-dimensional (3-D) see-through guidance system for precision surgery. Methods: To address the resolution and viewing angle limitation as well as the accuracy degradation problems of autostereoscopic 3-D display, we design a high quality and high accuracy 3-D integral videography (IV) medical image display method. Furthermore, a novel see-through microscopic device is proposed to assist surgeons with the superimposition of real 3-D guidance onto the surgical target is magnified by an optical visual magnifier module. Results: Spatial resolutions of 3-D IV image in different depths have been increased 50%similar to 70%, viewing angles of different image sizes have been increased 9%similar to 19% compared with conventional IV display methods. Average accuracy of real 3-D guidance superimposed on surgical target was 0.93 mm +/- 0.41 mm. Preclinical studies demonstrated that our system could provide real 3-D perception of anatomic structures inside the patient's body. Conclusion: The system showed potential clinical feasibility to provide intuitive and accurate in situ see-through guidance for microsurgery without restriction on observers' viewing position. Significance: Our system can effectively improve the precision and reliability of surgical guidance. It will have wider applicability in surgical planning, microscopy, and other fields.
Short-term Evaluation of Visual Quality, Amplitude of Accommodation, and Stereoacuity Between Patients with Moderate-to-High Myopia Who Underwent ICLV4c Implantation and SMILE.

Abstract:PURPOSE: To compare the visual quality, accommodation, and stereoacuity between patients with a myopia magnitude of-3.00 to-8.50 diopters (D) who underwent ICLV4c (STAAR Surgical) implantation and small incision lenticule extraction (SMILE). METHODS: The visual quality parameters, amplitude of accommodation (AMP), and stereoacuity were compared between and within the ICLV4c and SMILE groups (40 eyes/ group) with a mean spherical equivalent (SE) of-6.14 D. Differences in these parameters before and after surgery were analyzed using SPSS 22.0 software (SPSS, Inc).RESULTS: Modulation transfer function (MTF) cut-off increased significantly by 3.24 cycles/degree (t =-2.111, P =.041) and AMP decreased significantly by 1.66 D (t = 7.519, P < .001) from baseline to 3 months after surgery in the ICLV4c group. However, the MTF cut-off was significantly lower (t = 2.123, P = .037) and the AMP was significantly better (t =-3.605, P = .001) in the SMILE group than in the ICLV4c group at 3 months postoperatively. Other parameters such as Objective Scatter Index, Strehl ratio, and Optical Quality Analysis System (Visiometrics) under different contrast values did not show significant changes (P > .05). Stereoacuity did not improve significantly, except in a patient with anisometropia (2.50 D) in the ICLV4c group.CONCLUSIONS: The ICLV4c implantation yielded a better visual quality but worse AMP than did SMILE in patients with a myopia magnitude of-3.00 to-8.50 D in the short term.
Floating autostereoscopic 3D display with multidimensional images for telesurgical visualization

Abstract:Purpose We propose a combined floating autostereoscopic three-dimensional (3D) display approach for telesurgical visualization, which could reproduce live surgical scene in a realistic and intuitive manner. Methods A polyhedron-shaped 3D display device is developed for spatially floating autostereoscopic 3D image. Integral videography (IV) technique is adopted to generate real-time 3D images. Combined two-dimensional (2D) and 3D displays are presented floatingly around the center of the display device through reflection of semitransparent mirrors. Intra-operative surgery information is fused and updated in the 3D display, so that telesurgical visualization could be enhanced remotely. Results The experimental results showed that our approach can achieve a combined floating autostereoscopic display that presents 2D and 3D fusion images. The glasses-free IV 3D display has full parallax and can be observed by multiple persons from surrounding areas at the same time. Furthermore, real-time surgical scene could be presented and updated in a realistic and intuitive visualization platform. It is shown that the proposed method is feasible for facilitating telesurgical visualization. Conclusion The proposed floating autostereoscopic display device presents surgical information in an efficient form, so as to enhance operative cooperation and efficiency during operation. Combined presentation of imaging information is promising for medical applications.

Abstract:AIM: To determine the teaching effects of a real-time three dimensional (3D) visualization system in the operating room for early-stage phacoemulsification training. METHODS: A total of 10 ophthalmology residents of the first-year postgraduate were included. All the residents were novices to cataract surgery. Real-time cataract surgical observations were performed using a custom-built 3D visualization system. The training lasted 4wk (32h) in all. A modified International Council of Ophthalmology’s Ophthalmology Surgical Competency Assessment Rubric (ICO-OSCAR) containing 4 specific steps of cataract surgery was applied. The self-assessment (self) and expert-assessment (expert) were performed through the microsurgical attempts in the wet lab for each participant. RESULTS: Compared with pre-training assessments (self 3.2±0.8, expert 2.5±0.6), the overall mean scores of post-training (self 5.2±0.4, expert 4.7±0.6) were significantly improved after real-time observation training of 3D visualization system (P<0.05). Scores of 4 surgical items were significantly improved both self and expert assessment after training (P<0.05). CONCLUSION: The 3D observation training provides novice ophthalmic residents with a better understanding of intraocular microsurgical techniques. It is a useful tool to improve teaching efficiency of surgical education.

Abstract:There has been ongoing progress in the new technique and equipment in vitreoretinal surgery in recent years,contributing to the improvement of treatment of various vitreoretinal diseases.The application of 3D heads-up display viewing system (3D viewing system) has been one of the most fascinating breakthroughs in vitreoretinal surgery.Unlike the traditional method in which the surgeons have to look through the microscope eyepieces,this system allows them to turn their heads up and operate with their eyes on a high-definition 3D monitor.It provides the surgeons with superior visualization and stereoscopic sensation.And increasing studies have revealed it to be as safe and effective as the traditional microscopic system.Furthermore,the surgeons can keep a heads-up position in a more comfortable posture and lesson the pressure on cervical spine.Meanwhile,3D viewing system makes it easier for the teaching and learning process among surgeons and assistants.However,there are still potential disadvantages including the latency between surgeon maneuver and visualization on the display,learning curves and cost.We hope that the 3D viewing system will be widely used and become a useful new tool for various vitreoretinal diseases in the near future with rapid development in the technology and constant upgrade of the system.

Abstract:Three-dimensional (3D) visualization of preoperative and intraoperative medical information becomes more and more important in minimally invasive surgery. We develop a 3D interactive surgical visualization system using mobile spatial information acquisition and autostereoscopic display for surgeons to observe surgical target intuitively. The spatial information of regions of interest (ROIs) is captured by the mobile device and transferred to a server for further image processing. Triangular patches of intraoperative data with texture are calculated with a dimension-reduced triangulation algorithm and a projection-weighted mapping algorithm. A point cloud selection-based warm-start iterative closest point (ICP) algorithm is also developed for fusion of the reconstructed 3D intraoperative image and the preoperative image. The fusion images are rendered for 3D autostereoscopic display using integral videography (IV) technology. Moreover, 3D visualization of medical image corresponding to observer’s viewing direction is updated automatically using mutual information registration method. Abstract:Abstract Minimally invasive surgery (MIS) has attracted significant interest in current medicine for less operative pain and complications, smaller incisions, and faster recovery times. Image-guided surgery (IGS) is a general term used for any surgical procedure with indirect vision to realize the MIS. However, in current IGS, most three-dimensional (3D) medical images used for surgical navigation are displayed in 2D plane monitors, which lack intuitive 3D spatial information. Moreover, surgeons easily suffer from eye–hand coordination problems due to that the 2D navigation screen being away from the surgical area. This chapter focuses on 3D medical image rendering and display as well as augmented reality (AR)-based 3D image-guided techniques. Among the various 3D image rendering and display techniques, integral videography (IV) is promising due to its full parallax, continuous viewing points, autostereoscopic displayed images, and simple implementation without dedicated glasses or tracking devices. An AR-based 3D image-guided system using IV technique can present intuitive “see-through” surgical scenes to improve eye–hand coordination in diagnosis, surgical planning, operation, and telemedicine. With the assistance of IV image-guided techniques, MIS with enhanced safety, efficiency, and accuracy can be utilized widely for improving the quality of life.

Abstract:Purpose: The objective of this study is to demonstrate, based on objective clinical indicators, the advantages of depth of field provided by the 3D surgical video system compared to the traditional microscope during vitrectomy for treating epiretinal membranes(ERM) or macular holes(MH). Methods: A total of 38 patients were included in this study and randomly assigned to either the 3D surgical video group or the conventional microscope group. Surgical parameters such as the Focal plane adjustment frequency (FPAF), membrane peeling time, and number of attempts to peel the membrane were recorded for each patient. Additionally, patients were followed up for 3 months postoperatively. Results: No significant differences in age, gender, operated eyes, or follow-up rates between the groups. The 3D group had significantly lower Focal Plane Adjustment Frequency (FPAF) in macular hole surgery and epiretinal membrane surgery . No significant differences in peeling maneuvers, time, or total surgical time. Postoperative follow-up data showed no significant differences. Conclusion: In conclusion,the 3D surgical video system exhibits potential advantages in depth of field.The 3D surgical video system is a safe and effective technology in vitrectomy for macular diseases.

Visual Outcomes Early after Implantable Collamer Lens V4c Implantation in Different Preoperative Spectacle Correction: Full Correction Vs. under Correction

Abstract:PurposeTo investigate visual outcomes early after implantable collamer lens (ICL) V4c implantation between patients with fully corrected and under-corrected spectacles preoperatively.MethodsPatients who implanted ICL V4c were divided into the full correction (46 eyes/23 patients) and under-correction groups (48 eyes/24 patients) based on preoperative differences between the spherical diopter of the spectacles and the actual spherical diopter. Refractive outcomes, scotopic pupil size, higher-order aberrations, and subjective visual outcomes as assessed using a validated questionnaire were compared between the two groups 3 months postoperatively. Moreover, the relationships between the severity of haloes and postoperative ocular or ICL parameters were analyzed.ResultsAt the 3-month follow-up, the efficacy indices in the full correction and under-correction groups were 0.99 +/- 0.12 and 1.00 +/- 0.10, respectively; the safety indices were 1.15 +/- 0.16 and 1.15 +/- 0.15, respectively. Total-eye spherical aberration (p < 0.0001) and internal spherical aberration (p = 0.0005) were significantly different pre- and post-operatively in the under-correction group, while no differences were found in the full correction group. Total-eye spherical aberration (p = 0.002) and the severity of haloes (p = 0.03) were postoperatively different between the two groups. The severity of haloes was associated with postoperative spherical aberration (total-eye spherical aberration: r = -0.32, p = 0.002; internal spherical aberration: r = -0.24, p = 0.02).ConclusionGood efficacy, safety, predictability, and stability were obtained early after surgery regardless of preoperative spectacle correction. Patients in the under-correction group possessed a shift to negative spherical aberration and reported greater severity of haloes at the 3-month follow-up. Haloes were the most common visual symptoms after ICL V4c implantation and the severity of them was correlated with postoperative spherical aberration.

Abstract:To accurately guide surgical instruments during ophthalmic procedures, some necessary intraoperative depth perception is required, which standard surgical microscopes supply limitedly. Intraoperative optical coherence tomography (iOCT), combining optical coherence tomography (OCT) technology and surgical microscope, enables noninvasive, real-time and high-resolution cross-sectional imaging. Currently, though iOCT enables structural imaging, little research has been done on intraoperative angiography. In this work, we presented a swept-source intraoperative OCT angiography (SS-iOCTA) system based on a standard surgical microscope, which provides both structural and angiographic images. The feasibility of the proposed SS-iOCTA was confirmed through deep anterior lamellar keratoplasty (DALK) of ex vivo porcine eyes and blood perfusion imaging of in vivo rat cortex. High-resolution intraoperative feedback, including sub-surface structure and angiogram of biological tissue, can be visualized simultaneously with the SS-iOCTA system, which expand the surgeon’s capabilities and could be widely used in clinical surgery.

Abstract:We propose a bifocal computational near eye light field display (bifocal computational display) and structure parameters determination scheme (SPDS) for bifocal computational display that achieves greater depth of field (DOF), high resolution, accommodation and compact form factor. Using a liquid varifocal lens, two single-focal computational light fields are superimposed to reconstruct a virtual object's light field by time multiplex and avoid the limitation on high refresh rate. By minimizing the deviation between reconstructed light field and original light field, we propose a determination framework to determine the structure parameters of bifocal computational light field display. When applied to different objective to SPDS, it can achieve high average resolution or uniform resolution display over scene depth range. To analyze the advantages and limitation of our proposed method, we have conducted simulations and constructed a simple prototype which comprises a liquid varifocal lens, dual-layer LCDs and a uniform backlight. The results of simulation and experiments with our method show that the proposed system can achieve expected performance well. Owing to the excellent performance of our system, we motivate bifocal computational display and SPDS to contribute to a daily-use and commercial virtual reality display. Application of three-dimensional visualization technology in phacoemulsification benefits the dry eye symptoms of patients after cataract surgery

Abstract:Purpose To investigate the effects of the three-dimension visualization system on dry eye in patients after cataract phacoemulsification. Methods Seventy-one patients (104 eyes) were enrolled in this study and assigned to the conventional microscopic group (CM group) or three-dimension group (3D group). Ocular Surface Disease Index, The Schirmer I test (SIt), lipid layer thickness (LLT), noninvasive tear breakup time (NIBUT) and other factors associated with dry eye were recorded before and 1 week and 1 month after surgery. The operation time and intraoperative light intensity (ILI) were also recorded. Results The ILI in the 3D group was significantly lower than that in the CM group (33% vs. 60%, p < 0.01). There was an interaction (group and time) effect for first NIBUT (NIBUT-F), average NIBUT (NIBUT-Ave), tear meniscus height (TMH) and the score of eye redness ( P interaction < 0.05). The main effects of group on SIt, LLT, NIBUT-F, NIBUT-Ave and the score of eye redness were significant ( P group < 0.05). In the CM group, SIt, LLT, NIBUT-F, NIBUT-Ave, TMH were lower than those of the 3D group, the score of eye redness was higher than that of the 3D group at 1 week and 1 month after surgery (all p < 0.05). The changes in NIBUT-F and NIBUT-Ave between before surgery and 1 week after surgery showed negative correlations with ILI ( p < 0.01). Conclusion Compared with microscopic surgery, the 3D visualization system can provide better tear film stability for patients after cataract surgery.

Abstract:Image guided surgery has shown increasing importance as the public is in need of a surgical procedure that is minimally invasive, requires short recovery time, and can be performed in a safer and more efficient way. Current researches focus on how to utilize medical images to guide preoperative planning and to intraoperatively ensure real-time diagnosis, high-accurate operation, and high-efifcient treatment. This paper proposed a surgical navigation technique and system for speciifc clinical scenarios, and provided an integral videography three-dimensional (3D) image based high-precision augmented reality. Key technologies involved in the surgical navigation system were also introduced. The surgical tools can be tracked in real time and updated in 3D display. Using 3D image overlay system, 3D autostereoscopic images of critical anatomy structures and tools can be observed with naked eyes in surgical scene. The proposed system can provide intuitive and precise guidance for surgeons by providing a “see-through” 3D augmented scene.
Advanced stereoscopic projection technology significantly improves novice performance of minimally invasive surgical skills

Abstract:BackgroundThree-dimensional (3D) surgical imaging systems provide stereoscopic depth cues that are lost in conventional two-dimensional (2D) display systems. Recent improvements in stereoscopic projection technology using passive polarising displays may improve performance of minimally invasive surgical skills. This study aims to identify the effect of passive polarising stereoscopic displays on novice surgeon performance of minimally invasive surgical skills.Methods20 novice surgeons performed 10 repetitions of 4 surgical skills tasks using a new passive polarising stereoscopic display under 3D and 2D conditions. The previously validated tasks used were rope pass, paper cut, needle capping and knot tying. Outcome measures included total error rate and time for task completion.ResultsNovice surgeons demonstrated a significant reduction in error rates for sequential repetitions of each task using the passive polarising stereoscopic display compared with the 2D display. Mean errors for the 3D versus the 2D mode were 2.0 versus 4.3 for rope pass (P ≤ 0.001), 0.8 versus 1.6 for paper cut (P = 0.001), 1.3 versus 4.2 for needle capping (P ≤ 0.001) and 2.8 versus 8.0 for knot tying (P ≤ 0.001). Novice surgeons demonstrated a significant improvement in mean time for completion for all four tasks when using the 3D system. Mean time (in seconds) for 3D versus 2D were 106.5 versus 134.4 for rope pass (P ≤ 0.001), 116.1 versus 176.3 for paper cut (P ≤ 0.001), 76.3 versus 141.6 for needle capping (P ≤ 0.001) and 153.4 versus 252.6 for knot tying (P ≤ 0.001).ConclusionPassive polarising stereoscopic displays significantly improve novice surgeon performance during acquisition of minimally invasive surgical skills.
Enhance Medical Education and Training Using Interactive Autostereoscopic 3D Display by In Situ 3D Interaction

Abstract:In this study, we proposed to combine autostereoscopic 3D display and interactive display to enhance medical education and training, which could be an alternative to the traditional medical education. We developed an enhanced 3D rendering algorithm for medical education and training based on the traditional lens based rendering algorithm (LBR). We used the system to perform a user study of medical visualization and education with a heart phantom, and a human-computer interaction device to capture hand motions, rotations, and gestures. Structure of the heart was labeled and validated by physician ahead. Furthermore, we devided 40 users (20 women and 20 men) into two groups: control group and experimental group, to operate a user study on anatomical structure of the heart phantom. Experimental results showed that our algorithm can achieve higher rendering performance than the traditional LBR. User test showed that 3D education system equipped by the enhanced LBR can achieve interactive display, and can thus promote education experience and reduce education time consuming. Further work includes more user studies on medical education and increasing frame rate of rendering for more versatile hardware configurations.
